Abstract

This research project explored the use of new wearable and mobile technologies to support independent living and social interactions in the community for people living with dementia and their carers, more safely and for longer. The three key aims of the project were to: • Assess the potential acceptability, and usability of a mobile phone App and wearable wristband solution • Test the use of the technological in the everyday life contexts • Evaluate the usability and utility of a mobile phone App and digital wristband to reduce social isolation and improve health outcomes The research project had two stages: Stage 1 - Acceptability, utility and usability of the devices This stage of the project focused on developing initial understandings of the ways in which the proposed technological solutions work and potentially meet the needs of people living with dementia and their carers. This was achieved through: - Technology testing by the project team members. This gave the team a better understanding of the device used in the project. - Interviews with people with dementia and their carers, and with health and social care professionals. The interviews explored whether participants were receptive to, and what they thought about, the proposed solutions. Any concerns they had about the tracking technology in general or about the technology used in the project were also examined. Stage 2: Real world testing of the wristbands and smartphone App Research participant’s trialled the technology, as part of their everyday lives, for up to three months. During this period the research team worked closely with them to provide training and technological support and used a multi-method data collection focused on semi-structured interviews and observation to capture their experiences of using the technology. The participants living with dementia and their carers were given the opportunity to choose the technology that best suits them, either a wristbands or a smartphone application. Key findings In summary, through the interviews and focus groups and real world technology testing the research team and SME partners found there are issues around: - the use of technology for people living with dementia and their carers – for some people it was seen as a real benefit, others did not want to engage with the technology or have others know where they were. - education, information and support for people living with dementia and their carers to use new technologies; even when delivered via familiar technologies this can be challenging and needs support and time - support for family/other carers who are key to the support of mobile and wearable technologies. - support for people who do not have a local or remote family carer who can support the use of new technologies needs to be considered. - current GPS technologies not being accurate as a stand alone way of finding someone who may be lost or needing support. - safeguarding – which needs to take an holistic approach and include more traditional and ‘paper based’ safeguarding systems such as the Herbert protocol, - emerging and new technologies, which are developing constantly but a national approach is still missing; - the challenges for unpaid carers and people living with dementia to around exploring options around new technologies and deciding what are the best/cost effective options for their situations. - wearable and mobile technologies which can support people to be more independent in the community, but the technologies need to be: introduced early; affordable and be more easily supported by family and professional care givers as appropriate.

18. Associations Between Drosophila suzukii (Diptera: Drosophilidae) and Fungi in Raspberries.

Abstract

The invasive vinegar fly, Drosophila suzukii Matsumura, has emerged as one of the most serious arthropod pests of primocane red raspberries (Rubus ideaus L.) in the United States. In raspberries, D. suzukii encounter a diverse community of microbes, including fruit rot pathogens such as Botrytis cinerea Pers and Cladosporium cladosporioides de Vries. In this study, our primary objectives were to evaluate D. suzukii– fungal associations and determine D. suzukii 's influence on fungal communities in raspberry fruit. Through culture-based surveys of larval gut microbes, we isolated several yeast fungi (primarily Hanseniaspora spp.), as well as Cladosporium, Botrytis, and several other non-yeast fungi from larval frass, suggesting that D. suzukii larvae encounter and feed on these fungi. Subsequent field surveys confirmed that D. suzukii larvae occurred in berries affected by Botrytis fruit rot and Cladosporium fruit rot. Under laboratory conditions, D. suzukii may facilitate C. cladosporioides infections, likely through the introduction of epiphytic propagules on the fruit surface. We could not detect impacts on B. cinerea infections or establish a clear vectoring relationship for either fruit rot. These studies provide evidence for an association between D. suzukii and fungal fruit rot pathogens. Understanding interactions between raspberry fruit, D. suzukii, and fungal microbes—especially whether D. suzukii facilitates the development of fruit rots or conversely, if fruit rots influence D. suzukii infestation patterns—may improve pest and pathogen management programs. Abstract

PubMed ID: 12829583, Several studies have compared bone marrow (BM) and peripheral blood (PB) as stem cell sources in patients receiving allografts, but the cell doses infused have not been considered, especially for BM. Using the ALWP/EBMT registry, we retrospectively studied 881 adult patients with acute myelocytic leukemia (AML), who received a non-T-depleted allogeneic BM (n = 515) or mobilized PB (n = 366) standard transplant, in first remission (CR1), from an HLA-identical sibling, over a 5-year period from January 1994. The BM cell dose ranged from 0.17 to 29 × 10 8 /kg with a median of 2.7 × 10 8 /kg. The PB cell dose ranged from 0.02 to 77 × 10 8 /kg with a median of 9.3 × 10 8 /kg. The median dose for patients receiving BM (2.7 × 10 8 /kg) gave the greatest discrimination. In multivariate analyses, high-dose BM compared to PB was associated with lower transplant-related mortality (RR = 0.61; 95% CI, 0.39-0.98; P = .04), better leukemia-free survival (RR = 0.65; 95% CI, 0.46-0.91; P = .013), and better overall survival (RR = 0.64; 95% CI, 0.44-0. 92; P = .016). The present study in patients with AML receiving allografts in first remission indicates a better outcome with BM as compared to PB, when the dose of BM infused is rich. © 2003 by The American Society of Hematology.

Abstract

Several studies have compared bone marrow (BM) and peripheral blood (PB) as stem cell sources in patients receiving allografts, but the cell doses infused have not been considered, especially for BM. Using the ALWP/EBMT registry, we retrospectively studied 881 adult patients with acute myelocytic leukemia (AML), who received a non-T-depleted allogeneic BM (n = 515) or mobilized PB (n = 366) standard transplant, in first remission (CR1), from an HLA-identical sibling, over a 5-year period from January 1994. The BM cell dose ranged from 0.17 to 29 × 108/kg with a median of 2.7 × 108/kg. The PB cell dose ranged from 0.02 to 77 × 10 8/kg with a median of 9.3 × 108/kg. The median dose for patients receiving BM (2.7 × 108/kg) gave the greatest discrimination. In multivariate analyses, high-dose BM compared to PB was associated with lower transplant-related mortality (RR = 0.61; 95% CI, 0.39-0.98; P = .04), better leukemia-free survival (RR = 0.65; 95% CI, 0.46-0.91; P = .013), and better overall survival (RR = 0.64; 95% CI, 0.44-0. 92; P = .016). Abstract

Translation of drug candidates into clinical settings requires demonstration of preclinical efficacy and formal toxicology analysis for filling an Investigational New Drug (IND) application with the US Food and Drug Administration (FDA). Here, we investigate the membrane-associated glucose response protein 78 (GRP78) as a therapeutic target in leukemia and lymphoma. We evaluated the efficacy of the GRP78-targeted proapoptotic drug bone metastasis targeting peptidomimetic 78 (BMTP-78), a member of the D(KLAKLAK)2-containing class of agents. BMTP-78 was validated in cells from patients with acute myeloid leukemia and in a panel of human leukemia and lymphoma cell lines, where it induced dose-dependent cytotoxicity in all samples tested. Based on the in vitro efficacy of BMTP-78, we performed formal good laboratory practice toxicology studies in both rodents (mice and rats) and nonhuman primates (cynomolgus and rhesus monkeys). These analyses represent required steps towards an IND application of BMTP-78 for theranostic first-in-human clinical trials.

Abstract

There is a strong interest in maximizing the use of locally produced protein sources, like faba beans (FB), as a substitute for imported soybean meal (SBM). The aim of the present study was to determine the effects of dietary FB inclusion on diet digestibility. A further aim was to find an optimal inclusion level of locally produced unprocessed FB seeds in diets of broilers. A total of 196 male Ross 508 chicks were randomly assigned to four feeding treatments, and seven replications per treatment. FB seeds in proportions of 0, 80, 160, and 240 g/kg in the wheat- and SBM-based diets were tested in the grower period (bird age of 6–32 days). Digestibility and retention of nutrients were determined at the end of the experiment, and production performance was measured throughout the experiment. Apparent total tract digestibility of ash, nitrogen retention, apparent ileal digestibility of organic matter, and the nitrogen-corrected apparent metabolizable energy increased, with increased FB inclusion. The apparent ileal digestibilities of alanine, arginine, aspartic acid, histidine, and treonine increased by FB inclusion, while the apparent ileal digestibilities of cysteine and proline decreased. Digestibility differences were related to FB inclusion, but as well changes in proportion of wheat and SBM. Body weight, body weight gain, and feed consumption linearly decreased when dietary FB inclusion increased, while feed conversion ratio linearly improved. FB inclusion had no negative effect on the health of the broilers. In conclusion, 160 g/kg of FB can be used in broiler diets without major negative effects on production performance. Abstract

A critical step in cancer progression is the dissolution of the extracellular matrix surrounding the malignant tumor leading to invasion and dissemination of tumor cells. This process involves increased production of matrix-degrading proteinases, e.g. matrix metalloproteinases, plasminogens and plasminogen activators. Recently tumor-associated trypsins (TATs) have been implicated in this process. We now show that the major isoenzyme of tumor-associated trypsin (TAT)-2 directly activates latent human promatrix metalloproteinases (proMMP)-9 and -2. TAT-2 converted the proMMP-9 to the 77 kD MMP-9 species in a time-and dose-dependent manner causing a significant superactivation of MMP-9. The activation was inhibited by the tumor-associated trypsin inhibitor (TATI), a specific endogenous inhibitor of TATs. ProMMP-2 was also activated by TAT-2 but less efficiently. ProMMP-2 can also be activated by membrane-type MMP that is induced in human MDA-MB-231 breast carcinoma cells by Concavalin A (Con A). Addition of TATI to the Con A-induced MDA-MB-231 cells did not, however, inhibit the conversion of exogenous 72 kD proMMP-2 to 59 kD active MMP-2 species. The ability of TAT-2 to directly activate type IV procollagenases-especially proMMP-9 - suggests a previously unrecognized function for TAT-2 in the degradation of extracellular matrix and basement membrane associated with tumor progression and metastasis. Inhibition of human trypsin (TAT)-2 by TATI and/or potent synthetic inhibitors may prevent TAT-2-induced proMMP-9 activation cascade associated with human malignancies.

Abstract

Due to poor prognosis with conventional therapy, high-dose therapy (HDT) with autologous stem cell transplantation (ASCT) is considered for treatment in patients with primary amyloidosis (AL). Only single centre series are available on the feasibility and efficacy of this approach. Altogether 20 AL patients (11 males, 9 females, median age 54 years) were included in HDT protocols in 5 Finnish transplant centres between 1997 and 2003. Twelve patients were mobilized with granulocyte colony-stimulating factor (G-CSF) alone and 8 patients with a combination of cyclophosphamide and G-CSF. Sixteen patients (80%) went on to high-dose melphalan. Early transplant-related mortality was 25%. Nine out of 11 evaluable patients showed improvement or stabilization of AL. The overall survival of the transplanted patients is 69% (median follow-up 13 months). After a median follow-up of 26 months for the living patients, only 2 patients (18%) have shown progression of AL. This retrospective nation-wide analysis shows that HDT with ASCT leads to improvement or stabilization of AL in the majority of the patients who survive the immediate posttransplant period. A randomized multicentre trial is needed to show whether ASCT is superior to conventional therapy in patients with AL. [ABSTRACT FROM AUTHOR]

Abstract

We have previously shown that various benign and malignant natural killer (NK)-resistant monolayer cells inhibit endogenous human NK activity, probably by reducing the secretion of cytotoxic factors from the effector cells. The nature of the molecules responsible for the inhibition has been unclear. In this study we show that phosphate-buffered saline (PBS) extracts of ovarian cystadenocarcinoma tissue and normal uterine smooth muscle strongly inhibit NK activity. Fractionation of tumour extracts by gel chromatography revealed major inhibitory activity in the Mr range 160,000-180,000, and other weaker inhibiting activities. In the Mr ranges 50,000-70,000 and 20,000. The active material of Mr range 160,000-180,000 was adsorbed on anion exchange chromatography column at neutral pH and physiologic NaCl concentration, and it was eluted by 0.31-0.34 M NaCl. The inhibitory molecule was sensitive to proteolysis. No relation of this compound to immunoglobulins or trypsin and urokinase inhibitors was detected. The unfractionated extract inhibited NK activity apparently by the same mechanism as the monolayer target cells, i.e. by reducing the secretory capacity of effector cells. The data strongly suggest that the NK-inhibiting compounds described in this work are involved in the inactivation of NK cells by intact monolayer cells. [ABSTRACT FROM AUTHOR]

Abstract

The metabolism of RU 486 was studied in female volunteers following a single oral administration of 100, 400, 600 or 800 ing of RU 486. The serum concentrations of RU 486 were generally not affected by the dose within the range examined and they stayed at micromolar concentrations during the 48 h studied. RU 486 was metabolized extensively in a dose-dependent manner by two-step demethylation, and by hydroxyl-ation. Serum levels of the monodemethylated metabolite always exceeded those of RU 486. The concentrations of the didemethylated and hydroxylated metabolites equalled or exceeded those of RU 486 when the ingested dose was 400 mg or more. Monodemethylation and hydroxylation were rapid high-capacity reactions, whereas didemethylation was a tower-capacity reaction. In each group of different dosage, positive correlations were found between the individual mean α 1-acid glycoprotein (AAG) concentrations and the peak concentration of RU 486 measured at 1-2 h, versus the plateau concentration of RU 486 measured at 6 h. The in-vitro studies showed that the specific serum transport protein of RU 486, AAG, was saturated by RU 486 concentrations exceeding 2.5 /tM. In serum at 40 nM and 2.5 /tM RU 486 concentrations, 2.7% and 2.4%, respectively, of [H]RU 486 was not protein bound. Purified AAG in phosphate buffer was able to bind [H]RU 486 in a similar manner to that of serum. Thus our results suggest that AAG regulates in part the serum concentrations of RU 486, and RU 486 exceeding the specific serum transport capacity is effectively metabolized. Abstract

Aim and methods: The aim was to find out the number of patients with chronic wounds in Helsinki, Espoo and Vantaa (the Capital region in Finland). The survey was made cross-sectionally during one day in December 2008. It included all patients in primary health care wards, health centres and home care as well as in hospital wards and out-patient clinics. Questionnaires were sent to 765 units. Results: A total of 466 questionnaires (61%) were returned. A total of 1029 patients with chronic wounds were encountered. 524 (51%) of them were institutionalized. Chronic wound prevalence encoutered was 0.10% in the Capital region. 23 percent were pressure ulcers, 13% venous, 11% diabetic, 11% unhealed operative, 7% ischemic, 21% multifactorial and 14% wounds with unsettled aetiology. 36% were situated in the feet or at the ankle level, 27% in the legs and 19% in the back, hips or buttocks. Conclusions: When compared to previous data 29% increase in chronic wounds was observed in Helsinki in 10 years. The number of pressure ulcers seemed to be decreasing, whereas the number of venous, diabetic and ischemic, often included in the multifactorial group, was increasing. [ABSTRACT FROM AUTHOR]
