Kashiwanoha, Chiba-ken, Japan Background: There are very few effective, scientifically validated treatments with known mechanisms of action for treatment of hair loss in both men and women. Fibroblast growth factor 5 (FGF5) is an important factor in the irreversible transition from anagen to catagen, and inhibition of FGF5 prolongs anagen phase and reduces hair loss. Objective: We aimed to screen botanically derived molecules for FGF5 inhibitory activity in vitro and assess efficacy in a clinical setting. Methods: We screened for FGF5 inhibitory efficacy via a novel 2-step in vitro pipeline consisting of an engineered FGF5 responsive cell line, followed by an activated dermal papillae (DP) cell method. Efficacy in a clinical setting was assessed in a randomized, single-blind, placebo-controlled trial against early- to mid-stage pattern hair loss in men and women. Results: We observed FGF5 inhibitory activity for a number of compounds from the monoterpenoid family, many showing greater inhibitory efficacy than our previously reported crude plant extracts. Evaluation of a lead candidate in a clinical study over 112 days showed a significant improvement in anagen:telogen (AT) ratio (p = 0.002), reduced hair fall (p = 0.007) and improved visual grading (p = 0.004). Scientifically matched photography on a subgroup of randomly chosen participants highlighted significant improvement in hair density, with increases evident in all tested participants compared to baseline. Conclusion: Isolates from the monoterpenoid family displayed efficacy in FGF5 inhibition in vitro. A topical formulation containing a leading isolate significantly improved AT ratio, reduced hair fall and increased apparent hair density in the tested population of men and women. Thrombocytosis is associated with increased mortality in ESA-treated HD patients. We investigated variables affecting platelet count and its variability (platelet count increment [Δplatelet count]) in HD patients. This retrospective longitudinal and observational study of HD outpatients was carried out over 3 years. The outcome was independent determinants of platelet count and platelet count, which were associated with iron indices, ESA dose, and C-reactive protein. In univariate regression analysis, V-shaped relationship was observed between platelet count and transferrin saturation (TSAT), ferritin, serum iron, and hemoglobin (Hb) with the bottom of 0.21, 330 ng/mL, 49 µg/dL, and 10.3 g/dL, respectively. Mixed-effect multivariate regression analysis revealed that TSAT (inversely), Hb ≤10.3 g/dL (inversely), C-reactive protein, and ESA dose were independently associated with platelet count. Δplatelet count was independently and inversely correlated with ΔTSAT and directly correlated with Δferritin. TSAT was independently and inversely associated with ESA dose. ESA dose was directly correlated with iron dose and inversely correlated with TSAT, ferritin ≤330 ng/mL, and Hb ≤10.3 g/dL. ESA dose and TSAT were correlated in determining platelet count and Δplatelet count. Targets of iron indices that reflect iron supply sufficient to avoid platelet count increment and variability may be >21% of TSAT and 300 ng/mL of serum ferritin for appropriate ESA therapy in HD patients. Abstract

BACKGROUND: Diaphragmatic hernia (DH) presenting acutely can be a potentially life-threatening condition. Its management continues to be debatable. METHODS: A bibliographic search using major databases was performed using the terms "emergency surgery" "diaphragmatic hernia," "traumatic diaphragmatic rupture" and "congenital diaphragmatic hernia." GRADE methodology was used to evaluate the evidence and give recommendations. RESULTS: CT scan of the chest and abdomen is the diagnostic gold standard to evaluate complicated DH. Appropriate preoperative assessment and prompt surgical intervention are important for a clinical success. Complicated DH repair is best performed via the use of biological and bioabsorbable meshes which have proven to reduce recurrence. The laparoscopic approach is the preferred technique in hemodynamically stable patients without significant comorbidities because it facilitates early diagnosis of small diaphragmatic injuries from traumatic wounds in the thoraco-abdominal area and reduces postoperative complications. Open surgery should be reserved for situations when skills and equipment for laparoscopy are not available, where exploratory laparotomy is needed, or if the patient is hemodynamically unstable. Damage Control Surgery is an option in the management of critical and unstable patients. CONCLUSIONS: Complicated diaphragmatic hernia is a rare life-threatening condition. CT scan of the chest and abdomen is the gold standard for diagnosing the diaphragmatic hernia. Laparoscopic repair is the best treatment option for stable patients with complicated diaphragmatic hernias. Open repair is considered necessary in majority of unstable patients in whom Damage Control Surgery can be life-saving.

Abstract

Enhanced perioperative care protocols become the standard of care in elective surgery with a significant improvement in patients’ outcome. The key element of the enhanced perioperative care protocol is the multimodal and interdisciplinary approach targeted to the patient, focused on a holistic approach to reduce surgical stress and improve perioperative recovery. Enhanced perioperative care in emergency general surgery is still a debated topic with little evidence available. The present position paper illustrates the existing evidence about perioperative care in emergency surgery patients with a focus on each perioperative intervention in the preoperative, intraoperative and postoperative phase. For each item was proposed and approved a statement by the WSES collaborative group.

Abstract

Many countries are facing an aging population. As people live longer, surgeons face the prospect of operating on increasingly older patients. Traditional teaching is that with older age, these patients face an increased risk of mortality and morbidity, even to a level deemed too prohibitive for surgery. However, this is not always true. An active 90-year-old patient can be much fitter than an overweight, sedentary 65-year-old patient with comorbidities. Recent literature shows that frailty—an age-related cumulative decline in multiple physiological systems, is therefore a better predictor of mortality and morbidity than chronological age alone. Despite recognition of frailty as an important tool in identifying vulnerable surgical patients, many surgeons still shun objective tools. The aim of this position paper was to perform a review of the existing literature and to provide recommendations on emergency laparotomy and in frail patients. This position paper was reviewed by an international expert panel composed of 37 experts who were asked to critically revise the manuscript and position statements. The position paper was conducted according to the WSES methodology. We shall present the derived statements upon which a consensus was reached, specifying the quality of the supporting evidence and suggesting future research directions.

10. SiO Maser Spectra of V407 Cyg after the 2010 March Nova Outburst

Abstract

We report on the time variation of SiO maser emission from the symbiotic stellar system V407 Cyg after the classical nova outburst on 2010 March 10. Although both the SiO $J=1$--0 $v=1$ and 2 lines at 43.122 and 42.821 GHz were found previously in the envelope of a mira in the binary system, only weak emission of the $J=1$--0 $v=2$ line has continuously been detectable after the nova outburst. The line profile exhibited a dramatic change several weeks after the burst; the component on the higher-velocity side of the systemic velocity disappeared two weeks after the burst, and a new persistent component appeared on the lower-velocity side later. These observations indicate that the SiO emitting regions are wiped out in a time scale of two weeks by the nova shock, but a part of the masing region is quickly replenished by cool molecular gases expelled by the mira pulsation., Comment: 1 table, 3 figures; PASJ 63 no.1 (2011 Feb. 25 issue) in press. Abstract

We present the result of radio observations of red supergiants in the star cluster, Stephenson's #2, and candidates for red supergiants in the star clusters, Mercer et al. (2005)'s #4, #8, and #13, in the SiO and H$_2$O maser lines.The Stephenson's #2 cluster and nearby aggregation at the South-West contain more than 15 red supergiants. We detected one at the center of Stephenson's #2 and three in the south-west aggregation in the SiO maser line, and three of these 4 were also detected in the H2O maser line. The average radial velocity of the 4 detected objects is 96 km s^{-1}, giving a kinematic distance of 5.5 kpc, which locates this cluster near the base of the Scutum-Crux spiral arm. We also detected 6 SiO emitting objects associated with the other star clusters. In addition, mapping observations in the CO J=1--0 line toward these clusters revealed that an appreciable amount of molecular gas still remains around Stephenson's #2 cluster in contrast to the prototypical red-supergiant cluster, Bica et al.'s #122. Abstract

A concept for the fabrication of highly symmetric quantum dots that are coherently embedded in a single crystalline matrix is demonstrated. In this approach, the formation of the quantum dots is induced by a transformation of an epitaxial 2D quantum well into an array of isolated precipitates with dimensions of about 25 nm. The formation process is driven by the immiscibility of the constituent materials resulting from their different lattice structures. The investigated PbTe/CdTe heterosystem combines two different cubic lattices with almost identical lattice constants. Therefore, the precipitated quantum dots are almost strain free and near thermodynamic equilibrium they exhibit the shape of small-rhombo-cubo-octahedrons. The PbTe/CdTe quantum dots, grown on GaAs substrates, display intense room temperature luminescence at wavelength around 3.2 micrometer, which makes them auspicious for applications in mid-infrared photonic devices., Comment: 12 pages, 3 figures

Abstract

In a composite model of leptons and quarks, composite particles with heavy Majorana particles appear provided that the V-rishon is regarded as a Majorana particle. In this paper, the decay mode of such "Majorana partners" is isestigated. It is shown that the life time of Majorana partners are fairly long. Possible forsterite nanoparticle model of the Extended Red Emission(ERE) is proposed on the basis of photo-luminescence of forsterite after gamma-ray and neutron irradiation. Forsterite exhibits interesting thermoluminescence spectrum similar to ERE of Red Rectangle after irradiation in low temperature. It is shown that the forsterite after thermoluminescence is over exhibits photo-luminescence(PL) when Ultraviolet ray is irradiated. The structure of PL spectrum is almost similar to that of thermoluminescence. In order to explain small variations of the peak position of wavelength of ERE spectrum, possible nanoparticle model of forsterite is investigated. The possible extension of generation structure is investigated on the basis of a composite model, in which leptons and quarks are composed of rishons $T$ with charge 1/3 and purely neutral $V$. In this sub-system, we regard $V$ as the origin of Majorana particles. The seesaw mechanism implies the possible existence of heavy $V$, denoted as $V_B$ in addition to ordinary $V$, denoted as $V_s$. 18. Thermoluminescence of Simulated Interstellar Matter after Gamma-ray Irradiation

Abstract

Interstellar matter is known to be strongly irradiated by radiation and several types of cosmic ray particles. Simulated interstellar matter, such as forsterite $\rm Mg_{2}SiO_{4}$, enstatite $\rm MgSiO_{3}$ and magnesite $\rm MgCO_{3}$ has been irradiated with the $\rm ^{60}Co$ gamma-rays in liquid nitrogen, and also irradiated with fast neutrons at 10 K and 70 K by making use of the low-temperature irradiation facility of Kyoto University Reactor (KUR-LTL. Maximum fast neutron dose is $10^{17}n_f{\rm /cm^{2}}$). After irradiation, samples are stored in liquid nitrogen for several months to allow the decay of induced radioactivity. We measured the luminescence spectra of the gamma ray irradiated samples during warming to 370K using a spectrophotometer. For the forsterite and magnesite, the spectra exhibit a rather intense peak at about 645 -- 655 nm and 660 nm respectively, whereas luminescence scarcely appeared in olivine sample. We considere alternative possibility of combination of leptons and quarks to compose each generation, where the combination of $(\nu_{\tau},{\tau})$ and $(u,d)$ compose a generation. 20. On the Family of Pentagonal curves of genus 6 and associated modular forms on the Ball

Abstract

In this article we study the inverse of the period map for the family $\mathcal{F}$ of complex algebraic curves of genus 6 equipped with an automorphism of order 5. This is a family with 2 parameters, and is fibred over a certain type of Del Pezzo surace. The period satisfies the hypergeometric differential equation for Appell's $F_1({3/5},{3/5},{2/5},{6/5})$ of two variables after a certain normalization of the variable parameter. This differential equation and the family $\mathcal{F}$ are studied by G. Shimura (1964), T. Terada (1983, 1985), P. Deligne - G.D. Mostow (1986) and T. Yamazaki- M. Yoshida(1984). Recently M. Yoshida presented a new approch using the concept of configration space. Based on their results we show the representation of the inverse of the period map in terms of Riemann theta constants. Let us consider the rank 14 lattice $P=D_4^3\oplus < -2> \oplus < 2>$. We define a K3 surface S of type P with the property that $P\subset {\rm Pic}(S) $, where ${\rm Pic}(S) $ indicates the Picard lattice of S. In this article we study the family of K3 surfaces of type P with a certain fixed multipolarization. Many countries are facing an aging population. As people live longer, surgeons face the prospect of operating on increasingly older patients. Traditional teaching is that with older age, these patients face an increased risk of mortality and morbidity, even to a level deemed too prohibitive for surgery. However, this is not always true. An active 90-year-old patient can be much fitter than an overweight, sedentary 65-year-old patient with comorbidities. Abstract

Acute mesenteric ischemia (AMI) is a group of diseases characterized by an interruption of the blood supply to varying portions of the intestine, leading to ischemia and secondary inflammatory changes. If untreated, this process may progress to life-threatening intestinal necrosis. The incidence is low, estimated at 0.09–0.2% of all acute surgical admissions, but increases with age. Although the entity is an uncommon cause of abdominal pain, diligence is required because if untreated, mortality remains in the range of 50%. Early diagnosis and timely surgical intervention are the cornerstones of modern treatment to reduce the high mortality associated with this entity. The advent of endovascular approaches in parallel with modern imaging techniques is evolving and provides new treatment options. Lastly, a focused multidisciplinary approach based on early diagnosis and individualized treatment is essential. Thus, we believe that updated guidelines from World Society of Emergency Surgery are warranted, in order to provide the most recent and practical recommendations for diagnosis and treatment of AMI.

Emergency General Surgery (EGS) was identified as multidisciplinary surgery performed for traumatic and non-traumatic acute conditions during the same admission in the hospital by general emergency surgeons and other specialists. It is the most diffused surgical discipline in the world. To live and grow strong EGS necessitates three fundamental parts: emergency and elective continuous surgical practice, evidence generation through clinical registries and data accrual, and indications and guidelines production: the LIFE TRIAD.

Abstract

Background: Despite the call to enhance accuracy and value of operation records few international recommended minimal standards for operative notes documentation have been described. This study undertook a systematic review of existing operative reporting systems for laparoscopic cholecystectomy (LC) to fashion a comprehensive, synoptic operative reporting template for the future. Methods: A search for all relevant articles was conducted using PubMed version of Medline, Scopus and Web of Science databases in June 2021, for publications from January 1st 2011 to October 25th 2021, using the keywords: laparoscopic cholecystectomy AND operation notes OR operative notes OR proforma OR documentation OR report OR narrative OR audio-visual OR synoptic OR digital. Two reviewers (NOC, GMC) independently assessed each published study using a MINORS score of ≥ 16 for comparative and ≥ 10 for non-comparative for inclusion. This systematic review followed PRISMA guidelines and was registered with PROSPERO. Synoptic operative templates from published data were assimilated into one “ideal” laparoscopic operative report template following international input from the World Society of Emergency Surgery board. Results: A total of 3567 articles were reviewed. Following MINORS grading 25 studies were selected spanning 14 countries and 4 continents. Twenty-two studies were prospective. A holistic overview of the operative procedure documentation was reported in 6/25 studies and a further 19 papers dealt with selective surgical aspects of LC. A unique synoptic LC operative reporting template was developed and translated into Chinese/Mandarin, French and Arabic. Conclusion: This systematic review identified a paucity of publications dealing with operative reporting of LC. Abstract

Non-alcoholic fatty liver disease (NAFLD) is a potentially serious liver disease that affects approximately one-quarter of the global adult population, causing a substantial burden of ill health with wide-ranging social and economic implications. It is a multisystem disease and is considered the hepatic component of metabolic syndrome. Unlike other highly prevalent conditions, NAFLD has received little attention from the global public health community. Health system and public health responses to NAFLD have been weak and fragmented, and, despite its pervasiveness, NAFLD is largely unknown outside hepatology and gastroenterology. There is only a nascent global public health movement addressing NAFLD, and the disease is absent from nearly all national and international strategies and policies for non-communicable diseases, including obesity. In this global Delphi study, a multidisciplinary group of experts developed consensus statements and recommendations, which a larger group of collaborators reviewed over three rounds until consensus was achieved. The resulting consensus statements and recommendations address a broad range of topics — from epidemiology, awareness, care and treatment to public health policies and leadership — that have general relevance for policy-makers, health-care practitioners, civil society groups, research institutions and affected populations. These recommendations should provide a strong foundation for a comprehensive public health response to NAFLD. © 2021, Springer Nature Limited.
