Abstract

The safety and efficacy of diazoxide administered intravenously in the treatment of children with acute severe hypertension have been evaluated by a collaborative study. Observations of the response of blood pressure in 36 patients, ranging in age from two months to 18 years, during the initial episode of hospitalization reveal diazoxide treatment to be effective in lowering blood pressure in 94 per cent of the cases. No serious adverse circulatory, fluid and electrolyte, metabolic or hematologic effects were observed. Symptomatic and subjective reactions observed with diazoxide administered intravenously to children were identical with those described in adults. Reinstitution of other meant of antihypertensive therapy is safe and effective when delayed until the transiently induced period of hypotension has passed. Repeated use of diazoxide for subsequent recurrence of severe hypertension was equally effective and safe in 93 per cent of the instances. The results lead us to recommend the use of intravenous diazoxide for treatment of children with severe symptomatic hypertension especially when it is refractory to control by other hypertensive agents. 46. Growth in Children with End-stage Renal Disease Treated with Continuous Ambulatory Peritoneal Dialysis for at Least One Year

Abstract

Maintenance of adequate growth continues to be a major problem in the treatment of renal failure in children and adolescents. Poor growth seen with other forms of therapy for children with end-stage renal disease was the major reason we initiated our CAPD program at the Children's Hospital of Alabama in 1979. Since that time, we have treated 22 children with CAPD. We have an active transplant service and only 11 children have been maintained on CAPD for at least one year. This report concerns the growth of those 11 children, of whom eight grew at a rate greater than 80% of expected. The results to date with this form of treatment are very encouraging. The major determinants of growth in our patients seem to be maintenance of adequate caloric and protein intake, and control of hyperparathyroidism.

48. Continuous Ambulatory Peritoneal Dialysis: A Preliminary Pediatric Experience

Abstract

Continuous ambulatory peritoneal dialysis (CAPD) was first conceived in 1976,1 and its clinical application was demonstrated in an adult population by Popovich et al in 1978.2 Alexander et al3 reported results of a small series of children treated with CAPD in 1980. This is a form of home dialysis. A permanent catheter is surgically implanted into the peritoneal cavity. As opposed to chronic intermittent peritoneal dialysis, the dialysate is continuously present in the peritoneal cavity. In the usual form of peritoneal dialysis, dialysate is allowed to dwell only 20 to 40 minutes. In CAPD, dwell times are four to eight hours. This means only four or five exchanges are done per day. Because of the limited number, the exchanges can be done manually, at home, without expensive equipment. The CAPD system has been evaluated at Birmingham (Ala) Children's Hospital during the past year. We report our preliminary
