Abstract

The out-of-plane dielectric constant of the parent insulator of the high-temperature superconductor Bi_2Sr_2(Dy,Er)Cu_2O_8 was measured and analysed from 80 to 300 K in the frequency range of 10^6-10^9 Hz. All the samples were found to show a fairly large value of 10-60, implying some kind of charge inhomogeneity in the CuO_2 plane. Considering that the superconducting sample Bi_2Sr_2(Ca,Pr)Cu_2O_8 also shows a similar dielectric constant, the charge inhomogeneity plays an important role in the insulator-superconductor transition., Comment: RevTex4 format, 5 pages, 3 figures, submitted to J. Abstract

This thesis discusses Dutch diplomacy with respect to Japan in the period 1850-1863, predominantly on the basis of Dutch primary sources that were not widely known or not published until present. The political and economical importance of the Netherlands East Indies made Asian Affairs always essential for the Netherlands. Moreover only the Netherlands as Western power maintained a centuries long relationship with Japan. How special this relationship was showed particularly when Japan was in the process of being opened up in the middle of the nineteenth century. Dutch diplomacy at this crucial moment in time has been portrayed as rather passive and neutral. This study, however, examines The Hague’s diplomatic initiatives from the theory of Dutch imperialism, and reaches the opposite conclusion. The Netherlands engaged in an autonomous and positive diplomacy toward Japan in order to promote its prestige and profit, and in doing so it played an important role among the larger Western powers. Although The Netherlands judged by its size has often been ranked among the small countries of Europe, this thesis evaluates it as a middle-sized power on account of its political and economic interests as a prominent colonial power in Asia.

34. Efficacy of Rebamipide as Adjunctive Therapy in the Treatment of Recurrent Oral Aphthous Ulcers in Patients with Behçet's Disease: A Randomised, Double-Blind, Placebo-Controlled Study.

Abstract

Background: Behçet's disease (BD) is a recurrent inflammatory disease involving chronic recurrent oral aphthous ulcers (aphthae), uveitis, skin lesions and genital ulcers. We prospectively investigated the efficacy of rebamipide, a gastroprotective drug, against oral aphthous ulcers in BD patients. Methods: In a multicentre, double-blind, placebo-controlled study, 35 patients with BD, having as the main symptom oral aphthosis, were randomised to receive rebamipide 300 mg/day or placebo for 12 to 24 weeks between August 1994 and December 1996. Oral aphthosis must have occurred within 4 weeks prior to enrolment and must have been visible for at least 7 days during that time. Oral aphthae count and pain scores were recorded daily in a diary by the patients themselves. Monthly aphthae count and pain scores were defined as the sum of aphthae count and pain scores for a month, respectively. Investigators rated the global improvement in aphthae count and pain using a 6-point scale. The rate of change in monthly aphthae count and pain scores in the first 3 and last 3 months of treatment were assessed in patients with more severe symptoms whose aphthae count and pain score were >28 at baseline (trial entry). Results: The rate of moderate or marked improvement in aphthae count and pain was 36% (5 of 14 subjects) in the placebo group and 65% (11 of 17 subjects) in the rebamipide group. During months 2 to 6 of treatment, aphthae count tended to increase and reached a peak at month 4 in the placebo group but decreased in the rebamipide group. Abstract

The methods generally used in the second step screening of mass survey for gastric cancer are the direct, routine x-ray examination and/or the endoscopic examination. In order to improve the accuracy and efficiency of close examination (the second step examination), the results with two methods of close examination and in the institutions where they were carried out were respectively compared each other in the point of the detection rates of gastric cancer. The receiving rates of close examinations were also compared between the x-ray and the endoscopy, and between our institution and the others respectively.The results obtained from this study are as follows ;1) A total number of subjects receiving the first screening was 79064 and 29.9% of them was required to receive the close examinations.2) A number of subjects required to receive the endoscopic examination was 16134, showing eleven times more than the x-ray examination.3) A receiving rate of the close examinations was 77.4% in our institution and 69.9% in the others.4) In comparison of institution, a receiving rate of the endoscopy was 77.4% in ours and 61.1 % in the others. That of the x-ray was 76.7% in ours and 311.9% in the others.5) Regarding a detection rate of gastric cancer, our institution showed 1.1% with each methods of the close examinations, but the others revealed 0.3% with the x-ray and 0.7% with the endoscopy.6) A ratio of early stage cancer to the detected gastric cancer was 60.2% in our institution, but 40.5% in the others.From these results, the followings are concluded ; Compared with the other institutions, our institution obtained excellent results in the detection rates of gastric cancer as well as the receiving rate of close examinations. For the reasons, these are considered that, at first, the x-ray examination in our institution was less used than most of others ; second, most of the persons indicated to receive the endoscopic examination were examined with the x-ray instead of the endoscopy in the other institution.

Abstract

The intercalation reaction of natural phospholipids into synthetic mica was examined by means of chemical analysis and X-ray diffraction analysis. The rate of the intercalation was fast from a chloroform solution of lipids, and bilayer formation in the solid phase was perfect. On the other hand, the rate of intercalation was rather slow in ethanol solution, and bilayer formation was not always successful. Such a solvent effect was accountable due to the solvation of the hydrophilic quaternary ammonium head either in the solution phase or in the solid phase.

Abstract

An open-labeled study of once-daily administration of 40 mg famotidine was conducted in 24 patients with endoscopically confirmed reflux esophagitis. The mean age of the patients was 62.8 years. Most had stage I or stage II esophagitis of 1 years' duration or less. After 4, 8, 12, and 16 weeks of treatment, endoscopically documented healing was reported in 50%, 75%, 82%, and 83% of patients, respectively. Among specific endoscopic findings documented at the start of the study there was clearing of white exudate in 83% of patients, and redness cleared in 50% of patients after 16 weeks of treatment. Symptomatic improvement was observed in 73% of patients after 2 weeks, and this increased to 81% and 85% after 4 and 8 weeks of treatment, respectively. Painful symptoms cleared within 2 weeks in 68% of patients, and this increased to 77% after 8 weeks of once-daily therapy. Famotidine was well tolerated. Historically, reflux esophagitis has not responded satisfactorily to drug treatment. The reported symptomatic response and the response documented by endoscopy after therapy with 40 mg famotidine once daily at bedtime are encouraging. Further studies should be conducted to define the role of famotidine in the treatment of reflux esophagitis.
