Background and Purpose: Intensity modulated radiation therapy (IMRT) is an established technique for the treatment of prostate cancer. Volumetric modulated arc therapy (VMAT) has recently been introduced clinically. Nucletron BV (Veenendal, Netherlands) offers both optimisation algorithms on the same platform in Oncentra® MasterPlan 3.3. Methods: In this planning study five patients with localised prostate cancer are enrolled to compare these two algorithms. The planning target volume (PTV) and the rectum volume, urinary bladder and femoral heads are delineated as organs at risk (OAR). Collimator and table angles are kept fixed during the treatment. The planning is accomplished using the machine data of a SynergyS linear accelarator of the Elekta Company. For the IMRT seven equispaced beams are chosen, starting at a gantry angle of 0°, followed by 51°, 103°, 154°, 206°, 257° and 309°. The VMAT calculations are made based on a dual arc technique rotating from 182° to 178° and back. The dose-volume objectives are kept identical: Planning Target Volume minimum dose 68 Gy, maximum dose 72 Gy, both weighted 3000, urinary bladder 40 Gy to 50% of the volume, weight 1000, rectum 40 Gy to 30% of the volume, weight 1000, and both femoral heads set to a maximum dose of 30 Gy, weight 300. Average doses for the organs at risk and target dose homogeneity H are evaluated.The homogeneity is hereby defined as (D5-D95)/DAV. Furthermore the number of monitor units are compared. For one representative case the treatment times are measured. Results: The results of the dose distribution are rather similar: the average homogeneity for the VMAT plans is 0.088, in IMRT 0.094, both exceed the dose-volume objectives for the rectum (46,1 and 44,7 Gy) and the bladder (43,3 and 44.1 Gy). The objectives for the femoral heads are kept rather well although the weight is low (34,1 and 32,1 Gy). The number of monitor units is clearly higher for the VMAT plans than for the IMRT (1016 and 698 MU). But as the measurement of the treatment times in one representative case (1088 and 775 MU) shows, there is nevertheless an advantage in treatment time for the VMAT case (8,09 min versus 10,14 min). Conclusion: The results of the dose distribution are similar enough that VMAT is an interesting alternative for the treatment of prostate cancer. The treatment time, which is the crucial factor regarding intrafractional organ movements is advantageous for the VMAT technique.

The Institute of Photogrammetry has developed a peripheral system for the automatic derivation of digital terrain models for the analytical plotter Kern DSR 15; this project is carried out in close collaboration with Leica AG. The principal component of the system is a parallel processor based on Transputer technology. The correlation process itself runs off-line on the Transputer system according to the Multi-Templet-Matching procedure. In a second phase, the results of the image correlation are smoothed during a process which also filters out the terrain obstacles. This process runs completely automatically. The analytical plotter DSR 15 is used for the digitalization of aerial photographs and for the control and possible revision of digital terrain models; consequently, the data are integrated into an information system. The results of the image correlation can also be used for the production of digital orthophotos. The procedure has been conceived for large-scale photographs (-1:5000); according to various precision analyses, a height precision smaller than 0.1% of the flying height can be obtained.

Introduction Linear accelerators (Linacs) with Flatness Filter Free (FFF) mode obtain a much higher dose rate by omitting the flatness filter. The resulting inhomogeneous dose distribution is compensated by fluence modulating techniques like IMRT or VMAT. This planning study compares VMAT plans for patients with prostate carcinoma. Optimizations were performed using both modes: flattened beam (FB) and FFF. The aim of this study is to evaluate the plan quality and the number of monitor units (MU). Material and Methods Data sets of 10 patients with localized prostate cancer and reliable dose volume objectives were used for this retrospective planning study. A simultaneous integrated boost radiation therapy aims at a minimum dose of 71.0Gy and maximum of 74.2Gy in the CTV and a minimum dose of 59.4Gy in the PTV in 33 fractions. The posterior rectum wall is limited to 50.0Gy. The median values of rectum and urinary bladder are set to 50.0Gy. Additionally the maximum dose to the rectum is set to 74.2Gy. The VMAT parameters in the treatment planning system (TPS) Oncentra® External Beam v4.5 are set to: Single Arc rotation (182°-178°), collimator 45°, maximum delivery time 110s, gantry spacing 4°, and collapsed cone algorithm. The linac Synergy Agility offers a dose rate of 550MU/min (FB) and 1700MU/min (FFF). The leaves have a width of 5mm projected to the isocenter. The following parameters are evaluated: average dose DAv and homogeneity H = (D5–D95)/DAv in the CTV, minimum dose in the PTV represented by the D98PTV-CTV of the difference volume of PTV and CTV, maximum dose in the posterior rectum wall represented by the D2Rpost, median dose to rectum D50R and urinary bladder D50B, and the number of MU. Results Generally the dose volume statistics are very close for both modes. Taking the average dose in the CTV as normalization value, the maximum does not exceed 107%. The minimum dose in the PTV has been reached in one plan of each group only. The objectives for the median values in rectum and urinary bladder as well as maximum dose to the posterior rectum wall have been achieved in all cases. The only statistically significant difference for both groups is the number of MU which is about 10% lower for the FB plans. Discussion The plans for both modes show a good homogeneity in the CTV. The minimum dose to the PTV and the maximum dose to the posterior rectum wall are counterworking objectives which resulted in favour for the rectum for the given weights of the objectives. The high dose rate of the FFF mode aims at shorter treatment times which would reduce intrafractional organ motion. Abstract Background Since December 2009 a new VMAT planning system tool is available in Oncentra® MasterPlan v3.3 (Nucletron B.V.). The purpose of this study was to work out standard parameters for the optimization of prostate cancer. Methods For ten patients with localized prostate cancer plans for simultaneous integrated boost were optimized, varying systematically the number of arcs, collimator angle, the maximum delivery time, and the gantry spacing. Homogeneity in clinical target volume, minimum dose in planning target volume, median dose in the organs at risk, maximum dose in the posterior part of the rectum, and number of monitor units were evaluated using student’s test for statistical analysis. Measurements were performed with a 2D-array, taking the delivery time, and compared to the calculation by the gamma method. Results Plans with collimator 45° were superior to plans with collimator 0°. Single arc resulted in higher minimum dose in the planning target volume, but also higher dose values to the organs at risk, requiring less monitor units per fraction dose than dual arc. Single arc needs a higher value (per arc) for the maximum delivery time parameter than dual arc, but as only one arc is needed, the measured delivery time was shorter and stayed below 2.5 min versus 3 to 5 min. Balancing plan quality, dosimetric results and calculation time, a gantry spacing of 4° led to optimal results. Conclusion A set of parameters has been found which can be used as standard for volumetric modulated arc therapy planning of prostate cancer.

Abstract Background Several comparison studies have shown the capability of VMAT to achieve similar or better plan quality as IMRT, while reducing the treatment time. The experience of VMAT in a multi vendor environment is limited. We compared the plan quality and performance of VMAT to IMRT and we investigate the effects of varying various user-selectable parameters. Methods IMRT, single arc VMAT and dual arc VMAT were compared for four different head-and-neck tumors. For VMAT, the effect of varying gantry angle spacing and treatment time on the plan quality was investigated. A comparison of monitor units and treatment time was performed. Results IMRT and dual arc VMAT achieved a similar plan quality, while single arc could not provide an acceptable plan quality. Increasing the number of control points does not improve the plan quality. Dual arc VMAT delivery time is about 30% of IMRT delivery time. Conclusions Dual arc VMAT is a fast and accurate technique for the treatment of head and neck cancer. It applies similar number of MUs as IMRT, but the treatment time is strongly reduced, maintaining similar or better dose conformity to the PTV and OAR sparing.

Abstract Background and Purpose The purpose of this study was to assess plan quality and treatment time achievable with the new VMAT optimization tool implemented in the treatment planning system Oncentra MasterPlan® as compared to IMRT for Elekta SynergyS® linear accelerators. Materials and methods VMAT was implemented on a SynergyS® linear accelerator (Elekta Ltd., Crawley, UK) with Mosaiq® record and verify system (IMPAC Medical Systems, Sunnyvale, CA) and the treatment planning system Oncentra MasterPlan® (Nucletron BV, Veenendaal, the Netherlands). VMAT planning was conducted for three typical target types of prostate cancer, hypopharynx/larynx cancer and vertebral metastases, and compared to standard IMRT with respect to plan quality, number of monitor units (MU), and treatment time. Results For prostate cancer and vertebral metastases single arc VMAT led to similar plan quality as compared to IMRT. For treatment of the hypopharynx/larynx cancer, a second arc was necessary to achieve sufficient plan quality. Treatment time was reduced in all cases to 35% to 43% as compared to IMRT. Times required for optimization and dose calculation, however, increased by a factor of 5.0 to 6.8. Conclusion Similar or improved plan quality can be achieved with VMAT as compared to IMRT at reduced treatment times but increased calculation times.

Abstract Background To evaluate the effects of direct machine parameter optimization in the treatment planning of intensity-modulated radiation therapy (IMRT) for hypopharyngeal cancer as compared to subsequent leaf sequencing in Oncentra Masterplan v1.5. Methods For 10 hypopharyngeal cancer patients IMRT plans were generated in Oncentra Masterplan v1.5 (Nucletron BV, Veenendal, the Netherlands) for a Siemens Primus linear accelerator. For optimization the dose volume objectives (DVO) for the planning target volume (PTV) were set to 53 Gy minimum dose and 59 Gy maximum dose, in order to reach a dose of 56 Gy to the average of the PTV. For the parotids a median dose of 22 Gy was allowed and for the spinal cord a maximum dose of 35 Gy. The maximum DVO to the external contour of the patient was set to 59 Gy. The treatment plans were optimized with the direct machine parameter optimization ("Direct Step & Shoot", DSS, Raysearch Laboratories, Sweden) newly implemented in Masterplan v1.5 and the fluence modulation technique ("Intensity Modulation", IM) which was available in previous versions of Masterplan already. The two techniques were compared with regard to compliance to the DVO, plan quality, and number of monitor units (MU) required per fraction dose. Results The plans optimized with the DSS technique met the DVO for the PTV significantly better than the plans optimized with IM (p = 0.007 for the min DVO and p < 0.0005 for the max DVO). No significant difference could be observed for compliance to the DVO for the organs at risk (OAR) (p > 0.05). Plan quality, target coverage and dose homogeneity inside the PTV were superior for the plans optimized with DSS for similar dose to the spinal cord and lower dose to the normal tissue. The mean dose to the parotids was lower for the plans optimized with IM. Treatment plan efficiency was higher for the DSS plans with (901 ± 160) MU compared to (1151 ± 157) MU for IM (p-value < 0.05). Renormalization of the IM plans to the mean of the dose to 95% of the PTV (D95) of the DSS plans, resulted in similar target coverage and dose to the parotids for both strategies, at the cost of a significantly higher dose to the normal tissue and maximum dose to the target. The relative volume of the PTV receiving 107% or more of the prescription dose V107 increased to 35.5% ± 20.0% for the IM plan as compared to a mean of 0.9% ± 0.9% for the DSS plan. Conclusion The direct machine parameter optimization is a major improvement compared to the fluence modulation with subsequent leaf sequencing in Oncentra Masterplan v1.5. The resulting dose distribution complies better with the DVO and better plan quality is achieved for identical specification of DVO. An additional asset is the reduced number of MU as compared to IM.

50. Split Course Hyperfractionated Accelerated Radio-Chemotherapy (SCHARC) for patients with advanced head and neck cancer: Influence of protocol deviations and hemoglobin on overall survival, a retrospective analysis

Abstract Background The advantage of hyperfractionated accelerated radiation therapy for advanced head and neck cancer has been reported. Furthermore, randomized trials and meta-analyses have confirmed the survival benefit of additional chemotherapy to radiotherapy. We retrospectively analyzed the efficiency and toxicity of the Regensburg standard therapy protocol "SCHARC" and the overall survival of our patients. Methods From 1997 to 2004, 64 patients suffering from advanced head and neck cancer (88 % stage IV, 12 % stage III) were assigned to receive the SCHARC protocol. Around half of the patients were diagnosed with oro-hypopharynx carcinoma (52 %), one third with tongue and floor of mouth tumors (29 %) and one fifth (19 %) suffered from H & N cancer at other sites. The schedule consisted of one therapy block with 30 Gy in 20 fractions over a two week period with concomitant chemotherapy (d 1–5: 20 mg/m2/d DDP + 750–1000 mg/m2/d 5FU (cont. infusion). This therapy block was repeated after a fortnight break up to a cumulative dose of 60 Gy and followed by a boost up to 70 Gy (69–70.5 Gy). All patients assigned to this scheme were included in the survival evaluation. Results Forty patients (63 %) received both radiation and chemotherapy according to the protocol. The mean follow up was 2.3 years (829 d) and the median follow up was 1.9 years (678 d), respectively. The analysis of survival revealed an estimated 3 year overall survival rate of 57 %. No patient died of complications, 52 patients (80 %) had acute grade 2–3 mucositis, and 33 patients (58 %) suffered from acute grade 3 skin toxicity. Leucopenia was no major problem (mean nadir 3.4 g/nl, no patient < 1.0 g/nl) and the mean hemoglobin value decreased from 13.2 to 10.5 g/dl. Univariate analysis of survival showed a better outcome for patients with a hemoglobin nadir >10.5 g/dl and for patients who completed the protocol. Conclusion The SCHARC protocol was effective in patients diagnosed with advanced head and neck cancer. It led to long-term disease control and survival in about 50 % of the patients with significant but acceptable toxicity. Most patients were not anemic at beginning of therapy. Therefore, we could assess the influence of pre-treatment hemoglobin on survival. However, a low hemoglobin nadir was associated with poor outcome. This result suggests an influence of anemia during therapy on prognosis.
