Abstract

本研究は, 妊婦のセルフケア行動を促す保健指導を行うために, ①セルフケア行動の動機づけおよびセルフケア行動意図と妊婦の背景との関係を明らかすること, ②妊婦自身が日々の状況や行動を客観的に把握してチェックする介入を行い, セルフケア行動意図に与える効果を明らかにすることを目的とした. 妊娠中期の妊婦70名を対象に質問紙による調査を行い, 以下の結果と結論を得た.1. BMI25.0以上の妊婦は医療者から認められたいと考えていたが, 日常生活動作への配慮への意図は低かった.このことから, BMI25.0以上の妊婦にはできていることを肯定する指導が必要である.2. 妊娠経過異常の有る妊婦は母親役割や分娩への準備が遅れ, 食生活の配慮に対する意図が低かったことから,この2点についての指導が重要である.3. デイリーマタニティチェックを２週間記載することで, 母親役割や分娩への準備, 食生活や日常生活動作への配慮が上昇し, 特に初産婦の母親役割や分娩への準備, 仕事の持つ妊婦の日常生活への配慮に効果があった., The purpose of this study was toclarify the relationship between motivation and intention of self-care behavior in pregnant women and their background. In addition it was to clarify the effectiveness of maternity monitoring checks on self-care behavior intention. The subjects were 70 pregnant women. The results were as follows: 1. Pregnant women with BMI over 25.0 felt that they want recognition from their carers but as consideration of their behavior was low, positive affirmation is needed. 2. Pregnant women with abnormal pregnancy were under prepared for childbirth and their role as a mother, and had a low level of consideration for diet, and so guidance is needed in these two areas. 3. As a result of the two-week monitoring check, intention of self-care for preparation for childbirth and role as a mother, diet and consideration of behavior rose. It was especially effective for primipara mothers for intention of self-care for preparation for childbirth and role as a mother, and women in employment for consideration of their daily life behavior.

Abstract

漁村に暮らす住民の食品摂取の多様性の実態を把握し，高齢化，過疎化が進み，身近な商業施設等の生活資源が限られた地域において食生活に焦点を当てた健康づくり・介護予防を推進する方策を検討することを目的とした。研究方法はA県B市C地区在住の20歳以上の住民449人を対象に自記式質問紙調査を実施した。有効回答数（率）220（49.0％）であった。食品摂取の多様性得点は平均3.63（±2.27）点。食品群別摂取状況では「魚介類」の摂取頻度が高い一方で「いも類」「海藻類」「くだもの」は低く，年齢別では65歳以上で「肉類」「油脂類」，64歳以下で「くだもの」の摂取頻度が低かった。前年健診受診有の人は「緑黄色野菜」「くだもの」の摂取頻度が高く，多様性得点も高かった。C地区は先行研究と比べ多様性得点が低く，生活習慣病や要介護リスクを高める食生活をしている可能性が推察された。地場の農水産資源を活用した食文化を継承しながらも，ライフステージに応じた食事の必要性とその具体策を提示するともに，健診を契機に食品摂取の改善に向けた支援を行う必要がある。, This study aimed to clarify the state of Dietary variety among residents of a fishing village and investigate health-promotion in fishing regions where lifestyle resources are limited. Self-administered questionnaires were distributed to 449 residents aged 20 years or older of district C in City B of Prefecture A. A total of 220 (49.0%) valid responses were received. The average dietary variety score was 3.63 (±2.27) points. Investigation of intake by types of food revealed that seafood was most often eaten and tubers and roots, seaweed, and fruit were not commonly eaten. Investigation by age indicated that subjects aged 65 years and older ate meat and oils and fats less frequently and those aged younger than 64 years ate fruit less frequently. Those who reported undergoing a health checkup the previous year frequently ate green and yellow vegetables and fruit and had high dietary variety scores. Residents of district C had lower dietary variety scores than those reported in previous studies, suggesting that they were eating a diet putting them at high risk of developing lifestyle diseases and requiring long-term care. It is important to provide support that allows residents to inherit a food culture that utilizes local farm and marine resources but demonstrates the need for and specific measures for a diet that is appropriate to their life stage, with health check-ups becoming opportunities to identify areas of their diet that can be improved.

Abstract

Although malignancy is a fatal complication of end-stage renal disease (ESRD) requiring haemodialysis, successful treatment of haematological malignancies has been rarely reported. We describe the case of a 64-year-old man who presented with non-Hodgkin’s lymphoma (NHL; clinical stage, IVB) concomitant with ESRD. Before chemotherapy, haemodialysis was initiated, and one course of dose-adjusted CHOP (cyclophosphamide, doxorubicin, vincristine and prednisolone) therapy followed by eight courses of rituximab therapy were administered according to the performance status and degree of organ dysfunction. Consequently, the patient was disease free for 27 months. Thus, rituximab plus CHOP combination therapy was effective for NHL concomitant with ESRD. [ABSTRACT FROM PUBLISHER]
