ARE GENERALIZED RESISTANCE RESOURCES (GRRs) ASSOCIATED WITH FEAR OF MOVEMENT LEVEL? Janusz Kocjan Medical University of Silesia, School of Medicine, Katowice, Poland SUMMARY The generalized resistance resources (GRR) are defined as any characteristic of a person, primary group, subculture or society that is able to facilitate effective tension management. The attainment of a strength sense of coherence is dependent on the presence of GRR. A strong SOC is associated with good health condition. In this context, investigation whether sense of coherence is associated with fear of movement seems essential. The purpose of the study was to identife the role of sense of coherence in management of kinesiophobia (fear of movement) level. 115 participants with diagnosis of cardiovascular diseases in age of 40 - 84 years were examined. In this study sociometric techniques were used - Polish versions of the questionnaires: Sense of Coherence Scale (SOC-29), Kinesiophobia Causes Scale (KCS), and Baecke Habitual Physical Activity (HPA). Mean values obtained in SOC-29 were as follows: male: 132,59 ± 27,61 vs female: 120,76 ± 16,27; p=0,046. Overall KCS score was 45,88 ± 13,23 in case of males and 48,05 ± 15,61 in case of female (p=0,315). Streszczenie Zespół mięśnia gruszkowatego (ang. piriformis syndrome) jest stosunkowo rzadko występującym schorzeniem, które powstaje w wyniku przykurczu lub wzmożonego napięcia w obrębie mięśnia gruszkowatego. Diagnostyka tej jednostki chorobowej polega przede wszystkim na dokładnym badaniu fizykalnym. Przydatność poszczególnych testów funkcjonalnych wykorzystywanych w codziennej praktyce fizjoterapeutycznej nie jest dokładnie udokumentowana. Celem pracy jest ocena czułości i swoistości testu Patricka oraz pozostałych testów funkcjonalnych stosowanych w diagnostyce i rozpoznaniu zespołu mięśnia gruszkowatego. Grupę badaną stanowiło 20 osób w wieku 26-48 lat z rozpoznaniem zespołu mięśnia gruszkowatego. Grupa kontrolna składała się z 35 zdrowych osób w wieku 25-43 lat. Na potrzeby badania wykorzystano następujące testy kliniczne: test Freiberga, test Pace’a, test Beatty’ego, test FAIR, test Patricka oraz test opuszczania miednicy. Wykonano również badanie palpacyjne mięśnia gruszkowatego. Na podstawie uzyskanych rezultatów badania odnotowuje się, iż największymi wartościami czułości charakteryzuje się badanie palpacyjnie oraz test opuszczania miednicy. W przypadku pozostałych testów funkcjonalnych wartości czułości przedstawiały się na umiarkowanym poziomie, jednak wartość ich swoistości była wysoka. Wnioskuje się, iż bolesność palpacyjna oraz dodatni wynik w teście opuszczania miednicy i teście Freiberga przy jednoczesnym ujemnym wyniku pozostałych testów mogą być pomocne w wykrywaniu zespołu mięśnia gruszkowatego. Abstract The syndrome piriformis muscle (called. Piriformis syndrome) is a relatively rare disorder that arises as a result of contracture or increased tension within the piriformis muscle. Diagnosis of the disease is primarily thorough physical examination. The usefulness of the various functional tests used in everyday practice physiotherapy is not exactly documented. The aim of the study is to evaluate the sensitivity and specificity of the test Patrick and other functional tests used in the diagnosis and diagnosis of piriformis muscle syndrome. The study group consisted of 20 people aged 26-48 years diagnosed with piriformis muscle syndrome. The control group consisted of 35 healthy individuals aged 25-43 years. For the study we used the following clinical trials: Freiberg test, Pace test, Beatty test, FAIR test and Patrick test lower pelvis. Also performed palpation piriformis muscle. Based on the results of the examination it recorded that the highest values of sensitivity is characterized by palpation examination and test lower pelvis. In the case of other functional tests of sensitivity presented at a moderate level, but the value of their specificity was high. It is proposed that the pain palpation and the positive result in the test, leaving the pelvis and Freiberg test, while a negative result of other tests may be helpful in detecting team piriformis muscle.

EFFICACY OF NEURAL MOBILIZATION AND MID-CARPAL MOBILIZATION IN THE TREATMENT OF CARPAL TUNNEL SYNDROME SKUTECZNOŚĆ NEUROMOBILIZACJI ORAZ MOBILIZACJI STAWU ŚRÓDNADGARSTKOWEGO W LECZENIU ZESPOŁU KANAŁU NADGARSTKA Janusz Kocjan Medical University of Silesia, Doctoral Studium of School of Medicine, Katowice, Poland Śląski Uniwersytet Medyczny, Studium Doktoranckie Wydziału Lekarskiego w Katowicach SUMMARY Introduction: Carpal Tunnel Syndrome (CTS) is a cause of functional impairment and chronic wrist pain of the hand. It results from compression of the median nerve as it passes through the carpal tunnel. Purpose: The purpose of this study was to determine the effect of a manual therapy techniques (median nerve neural mobilization and wrist traction) on measures of pain and hand function in individuals with carpal tunnel syndrome. Material and methods: 36 patients of ages between 35-50 years with clinical and electrodiagnostic evidence of carpal tunnel syndrome, as well as positive Phalen test participated in this study. Subjects were randomly assigned to one of the two physiotherapy intervention groups: Group I - only median nerve neuromobilization was applicated. Group II - median nerve neuromobilization technique with additionally performed mid-carpal distraction using manual therapy technique. Psychometric measures, Boston Carpal Tunnel Questionnaire (BCTQ), The Disabilities of the Arm, Shoulder and Hand (DASH) and Visual Analogue Scale (VAS) were used. Results: Both methods were very effective in the treatment of CTS. The results of the study demonstrated that a combination of neuromobilisation and mid-carpal distraction brought slightly greater gains in outcome measures: symptom severity, hand functional status and pain intensity, than neural mobilization single performed. although in some cases the differences were not statistically significant. Conclusions: It is concluded that the combination of these two techniques may be effective in the treatment of CTS, especially in second degree of CTS where patients have persistent numbness in the area supplied by the median nerve. Dane literaturowe wskazują, że czynniki psychospołeczne mają istotny wpływ na przewlekłe utrzymywanie się dolegliwości bólowych. Cel: Celem pracy było określenie związku pomiędzy poczuciem koherencji i stopniem akceptacji choroby a nasileniem dolegliwości bólowych dolnego odcinka kręgosłupa. Materiał i metody: Badania przeprowadzono wśród 93 pacjentów (43 kobiety i 50 mężczyzn) z rozpoznaniem choroby dyskowej odcinka lędźwiowo-krzyżowego kręgosłupa. Poczucie koherencji oceniono za pomocą kwestionariusza SOC-29, poziom akceptacji choroby - skalą AIS, nasilenie bólu - skalą VAS, a stopień niepełnosprawności spowodowanej bólem kręgosłupa – kwestionariuszem ODI. Wyniki: Średnie wartości punktowe wraz z odchyleniem standardowym dla poszczególnych kwestionariuszy prezentowały się następująco: SOC-29: 135,54 ± 25,61; AIS: 30,87 ± 6,96; VAS: 6,35 ± 3,16; ODI: 29,59 ± 16,99. U większości badanych stwierdzono mierny (37,63%) i poważny (41,93) stopień niesprawności kręgosłupa. Brak akceptacji choroby odnotowano u 11,82% badanych. Analiza zależności wykazała istotne statystycznie korelacje między poczuciem koherencji i stopniem akceptacji choroby a nasileniem dolegliwości bólowych w skali VAS oraz stopniem niepełnosprawności funkcjonalnej w skali ODI. Wnioski: Badani o wyższym poczuciu koherencji oraz wysokim stopniu akceptacji choroby odczuwają mniejsze nasilenie dolegliwości bólowych oraz charakteryzują się mniejszym stopniem niepełnosprawności funkcjonalnej w porównaniu do osób z niskim poczuciem globalnej orientacji życiowej oraz niskim stopniem akceptacji choroby. Literature data indicate, that psychosocial factors have an significant influence on the persistence of pain. Aim: The aim of the study was to determine the relationship between sense of coherence and degree of acceptance of an illness with the intensity of low back pain. Material and methods: The study was conducted among 93 patients (43 women and 50 men) diagnosed with lumbosacral spine discopathy. The sense of coherence was evaluated using the SOC-29 questionnaire, illness acceptance degree - using AIS scale, pain intensity by means of VAS scale, while the perceived level of disability associated with low back pain was evaluated with the ODI Scale. Results: Mean values obtained from questionnaires were as follows: SOC-29: 135,54 ± 25,61; AIS: 30,87 ± 6,96; VAS: 6,35 ± 3,16; ODI: 29,59 ± 16,99. In case of the majority of respondents, mediocre (37,63%) and serious (41,93%) degree of spine disability was found. Lack of acceptance of an illness was observed in 11.82% of patients. Significant dependencies between sense of coherence, illness acceptance with intensity of feeling of pain and degree of disability were found. Conclusions: Subjects with a high sense of coherence and with a high degree of illness acceptance experience lower pain intensity and are characterized by a lower degree of functional disability compared to subjects with low self global life orientation and a low degree of acceptance of the disease. Article attempts to discuss this issue from the clinical and rehabilitation perspective. W ostatnich latach związaną z tym problematykę określono w literaturze mianem jednego z najbardziej kontrowersyjnych i trudnych obszarów nowoczesnej psychiatrii. W świetle dotychczasowych podziałów taksonomicznych, hipochondria przez długi czas klasyfikowana była jako zaburzenie somatyczne. Koncepcyjnie jednak, najlepiej rozumiana jest jako zaburzenie lękowe (lęk o zdrowie). Kilka teoretycznych modeli stara się wyjaśnić przyczyny występowania oraz utrzymywania ciężkiego lęku o zdrowie (kliniczna forma hipochondrii), m.in. model poznawczo-behawioralny oraz model interpersonalny. Słowa kluczowe: hipochondria, lęk o zdrowie, DSM-V, ICD-10. SUMMARY Classification and diagnosis of hypochondria has always been a serious problem and raised a many doubts. In literature, this problem have been described recently as one of the most difficult and challenging areas of modern psychiatry. In the light of previous taxonometric studies, hypochondria was classified as a somatoform disorders for a long time. However, conceptually is best understood as an anxiety disorder (health anxiety). Several theoretical model endavour to explain the development and maintenance of severe health anxiety (clinical form of hypochondriasis), including cognitive-behavioral model and interpersonal model. KINESIOPHOBIA DETERMINANTS IN PATIENTS AFTER MYOCARDIAL INFARCTION Janusz Kocjan Medical University of Silesia, School of Medicine, Katowice, Poland Janusz Kocjan: http://orcid.org/0000-0003-0630-602X; j_kocjan@wp.pl Abstract Introduction: In the most cases, the causes of morbidity in cardiovascular diseases is lack of regular physical activity. Kinesiophobia, defined as fear before movement may significantly contribute to the occurence and intensification of hypokinesis. Aim of the study: Determine the level of barriers of a physical activity (kinesiophobia) in patients after Myocardial Infarction (MI). Decided also examine that selected variables: age, BMI, elapsed time since MI and health self-assessment have impact to the general level of kinesiophobia (KCS) and domains: biological (BD) and psychological (PD). Materials and methods: 64 patients, in age of 47-84 years, after MI: 32 females (age: mean 64.0 ± 9.9 years; BMI: mean 27.1 ± 4,3; Time since MI: mean 10.6 ± 8.5 months) and 32 males (age: mean 67.1 ± 9.7 years; BMI: mean 27.1 ± 3.18; Time since MI: mean 8.7 ± 6.9 months) – patients undergoing cardiology treatment – were examined. Kinesiophobia Causes Scale (KCS), SF-36 ( Short Form Health Survey) and author metrics were used. Results: Patients presented elevated level of kinesiophobia, in general (females: x = 48.7 ± 15.1; males: x = 46.2 ± 10.6) as well as in biological domain (females: x = 42.6 ± 15.9; males: 40.5 ± 10.9) and psychological domains (females: 54.8 ± 16.3; mężczyźni: 51.8 ± 14.9). In women, BMI index correlated with: BD (r=0.541), PD (r=0.560) and KCS (r=0.566) and were also noted correlations between time since MI and: DB: r=0.408; PD: r=0.591 and KCS: r=0.524). Besides, health factor: physical functioning correlates with: BD: r= – 0,528; DP: r= -0.619 and KCS: r=-0.661. In men, only KCS correlated with pain factor r=-0.407. Conclusions: In females, BMI and time since MI strongly determine the level of kinesiophobia. In men, disposition seems to be constans. Higher intensification of kinesiophobia in psychological domain suggest take into account psychological sphere in cardiac rehabilitation process. Identification and early diagnosis of the causes of motor passivity is very important, both in primary and secondary prevention. IS HYPOCHONDRIASIS A SIGNIFICANT PROBLEM AMONG POLISH ADOLESCENTS? AN ATTEMPT OF ASSESSMENT OF SEVERE FORM OF HEALTH ANXIETY IN POLISH POPULATION JANUSZ KOCJAN Medical University of Silesia, School of Medicine, Katowice, Poland SUMMARY [Background] Hallmark features of health anxiety include persistent concern that one may have or contract an illness. People with health anxiety persist in looking for possible organic causes for their symptoms despite the negative results of medical examinations. It is estimated that 5 percent of the general population suffers hypochondria. However, still is little known about prelevance of hypochondriasis in polish population. [Aim of the study] The aim of this study was to investigate health anxiety level among polish adolescents and to examine the self-reported experience of health anxiety in medical students compared with non-medical students. [Material and methods] A total of 510 students aged 18–24 were anonymously examined. Health anxiety was measured using a polish language questionnaire known as the Short Health Anxiety Inventory (SHAI). [Results] Mean values of SHAI total score were as follows: overall sample: 15,7; medical group: 19,2 non-medical group:10,5; II year students: 22,8; VI year students: 14,7. Elevated health anxiety was reported by 26,7 percent of the sample, with reference to 18,8% of medical population and 8% of non-medical participants. Significant differences in health anxiety between medical and medical populatioan, as well as, between II year and VI year medical students were also found. [Conclusions] The study provides evidence about moderate intensification of health anxiety among polish adolescent. Health anxiety level was significantly higher among medical students versus non-medical students group. Key words: health anxiety, hypochiondriasis, SHAI, polish population, polish adolescents

STRONG SENSE OF COHERENCE CONTRIBUTES TO SUCCESSFUL AGING AND HIGHER SATISFACTION WITH LIFE SILNE POCZUCIE KOHERENCJI PRZYCZYNKIEM DO POMYŚLNEGO STARZENIA I LEPSZEJ SATYSFAKCJI Z ŻYCIA JANUSZ KOCJAN Medical University of Silesia, School of Medicine, Katowice, Poland Summary Sense of coherence (SOC) is a core concept within Antonovsky’s salutogenic theory and is argued to be a psychological determinant of health. Salutogenic approach proposed by Antonovsky emphasized the importance of coping resources in dealing with stress. The general objective of the study was to empirically explore the relationship between the generalized resistance resources of sense of coherence with successful aging and satisfaction with life. The sense of coherence (SOC-29), life satisfaction (SWLS) and successful aging (SAS) was assessed in a sample of N = 67 participants aged 80 years or older. The present study reveals that strong SOC is associated with better life satisfaction and better successful aging. Interaction effect of age to SOC was no found. Overall, the findings provided support that SOC is an important salutary resource in association with life satisfaction and successful aging among older people.
