- Abstract
BACKGROUND: Exposure to repetitive head impacts (RHI) is associated with an increased risk of later-life neurobehavioral dysregulation and neurodegenerative disease. The underlying pathomechanisms are largely unknown. PURPOSE: To investigate whether RHI exposure is associated with later-life corpus callosum (CC) microstructure and whether CC microstructure is associated with plasma total tau and neuropsychological/neuropsychiatric functioning. STUDY TYPE: Retrospective cohort study. POPULATION: Seventy-five former professional American football players (age 55.2 ± 8.0 years) with cognitive, behavioral, and mood symptoms. FIELD STRENGTH/SEQUENCE: Diffusion-weighted echo-planar MRI at 3 T. ASSESSMENT: Subjects underwent diffusion MRI, venous puncture, neuropsychological testing, and completed self-report measures of neurobehavioral dysregulation. RHI exposure was assessed using the Cumulative Head Impact Index (CHII). Diffusion MRI measures of CC microstructure (i.e., free-water corrected fractional anisotropy (FA), trace, radial diffusivity (RD), and axial diffusivity (AD)) were extracted from seven segments of the CC (CC1-7), using a tractography clustering algorithm. Neuropsychological tests were selected: Trail Making Test Part A (TMT-A) and Part B (TMT-B), Controlled Oral Word Association Test (COWAT), Stroop Interference Test, and the Behavioral Regulation Index (BRI) from the Behavior Rating Inventory of Executive Function, Adult version (BRIEF-A). STATISTICAL TESTS: Diffusion MRI metrics were tested for associations with RHI exposure, plasma total tau, neuropsychological performance, and neurobehavioral dysregulation using generalized linear models for repeated measures. RESULTS: RHI exposure was associated with increased AD of CC1 (correlation coefficient (r) = 0.32, P < 0.05) and with increased plasma total tau (r = 0.34, P < 0.05). Der Artikel bewertet die Beiträge von FlexCargoRail für eine Verbesserung des Einzelwagenverkehrs aus logistischer Sicht, stellt eine Wirtschaftlichkeitsbetrachtung dazu auf und leistet aus den Betrachtungen Handlungsempfehlungen für die gezielte weitere Untersuchung des Einzelwagenverkehrs ab. FlexCargoRail kann die Flexibilität an der Ladestelle erhöhen, indem eine Zustellung der Wagen von zentralen Punkten des Gleisanschlusses zu den Ladenstellen unabhängig von Rangierlokomotiven just-in-time oder just-in-sequence erfolgen kann. Damit wird die Logistikfähigkeit des Einzelwagenverkehrs verbessert. Aus wirtschaftlicher Sicht wird eine Target-Costing-Betrachtung durchgeführt- Hierbei wird ermittelt, wie hoch die Investitionskosten für eine FlexCargoRail -Fahrzeuge sein dürfen, um Kostengleichheit mit dem Einzelwagenverkehr zu erreichen. Die möglichen Gesamtinvestitionen sind abhängig von der Anzahl der Wagennutzung pro Jahr und dem Verhältnis der Kosten der FCR-Fahrzeuge zu denen konventioneller (nicht angetriebener) Güterwagen. Entscheidend ist, die Umlaufzeit der Wagen zu verkürzen, was zu Überlegungen führt, mehr Direktzüge zwischen Gleisanschlüsse bzw. Satellitenbahnhäfen einzusetzen. Hierfür wird ermittelt, ab welcher Anzahl Wagen ein Direktzug günstiger wird als die Nutzung des Einzelwagensystems. Im Ergebnis wird festgestellt, dass eine Kombination von FlexCargoRai-Fahrzeugen auf der letzten Meile mit Direktzügen zwischen Gleisanschlüssen und/oder Satellitenbahnhöfen Ziel führend für eine Ergänzung des Einzelwagenverkehrs ist.

Dabei entstanden für Verlader und Empfänger- sowie für mit den neuen kmEVU zusammenarbeitende Speditionen und Logistikdienstleister- bereits zahlreiche innovative Schienenverkehrsprodukte und -dienstleistungen. Neben Flexibilität und Qualität der Leistungserstellung- bei gleichzeitig notwendiger hoher Ressourcenauslastung, um im Wettbewerb zur Straße konkurrenzfähige Preise anbieten zu können- spielt dabei auch der den Transport begleitete Informationsfluss eine wesentliche Rolle: Transportangebote müssen in moderne Logistiksysteme integriert sein, um am Markt bestehen zu können.

Die Integration des Schienengüterverkehrs (SGV) in Supply-Chain-Management-Systeme (SCM) ist ein notwendiger Schritt, um die Wettbewerbsfähigkeit des SGV zu erhalten, zu steigern und damit ein Mehraufkommen zu erreichen. Eine Supply-Chain ist ein bedarfsgetriebenes Netzwerk von Organisationen, die durch Beziehungen im Material- und Informationsfluss in verschiedenen Prozessen und Aktivitäten verbunden sind. Diese Organisationen können Firmen sein, die Teile, Komponenten, Endprodukte herstellen, Logistikleistungen anbieten, oder sogar der Endkunde selbst sein.Das Supply-Chain-Management hat zum Ziel, alle Partner einer Supply-Chain in die Planung und Steuerung zu integrieren, um die Informationsbarrieren zwischen Abteilungen und Unternehmen abzubauen, mit genauen Datenplanen und koordinierte Entscheidungen treffen zu können. SCM-Systeme unterstützen diese prozessorientierte Planung und Steuerung der Waren-,Informations- und Geldfl üsse entlang der gesamten Wertschöpfungskette. Dazu existieren bereits IT-gestützte Teilsysteme, die eine SCM-Integration des Schienengüterverkehrs, d.h. die Einbindung des Verkehrsträgers Schiene in die Supply-Chain-Management-Systeme der Industrie, ermöglichen oder verbessern.Die Verbesserung der Logistikfähigkeit ist eine wesentliche Voraussetzung, um Gleisanschlüsse und Einzelwagenverkehre zukünftig erhalten zu können und den Marktanteil des Schienengüterverkehrs insgesamt zu steigern. Logistikfähigkeit beschreibt hier die Integrationsfähigkeit des Verkehrsträgers Schiene in zumeist übergeordnete logistische Prozesse. Damit soll die Schiene stärker den Ansprüchen moderner Logistiksysteme gerecht werden und zu einem integrativen Bestandteil durchgängiger und unternehmensübergreifender Logistikketten werden. Die Verbesserung der Prozesseffizienz kann hierzu einen wichtigen Beitrag leisten. Zielführende Ansätze für den Erhalt und Mehrverkehr im Einzelwagen- und Gleisanschlussverkehr sind die Kooperation mit Speditionen, die Bündelung von Verkehren, effizientere Betriebsverfahren und effizientere Ressourcennutzung sowie der unternehmensübergreifende Informationsfluss im Sinne des SCM. Der Zugang zum System Schiene kann insbesondere für Verlader und Spediteure ohne bahnspezifische Vorkenntnisse durch SCM erleichtert und attraktiver gestaltet werden. Die Bahnen ihrerseits benötigen rechtzeitige und valide Informationen über künftige Aufträge, um ihre Ressourcen effizient einsetzen und Ihre Reaktionsgeschwindigkeit zu erhöhen zu können. Die Chancen bestehen vor allem in der Nutzung und Adaption vorhandener Lösungen aus industrieller Produktion für einen logistik- und damit zukunftsfähigen Schienengüterverkehr.

- Abstract
Military service members are at increased risk for mental health issues, and comorbidity with mild traumatic brain injury (mTBI) is common. Largely overlapping symptoms between conditions suggest a shared pathophysiology. The present work investigates the associations among white matter microstructure, psychological functioning, and serum neuroactive steroids that are part of the stress-response system. Diffusion-weighted brain imaging was acquired from 163 participants (with and without military affiliation) and free-water-corrected fractional anisotropy (FA
T ) was extracted. Associations between serum neurosteroid levels of allopregnanolone (ALLO) and pregnenolone (PREGNE), psychological functioning, and whole-brain white matter microstructure were assessed using regression models. Moderation models tested the effect of mTBI and comorbid post-traumatic stress disorder (PTSD) and mTBI on these associations. ALLO is associated with whole-brain white matter FAT ( β = 0.24, t = 3.05, p = 0.006). This association is significantly modulated by PTSD+mTBI comorbidity ( β = 0.00, t = 2.50, p = 0.027), although an mTBI diagnosis alone did not significantly impact this association ( p = 0.088). There was no significant association between PREGNE and FAT ( p = 0.380). Importantly, lower FAT is associated with poor psychological functioning ( β = -0.19, t = -2.35, p = 0.020). This study provides novel insight into a potential common pathophysiological mechanism of neurosteroid dysregulation underlying the high risk for mental health issues in military service members. Further, comorbidity of PTSD and mTBI may bring the compensatory effects of the brain's stress response to their limit. Future research is needed to investigate whether neurosteroid regulation may be a promising tool for restoring brain health and improving psychological functioning.- Published

- Abstract
Repetitive head impacts (RHI) are commonly observed in athletes participating in contact sports such as American football, ice hockey, and soccer. RHI usually do not result in acute symptoms and are therefore often referred to as subclinical or "subconcussive" head impacts. Epidemiological studies report an association between exposure to RHI and an increased risk for the development of neurodegenerative diseases. Diffusion magnetic resonance imaging (dMRI) has emerged as particularly promising for the detection of subtle alterations in brain microstructure following exposure to sport-related RHI. The purpose of this study was to perform a systematic review of studies investigating the effects of exposure to RHI on brain microstructure using dMRI. We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) to determine studies that met inclusion and exclusion criteria across three databases. Seventeen studies were identified and critically evaluated. Results from these studies suggest an association between white matter alterations and RHI exposure in youth and young adult athletes. The most consistent finding across studies was lower or decreased fractional anisotropy (FA), a measure of the directionality of the diffusion of water molecules, associated with greater exposure to sport-related RHI. Whether decreased FA is associated with functional outcome (e.g., cognition) in those exposed to RHI is yet to be determined. This review further identified areas of importance for future research to increase the diagnostic and prognostic value of dMRI in RHI and to improve our understanding of the effects of RHI on brain physiology and microstructure., (© 2022. - Abstract
Younger age at first exposure (AFE) to repetitive head impacts while playing American football increases the risk for later-life neuropsychological symptoms and brain alterations. However, it is not known whether AFE is associated with cortical thickness in American football players. Sixty-three former professional National Football League players (55.5 ± 7.7 years) with cognitive, behavioral, and mood symptoms underwent neuroimaging and neuropsychological testing. First, the association between cortical thickness and AFE was tested. Second, the relationship between clusters of decreased cortical thickness and verbal and visual memory, and composite measures of mood/behavior and attention/psychomotor speed was assessed. AFE was positively correlated with cortical thickness in the right superior frontal cortex (cluster-wise P value [CWP] = 0.0006), the left parietal cortex (CWP = 0.0003), and the occipital cortices (right: CWP = 0.0023; left: CWP = 0.0008). A positive correlation was found between cortical thickness of the right superior frontal cortex and verbal memory (R = 0.333, P = 0.019), and the right occipital cortex and visual memory (R = 0.360, P = 0.012). In conclusion, our results suggest an association between younger AFE and decreased cortical thickness, which in turn is associated with worse neuropsychological performance. Furthermore, an association between younger AFE and signs of neurodegeneration later in life in symptomatic former American football players seems likely., (© The Author(s) 2021. - Abstract
Sports-related concussion is a serious health challenge, and females are at higher risk of sustaining a sports-related concussion compared to males. Although there are many studies that investigate outcomes following concussion, females remain an understudied population, despite representing a large proportion of the organized sports community. In this review, we provide a summary of studies that investigate sex-related differences in outcome following sports-related concussion. Moreover, we provide an introduction to the methods used to study sex-related differences after sports-related concussion, including common clinical and cognitive measures, neuroimaging techniques, as well as biomarkers. A literature search inclusive of articles published to March 2020 was performed using PubMed. The studies were reviewed and discussed with regard to the methods used. Findings from these studies remain mixed with regard to the effect of sex on clinical symptoms, concussion-related alterations in brain structure and function, and recovery trajectories. Nonetheless, there is initial evidence to suggest that sex-related differences following concussion are important to consider in efforts to develop objective biomarkers for the diagnosis and prognosis of concussion. Additional studies on this topic are, however, clearly needed to improve our understanding of sex-related differences following concussion, as well as to understand their neurobiological underpinnings.
