Abstract Background Wild-type and hereditary transthyretin-mediated amyloidosis (ATTRwt and ATTRv amyloidosis, respectively) are progressive, fatal diseases with a broad range of clinical presentations and multisystem effects. Despite having a higher prevalence, ATTRwt amyloidosis is less well characterized due to its non-hereditary nature, and its relatively poorer disease awareness delays diagnosis. Understanding of its natural history has evolved in recent years, but this is largely based on physician-collected data rather than patients’ reports of their own experiences. A mixed methods approach was used to evaluate how the healthcare journeys of patients with ATTRv and ATTRwt amyloidosis compare. Methods A quantitative survey was administered to US-patients diagnosed with both ATTRwt amyloidosis and ATTRv amyloidosis identified through a patient support group. Subsequent in-depth interviews with participants with ATTRwt amyloidosis were conducted. Quantitative data with related qualitative quotes from patients were produced to characterize their paths to diagnosis and the disease burden experienced. Results A total of 47 respondents completed the survey (ATTRv, n = 20 and ATTRwt, n = 27) and a total of 14 survey respondents with ATTRwt amyloidosis were interviewed. Survey results reported a high disease burden for patients with both conditions, with patients with ATTRwt amyloidosis reporting more diagnoses and procedures prior to their final diagnosis. Interviews with participants with ATTRwt amyloidosis revealed that patients face a high symptomatic burden of disease. Diagnosis was often delayed due to three key factors: (1) early signs of ATTRwt amyloidosis were often assumed to be related to old age; (2) many medical specialists working in silos were involved in participants’ diagnostic; and (3) there was a general lack of disease awareness. Early indicators such as carpal tunnel syndrome were often overlooked. Participants were typically diagnosed after the disease had progressed to include severe cardiac symptoms such as atrial fibrillation and severe shortness of breath. Sleep apnoea was also reported by a number of participants, with a considerable impact on quality of life. Conclusions Our study provides insight into the overall impact of the patient journey on their quality of life and demonstrates how increased awareness of ATTRwt amyloidosis and more coordinated engagement with physicians could reduce the time to diagnosis.

Abstract Siliceous rocks served as raw materials in the production of stone tools from the Middle Paleolithic onwards. Due to migration, the provenance of archaeological artefacts can differ from their natural outcrop location. The aim of this work was the application of 1D and 2D low-field nuclear magnetic resonance (LF-NMR) relaxometry to distinguish cherts by their original source. Herein, bedded cherts and accompanying nodular cherts coming from three different outcrops of Kraków-Częstochowa Upland were investigated. 1D and 2D (T 1 -T 2 ) experiments of water-saturated and dry rock sample states delivered T 1 , T 2 times and T 1 /T 2 ratios of distinct hydrogen populations – parameters sensitive to pore size, surface properties, and hydrogen bonding length. In-depth analysis of NMR data showed substantial differences in the porosity, pore surface and pore structure properties of investigated chert samples tested in the three different saturation levels (100% water-saturated, dried and differential). Finally, principal component analysis (PCA) was performed to reduce the number of correlations obtained and highlight the most important NMR properties specific to the particular outcrop localization."Please check captured corresponding author email if correct.""The email is correct"

Introduction. Gastrointestinal infections in young children in the form of acute diarrhoea of viral aetiology constitute a serious epidemiological and therapeutic problem. Aim. The aim of the study is to determine the severity of symptoms of acute rotavirus diarrhoea in children and to evaluate the level of parental knowledge about the factors predisposing to this condition and the preventive measures. Material and methods. A group of 100 individuals aged 18 to 40, parents of children under the age of 4 who were hospitalised due to acute diarrhoea, was surveyed. The questions concerned determining the severity of symptoms of acute diarrhoea in children of rotavirus aetiology and assess the level of parental knowledge about the factors predisposing to this condition and preventive measures. The study utilised the diagnostic survey method with a self-designed questionnaire, as well as a standardised questionnaire for assessing the child's condition on admission and the CDS scale. Results. The analysis of the results showed that parental knowledge about the course, management, and prevention of infectious diarrhoea in children is low. Parents tend to avoid seeking information from nurses and physicians, instead relying on media and Internet sources, which they consider as credible. The study identified sociodemographic factors that influence parental knowledge. Conclusions. Age, education, and place of residence significantly influence parental knowledge about the course, management, and prevention of infectious diarrhea in children. It was found that the gender of the parents does not have a significant impact on the level of knowledge regarding the disease in question.

Patients with DM and prediabetes (preDM) face an increased ACS risk. Inflammation plays a significant role in the pathogenesis of both CCS and ACS. This study delves into novel inflammatory markers, such as the systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), and aggregate index of systemic inflammation (AISI, also known as SIIRI or PIV), to explore their relationship with ACS and CCS in patients that have been or have not been diagnosed with DM or preDM.Patients and Methods: This study included data of 493 patients with chest pain undergoing coronary angiography. They were categorized into four groups: 1) without DM/preDM and with CCS; 2) with both DM/preDM and CCS; 3) without DM/preDM and with ACS, 4) with both DM/preDM and ACS. Standard methods of statistical analysis were used to reveal possible differences between groups and to find the most influential ACS risk factors in groups with DM/preDM and without DM/preDM.Results: The analysis showed no significant differences in SII, SIRI, or AISI between the respective patient groups. A logistic regression analysis generated a model incorporating SII, high-density lipoprotein, and low-density lipoprotein levels as the influential ACS risk factors for patients with DM/preDM. The model demonstrated 71.0% accuracy, 37.0% sensitivity, and 89.4% specificity.Conclusion: The findings suggest that the aforementioned inflammatory markers may have potential for distinguishing DM/preDM patients at higher risk of ACS at a low financial cost. However, further comprehensive and well-designed research is required to validate their clinical utility.Plain Language Summary: People with type 2 diabetes (DM) and prediabetes (preDM) have a higher risk of heart problems. These include chronic coronary syndrome (CCS) and acute coronary syndrome (ACS). Inflammation is a key element in these issues. We looked at 493 patients with chest pain. We divided them into groups based on diabetes status (DM/preDM vs no diabetes) and heart conditions (ACS and CCS). We explored new markers related to inflammation. These include the systemic immune-inflammation index (SII), systemic inflammation response index (SIRI), and aggregate index of systemic inflammation (AISI) that all can be calculated from simple blood tests. We found no differences in these markers between groups. To understand ACS risk factors better, we used statistical analysis. The model found key factors for patients with DM/preDM: SII, LDL, and low-density lipoprotein levels. It was accurate (71.0%), but sensitivity was 37.0%, and specificity was 89.4%. These markers could be helpful in identifying DM/preDM patients at risk of ACS with low cost tests. Subclinical leaflet thrombosis is an imaging phenomenon observed after transcatheter aortic valve implantation (TAVI) and characterized by hypoattenuating leaflet thickening (HALT) on computed tomography angiography. The clinical implications and underlying causes remain uncertain. Hypercoagulability, a component of Virchow’s triad, may contribute to thrombus formation on bioprosthetic leaflets, but data on hypercoagulable disorders in TAVI patients and their impact on HALT are limited. The PROSTHESIS study (Prothrombotic States in Transcatheter Heart Valve Subclinical Leaflet Thrombosis) is a single-center observational cohort study aiming to include 130 TAVI patients. This pilot study aimed to (i) assess the effect of hypercoagulable disorders on HALT prevalence and (ii) evaluate their impact on the natural history of HALT. Patients were screened for common hypercoagulable disorders using genetic and functional assays and underwent multimodal imaging one year after TAVI to detect HALT. In patients with HALT, post-implant imaging was repeated after three months to assess its progression. Early results comparing 52 TAVI patients with 52 matched controls undergoing coronary angiography showed similar thrombophilia prevalence between the groups (16% vs. 12%, p = 0.565). HALT occurred in 15% of TAVI patients, more extensively in those with thrombophilia (712 mm3 vs. 135 mm3, p = 0.036). However, thrombophilia was not an independent predictor of HALT. One-year follow-up CTA revealed spontaneous HALT resolution in 63%, stability in 25%, and progression in 12%. This study aims to provide insights into HALT and its mechanisms, which may help prevent complications and improve bioprosthesis durability.

For sustained vision, photoactivated rhodopsin (Rho*) must undergo hydrolysis and release of all-trans-retinal, producing substrate for the visual cycle and apo-opsin available for regeneration with 11-cis-retinal. The kinetics of this hydrolysis has yet to be described for rhodopsin in its native membrane environment. We developed a method consisting of simultaneous denaturation and chromophore trapping by isopropanol/borohydride, followed by exhaustive protein digestion, complete extraction, and liquid chromatography-mass spectrometry. Using our method, we tracked Rho* hydrolysis, the subsequent formation of N-retinylidene-phosphatidylethanolamine (N-ret-PE) adducts with the released all-trans-retinal, and the reduction of all-trans-retinal to all-trans-retinol. We found that hydrolysis occurred faster in native membranes than in detergent micelles typically used to study membrane proteins. The activation energy of the hydrolysis in native membranes was determined to be 17.7 ± 2.4 kcal/mol. Our data support the interpretation that metarhodopsin II, the signaling state of rhodopsin, is the primary species undergoing hydrolysis and release of its all-trans-retinal. In the absence of NADPH, free all-trans-retinal reacts with phosphatidylethanolamine (PE), forming a substantial amount of N-ret-PE (∼40% of total all-trans-retinal at physiological pH), at a rate that is an order of magnitude faster than Rho* hydrolysis. However, N-ret-PE formation was highly attenuated by NADPH-dependent reduction of all-trans-retinal to all-trans-retinol. Neither N-ret-PE formation nor all-trans-retinal reduction affected the rate of hydrolysis of Rho*. Our study provides a comprehensive picture of the hydrolysis of Rho* and the release of all-trans-retinal and its reentry into the visual cycle, a process in which alteration can lead to severe retinopathies.

Wstęp. Praca z dziećmi i ich rodzicami to szczególne wyzwanie zawodowe dla pielęgniarki. To właśnie pielęgniarka jest pierwszym źródłem informacji dla rodziców. Obecnie musi sprostać coraz większym wymaganiom z ich strony. Zakres działań profilaktycznych i promujących zdrowie, podejmowanych przez pielęgniarkę POZ jest ściśle określony przepisami, a swoją pracę organizuje w sposób niezależny. Najważniejszym elementem w tym aspekcie pielęgniarki POZ jest edukacja zdrowotna, bez której nie można mówić o profilaktyce. Cel. Celem pracy jest ocena działań zawodowych podjętych przez pielęgniarkę POZ na rzecz profilaktyki i promocji zdrowia wobec dzieci w wieku przedszkolnym oraz wskazanie kwestii, w których jakość usług powinna ulec poprawie. Wyniki. Większość badanych rodziców stanowiły kobiety- 84%, a także osoby w przedziale wiekowym 26-35 lat – 51%, posiadające wyższe wykształcenie – 51%. Z przeprowadzonych badań wynika, że poziom satysfakcji z opieki pielęgniarskiej wśród respondentów jest wysoki. Najwyżej ocenionym kryterium szczegółowym dotyczącym profesjonalnej opieki pielęgniarskiej było „Czy pielęgniarka odnosiła się z szacunkiem do rodziców/opiekunów”, średnia ocen wynosiła 3,99 punktu. Spośród ogółu badanych aż 71% wykazało wysoki poziom satysfakcji w tej kwestii. W doniesieniu do wykonywania przez pielęgniarkę zadań profilaktycznych i promujących zdrowie, rodzice/ opiekunowie dzieci byli mniej usatysfakcjonowani. Najniżej ocenionym kryterium szczegółowym było „wpływ podjętych przez pielęgniarkę działań profilaktycznych wpływających na zdrowie dziecka” – średnia odpowiedzi wynosiła 3,26 punktu. Wnioski. Podczas wizyty z dzieckiem w placówce, rodzice/opiekunowie obserwują pracę pielęgniarki i dokonują jej oceny w wielu aspektach. Zwracają szczególną uwagę na profesjonalizm pielęgniarki w sprawowaniu opieki nad dzieckiem. W tej kwestii wykazują duże zadowolenie, co świadczy o rzetelnym wykonywaniu swoich obowiązków przez pielęgniarki. Aspektem, na który należy szczególnie zwrócić uwagę i usprawnić to jakość działań profilaktycznych. Należy wzmocnić zaangażowanie pielęgniarek w wykonywanie zadań profilaktycznych i promujących zdrowie, a przede wszystkim w edukację zdrowotną, w jakość przekazywanych wskazówek rodzicom.

Background and aims – The genus Orthoseira is generally known to be typically aerophilous. Despite the worldwide occurrence of the genus, most of the Orthoseira species seem to have a rather restricted biogeographic distribution, often being part of an endemic diatom flora. During a survey of the aerophilic diatom flora in Papua New Guinea (Karkar Island, Boisa Island), four morphologically distinct Orthoseira taxa have been observed. Material and methods – The morphology of four Orthoseira species was investigated in detail using light microscopy (LM) and scanning electron microscopy (SEM). Key results – The morphology of all four species is discussed and compared to known species worldwide. The main differences between the species include: the presence and shape of linking structures, the number of rows of perforations on the copulae, presence/absence of internal caverns and the external structure of the carinoportulae. Two species, with a unique set of features, are described as new to science: Orthoseira iserentantii sp. nov. and Orthoseira papuensis sp. nov. Conclusions – The observation of four Orthoseira species, including two species new to science, confirms that the diversity of the genus Orthoseira is underestimated.

Rationale: Acute otitis media is a common disease in early childhood, and is usually caused by Streptococcus pneumoniae (S. pneumoniae). Acute mastoiditis is a complication of acute otitis media and can involve not only the mucoperiosteum of the middle ear but can also spread to the periosteum by destroying the mastoid bone (acute coalescent mastoiditis). In addition, the infection can extend through the surrounding bones or the emissary veins beyond the mastoid's air cells, leading to subperiosteal abscesses. Patient's Concern: A 16-month-old female patient was hospitalized due to the purulent discharge of the left ear and the symptoms of right mastoiditis (swelling and redness of the skin). Diagnosis: Bilateral acute coalescent mastoiditis caused by S. pneumoniae infection. The computer tomography revealed bilateral bone destruction of the mastoid and abscesses found behind the auricle on both sides. Interventions: The patient underwent intravenous antibiotic therapy and surgical treatment. Outcomes: The patient was discharged 14 days after hospitalization with an improved condition. Lessons: Improperly treated acute coalescent mastoiditis can lead to extracranial and intracranial complications, sometimes serious and even life-threatening. Complications are prevalent in children under 2 years, in whom the disease progresses more rapidly and severely. The vaccination with a 13-valent vaccine may not result in sufficient immunity against S. pneumoniae, a predominant pathogen in children affected by acute coalescent mastoiditis.

Fibrous dysplasia is an uncommon bone disorder affecting various parts of the skeleton, often affecting facial and cranial bones. In this case, a 10-year-old patient was diagnosed with fibrous dysplasia of the ethmoid sinus at an early age. The patient has experienced nasal congestion, snores, and worsening nasal patency since 2019. A CT scan revealed an expansive proliferative lesion, likely from the frontal or ethmoid bone, protruding into the nasal cavity, ethmoid sinus, and right orbit. The tumor causes bone defects in the area of the nasal bone, leading to fluid retention in the peripheral parts of the right maxillary sinus. The patient’s parents decided not to undergo surgery to remove the diseased tissue and reconstruct the area, as it would be very extensive, risky, and disfiguring. The patient is being treated conservatively with an MRI, with a contrast performed approximately every six months and infusions of bisphosphonates. Despite the lesion’s size, the patient does not experience pain characteristic of dysplasia, and functions typically. Fibrous dysplasia of bone is a rare condition that presents with the most visually apparent manifestations, often mistaken for other bone conditions. Advanced diagnostic tools, like CT and MRI, are used to identify conditions affecting the ethmoid sinus more frequently. However, diagnostic errors often occur in imaging studies, leading to confusion. The most common period for clinical manifestations and diagnosis is around 10 years of age. The preferred approach in managing fibrous dysplasia involves symptomatic treatment, which can alleviate airway obstruction, restore normal globe position and visual function, and address physical deformities. Surgical intervention is recommended only for patients with severe functional impairment, progressive deformities, or malignant transformation.

The human endogenous retroviruses (HERVs) are ancient exogenous retroviruses that were embedded in the germline over 30 million years ago and underwent an endogenization process. They make up roughly 8% of the human genome. HERVs exhibit many physiological and non-physiological functions; for example, they play a role in the development of many diseases. They have been shown to affect carcinogenesis by modifying the expression of host genes through their functions as enhancers and promoters. Additionally, some molecules derived from HERVs may stimulate the immune system. Recently research has been focused on the effect of human endogenous retroviruses on the development of neurodegenerative diseases, including Alzheimer’s disease (AD), which is the most common cause of dementia. AD is also linked to a significant deterioration in quality of life. The article aims to highlight the potential role of HERVs in neurodegenerative diseases such as Alzheimer’s disease and senescence. Moreover, it is estimated that HERVs may be potential targets for diagnosis and therapy of AD.

Introduction. Currently, back pain syndromes are considered to be widely known problems in the medical, but above all, social and economic aspects. The specific nature of the profession performed by nurses puts them among the people most at risk of spine dysfunction. Aim. The aim of this study is to assess the occurrence of pain in the lumbar spine pain among nurses working at the Provincial Specialist Hospital in Włocławek. Material and Methods. The research was conducted among 120 nurses working in hospital wards of the Provincial Specialist Hospital in Włocławek. The research used an own survey questionnaire and the Revised Oswestry Low Back Pain Disability Scale (ODI) — the Polish version. Consent to conduct the research was obtained from the Management of the Provincial Specialist Hospital in Włocławek and the Bioethics Committee of the State Academy of Applied Sciences in Włocławek. Results. The majority of respondents indicated that they connect back pain with work — 104 people (86.7%) and received the results of slight limitations — 60 people (50.0%). Most respondents indicated that they had experienced pain for 1–5 years — 42 people (35.0%). Respondents aged up to 45 years indicated that they had been experiencing pain for 1–5 years — 15 people (48.4%). In the group with 30–40 years of experience, the most respondents indicated that they had more than 10 years of experience — 19 people (34.5%). In the group with 30–40 years of experience, the majority indicated that often several times a month — 21 people (38.2%). The highest degree of disability was recorded at the age of 56–62, serious disruptions — 8 people (33.3%), and at the age of 51–55, serious disruptions — 6 people (14.03%). Conclusions. The work performed affects the occurrence of pain in the lumbar spine. Age groups up to 45 years and 46–50 years have been experiencing pain for 5–10 years. Pain is most often experienced by respondents with 30–40 years of work experience. The vast majority of respondents experience slight limitations. The highest rate of disability is experienced by nurses aged 51–55. (JNNN 2023;12(4):164–169)

The present paper, adopting some of the tools offered by Cognitive Linguistics, namely the mechanisms of conceptual metaphor and metonymy, is a qualitative study of a sociolinguistic nature. Its overall purpose is an attempt at exhibiting a paradigm shift in the representation of women in Anglo-American proverbs. Combining the potential of the cross-fertilisation between Cognitive Linguistics and paremiological studies, the study appertains to the sense-threads embedded in the figurative language of proverbs, with the main focus on a cognitive semantic analysis of selected Anglo-American paremias directed towards women and animals. The main goal of the research is the juxtaposition of the meaning coded in two proverbs of traditional status, as representatives of a larger group of paremiological units (i.e. A woman, a cat, and a chimney should never leave the house; A whistling girl and a crowing hen always come to no good end), reflecting the deep-rooted gender-biased ideology in patriarchal Anglo-American society, with the content of the selected anti-proverb (i.e. The early bird gets up to serve his wife breakfast in bed) and a contemporary proverb (i.e. A woman without a man is like a fish without a bicycle), serving as sample evidence of the heralds of a paradigm shift in the issue of gender stereotyping stored in paremiological wisdom. The paper shows that the motivation behind the use of the analysed proverbs is to be accounted for by reference to the mechanism of metaphor-metonymy interaction, while the rise of new gender-related proverbs can be regarded as a sign of socio-cultural changes. Specifically, through the medium of modern paremiology, asymmetrical representation of male and female gender, coupled with traditional masculine and feminine characteristics as well as social roles, appears if not endangered then, at least, to be taking a promising path.

Introduction24-Hydroxylase, encoded by the CYP24A1 gene, is a crucial enzyme involved in the catabolism of vitamin D. Loss-of-function mutations in CYP24A1 result in PTH-independent hypercalcaemia with high levels of 1,25(OH)2D3. The variety of clinical manifestations depends on age, and underlying genetic predisposition mutations can lead to fatal infantile hypercalcaemia among neonates, whereas adult symptoms are usually mild.Aim of the studyWe report a rare case of an adult with primary hyperparathyroidism and loss-of-function mutations in the CYP24A1 gene and a review of similar cases.Case presentationWe report the case of a 58-year-old woman diagnosed initially with primary hyperparathyroidism. Preoperatively, the suspected mass adjoining the upper pole of the left lobe of the thyroid gland was found via ultrasonography and confirmed by 99mTc scintigraphy and biopsy as the parathyroid gland. The patient underwent parathyroidectomy (a histopathology report revealed parathyroid adenoma), which led to normocalcaemia. After 10 months, vitamin D supplementation was introduced due to deficiency, and the calcium level remained within the reference range. Two years later, biochemical tests showed recurrence of hypercalcaemia with suppressed parathyroid hormone levels and elevated 1,25(OH)2D3 concentrations. Further investigation excluded the most common causes of PTH-independent hypercalcaemia, such as granulomatous disease, malignancy, and vitamin D intoxication. Subsequently, vitamin D metabolites were measured using LC–MS/MS, which revealed high levels of 25(OH)D3, low levels of 24,25(OH)2D3 and elevated 25(OH)2D3/24,25(OH)2D3 ratios, suggesting a defect in vitamin D catabolism. Molecular analysis of the CYP24A1 gene using the NGS technique revealed two pathogenic variants: p.(Arg396Trp) and p.(Glu143del) (rs114368325 and rs777676129, respectively).ConclusionsThe diagnostic process for hypercalcaemia becomes complicated when multiple causes of hypercalcaemia coexist. The measurement of vitamin D metabolites using LC–MS/MS may help to identify carriers of CYP24A1 mutations. Subsequent molecular testing may contribute to establishing the exact frequency of pathogenic variants of the CYP24A1 gene and introducing personalized treatment.

The compound 9-cis-retinyl acetate (9-cis-RAc) is a precursor to 9-cis-retinal, which has potential application in the treatment of some hereditary diseases of the retina. An attractive synthetic route to 9-cis-RAc is based on the photoisomerization reaction of the readily available all-trans-RAc. In the present study, we examine the mechanism of the photoisomerization reaction with the use of state-of-the-art electronic structure calculations for two polyenic model compounds: tEtEt-octatetraene and tEtEtEc-2,6-dimethyl-1,3,5,7,9-decapentaene. The occurrence of photoisomerization is attributed to a chain-kinking mechanism, whereby a series of S1/S0 conical intersections associated with kinking deformations at different positions along the polyenic chain mediate internal conversion to the S0 state, and subsequent isomerization around one of the double bonds. Two other possible photoisomerization mechanisms are taken into account, but they are rejected as incompatible with simulation results and/or the available spectroscopic data.

In general, “stress” is the reaction of the body to mental and physical demands placed on it. Stress disrupts mental balance, and reduces the ability to work and function, which negatively affects the performance of duties. The aim of this study was to analyse the level of stress and ways of coping with it among nursing staff. The study covered 220 nurses employed at the Provincial Specialist Hospital in Włocławek. The research tool was the Perceived Stress Scale (PSS-10) and the Brief-COPE. The results of this survey showed the occurrence of average and high levels of experiencing stress in 36% and 40%of staff, respectively. Brief-COPE scale results show that substance use/gender (men) and use of emotional support/place of work (internal medicine department) are significant at p < 0.01. Considering the impact of the workplace on the use of psychoactive substances, it can be seenthat people working in the surgical ward are more likely to use psychoactive substances. Furthermore, nurses in the surgical ward find it easier to think and plan what to do when faced with a difficult life situation. Most often, the respondents with the highest work experience, i.e., the elderly, declared a return to religion. The results of the research indicate that the nursing community experiences stress to an average or significant degree. The strategies are mainly based on active coping and seeking emotional and instrumental support. Further research is needed in this field.
