Background: Stroke account for 15% of all deaths in Turkey. In acute ischemic stroke (AIS), neuronal damage continues every minute until the occluded vessel is not opened. Currently, the most important treatment of AIS is the rapid and effective opening of the occluded vessel. In this study, we aimed to share our experience of endovascular treatment of AIS patients in our center.Materials and Methods: Between January 2015 - November 2018, the first 100 consecutive patients who presented with the diagnosis of AIS and received endovascular treatment were included in this study. We recorded the clinical and demographic characteristics and endovascular procedure characteristics of all patients.Results: Of the 100 patients included in the study, 49 were female and 51 were male, and the median age was 65 (55-74). Hypertension was present in 74%, hyperlipidemia in 25%, diabetes mellitus in 38%, atrial fibrillation in 26%, coronary artery disease in 36%, and smoking in 39% patients. The Median NIHSS score was 15.5 (13-19) prior to treatment. At the 3-month follow-up, 31 patients died and the mortality rate was calculated as 31%. At the end of the 3-month, good outcome was observed in 40% of patients (modified Rankin scale score 0-2). Our effective recanalization rate (Thrombolysis in Cerebral Infarction [TICI2b-3]) was found to be 75%.Conclusions: AIS patients with suspected large artery occlusion should be transferred to centers where endovascular treatment is available, as soon as possible. Better results can be achieved as experience of endovascular treatment in centers increases., Amaç: İnme, Türkiye’de tüm ölümlerin %15 nedenidir. Akut iskemik inmede(Aİİ) tıkanın damarın açılmadığı her dakikada nöron harabiyeti devam etmektedir ve bugün için Aİİ’nin erken dönem en önemli tedavisi tıkalı damarın hızlı ve etkili şekilde açılmasıdır. Biz bu çalışmada merkezimizdeki inme hastalarına uygulanan endovasküler tedavi deneyimlerimizi paylaşmayı amaçladık. Materyal ve method: Ocak 2015 ve Kasım 2018 arasında başvuran, Aİİ tanısı konarak endovasküler tedavi alan ardışık ilk 100 hastanın klinik ve demografik özelliklerini ve endovasküler işlem özelliklerini kaydettik.Bulgular: Çalışmaya dahil edilen 100 hastanın 49’u kadın, 51’i erkekti ve yaş ortalamaları 65(55-74) idi. Hastaların %74’ünde hipertansiyon, %25’inde hiperlipidemi, %38’inde diabetes mellitus, %26’sınde atrial fibrilasyon, %36’ünde koroner arter hastalığı ve %39’ünde sigara kullanımı mevcuttu. Hastaların tedavi öncesi National Institutes of Health Stroke Scale (NIHSS) skoru 15,5 (13-19) idi. Üç aylık takipte 31 hasta kaybedildi ve mortalite oranı %31 olarak hesaplandı. Üçüncü ay sonunda %40 hastada iyi sonuç gözlemlendi (modifiye Rankin skalası skoru 0-2). Etkili rekanalizasyon oranımızın [ Thrombolysis in Cerebral Infarction (TICI2b-3)] %75 olduğu görüldü.Sonuç: Büyük arter tıkanıklığı düşünülen hastaların en erken dönemde endovasküler tedavi imkanlarının olduğu merkezlere sevki sağlanmalıdır. Merkezlerde endovasküler tedavi deneyimi artıkça daha iyi sonuçlar elde edilebilir.

Background: Migraine is associated with atherosclerosis and white matter hyperintensities. This study aims to evaluate the relationship between white matter hyperintensities and carotid intima-media thickness in patients who have migraines without auras. Methods: The study enrolled 105 patients; of these, 43 patients had migraine without white matter hyperintensity (WMH) and 32 had migraine with WMH. There were also 30 healthy control subjects. The patients were divided into two groups according to whether or not they had WMHs on their brain magnetic resonance imaging (MRI). All subjects, including the control group, underwent brain MRI and carotid ultrasonographic examination to evaluate WMH and carotid intima-media thickness (CIMT), respectively. Results: The groups did not differ with regard to demographics and clinical findings. The CIMT was significantly greater in patients who had migraine without WMH than in the control group (p < 0.001) and in those who had migraine with WMH than in those who had migraine without WMH (p = 0.004) Conclusion: CIMT values were higher in migraine patients with WMH than in migraine patients without WMH. Background: Chronic cerebral hypoperfusion was observed in patients with transient ischemic attack. Inorder to prevent a subsequent ischemic stroke, early diagnosis and proper management of transient ischemicattack are essential. Transient monocular blindness (TMB) accounts for 25% of transient ischemic attacks.Ischemic TMB occurs in the internal carotid territory and is a risk for ischemic stroke. Among patients withTMB, 27-67% of them had carotid artery stenosis and associated with a risk of ischemic stroke. Opticalcoherence tomography (OCT) which is widely used in ophthalmology is a noninvasive and easy techniquethat allows imaging of the retinal nerve fiber layer thickness (RNFT) and macular volume.In the present study, we aimed to evaluate the OCT findings of RNFT, central foveal thickness (CFT) andmacular volume of patients with TMB.Methods: This is a prospective, comparative study of the 14 patients with TMB and 16 age and sex-matchedhealthy subjects. After routine eye examination; RNFT, macular volume and CFT measurements wereperformed by OCT.Results: There was no significant difference between two groups in respect of age and sex (p=0,60 andp=0,71 respectively). In our study group, the mean total RNFT was significantly higher in comparison to thecontrol (p=0,008). In respect to sectoral evaluation, RNFT did not differ significantly between the study and3 the control group (p>0,05). The mean macular volume was 8,6±0,2 mm in the control group and it was3 9,0±0,6 mm in the study group and this was significantly higher (p, Amaç: Geçici iskemik atak olgularında kronik serebral hipoperfüzyonu saptanmıştır. Takip edebilecekiskemik serebrovasküler hastalık gelişimini engellemek için geçici iskemik atağın erken tanısının konmasıve gerekli müdahalenin yapılması önemlidir. Geçici görme kaybı (GGK) geçici iskemik atakların %25'inioluşturmaktadır. İskemiye bağlı oluşan GGK de internal karotid arterin perfüzyon alanında olup serebraliskemi için risk oluşturmaktadır. Geçici görme kaybı hastalarının %27-67'sinde zeminde karotis arterhastalığı vardır ve iskemik serebrovasküler hastalık açısından önemli bir risk oluşturur. Optik koherenstomografi (OKT), oftalmolojide yaygın olarak kullanılan retina sinir lifi kalınlığı (RSLK) ve makülerhacmin ölçüldüğü noninvasif, kolay bir yöntemdir.Çalışmamızda GGK tanısı alan olguların OKT ile RSLK, santral foveal kalınlık ve maküla hacminindeğerlendirimesi amaçlanmıştır.Materyal ve Metod: Prospektif olarak yapılan bu çalışmaya 14 GGK hastası ve kontrol grubu olarak 16sağlıklı birey dahil edilmiştir. Her iki gruba da rutin oftalmolojik muayeneyi takiben OKT kullanılarakRSLK, maküla hacmi ve santral foveal kalınlık ölçümleri alınmıştır.Bulgular: Yaş ve cinsiyet açısından iki grup arasında istatistiksel açıdan anlamlı bir farklılık yoktu (sırasıylap=0,60 ve p=0,71). Objectives: This study aimed to research the value of ultrasonography in the diagnosis of carpal tunnel syndrome (CTS) in patients with diabetes mellitus (DM) and to examine the flow characteristics of the radial and ulnar arteries in diabetic patients with CTS. Methods: A total of 23 diabetic hands diagnosed with CTS from electrophysiological evaluation (DM-CTS), 47 asymptomatic diabetic hands (DM) and 50 healthy hands (C) as the control group were evaluated with high resolution ultrasonography. The median nerve was measured in the cross-sectional area (CSA), flattening ratio (FR) and at the level of the carpal tunnel inlet [proximal (p)] and the wrist crease [distal (d)]. The radial and ulnar arteries were evaluated with both hands in a neutral position. Objective: Stroke ranks second among the diseases that cause death and third among the causes of disability in Turkey. An average of 1.9 million neurons die every minute due to acute ischemic stroke and therefore 'time is brain'. Intravenous thrombolytic treatment (ITT) and, if possible, endovascular treatment should be initiated quickly. We aimed to share our ITT experience in Sanliurfa with this study. Materials and Methods: We recorded the clinical and demographic characteristics, risk factors, and three-month follow-up of 21 patients with acute ischemic stroke who were given ITT between June 2015 and July 2017. Results: Of the 21 patients, nine were male and 12 were female, and the mean age was 72.1±11.8 (range, 48-87) years. Large vessel occlusion was found in five (23.8%) patients, cardioembolism in 9 (42.9%), and small vessel occlusion in 2 (9.5%) patients; etiology was not determined in five (23.8%) patients. Atrial fibrillation was found in 38.1%, hypertension in 57.1%, diabetes mellitus in 23.8%, coronary artery disease in 33.3%, hyperlipidemia in 19%, and smoking in 33.3% of the patients.The NIHSS score was 11.7±6.7 (range, 2-24) prior to treatment. The symptom-onset to needle time was 185±55.8 minutes when 19 patients were evaluated. Intracerebral hemorrhage was observed in two (9.5%) patients after treatment. One patient was asymptomatic and the symptomatic patient (4.75%) died despite decompression surgery. Ten patients died and the mortality rate was 47.6% at the end of the three-month follow-up period. The modified Rankin scale score was 0-2 in eight patients (38.1%) and was 0-1 in seven patients (33.3%). ITT was given to seven patients aged >80 years, five patients with NIHSS <5, and two patients in whom the treatment window was exceeded, which was non-adherent to the label. Conclusion: ITT is effective and safe. It increases the number of independent living patients. We must strive to perform this treatment all over Turkey and encourage our colleagues. Amaç: Bu çalışmada, posterior sirkülasyon infarktı (POCI) olan hastaların demografik özellikleri, riskfaktörleri, etiyolojik ve klinik profilleri gözden geçirerek bu parametreler bakımından cinsiyetler arasındafarklılık olup olmadığını değerlendirmeyi amaçladık.Materyal ve Metod: Bu çalışmaya Başkent Üniversitesi Adana Araştırma ve Uygulama Hastanesi,Kahramanmaraş Özel Hayat Hastanesi ve Şanlıurfa Mehmet Akif İnan Eğitim Araştırma Hastanesine 2014yılı ilk 6 aylık süre içerisinde başvuran ve posterior sirkülasyon infarktları olan ardışık 11'i (% 26.2) kadın ve31'i (%73.8) erkek toplam 42 hasta dahil edildi. Yaş, eğitim durumu, risk faktöleri, inme etiyolojisi ve ölümoranı bakımından kadın ve erkek hastalar karşılaştırıldı.Bulgular: Kadın hastalarda erkek hastalara göre; geçici iskemik atak, mortalite oranı daha yüksek saptandı.Risk faktörlerinden koroner arter hastalığı, konjestif kalp yetmezliği, atrial fibrilasyon, hiperlipidemi,diabetes mellitus, hiperlipidemi varlığı daha yüksek oranda saptandı.Sonuç: Çalışmamızdaki bulgular, POCI sendromlu hastalarda cinsiyetin kadın olmasının inmenin klinikseyrini olumsuz yönde etkileyebileceğini düşündürmektedir., Objective: In this study, we evaluated the risk factors, demographic, clinical and etiological characteristicsof patients with total posterior circulation infarct (POCI) and we aimed to elucidate the impact of genderbased differences on these parameters.Material and Methods: In this study, 42 ( 11 (26.2%) famele, 31 (73.8%) male) consecutive posteriorcirculation infarct patients from which Başkent University Research and Training Hospital, KahramanmaraşPrivate Hayat Hospital, and Şanlıurfa Mehmet Akif İnan Research and Training Hospital were included forthe first six months of 2014. We compared the age, previous stroke, risk factors, education and the rate ofdeath between genders.Results: Transient ischemic attack, the mortality rate was higher than female patients compared to malepatients; Risk factors for coronary artery disease, congestive heart failure, atrial fibrillation, hyperlipidemia,diabetes mellitus and hyperlipidemia were higher in patints with total posterior circulation infarct (POCI).Conclusion: The result of the present study indicated that female gender has a negative effect on clinicaloutcome of POCI.

Tanımlayıcı tipteki bu çalışmada, HÜTF Erişkin Hastanesi’nde 2004 yılındaki 674 ölüm vakasından kayıtlarına ulaşılabilen 659 ölüm vakası (%97.8) incelenmiş, hastaların epikriz bilgileri ile Devlet İstatistik Enstitüsü (DİE) formuna işlenen bilgi arasındaki tutarlılık değerlendirilmiştir. Çalışma bulgularına göre, HÜTF Erişkin Hastanesi’nde ölüm kayıtlarının daha iyi tutulmalıdır ve epikriz formunu yazan hekim ve DİE formunu dolduran görevli daha dikkatli olmalıdır. Epikrizin hekimler tarafından “doğru ve eksiksiz olarak”, ölüm formunun da servis sekreteri tarafından epikrize bakılarak doldurulması olası hataları azaltacaktır

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy. Carpal tunnel syndrome is acompression neuropathy which occurs due to compression of median nerve at the wrist level. CTS is mostcommonly seen in women over 30 years of age, frequently bilaterally, with nocturnal pain, paresthesia and/orsensory deficit in the median nerve innervated region of the hand. CTS is a clinical diagnosis based on thepatient's history and physical examination; electrodiagnostic tests including nerve conduction studies andneedle electromyography are the most commonly used auxillary diagnostic methods., Karpal tünel sendromu (KTS) en sık rastlanan tuzak nöropatidir. Karpal tünel sendromu, median sinirin elbileği düzeyinde tuzaklanmasına bağlı gelişen kompresyon nöropatisidir. En sık 30 yaşından büyükkadınlarda, sıklıkla çift taraflı olarak, median sinirin elde inervasyon alanına uyan bölgede geceleri ortayaçıkan ya da artan ağrı, uyuşma, ve/veya duyu kaybı belirtileri ile görülür. KTS hastanın öyküsü ve fizikmuayenesi ile ulaşılan bir klinik tanıdır ve sinir ileti çalışmalarına ilave iğne elektromiyografisinden oluşanelektrodiagnostik incelemeler tanıda en sık kullanılan yardımcı yöntemlerdir.

AMAÇ: Serebral venöz tromboz (SVT) az rastlanılan bir hastalıktır. Farklı klinik tablolarla karşımıza çıkabilir. Hastalığın etyolojisinde pekçok sebep vardır. Bu makalede biz, SVT tanısı alan hastaların hastaneye başvuru semptomlarını ve prognozunu tartışacağız.GEREÇ ve YÖNTEM: Bu çalışma, hastanemizde sinüs ven trombozu nedeniyle takip edilen hastaların hastane verilerinin retrospektif olarak taranması ile yapıldı. Çalışmaya 29 hasta dahil edildi. Tüm hastaların kranial magnetik rezonans (MR) görüntüleme ve MR veya bilgisayarlı tomografi venografi tetkikleri vardı. BULGULAR: Çalışmamızda 27 kadın, 2 erkek toplam 29 hasta vardı. Hastalarımızın yaş ortalaması 34.97 ±12.79 idi. Sadece 8 hastada anormal nörolojik muayene bulgusu vardı (4 hastada papil stazı, 2 hastada konfüzyon, 1 hasta serebeller bozukluk, 1 hasta homonim hemianopsi). Hastaların başvuru semptomları başağrısı (n=19), epileptik nöbetler (n=6), görme bulanıklığı (n=2), diplopi (n=2) ve bir hastanın da vertigosu vardı. Trombotik risk faktörüne 21 hastada rastlandı. En önemli risk faktörleri gebelik, postpartum dönem, oral kontraseptif kullanımı ve kazanılmış hiperkoagülabilitelerdi. SONUÇ: SVT nadir fakat önemli bir durumdur. Bazen hiçbir özelliği olmayan başağrısı bu hastalığın habercisidir. Özellikle gebelik ve postpartum döneminde SVT'ye yatkınlık söz konusudur, PURPOSE: Cerebral venous thrombosis (CVT) is not a common disease. It is known to have a varied clinical spectrum. There is too much cause of the etiology of disease. In this manuscript we discussed our patients initial symptoms, cause of CVT and our patients' prognosis. MATERIALS and METHODS: This study was a retrospective database analysis of 29 patients who had CVT followed from 2004 to 2010. All of the patients had cranial magnetic resonans imaging (MR) and Mr /Computarized Tomography (CT) venography. RESULTS: There were 27 female, 2 male in this study. Mean age of the patients was 34.97 ±12.79 years old. Only eight patients had abnormal neurological examination (4 patients papilloedema, 2 patients confusion, 1 patient cerebellar dysfunction, 1 patient homonymous hemianopsia). Initial symptoms of patients were headache(n=19), epileptic seizures (n=6), blurred vision (n=1), diplopia (n=2) and one of the patient had vertigo. Ten patients had only one sinus thrombosis however 19 patients had more than one sinus thrombosis. Thrombotic risk factors were found in 21 patients. Most important risc factor were pregnancy, postpartum condition, oral contraceptive usage and acquired hypercoagulable disease. CONCLUSION: CVT is rare but important condition. Sometimes non specific headache is a herald of this condition. Especially pregnancy and postpartum period are susceptibility of this condition.

Serebral kanamanın en sık gözlendiği bölgeler putamen, temporal lob, parietal lob veya frontal lobların santralmaddesi, thalamus, serebllar hemisfer ve beyin sapında ponstur. Bazal ganglionlar hipertansiyona bağlıintraserebral hemorajinin (ISH) en yaygın gözlendiği bölgedir. Etyolojisinde hipertansiyon, vasküler anomaliler,vaskülopatiler, hematolojik bozukluklar, tümörler, antikoagülanlar, ilaçlar rol oynar. Bilateral intraserebralhemoraji (BİH) gelişiminde hipertansiyondan başka metanol intoksikasyonu, etilen glikol intoksikasyonu,neonatal asfiksi, migren ve hiperglisemik hiperosmolar koma ile ilişkili vakalar da bildirilmiştir. Hipertansiyonabağlı olarak bilateral bazal ganglion kanaması az rastlanılan bir tablodur. Bu olgu sunumunda, sol hemiparezisiolan, risk faktörü olarak hipertansiyon ve diabeti bulunan ve hipertansiyona bağlı bilateral intraserebral hemorajigelişen bir hastadan bahsedeceğiz, Cerebral hemorrhages are mainly seen in the area of the central matter of putamen, temporal lobe, parietal lobe or frontal lobe, thalamus, cerebellar hemisphere and pons in the brainstem. Hypertension related intracranial hemorrhages are most commonly seen in basal ganglia. The etiology of the hemorrhages are hypertension, vascular anomalies, vasculopaties, hematological disease, tumors, anticoagulant drugs. Besides hypertension, methanol or ethylene glycol intoxication, neonatal asphyxia, migraine and hyperglycemic hyperosmolar coma are also related with this condition. In this case report we discuss a patient with hypertension and diabetes mellitus who developed bilateral intracerebral hemorrhages of the basal ganglia.

30. Comparison of the Short-Term Effectiveness of Short-Wave Diathermy Treatment in Patients With Carpal Tunnel Syndrome: A Randomized Controlled Trial.

Objectives: This study aims to compare the effects of pulsed and continuous short wave diathermy (SWD) treatment in carpal tunnel syndrome. Patients and methods: This double-blind, randomized controlled trial included 30 patients (55 wrists) diagnosed with mild or moderate carpal tunnel syndrome. Patients were randomized into three groups and administered splinting and one of the following treatments: group 1: continuous SWD, group 2: pulsed SWD, and group 3: placebo SWD. The treatment was applied five days a week for 15 sessions. Patients were evaluated by the Tinel's test, Phalen's and reverse Phalen's maneuvers, carpal compression test, the Boston Carpal Tunnel Questionnaire (BCTQ: includes 2 parts, namely the symptom severity scale and functional status scale), and a visual analog scale. Median motor distal latency, median sensory distal latency, and nerve conduction velocity were measured. Electrophysiological findings, clinical tests, and scales were evaluated at the beginning and end of the therapy. Results: A significant difference was observed in Phalen's maneuver positivity in the pulsed SWD group (p<0.05). Visual analog scale, BCTQ- functional status scale, BCTQ-symptom severity scale, median nerve motor and sensory latencies, and sensory nerve conduction velocity values were improved in both continuous and pulsed SWD groups (p<0.05). The placebo group also showed improved BCTQ-functional status scale and median nerve motor distal latency (p<0.05). BCTQ-symptom severity scale improved more in the continuous SWD group than the other groups (p<0.01), and improved median nerve sensory distal latency and nerve conduction velocity parameters were observed in both SWD groups than the control group (p<0.01). Conclusion: Continuous and pulsed SWD therapies supported with splinting were effective for improving pain, hand function, and electrophysiological findings. In addition, continuous SWD seemed to be more effective in reducing symptom severity than pulsed and placebo SWD. [ABSTRACT FROM AUTHOR]

Stroke remains one of the leading causes of death and disability in Europe. The European Stroke Action Plan (ESAP) defines four main targets for the years 2018 to 2030. The COVID-19 pandemic forced the use of innovative technologies and created pressure to improve internet networks. Moreover, 5G internet network will be helpful for the transfer and collecting of extremely big databases. Nowadays, the speed of internet connection is a limiting factor for robotic systems, which can be controlled and commanded potentially from various places in the world. Innovative technologies can be implemented for acute stroke patient management soon. Artificial intelligence (AI) and robotics are used increasingly often without the exception of medicine. Their implementation can be achieved in every level of stroke care. In this article, all steps of stroke health care processes are discussed in terms of how to improve them (including prehospital diagnosis, consultation, transfer of the patient, diagnosis, techniques of the treatment as well as rehabilitation and usage of AI). Aim and Clinical Rationale for the Study: It is now known that inflammation is involved in the pathophysiology of acute ischaemic stroke (AIS). It has been proven that CRP and albumin alone are useful in predicting a prognosis for stroke patients. A combination of these two parameters, namely the ratio of CRP to albumin (CAR), is believed to be a more accurate indicator of inflammatory status than CRP or albumin alone, and may be more valuable than either of them separately in predicting the prognosis of ıschaemic stroke patients. However, the role of CAR as a predictor of mortality in patients with AIS remains unclear., Materials and Methods: We retrospectively enrolled 260 patients who were referred to our clinic within the first 24 hours of symptom presentation and who were diagnosed with AIS between January 2015 and December 2018. The patient group was classified into two groups according to 90-day mortality. These groups were compared in terms of C-reactive protein, albumin, and CAR., Results: The C-reactive protein and CAR values were higher, and the albumin level was lower, in non-surviving patients. The CAR value was also found to be a significant independent variable of 90-day mortality in patients with AIS (p < 0.001). The optimum cut-off value of CAR in predicting the 90-day mortality for patients with AIS was 0.50, with 64.1% sensitivity and 56.2% specificity., Conclusions and Clinical Implications: Our study demonstrated that a high CAR value is an independent predictor of 90-day mortality in patients with AIS.

- Abstract
Sjögren's syndrome (SS) is an autoimmune disease with mononuclear cell infiltration and destruction of the lacrimal gland and salivary glands, which cause dryness of the eyes and mouth. The most common neurological condition seen in SS is peripheral neuropathy. Initial manifestation of SS as an acute fulminant peripheral neuropathy is extremely rare. We report a 42-year-old patient presenting with acute motor sensory-axonal neuropathy in the presence of SS. She showed partial response to intravenous immunoglobulin but favourable clinical improvement was seen after initiation of corticosteroid treatment.
