8. The effectiveness of telerehabilitation‐based exercise combined with pain neuroscience education for patients with facet joint arthrosis: A randomized controlled study.

Abstract

Objectives: This study aimed to investigate the short‐term effectiveness of exercise combined with PNE and exercise alone via telerehabilitation for patients with low back pain (LBP) caused by facet joint arthrosis (FJA). Methods: This is a prospective, single‐blind, randomized‐controlled clinical trial. Forty–five patients with LBP caused by FJA were randomly allocated into three groups. The exercise (n = 15) and the exercise with PNE (n = 15) groups were provided interventions twice a week for six weeks via telerehabilitation. The control group (n = 15) was placed on the waiting list. The primary outcome measures were the numeric pain‐rating scale (NPRS) at rest and with activity, The Oswestry disability index (ODI), the pain beliefs questionnaire (PBQ), and secondary outcome measures were short form‐12v2 (SF‐12v2) and the global rating of change score (GROC). Results: The groups (52.00 ± 4.86, 46.7% female) were similar at baseline except for gender (p = 0.029). There was a significant group‐by‐time interaction for NPRS‐rest (F = 4.276, p = 0.021), NPRS‐activity (F = 12.327, p = 0.0001), the ODI (F = 23.122, p = 0.0001) and organic pain belief (F = 39.708, p = 0.0001). Further comparison with ANOVA showed that the exercise with PNE group showed better improvement in only organic pain belief (p = 0.0001). All groups reported improvements according to GROC, but it was higher in the intervention groups (p = 0.001). Conclusion: The patients who received exercise combined with PNE and exercise alone significantly improved pain intensity, disability and organic pain beliefs via telerehabilitation for patients with LBP caused by FJA compared to the control group. The PNE in addition to exercise may exhibit greater benefit in improving organic pain beliefs in short term. Significance: This study highlights that combining exercise with PNE can lead to greater improvements compared to exercise alone or no intervention for FJA patients. The implementation of PNE in physiotherapy sessions has the potential to offer significant benefits. Furthermore, our results highlight the promising role of telerehabilitation as an effective method for delivering interventions to individuals with FJA. [ABSTRACT FROM AUTHOR]

11. Three region spinal decompression in the same session: a case report.

Abstract

Background and importance: Multiregional spinal stenosis [tandem spinal stenosis (TSS)] is not rare but operating on multiple regions at the same sitting is. Decompression of cervical and lumbar spine in the same session has a frequency of 5-25% all TSS cases, the most frequent one is TSS. Decompression in three different regions is so rare that there is only one case in the literature. We report the second. Clinical presentation: A 72-year-old man with pain in legs and arms, neurogenic claudication, progressive loss of balance, radiculopathy and myelopathy in lower and upper extremities whose cervical-thoracic and lumbar spinal stenosis were treated with decompressive surgery in the same session. Total time of surgery for three regions was 330 min. No complications were observed. The patient was mobilized on day 1 postoperative and was discharged from the hospital on day 3. By month 3, motor function had improved almost completely, and pyramidal findings have decreased. Conclusion: Multiregional spinal stenosis of three spinal regions at the same time is a rare case and these cases, surgery of three regions can be carried out in the same session respectively. Abstract

ÖZETAmaç: Tek merkezde lomber dejeneratif hastalıklarda uygulanan lomber korpuslar arası füzyon kafeslerinin(TLIF) anteriora (anterior longtdiunal ligamanı geçmiş şekilde) yerleştirilme sebeplerini ve oranlarını tanımlamak.Yöntemler: 2014 -2017 yılları arasında 270 hasta ve bu hastaların 366 disk mesafesine uygulanan TLIF kafesuygulaması incelendi. TLIF için uygulanan kafeslerin boyutuna ve uygulanış yerlerine göre erken operasyonsonrası dönemde anterior yerleştirme oranlarına ve nedenlerine bakıldı.Bulgular: Buna göre anteriora yerleştirme toplamda 3 hastada görüldü. Anteriora yerleştirme oranı tek merkezliyapılan bu çalışmada %0.008 civarındadır. Bunlardan üçünde de anteriora yerleştirme seviyesinin L5-S1seviyesi olduğu görüldü.Bu hastalarda ve diğer hastalarda kullanılan kafeslerin boyutları da ele alındı. Boyutlar ele alındığında 3hastada da disk mesafesinin ön arka çapının orta noktasının sagittal görüntüdeki yüksekliğinin kafes yüksekliğindendaha fazla olduğu görüldü.Sonuç: L5-S1 seviyesi diskin ön yüksekliği diğer disk mesafelerine göre daha fazla olduğu için burada kafesinanteriora yerleştirilmesi olasılığı yüksektir. Bunu da diskin orta noktasındaki sagittal boyutundan daha büyükbir kafes kullanılması önleyebilir.Disk mesafesinin anteroposterior çapının orta noktasında geçen disk yüksekliğinin boyutundan daha küçükbir boyuta sahip kafes uygulamasının, anteriora yerleşimi için bir risk olacağı sonucuna varılmıştır.Anahtar Kelimeler: Öne doğru çıkma; Komplikasyon; TLIFABSTRACTAim: The aim of this study was to define the anterior misplacement rates and causes of lumbar transforaminalinterbody fusion (TLIF) cages applied in spinal degenerative diseases in a single centerMaterial-Method: Between 2014 and 2017, 366 intervertebral disc spaces in 270 patients which underwentTLIF cages application were evaluated. The anterior misplacement rates and causes of cages applied forfusion were assessed according to the size and location. All patients had lumbar degenerative diseases.Cage were performed in all cases where instability was obvious or the foraminal height should be increased.Peroperative scopy or early postoperative images were evaluated in the evaluation of anterior misplacement.Postoperative computed tomography (CT) was performed in all patients.The dimensions of the cages used in these patients and other patients were also discussed.Results: According to this, anterior misplacement was seen in 3 patients in total. It was seen that the levelof misplacement was L5-S1 level in these three patients. Anterior misplacement rate is nearly 0.008% in thisstudy which carried out in a single center.Conclusion: Since the frontmost height of the L5-S1 disc space is wider than the other disc spaces, the cageis more likely to be applied in a misplacement location. Application of a larger cage than the sagittal heightof the midpoint of disc space can also prevent this. It was concluded that a cage application with a smallersize than the size of height of the disc space crossing at the midpoint of anteroposterior diameter of the discspace would be a risk for anterior misplacement.Key Words: Antepulsion; Complication; TLIF

Abstract

Suicide is the process that an individual ends his/her own life, and usually methods providing a quick death are chosen. The most common methods are jumping from heights and firearm shots. However, in some suicide attempts, some unusual methods are used. These can be seen in patients with psychiatric disorders such as severe depression and schizophrenia. The idea of suicide by hammering nail into the head is uncommon because it is a method that cannot lead to a quick death and cause suffering. Beside these, it does not lead to death with a single action. We present a case of a young male patient who attempted suicide with hammering nail into the head and review the literature on this subject. / İntihar insanın kendi hayatına son vermesi işlemidir ve genellikle ölümü aniden getirecek yöntemler seçilmektedir. Bunlardan en sık olanları yüksekten atlama, ateşli silah kullanımıdır. Ancak bazı intihar şekilleri de hiç tahmin edilemeyecek yöntemlerle olabilir. Ağır depresyon, şizofreni gibi psikiyatrik hastalıkları olan hastalarda bunlar görülebilmektedir. Başına çivi çakarak intihar etme düşüncesi de sık görülen bir durum değildir. Çünkü ani ölüm getirmeyecek ve acı çekerek ölüme sebep olabilecek bir yöntemdir ve tek bir hamlede bitirelecek bir intihar girişimi değildir. 32. The effect of technological devices on cervical lordosis

Abstract

Purpose: There is a need for cervical flexion and even cervical hyperflexion for the use of technological devices, especially mobile phones. We investigated the effect of this use on the cervical lordosis angle. Material and methods: A group of 156 patients who applied with only neck pain between 2013–2016 and had no additional problems were included. Patients are specifically questioned about mobile phone, tablet, and other devices usage. The value obtained by multiplying the year of usage and the average usage (hour) in daily life was determined as the total usage value (an average hour per day x year: hy). Cervical lordosis angles were statistically compared with the total time of use. Results: In the general ROC analysis, the cut-off value was found to be 20.5 hy. When the cut-off value is tested, the overall accuracy is very good with 72.4%. The true estimate of true risk and non-risk is quite high. The ROC analysis is statistically significant. Conclusion: The use of computing devices, especially mobile telephones, and the increase in the flexion of the cervical spine indicate that cervical vertebral problems will increase even in younger people in future.
