During last years a few new directions appeared in therapy of advanced colorectal cancer. Topotecan, a camptothecin analog, seems to be one of the promising novel drug in these cases. Its unique mechanism of action is connected with inhibition of the nuclear enzyme topoizomerase I. Assessing therapeutic effects of new cytotoxic drugs we should consider their impact on survival time and quality of life as well. The aim of our study was the attempt to assess the quality of life of patients with advanced colorectal cancer (IV stage in TNM scale), treated by topotecan. Clinical trial was performed in the group of 10 patients. Topotecan was administered intravenously at 1.5 mg/m2/day for 5 days and repeated every 21 days. Quality of life assessment was performed at special time points using Rotterdam Symptom Checklist. We observed improvement in quality of life in six out ten patients having advanced colorectal cancer. Two patients did not show any change in quality of life and two patients with progression of disease demonstrated lower quality of life during topotecan treatment.Topotecan has a positive influence on quality of life of patients with advanced colorectal cancer. Further study are needed to confirm this observation.

- Abstract
Purpose: Past research studies have focused on perceptions of stuttering by various age groups and only a few have examined how children react to a peer who stutters. All of these studies used a quantitative analysis but only one included a qualitative analysis of elementary school age children's responses to stuttering. The aim of this study was to further explore the perceptions of elementary school students toward a peer who stutters using both quantitative and qualitative analyses of three levels of stuttering., Methods: Participants included 88 elementary school children between 8 and 12 years of age. Each participant viewed one of four audiovisual samples of a peer producing fluent speech and mild, moderate, and severe simulated stuttering. Each participant then rated five Likert statements and answered three open-ended questions., Results: Quantitative and qualitative results indicated that negative ratings and the percentage of negative comments increased as the frequency of stuttering increased. However, the children in this study indicated that they were comfortable listening to stuttering and would be comfortable making friends with the peer who stutters., Conclusion: The findings of this study together with past research in this area should help clinicians and their clients appreciate the range of social and emotional reactions peers have of a child who stutters., Educational Objectives: After reading this article, the reader will be able to: (a) discuss past research regarding children's perceptions of stuttering; (b) summarize the need to explore the perceptions of elementary-aged children toward a peer who stutters; (c) describe the major quantitative and qualitative findings of children's perceptions of stuttering; and (d) discuss the need for disseminating more information about stuttering to children and teachers., (Copyright © 2015 Elsevier Inc. 7. 23-year experience on diagnosis and surgical treatment of benign and malignant cardiac tumors.

- Abstract
Introduction: Although myxoma is the most frequent cardiac tumor, other conditions should be taken into consideration in the differential diagnosis. Transthoracic echocardiography (TTE), followed by transesophageal echocardiography (TEE) remain the principal methods for cardiac tumor screening and visualizing. The aim of the study was to compare the diagnostics, surgical treatment and prognosis of malignant and benign cardiac tumors., Material and Methods: From 1986 to 2009 there were 121 patients with cardiac tumors operated on in the Cardiac Surgery Clinic of the Medical University in Lodz. Patients were referred to surgery mainly on the basis of the TTE and TEE image. In 4 cases valvular prosthesis implantation or valve repair were carried out. Patients remained under long-term observation in the Cardiac Surgery Outpatient Clinic., Results: Myxoma was diagnosed in 114 cases. Malignancies were discovered in 7 cases. The left atrium was the most frequent localization. The echocardiographic image differed significantly in benign and malignant tumors. The postoperative period was complicated by embolic events or myocardial infarctions. Only malignant tumors were associated with mortality due to cardiovascular events. The survival for malignant tumors was significantly shorter., Conclusions: Short and long-term results of operative treatment are very good for benign tumors in contrast to cardiac malignancies. The TTE and TEE image can be very significant in the final diagnosis.

- Abstract
Unlabelled: Renal failure of patients qualified for open-heart surgical treatment seems to be very important problem since years. The operation with extracorporeal circulation is large burden for kidneys and there is huge risk of intensifying of existing failure. The aim of the study was the evaluation of renal parameters, complications occurrence and mortality of patients with chronic renal failure (ChRF) (grade II--mild ChRF) subjected to mitral valve replacement., Materials and Methods: To retro- and prospective analysis were subjected 34 patients with severe mitral valve defect, subjected to operation between 1998 and 2002 in Department of Cardiac Surgery in Lodz. All patients included to study had increased level of creatinine--1, 5-3, 0 mg/dl (at least in three measurements). All examined patients were subjected to artificial mitral valve replacement and 13 patients to simultaneous tricuspid valve plastic surgery. Postoperative evaluation of clinical condition and renal parameters was performed twice: early (till 30 day after operation) and long-term (after meanly 14 months)., Results: In seven patients, in early postoperative period, it was necessary to implement extracorporeal dialysis with artificial kidney dialysis machine. Among these patients we ascertained three deaths (intrahospital mortality was 8.8%). During long-term observation we noted two cases of infective endocarditis requiring reoperation and re-implantation of mitral valve. During this time of observation only one death was noted (post-hospital mortality was 3.2%)., Conclusions: Dysfunction of kidneys, even in mild degree, causes significant increase of risk of early death (about 2.5 times). In most research population in long-term observation we ascertained the large improvement of kidney functions (significant decrease of creatinine level; p < 0.001). Concomitant tricuspid valve regurgitation, requiring plastic surgery is an additional factor incriminating long-term results of surgical treatment.

- Abstract
Among several topoisomerase I inhibitors topotecan is worth taking into account because of its little toxicity, good tolerance and quick excretion. The study compared functioning of bone marrow, liver and kidneys during and after the treatment, as well as survival period and quality of life of patients treated with topotecan and patients who underwent symptomatic treatment. 35 patients aged 43-71 of both sexes, with advanced stage of large intestine cancer (IV grade in TNM scale) were included in the study. 20 hospitalized patients were administered topotecan, topoisomerase I inhibitor, intravenously in 5-day courses (1.5 mg/m2 of body surface daily) repeated 5 times with 21-day intervals. Before the beginning of therapy and after it the following were conducted: medical check-ups as well as surveys and test research including groups of somatic and vegetative symptoms connected with the disease (based on the questionnaire QLQ-C30). 15 patients with the similar stage of the disease who had not agreed to palliative treatment with chemotherapy were treated only symptomatically. Applying antineoplastic treatment with topotecan contributes to the extension of the survival time by 4 months on average, and mortality during and after the treatment is lower than in the group of patients with the symptomatic treatment only. Antineoplastic treatment improves the quality of life and delays the disease progression. The quality of life is better until the preterminal period of the disease and enables patients maintaining motor activity and independence.

Unlabelled: Anxiety accompanies patients at all stages of severe somatic diseases as well in the course of diagnostic procedures. It affects significantly quality of life of these patients and their relations with their physicians. It may also induce or exacerbate wide range of somatic complaints., Objective: Evaluation of usefulness of anxiolytic therapies in patients with diagnosed neoplastic diseases. Comparison of anti-anxiety and antidepressant effects of alprazolam and diazepam in patients treated with systemic chemotherapy., Material and Methods: Final analysis included 45 patients with various malignancies randomly assigned to receive alprazolam 3 x 0.5 mg or diazepam 3 x 2 mg. Anti-anxiety effects of both drugs was evaluated four times during the 21-day follow-up period at one week intervals with help of Clinical Global Impression scale (CGI), Hamilton Anxiety Rating Scale (HARS), Hospital Anxiety and Depression Scale (HADS). Analysis of these questionnaires was performed with Mann-Whitney test and Wilcoxon test., Results: Statistically significant reduction of anxiety was found in both study groups. Alprazolam was proven to be significantly more effective than diazepam in reduction of anxiety (HARS, HADS) and depressive symptoms accompanying anxiety (HADS). Alprazolam vs. diazepam showed better tolerance by patients., Conclusions: Alprazolam effectively reduced anxiety in patients suffering from malignant diseases. Alprazolam was found to reduce depressive symptoms in patients with neurotic disorders whereas no effects of this kind were observed for diazepam.

- Abstract
Unlabelled: The aim of the thesis is the evaluation of efficacy of surgical treatment for complex valvular heart diseases. An inclusion criterion to this analysis was the performance of a mitral valve replacement (MVR) and aortic valve replacement (AVR) for an artificial valve and a tricuspid valve replacement or plastic operation (TVR or pl. TV). Such operative procedures were carried out simultaneously. Twenty five operations were performed from 1995 to 1999. TVR was carried out in 2 patients, pl. TV in 21 patients and the Revuelt method was adopted for this purpose, whereas in two cases a tricuspid valve plastic operation was performed sewing in the annulus. In the two cases the revascularization of the right coronary artery was carried out simultaneously. In another case a myxoma removal from the left atrium was additionally carried out. One patient died due to renal failure during hospitalization on the 9th day after the operation. Three patients died subsequently in the first, second and third year after operation. The causes of subsequent deaths among the patients examined in the period of five years after the operation were thrombotic-embolic complications., Conclusions: Early and subsequent results of simultaneous tri-valvular operations are good. The most frequent and difficult postoperative complication in this group of patients is a low output syndrome. Subsequent complications and deaths are most often due to thromboembolic complications, therefore a particular attention should be paid to the anticoagulant treatment in this group of patients.

- Abstract
The aim of this study was an attempt to estimate depression prevalence in medical students. Simultaneously we tried to asses the impact of early negative experiences on the level of depression in that population. We examined 92 third year medical students of the Silesian Academy of Medicine (54 women and 38 men) at the age from 20 to 24. Frequency and intensification of depression were assessed by using Beck Depression Inventory. Early experiences including negative and positive ones were estimated with the use of a self prepared repertory test called "biographical path", based on Personal Construct Theory of George Kelly. The obtained results indicated that 25% of examined students had depression symptoms. These students were exposed more frequently to early negative experiences than students without depression. In childhood depressive students were under pressure of their environment to score successes. If they did not realise these expectations they were exposed to discontent and frustration of their parents. Depressive students came less frequently into contact with other people and acquired independence later than non depressive students.

- Abstract
In the years 1981-1989 in the Department of Cardiosurgery, Institute of Cardiology, Medical Academy in Lódź, 39 patients with Fallot's syndrome were subjected to radical correction of the congenital heart disease. In 18 cases the correction was preceded by a palliative operation carried out on the average four years before the radical correction of the congenital heart disease. During the early postoperative period seven patients died which accounted for 17%. Out of 39 patients treated surgically, 32 were in I or II haemodynamic grade according to NYHA.
