Abstract

The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. The chapters on paediatric acute and chronic rhinosinusitis are totally rewritten. All available evidence for the management of acute rhinosinusitis and chronic rhinosinusitis with or without nasal polyps in adults and children is systematically reviewed and integrated care pathways based on the evidence are proposed. Despite considerable increases in the amount of quality publications in recent years, a large number of practical clinical questions remain. It was agreed that the best way to address these was to conduct a Delphi exercise . The results have been integrated into the respective sections. Last but not least, advice for patients and pharmacists and a new list of research needs are included. Abstract

A new approach for the preparation of nitrogen bridgehead bicyclic heterocyclic compounds such as 1,5-dimethyl-1-azoniabicyclo-[3.2]octane iodide in excellent yield is discussed. The method involves ring closure of beta-ammonio 5-hexenyl radicals to achieve complete regioselectivity and enhance the rate of cyclization.

Abstract

A study of the behavior of the (3-methylenecyclohexyl)ethyl radical is presented. It is shown that cyclization of the radical to the isomeric 1-bicyclo(3.2.1)octylmethyl radical occurs with an activation energy of 11.6 kcal mol-1. This barrier tallies with the calculated value of 12.6 kcal mol-1. The ring closure behavior of the radical can be utilized for high-yield synthesis of bicyclo(3.2.1) ocanes with useful functionality at the bridgehead.

Abstract

A theoretical analysis of ring-closure of the (4-methylenecyclohexyl)methyl radical was done using molecular mechanics calculations of activation energy. Results obtained using the Beckwith-Schiesser model showed an energy barrier equal to 13.5 kcal mol-1 while the Spellmeyer-Houk method showed a value of 12.5. An easy route to a norbonyl ester was suggested. This included a three-step reaction with an overall yield of 46%. Ring closure reactions of substituted methyl radical precursors led to the synthesis of bicyclo(2.2.1)heptyl systems.

16. Solid-phase extraction and HPLC-MS profiling of pyrrolizidine alkaloids and their N-oxides: a case study of Echium plantagineum.

Abstract

Pyrrolizidine alkaloids and their N-oxides can be extracted from the dried methanolic extracts of plant material using dilute aqueous acid. The subsequent integration of solid-phase extraction (with a strong cation exchanger) of the alkaloids and N-oxides from the aqueous acid solution, together with analysis using HPLC-ESI[sol ]MS, provides a method for the simultaneous profiling of the pyrrolizidine alkaloids and their N-oxides in plant samples and the collection of useful structural data as an aid in their identification. The N-oxide character of the analytes may be confirmed by treating analytical samples with a redox resin and observing the formation of the corresponding parent pyrrolizidine alkaloids. The present case study of Echium plantagineum highlighted a higher ratio of N-oxides to the parent tertiary bases than has been previously reported. Furthermore, a higher proportion of acetylated pyrrolizidine- N-oxides was observed in the flower heads relative to the leaves. Six pyrrolizidine alkaloids or pyrrolizidine- N-oxides, not previously reported from E. plantagineum, were tentatively identified on the basis of MS and biogenetic considerations. Three of these, 3′- O-acetylintermedine[sol ]lycopsamine, leptanthine- N-oxide and 9- O-angelylretronecine- N-oxide, have been reported elsewhere, whilst three others, 3′- O-acetylechiumine- N-oxide, echimiplatine- N-oxide and echiuplatine- N-oxide, appear unreported from any other source.
