Abstract

Case description Our patient, a 13-year-male with PWS, initially presented with pancytopenia and was subsequently diagnosed with MDS with excess blasts (19% bone marrow blasts). Prader-Willi Syndrome (PWS) is a neurodevelopmental genetic disorder characterized by behavioral, developmental, and endocrine abnormalities, and a characteristic physical phenotype.[1] PWS arises due to genomic imprinting errors resulting in loss of expression of paternal genes in the chromosome 15q11-q13 region. The dietetics service was utilized to support patient food choices and nutrition, simultaneously balancing nutritional requirements and PWS-related hyperphagia with transplant-related reduced intake, nausea, and mucositis. A 14-month-old male presented with paroxysmal nocturnal dyspnea and grade III/VI systolic ejection murmur at the upper left sternal border with an S4 gallop and was subsequently found to have a right ventricular cardiac myxoma. Prior presentations of these tumors have been with exertional syncope and murmur, asymptomatic murmur, or exertional dyspnea; the presentation of such a tumor with paroxysmal nocturnal dyspnea is novel.

Abstract

Autologous hematopoietic stem cell transplant (aHSCT) is a central component of paediatric brain tumours and high-risk neuroblastoma management. Standardly, chemotherapy and G-CSF are used to mobilize the CD34+ hematopoietic stem/progenitor cells (HSC), which are used to support hematopoietic recovery following dose chemotherapy. Clinically, the graft infused is defined by solely by the CD34+ content. The non-hematopoietic passenger cells makeup most of the graft and are a mixture of myeloid and lymphoid cells that are potentially biologically active in the post-transplant setting. Here we study the content of the graft, focusing on the phenotypes of the cell populations, to begin to look at how the non-hematopoietic cells may facilitate anti-tumor activity, affect the tumour microenvironment, and/or immune recovery post-transplant.
