Background. COVID-19 is a disease that has changed the realities of society, medical personnel, and patients worldwide. The long-term course of the COVID-19 pandemic has allowed us to observe the many changes occurring during this disease. Lesions resulting from SARS-CoV-2 infection manifest in the lungs and other systems, including the skin. Methods. The paper presents a comprehensive description of skin lesions occurring. The course of COVID-19 was based on a literature review and my experience acquired in the intensive care unit. Results. The mechanism of formation of the cutaneous manifestations in SARS-CoV-2 infected patients due to the course of the disease was discussed. Skin lesions from the treatment, hospitalisation and immobilisation of the patient were also considered. The causes of developing bedsores are also highlighted, with examples provided with photographs. In addition, the paper includes tables comparing the appearance of skin lesions with the severity of the course and caused by pharmacotherapy, which may be a practical instrument in clinical practice. Conclusions. Cutaneous manifestations may be the single symptom of COVID-19, which may facilitate the diagnosis of this disease. It seems necessary to extend the diagnostics of skin lesions during COVID-19 to understand their pathogenesis. Inadequate care due to staff shortages or inadequate education may lead to the development of pressure sores. Implementing solutions that could protect patients and staff is imperative. We present a case of the Lazarus phenomenon, which is to our knowledge first report from Poland. This is a phenomenon of the heart, which can resume its spontaneous activity and generate circulation. It can happen after ineffective cardiopulmonary resuscitation (CPR) or in a departed person when a resuscitation attempts were not undertaken but a circulation returned spontaneously. The victim was 86 years old woman. She suffered cardiac arrest in a church. The rescuers began CPR, administered one shock and continued pharmacotherapy. After 40 minutes, the CPR was discontinued and death was confirmed. Suddenly after 15 minutes return of spontaneous breathing was recognized. Upon arrival to the hospital her vital signs were: HR 84/min, NIBP 106/70 mmHg, saturation 93%, temperature 36,5 °C. An increase of troponin in repeatable laboratory tests was noticed, but taking into account the extended duration of the cardiac arrest, and the amount of time before CPR was administered as well as patient's age and unfavorable prognosis, the decision was made not to apply any invasive cardiologic intervention. At third day of hospitalization sudden ventricular tachycardia with narrow QRS complexes and destabilization of circulation was observed. At the beginning of a therapy an improvement of a condition of the patient was reported, but later sudden drop in blood pressure appeared with no reaction to the treatment. [ABSTRACT FROM AUTHOR]

Background health-related quality of life (HRQoL) is an important patient-centred outcome in patients surviving ICU admission for COVID-19. It is currently not clear which domains of the HRQoL are most affected. Objective to quantify HRQoL in order to identify areas of interventions. Design prospective observation study. Setting admissions to European ICUs between March 2020 and February 2021. Subjects patients aged 70 years or older admitted with COVID-19 disease. Methods collected determinants include SOFA-score, Clinical Frailty Scale (CFS), number and timing of ICU procedures and limitation of care, Katz Activities of Daily Living (ADL) dependence score. HRQoL was assessed at 3 months after ICU admission with the Euro-QoL-5D-5L questionnaire. An outcome of ≥4 on any of Euro-QoL-5D-5L domains was considered unfavourable. Results in total 3,140 patients from 14 European countries were included in this study. Three months after inclusion, 1,224 patients (39.0%) were alive and the EQ-5D-5L from was obtained. The CFS was associated with an increased odds ratio for an unfavourable HRQoL outcome after 3 months; OR 1.15 (95% confidence interval (CI): 0.71–1.87) for CFS 2 to OR 4.33 (95% CI: 1.57–11.9) for CFS ≧ 7. The Katz ADL was not statistically significantly associated with HRQoL after 3 months. Conclusions in critically ill old intensive care patients suffering from COVID-19, the CFS is associated with the subjectively perceived quality of life. The CFS on admission can be used to inform patients and relatives on the risk of an unfavourable qualitative outcome if such patients survive.

PURPOSE: Critically ill old intensive care unit (ICU) patients suffering from Sars-CoV-2 disease (COVID-19) are at increased risk for adverse outcomes. This post hoc analysis investigates the association of the Activities of Daily Living (ADL) with the outcome in this vulnerable patient group. METHODS: The COVIP study is a prospective international observational study that recruited ICU patients ≥ 70 years admitted with COVID-19 (NCT04321265). Several parameters including ADL (ADL; 0 = disability, 6 = no disability), Clinical Frailty Scale (CFS), SOFA score, intensive care treatment, ICU- and 3-month survival were recorded. A mixed-effects Weibull proportional hazard regression analyses for 3-month mortality adjusted for multiple confounders. RESULTS: This pre-specified analysis included 2359 patients with a documented ADL and CFS. Most patients evidenced independence in their daily living before hospital admission (80% with ADL = 6). Many diseases compromise the immune status. Some of them are congenital, and environmental factors induce others. As a result, patients are more prone to other diseases that would not match their clinical presentation at first sight. The main categories include primary and secondary immunodeficiency. Inborn errors of immunity (IEI) are a group of congenital diseases characterised by incomplete innate or acquired immunity pathways. Secondary immunodeficiency is the result of the immune system being compromised due to an environmental factor, such as specific viruses. The clinical presentation of immunodeficiency tends to be apparent at first sight as diagnostic criteria include recurrent infections of severity that are incoherent with age-related risk factors. Late diagnosis may lead to conditions such as sepsis due to severe infection. Every specialist has his own points of view on the factors that lead to sepsis, but all of them should keep in mind the possibility that a patient develops sepsis. To see the different points of view, we reviewed the literature on patients with immunodeficiency-related sepsis treated by clinicians of different specialities to find factors leading to such complications. Background: Intensive care unit (ICU) patients age 90 years or older represent a growing subgroup and place a huge financial burden on health care resources despite the benefit being unclear. This leads to ethical problems. The present investigation assessed the differences in outcome between nonagenarian and octogenarian ICU patients. Methods: We included 7900 acutely admitted older critically ill patients from two large, multinational studies. The primary outcome was 30-day-mortality, and the secondary outcome was ICU-mortality. Baseline characteristics consisted of frailty assessed by the Clinical Frailty Scale (CFS), ICU-management, and outcomes were compared between octogenarian (80-89.9 years) and nonagenarian (>= 90 years) patients. We used multilevel logistic regression to evaluate differences between octogenarians and nonagenarians. Results: The nonagenarians were 10% of the entire cohort. They experienced a higher percentage of frailty (58% vs 42%; p < 0.001), but lower SOFA scores at admission (6 +/- 5 vs. 7 +/- 6; p < 0.001). ICU-management strategies were different. Octogenarians required higher rates of organ support and nonagenarians received higher rates of life-sustaining treatment limitations (40% vs. 33%; p < 0.001). ICU mortality was comparable (27% vs. 27%; p = 0.973) but a higher 30-day-mortality (45% vs. 40%; p = 0.029) was seen in the nonagenarians. After multivariable adjustment nonagenarians had no significantly increased risk for 30-day-mortality (aOR 1.25 (95% CI 0.90-1.74; p = 0.19)). Conclusion: After adjustment for confounders, nonagenarians demonstrated no higher 30-day mortality than octogenarian patients. In this study, being age 90 years or more is no particular risk factor for an adverse outcome. This should be considered- together with illness severity and pre-existing functional capacity - to effectively guide triage decisions.

Postoperative delirium is one of the types of cognitive impairment. It occurs in elderly patients, usually after the age of 65. It is the cause of longer convalescence, worse treatment effects and prognosis, and sometimes premature death. Delirium usually occurs in the hypoactive form, which is usually undiagnosed and left untreated. Tools are available to effectively diagnose delirium by people who care for patients, and recent years bring new pharmacological therapies that are effective in inhibiting the symptoms of the developing disease. Background: Human cognitive functioning can be assessed using different methods of testing. Age, level of education, and gender may influence the results of cognitive tests., Material and Methods: The well-known Trail Making Test (TMT), which is often used to measure the frontal lobe function, and the experimental test of Interval Timing (IT) were compared. The methods used in IT included reproduction of auditory and visual stimuli, with the subsequent production of the time intervals of 1-, 2-, 5-, and 7-seconds durations with no pattern. Subjects included 64 healthy adult volunteers aged 18-63 (33 women, 31 men). Comparisons were made based on age, education, and gender., Results: TMT was performed quickly and was influenced by age, education, and gender. All reproduced visual and produced intervals were shortened and the reproduction of auditory stimuli was more complex. Age, education, and gender have more pronounced impact on the cognitive test than on the interval timing test. The reproduction of the short auditory stimuli was more accurate in comparison to other modalities used in the IT test., Conclusions: The interval timing, when compared to the TMT, offers an interesting possibility of testing. Further studies are necessary to confirm the initial observation.

Introduction. The aim of this study was to evaluate two measures in a cognitive examination: psychomotor function and the perception of time (PT) in patients after intravenous anesthesia for endoscopic procedures. Material and Methods. We tested 23 anesthetized patients (Anesthesia Group, AG) and 17 not anesthetized patients (Control Group, CG). The Dufour Cross-Shaped Apparatus (DA) was used to assess quick reactions. Perception of time (PT) was measured for 1-, 2-, 5-, and 7-second intervals. The tests were performed before the anesthesia was administered and 1.5, 3, and 6 hours after the procedure was completed. Results. The intervals that were generated and the reproduced visual stimuli were shorter than the patterns. The reproduced 1- and 2-second auditory stimuli were longer than the patterns. The remaining reproduced auditory impulses were shorter than the patterns. Conclusions. In anesthetized patients, quick psychomotor reactions and the ability to time intervals are preserved 1.5 h and later after intravenous anesthesia for endoscopy.

- Abstract
Background: Surgery and anaesthesia may account for postoperative complications such as cognitive impairment, depression, and delay of convalescence. The purpose of the study was to assess the influence of different risk factors on cognitive performance and emotional state., Methods: This study included patients received general anaesthesia for various abdominal, urogenital and vascular surgery. All patients were induced with thiopentone and fentanyl. Succinylcholine was used to facilitate endotracheal intubation. Anaesthesia was maintained with sevoflurane, fentanyl and rocuronium. We assessed the effect of duration of anaesthesia, arterial blood oxygenation, body temperature, pain and neoplastic disease on cognitive function, evaluated using the Mini-Mental State Examination (MMSE) questionnaire., Results: Forty-two women and thirty-three men, aged 65-83 years (mean 71.4) were enrolled in the study. Fourteen women and eleven men, aged 67-84 years (mean 74.2) and hospitalized for nonsurgical disease, served as controls. We found a positive correlation between the MMSE test and impaired SaO2 on the first day after surgery, and a negative correlation with the VAS pain score on the 1st and 5th days. The presence of neoplastic disease, body temperature and duration of anaesthesia had no influence on cognitive function., Discussion: The relationship between low arterial oxygenation and cognitive impairment has been described previously, with the deterioration usually being observed in the immediate postoperative period. We could not find any relationship between severity of pain and cognitive function. Perhaps this was because patients did not properly understand the VAS score and could not rate severity o pain properly.
