Abstract

Background Pathogenic variants in SPTAN1 have been linked to a remarkably broad phenotypical spectrum. Clinical presentations include epileptic syndromes, intellectual disability, and hereditary motor neuropathy. Objectives We investigated the role of SPTAN1 variants in rare neurological disorders such as ataxia and spastic paraplegia. Methods We screened 10,000 NGS datasets across two international consortia and one local database, indicative of the level of international collaboration currently required to identify genes causative for rare disease. We performed in silico modeling of the identified SPTAN1 variants. Results We describe 22 patients from 14 families with five novel SPTAN1 variants. Of six patients with cerebellar ataxia, four carry a de novo SPTAN1 variant and two show a sporadic inheritance. In this group, one variant (p.Lys2083del) is recurrent in four patients. Two patients have novel de novo missense mutations (p.Arg1098Cys, p.Arg1624Cys) associated with cerebellar ataxia, in one patient accompanied by intellectual disability and epilepsy. We furthermore report a recurrent missense mutation (p.Arg19Trp) in 15 patients with spastic paraplegia from seven families with a dominant inheritance pattern in four and a de novo origin in one case. One further patient carrying a de novo missense mutation (p.Gln2205Pro) has a complex spastic ataxic phenotype. Through protein modeling we show that mutated amino acids are located at crucial interlinking positions, interconnecting the three-helix bundle of a spectrin repeat. Conclusions We show that SPTAN1 is a relevant candidate gene for ataxia and spastic paraplegia. We suggest that for the mutations identified in this study, disruption of the interlinking of spectrin helices could be a key feature of the pathomechanism. © 2022 International Parkinson and Movement Disorder Society

Abstract

Type I interferons (IFNs) mediate antiviral activity, modulate immune responses and have anti-tumor activity. They impart their activities by binding to the IFN-alpha receptor on the cell surface and inducing the expression of IFN-stimulated genes (ISGs). The best-studied type I IFNs, IFN-alpha and -beta, are only expressed at very low levels in the absence of appropriate stimuli but can be strongly induced in most cell types upon activation of pattern-recognition receptors (PRRs). In contrast, IFN-kappa, another type I IFN, is expressed in a constitutive and cell type-specific manner in keratinocytes and is only weakly induced by activation of PRRs. IFN-kappa possesses activity against high risk human papillomaviruses (HR-HPV) which infect keratinocytes and IFN-kappa expression is reduced by HR-HPVs. HR-HPV represent a major health issue as they are one of the most frequently sexually transmitted infectious agents and cause virtually all cervical carcinomas, the world’s third most common cancer in women, which leads to more than 300,000 deaths per year. In the present work, regulatory mechanisms underlying the unusual pattern of IFN-kappa expression have been uncovered, using both normal and HPV16-positive human keratinocytes. It could be shown that IFN-kappa expression is mediated by the transcription factors AP-2alpha and p63, which are critically involved in keratinocyte biology and might explain the cell type-specific expression of this IFN. In addition, two signaling pathways which fine-tune IFN-kappa expression have been identified: TGF-beta1 signaling increases whereas ERK1/2 activity decreases IFN-kappa and ISG expression, both in normal and HPV16-positive keratinocytes, suggesting that IFN-kappa expression can be manipulated in pathological settings. Furthermore, increased IFN-kappa and ISG expression in HPV16-positive keratinocytes was accompanied by a decrease in viral transcription.

Abstract

Type I IFNs have antiviral and immune-modulating activities. IFN-a/-b have very low basal expression levels but are strongly induced upon activation of pattern recognition receptors. In contrast, IFN-k is constitutively expressed in uninfected keratinocytes and responds only weakly to pattern recognition receptor activation. IFN-k expression has been implicated in the pathogenesis of inflammatory skin diseases and in limiting human papillomavirus replication in human keratinocytes. We have identified an enhancer ~5 kb upstream of the IFNK gene driving its expression in keratinocytes. The enhancer consists of binding sites for the transcription factors jun-B, SMAD3/4, AP-2a/g, and p63, of which the latter two are key regulators of keratinocyte biology. The jun-B and SMAD3/4 elements confer activation by the TGF-b pathway. Furthermore, inhibition of ERK1/2 kinases activates IFN-k expression. Our study provides a framework for the cell type-specific, constitutive expression of IFN-k and its modulation by signal transduction pathways in human keratinocytes. [ABSTRACT FROM AUTHOR]

Abstract

Genetic variation in glutamatergic signalling pathways is believed to play a substantial role in the aetiology of schizophrenia. The N-methyl-D-aspartate receptor subunit gene GRIN1has been proposed as a candidate gene for schizophrenia. We tested for a potential association between schizophrenia and four single nucleotide polymorphisms (rs4880213, rs11146020, rs6293, and rs10747050) and one microsatellite marker at GRIN1in a German sample of 354 patients and 323 controls. We found significant associations in single-marker and haplotype-based analyses (P<0.05). Significance was more pronounced (P<0.01) in the subset of patients with a lifetime history of major depression, a subgroup of schizophrenia described previously as a promising phenotypic subtype in genetic studies of schizophrenia. Although significances did not withstand correction for multiple testing, the results of our exploratory analysis warrant further studies on GRIN1and schizophrenia.

Abstract

Seroconversion after COVID-19 vaccination is impaired in kidney transplant recipients. Emerging variants of concern such as the B.1.617.2 (delta) and the B.1.1.529 (omicron) variants pose an increasing threat to these patients. In this observational cohort study, we measured anti-S1 IgG, surrogate neutralizing, and anti-receptor-binding domain antibodies three weeks after a third mRNA vaccine dose in 49 kidney transplant recipients and compared results to 25 age-matched healthy controls. In addition, vaccine-induced neutralization of SARS-CoV-2 wild-type, the B.1.617.2 (delta), and the B.1.1.529 (omicron) variants was assessed using a live-virus assay. After a third vaccine dose, anti-S1 IgG, surrogate neutralizing, and anti-receptor-binding domain antibodies were significantly lower in kidney transplant recipients compared to healthy controls. Only 29/49 (59%) sera of kidney transplant recipients contained neutralizing antibodies against the SARS-CoV-2 wild-type or the B.1.617.2 (delta) variant and neutralization titers were significantly reduced compared to healthy controls (p < 0.001). Vaccine-induced cross-neutralization of the B.1.1.529 (omicron) variants was detectable in 15/35 (43%) kidney transplant recipients with seropositivity for anti-S1 IgG, surrogate neutralizing, and/or anti-RBD antibodies.
