This article discusses a case of tacrolimus-associated phototoxicity in a patient with vitiligo. The patient, an 18-year-old male, developed erythematous plaques with bullae and serosanguinous drainage on his upper extremities, face, and neck after applying tacrolimus ointment to his depigmented areas and being exposed to sunlight. The affected areas corresponded to the distribution of vitiligo patches where the tacrolimus was applied. The patient was managed by unroofing the bullae and discontinuing tacrolimus, which resulted in resolution of symptoms. Although tacrolimus-induced phototoxicity has not been previously reported, this case suggests that it may be a rare cause of phototoxic reactions.
