A 62-year-old man with myasthenia gravis (MG) was admitted with dyspnea, bulbar symptoms (dysphagia and dysarthria), and subsequent muscular weakness of the limbs. Results for anti-acetylcholine receptor antibody and muscle-specific kinase antibody were negative. We introduced periodic plasma exchange as a new therapeutic strategy. Once double-filtration plasmapheresis (DFPP) was adopted, effects became notable, but the response shortened over time. After stepwise increases in PSL up to 60 mg/day with insulin, the interval between DFPP was prolonged from 1 week to 2 weeks. DFPP was significantly beneficial in this case. Combining several effective methods can optimize therapeutic effects against refractory MG.

A 23-year-old man was admitted to our hospital due to loss of consciousness and a generalized convulsive seizure. He was diagnosed as having primary epilepsy and treated with antiepileptic drugs. Emergency CT scan of the head showed no abnormality. However, MRI scan of the head several days after admission revealed fresh infarctions caused by occlusion of the basilar artery, i.e., "top of the basilar" syndrome. This case indicates the need for precise differential diagnosis of convulsive seizure in an emergency situation. It should also be borne in mind that basilar occlusion with 'onset seizure' can occur even in young adults who have no risk factors for stroke.

Pain-related somatosensory evoked potentials (pain SEPs) following CO2 laser stimulation as well as conventional electrically stimulated SEPs (electric SEPs) were examined in 10 patients with peripheral neuropathies in whom the histopathological examination of the sural nerve was done. Results of pain SEPs showed a positive relationship with clinical impairment of pain sensation and densities of small myelinated fibers of the sural nerve. In contrast, results of electric SEPs showed a positive relationship with clinical impairment of deep and tactile sensations and with densities of large myelinated fibers of the sural nerve. Therefore, pain SEPs are considered to be generated by ascending signals mediated through nociceptive receptors and A delta fibers. The pain SEP is only one noninvasive and objective method currently available to investigate a physiological condition of the sensory pathway responsible for pain sense, and is especially useful when combined with the conventional electric SEPs.
