A 42-year-old male presented with a 4-week history of a mass in the right inferior palpebral conjunctiva close to the punctum. An excisional biopsy of the lesion and histopathological examination revealed that the mass was composed of Schwann cells with thin conical nuclei, fine chromatin, and unnoticeable nucleoli. Immunohistochemically, the spindle cells were diffusely and strongly positive for S100 protein. Neurofilament immunostaining was also positive, which highlighted axons. In light of these findings, the tumor was diagnosed as solitary circumscribed neuroma. A comprehensive evaluation for multiple endocrine neoplasia type 2b was performed. However, no multiple endocrine neoplasia type 2b stigmata and no family history were detected. The diagnosis was therefore finalized as solitary circumscribed neuroma, which is considered as a rare condition. The differential diagnosis is based on the histopathological examination and immunohistochemical evaluation. As the tumor can be related with multiple endocrine neoplasia type 2b, it is essential to systematically investigate for multiple endocrine neoplasia type 2b in such cases. RESUMO Um homem de 42 anos apresentou uma massa na conjuntiva palpebral inferior direita, próxima ao punctum, com evolução de 4 semanas. Uma biópsia excisional da lesão e o subsequente exame anatomopatológico revelaram que a massa era composta de células de Schwann com núcleos cônicos, cromatina fina e nucléolos não visíveis. Ao exame imuno-histoquímico, as células fusiformes mostraram-se difusa e fortemente positivas para a proteína S100. A imunocoloração também foi positiva para neurofilamentos e evidenciou os axônios. Considerando esses achados, o tumor foi diagnosticado como um neuroma circunscrito solitário. Procedeu-se uma investigação completa para neoplasia endócrina múltipla tipo 2b, entretanto, estigmas característicos e história familiar não foram detectados. Assim, o diagnóstico foi firmado como neuroma circunscrito solitário, condição rara cujo diagnóstico diferencial baseia-se no exame anatomopatológico e na avaliação imuno-histoquímica. Já que esse tumor pode estar relacionado à neoplasia endócrina múltipla tipo 2b, torna-se essencial, nesses casos, a investigação da neoplasia de forma sistemática.

AMAÇ: Yeni tanı almış ve tedavi uygulanmamış neovasküler tip yaşa bağlı makula dejenerasyonu (nvYBMD) olgularında intravitreal (İV) aflibercept (A) tedavisi ile elde edilen anatomik sonuçları değerlendirmek ve anatomik cevabı etkileyen başlangıç lezyon özelliklerini araştırmak. GEREÇ-YÖNTEM: Bu prospektif, kesitsel klinik araştırmaya, aktif nvYBMD tanısı konulan daha önce hiç tedavi uygulanmamış ardışık 166 göz dahil edilmiştir. Gözlerin tamamına tam oftalmolojik muayene, spektral-domain optik koherens tomografi (SD-OKT) ve floressein anjiyografi (FA) tetkikleri yapılmıştır. Boya temin edilebilen hastalara ek olarak İSYA tetkiki yapılmış ve başlangıç lezyon özellikleri tanımlanmıştır. Aktif nvYBMD'li gözlere tedavi et ve uzat rejimine göre İV A tedavisi uygulanmıştır. Her enjeksiyondan 4-6 hafta sonra kontrol muayeneleri yapılmış ve SD-OKT tetkikleri tekrarlanmıştır. Çalışmaya dahil edilen 166 gözün tümünde tedavi ile elde edilen anatomik cevaplar değerlendirilmiş olup ilaveten 3. ve 6. İV A enjeksiyonundan 4-6 hafta sonraki kontrol muayenesinde saptanan anatomik cevaplar kesitsel olarak incelenmiştir. İV A tedavi sonuçlarına etki eden başlangıç lezyon özellikleri de ayrıca değerlendirilmiştir. BULGULAR: Çalışmaya dahil edilen 150 hastanın 82'si (%54.7) erkek, 68'i (%45.3) kadın olup; olguların ortama yaşı 71.7±9.4 (51-90) yıldır. İV A tedavisi öncesi 166 gözün ortalama EİDGK'sı 0.88±0.59 (0-2.1) LogMAR'dır, ortalama 5.31±3.36 (1-17) kez İV A enjeksiyonu yapılmış olup ortalama takip süresi 10.74± 6.27 (2-30) aydır. En az 3 İV A enjeksiyonu yapılan 139 gözde, 3. tedaviden 4-6 hafta sonraki kesitte gözlerin 76'sı (%54.6) 'Tam Cevaplı' olarak saptanırken; en az 6 doz İV A yapılan 62 gözde ise 6. tedaviden 4-6 hafta sonraki kesitte gözlerin 36'sı (%58) 'Tam Cevaplı' olarak izlenmiştir. Tüm olguların (166 göz) en son İV A enjeksiyonundan 4-6 hafta sonra yapılan kontrol muayenesine ait kesitte ise 116 (%69.9) göz 'Tam Cevaplı' olarak saptanmıştır. EİDGK, İV A tedavisi ile istatistiksel olarak anlamlı artış gösterirken; PED oranı, başlangıca göre istatiksel olarak anlamlı derecede azalmıştır. Tanı sırasında saptanan FA'daki nv patternleri, SD-OKT'ye göre nv tipleri, PED varlığı, seröz PED olması, seröz ve FV PED'in birlikte izlenmesi veya PKV varlığı ile İV A tedavisi ile alınan anatomik cevap arasında istatistiksel olarak anlamlı ilişki bulunmamıştır. Tanı sırasında FV PED olan olgularda diğer PED tipleri bulunan olgulara göre 'Tam Cevaplı' olma ihtimali yaklaşık 5 kat daha fazla izlenmiştir. FV PED varlığı başarıyı etkileyen bağımsız bir faktör olarak ortaya konmuştur. SONUÇ: Çalışmamızda yeni tanı almış ve tedavi uygulanmamış nvYBMD'li gözlerde İV A tedavisi ile yüksek oranda anatomik fayda sağlandığı tespit edilmiştir. Ek olarak İV A tedavisi ile anlamlı görme artışı da sağlanabilmiştir. Başlangıç lezyon özelliklerinin tedavi ile elde edilen anatomik cevaplar üzerindeki etkisi ile ilgili önemli sonuçlara varılmıştır. Günümüzde nvYBMD tedavisinde anti-VEGF tedaviler standart tedavi yöntemi olarak kabul edilmekle birlikte, tedavi cevabını öngörmede rehberlik edecek bulguların saptanması klinik uygulamalarda uygun tedavi stratejilerinin belirlenmesi ve tedaviden beklentilerin rasyonelize edilmesi açısından oldukça önemlidir. OBJECTIVE: To evaluate the anatomic findings obtained from intravitreal (IV) aflibercept (A) treatment in cases of newly diagnosed and untreated neovascular type age-related macular degeneration (nvAMD) and to investigate initial lesion properties affecting anatomic response. MATERIALS-METHODS: Consecutive 166 untreated eyes yet diagnosed with active nvAMD were included in this cross-sectional clinical trial. All eyes were subjected to complete ophthalmologic examination, spectral-domain optical coherence tomography (SD-OCT) and fluorescein angiography (FA). Indocyanine green angiography was performed in addition for the patients who obtained the stain and initial lesion properties were identified. IV A treatment was performed on the eyes diagnosed with active nvAMD, according to treat and extend regimen. Control examinations were performed after 4-6 weeks later than every injection and SD-OCT imaging was repeated. Anatomic responses obtained from treatment of 166 eyes included in the study were evaluated in addition to cross sectional investigation of anatomic responses determined in the control examination on 4-6 weeks after 3rd and 6th IV A injection. Initial lesion properties affecting IV A treatment outcome were also evaluated. RESULTS: 82 of the 150 patients (54.7%) included in the study were male, 68 of them (45.3%) were female and average of their ages was 71.7±9.4 (51-90). Average best corrected visual acuity (BCVA) of 166 eyes before IV A treatment was 0.88±0.59 (0-2.1) LogMAR, IV A injection was performed 5.31±3.36 (1-17) times and average length of follow-up was 10.74± 6.27 (2-30) months. In 139 eyes subjected to IV A injection at least thrice, 76 eyes (54.6%) were determined as 'Fully Responsive' in cross-section after 4-6 weeks followed by 3rd treatment, whereas in 62 eyes subjected to IV A injection at least six times, 36 eyes (58%) were observed as 'Fully Responsive' in cross-section after 4-6 weeks followed by 6th treatment. 116 eyes (69.9%) of all cases (166 eyes) were identified as 'Fully Responsive' in the cross-section of control examination performed after 4-6 weeks followed by the last IV A injection. BCVA showed statistically significant increase with IV A treatment; whereas pigment epithelial detachment (PED) ratio showed statistically significant decrease compared to baseline. There was no statistically significant correlation between neovascularization patterns in FA, neovascularization types according to SD-OCT, PED presence, presence of polypoidal choroidal vasculopathy determined during diagnosis and anatomic response obtained from IV A treatment. The possibility of being 'Fully Responsive' was five times higher in FV PED cases during diagnosis compared to cases of other PED types. Presence of FV PED was introduced as an individual factor affecting the success. CONCLUSION: In our study it was determined that newly diagnosed, untreated nvAMD eyes benefit from IV A treatment at high level. In addition, vision was significantly improved. Important conclusions were drawn about the effects of initial lesion properties on anatomic responses obtained by treatment. Although anti-VEGF therapies are accepted as standard method in nARMD treatment recently, identification of the findings to guide the prediction of treatment outcome is rather important in terms of determining the appropriate treatment strategies in clinical practice and rationalizing the treatment expectations. 90
