Abstract

Background Headaches are frequent neurological disorders that are yet to be unveiled and treated comprehensively worldwide. Bearing in mind that the distribution of headache subtypes in neurology clinics (NC) is essential for planning appropriate diagnostic and therapeutic approaches, the primary goals of this multi-centric study are to carry out inter-regional comparisons by using current diagnostic criteria with evaluations of neurologists to delineate headache burden. Methods A cross-sectional study between April 1 and May 16, 2022 was conducted with the participation of 13 countries from the Middle East, Asia, and Africa. Patients were included in the study on a specific day each week during five consecutive weeks. All volunteers over the age of 18 and whose primary cause for admission was headache were examined. The patients admitted to NC or referred from emergency services/other services were evaluated by neurologists by means of the International Classification of Headache Disorders (ICHD-3) criteria. Results Among the 13,794 patients encountered in NC, headache was the primary complaint in 30.04%. The headache patients’ mean age was 42.85 ± 14.89 (18–95 years), and 74.3% were female. According to the ICHD-3 criteria, 86.7% of the main group had primary headache disorders, 33.5% had secondary headaches, 4% had painful cranial neuropathies along with other facial and headaches, and 5.2% had headaches included in the appendix part showing some overlapping conditions. While the most common primary headache was migraine without aura (36.8%), the most common secondary headache was medication-overuse headache (MOH) (9.8%). Headaches attributed to COVID-19, its secondary complications, or vaccines continue to occur at rates of 1.2%-3.5% in current neurology practice. Pain severity was significantly lower in Ivory Coast and Sudan than in Türkiye, Turkish Republic of Northern Cyprus, Iran, Egypt, Senegal, Tatarstan, and Azerbaijan (p Conclusions The study showed that migraine is still the most common motive for admissions to NC in different regions. Furthermore, MOH, an avoidable disorder, is the most common secondary headache type and appears to be a significant problem in all regions. Remarkably, pain perception differs between regions, and pain intensity is lower in Africa than in other regions.

The first cerebral computed tomography scan was normal. Lumbar puncture showed hyperproteinorachy at 2 g/L with six lymphocytic cells. The headache became very intense. At admission, clinical examination was normal. Ophthalmological examination did not show any abnormalities. Encephalic magnetic resonance imaging (MRI) showed bilateral subdural hematoma with tonsillar descent simulating Chiari type I malformation. After surgical drainage and symptomatic treatment, the patient was discharged with no recurrence.Conclusion: Spontaneous intracranial hypotension is associated with simple clinical presentation, orthostatic headache, and characteristic MRI findings. Abstract

Neuromyelitis optica spectrum disorder (NMOSD) is an uncommon inflammatory disease of the central nervous system, manifesting clinically as optic neuritis, myelitis, and certain brain and brainstem syndromes. Cases clinically diagnosed as NMOSD may include aquaporin 4 (AQP4)-antibody-seropositive autoimmune astrocytopathic disease, myelin oligodendrocyte glycoprotein (MOG)-antibody-seropositive inflammatory demyelinating disease, and double-seronegative disease. AQP4-antibody disease has a high female-to-male ratio (up to 9:1), and its mean age at onset of ~40 years is later than that seen in multiple sclerosis. For MOG-antibody disease, its gender ratio is closer to 1:1, and it is more common in children than in adults. Its clinical phenotypes differ but overlap with those of AQP4-antibody disease and include acute disseminated encephalomyelitis, brainstem and cerebral cortical encephalitis, as well as optic neuritis and myelitis. Double-seronegative disease requires further research and clarification. Population-based studies over the past two decades report the prevalence and incidence of NMOSD in different populations worldwide. Abstract

Neuromyelitis optica (NMO) (and NMO spectrum disorder) is an autoimmune inflammatory disease of the CNS primarily affecting spinal cord and optic nerves. Reliable and sensitive biomarkers for onset, relapse, and progression in NMO are urgently needed because of the heterogeneous clinical presentation, severity of neurologic disability following relapses, and variability of therapeutic response. Detecting aquaporin-4 (AQP4) antibodies (AQP4-IgG or NMO-IgG) in serum supports the diagnosis of seropositive NMO. However, whether AQP4-IgG levels correlate with disease activity, severity, response to therapy, or long-term outcomes is unclear. Moreover, biomarkers for patients with seronegative NMO have yet to be defined and validated. Collaborative international studies hold great promise for establishing and validating biomarkers that are useful in therapeutic trials and clinical management. In this review, we discuss known and potential biomarkers for NMO.

47. Meningomyelite d’origine syphilitique: Etude de cinq cas

Abstract

IntroductionLes manifestations neurologiques de la syphilis sont polymorphes et sont actuellement en recrudescence. Nous rapportons cinq cas de méningomyélites en rapport avec une neurosyphilis.ObservationsIl s’agit de cinq hommes âgés respectivement de 38, 40, 41, 42 et 44 ans. La notion de chancre était présente dans 1 cas. Le tableau fut celui d’un syndrome de compression médullaire dans 3 cas, syndrome de sclérose combinée de la moelle dans 2 cas. Le signe d’Argyll Robertson retrouvé dans 1 cas. L’imagerie par résonance magnétique réalisée chez tous les patients confirma le diagnostic de myélopathie non compressive. L’étude du liquide céphalorachidien (LCR) montra une méningite lymphocytaire avec des sérologies syphilitiques positives. L’utilisation de la pénicilline G améliora l’état clinique dans 4 cas.ConclusionLa recherche d’une syphilis doit être systématique devant toute myélopathie non compressive.

Abstract

Introduction : C’est une affection de l’adulte jeune. Elle débute le plus souvent entre 20 et 40 ans. Les formes débutant après 50 ans sont rares constituant 1,3 à 6 p. 100 des scléroses en plaque selon les études. Observation : Deux patients dont un homme âgé de 68 ans et une femme âgée de 63 ans présentaient des épisodes neurologiques évoluant par poussées - rémissions. Le diagnostic de sclérose en plaque rémittente a été retenu selon les critères cliniques, radiologiques et biologiques. Des bolus de corticoïdes, administrés comme traitement de poussées ont permis l’amélioration des symptômes. De même, le traitement de fond chez un patient a permis de diminuer la fréquence et la sévérité des poussées. Discussion : Le diagnostic des formes tardives de sclérose en plaque est souvent difficile car une étiologie vasculaire est recherchée en priorité dans cette tranche d’âge. La principale caractéristique de ces patients est la forte prévalence des formes progressives d’emblée (plus d’un tiers) et, par conséquent, des symptômes essentiellement moteurs à type de paraparésie. La distribution oligoclonale du LCR semble aussi fréquente que pour les Sclérose en plaque à révélation plus précoce. Par ailleurs, les anomalies radiologiques sont moins spécifiques que chez l’adulte jeune. La progression rapide des scléroses en plaque de début tardif pourrait inciter à une prise en charge thérapeutique précoce chez ces patients tout en tenant compte de la toxicité de certains médicaments., Maroc Médical, Vol. 31, No 1 (2009)

Abstract

Background: Ischemic stroke is the leading cause of mortality worldwide and a major contributor to neurological disability and dementia. Terutroban is a specific TP receptor antagonist with antithrombotic, antivasoconstrictive, and antiatherosclerotic properties, which may be of interest for the secondary prevention of ischemic stroke. This article describes the rationale and design of the Prevention of cerebrovascular and cardiovascular Events of ischemic origin with teRutroban in patients with a history oF ischemic strOke or tRansient ischeMic Attack (PERFORM) Study, which aims to demonstrate the superiority of the efficacy of terutroban versus aspirin in secondary prevention of cerebrovascular and cardiovascular events. Methods and Results: The PERFORM Study is a multicenter, randomized, double-blind, parallel-group study being carried out in 802 centers in 46 countries. The study population includes patients aged ≥55 years, having suffered an ischemic stroke (≤3 months) or a transient ischemic attack (≤8 days). Participants are randomly allocated to terutroban (30 mg/day) or aspirin (100 mg/day). The primary efficacy endpoint is a composite of ischemic stroke (fatal or nonfatal), myocardial infarction (fatal or nonfatal), or other vascular death (excluding hemorrhagic death of any origin). Safety is being evaluated by assessing hemorrhagic events. Follow-up is expected to last for 2–4 years. Assuming a relative risk reduction of 13%, the expected number of primary events is 2,340. To obtain statistical power of 90%, this requires inclusion of at least 18,000 patients in this event-driven trial. The first patient was randomized in February 2006. Conclusions: The PERFORM Study will explore the benefits and safety of terutroban in secondary cardiovascular prevention after a cerebral ischemic event.
