Cardiovascular disease (CVD) is the number one cause of death globally. Cardiac rehabilitation (CR) is an essential component of long-term recovery following a cardiac event. Typical CR may not be optimal for patients presenting with sarcopenic obesity (SO) who present with reduced muscle mass and elevated adipose tissue, and may indicate greater cardiovascular disease risk. Resistance exercise and high-protein diets are known to increase muscle mass, while Mediterranean-style diets have been shown to reduce CVD risk. A high-protein Mediterranean-style diet combined with resistance exercise intervention is yet to be trialled in cardiac rehabilitation populations. As such, this thesis, provides rationale for the development of such a protocol using multiple research methods including literature review, systematic review and meta-analysis, Mendelian randomization and qualitative analysis. A wide variety of topics are discussed including: the importance of muscle in the development of chronic disease; the effects of protein and resistance exercise on muscle mass in older adults; the effects of lifetime vitamin D levels on muscle mass; the effect of lean mass on lipoprotein particle size; the impact of COVID-19 on CR practice; the protoocl development for a diet and exercise intervention in CR; and investigating the feasibility of said protocol. As such, this thesis provides a comprehensive rationale for and methodology to achieve a high-protein Mediterranean-style diet and RE intervention in a CR population.
