Abstract

Objectives: The goal of this study was to develop a risk score model for patients with Brugada syndrome (BrS). Background: Risk stratification in BrS is a significant challenge due to the low event rates and conflicting evidence. Methods: A multicenter international cohort of patients with BrS and no previous cardiac arrest was used to evaluate the role of 16 proposed clinical or electrocardiogram (ECG) markers in predicting ventricular arrhythmias (VAs)/sudden cardiac death (SCD) during follow-up. Predictive markers were incorporated into a risk score model, and this model was validated by using out-of-sample cross-validation. Results: A total of 1,110 patients with BrS from 16 centers in 8 countries were included (mean age 51.8 ± 13.6 years; 71.8% male). Median follow-up was 5.33 years; 114 patients had VA/SCD (10.3%) with an annual event rate of 1.5%. Of the 16 proposed risk factors, probable arrhythmia–related syncope (hazard ratio [HR]: 3.71; p < 0.001), spontaneous type 1 ECG (HR: 3.80; p < 0.001), early repolarization (HR: 3.42; p < 0.001), and a type 1 Brugada ECG pattern in peripheral leads (HR: 2.33; p < 0.001) were associated with a higher risk of VA/SCD. A risk score model incorporating these factors revealed a sensitivity of 71.2% (95% confidence interval: 61.5 to 84.6) and a specificity of 80.2% (95% confidence interval: 75.7 to 82.3) in predicting VA/SCD at 5 years. Calibration plots showed a mean prediction error of 1.2%. The model was effectively validated by using out-of-sample cross-validation according to country. Conclusions: This multicenter study identified 4 risk factors for VA/SCD in a primary prevention BrS population. A risk score model was generated to quantify risk of VA/SCD in BrS and inform implantable cardioverter-defibrillator prescription.

Abstract

Using a 35 Gc/sec spectrometer, E.P.R. linewidths have been measured in a number of synthetic ruby specimens grown by various means. In addition, measurements of both linewidth and relaxation time have been made on a number of specimens of calcium tungstate doped with neodymiura. Using a mathematical treatment of the second moment, the homogeneously broadened ruby line has been resolved into concentration-dependent and concentration-independent components. The observed constant nature of the former suggests that the effective concentration of paramagnetic centres is not indicative of the true chemical concentration. Analysis of the inhomogeneous ruby line results in broadening components representative of axial misorientation and strain, A moderate correlation is obtained between misorientation figures derived from x-ray and e.p.r. measurements, and a similar correlation exists between strain and spin-lattice relaxation times. There appear to be no significant differences between the various types of flame-fusion material available. Preliminary analysis suggests that the homogeneous line in calcium tungstate owes its width to processes similar to those occuring in ruby, but strain does not appear to be predominant in the inhomogeneous broadening. Relaxation times in this material exhibit concentration and orientation dependence and a temperature dependence has been fitted empirically in the Direct and Raman regions.

Abstract

Intact cores were collected from a variety of soils in the Adelaide Hills, South Australia, and tested for phosphorus retention and mobility (P in drainage) under various rainfall intensities (5, 25, and 50 mm/h). Phosphorus mobility was high in soils with significant macropore structure. However, all soils exhibited some degree of preferential flow of P, including the heavy-textured soils with high P adsorption that were not P saturated. A phosphorus adsorption index based only on the chemical properties of the soil did not accurately predict the mobility of P through soils with macroporosity. A phosphorus mobility index was developed encompassing both soil chemical and physical parameters. Results showed the sandy soils, and the loams over clays with high macroporosity that are located in the more elevated parts of the Adelaide hills, are most susceptible to P leaching. Management to reduce P loss to groundwater, streams, or surface water storages must aim to increase the residence time of P within soils and thereby allow mineral and organic fractions time to sorb P. Phosphorus loss through wet soils was significantly less than P loss through dry soils with high macroporosity. Application of P fertiliser to soils with high macroporosity may need to be delayed until later in the growing season than is currently practised. 46. Impact of a change in tillage and crop residue management practice on soil chemical and microbiological properties in a cereal-producing red duplex soil in NSW, Australia

Abstract

The effect of a change of tillage and crop residue management practice on the chemical and micro-biological properties of a cereal-producing red duplex soil was investigated by superimposing each of three management practices (CC: conventional cultivation, stubble burnt, crop conventionally sown; DD: direct-drilling, stubble retained, no cultivation, crop direct-drilled; SI: stubble incorporated with a single cultivation, crop conventionally sown), for a 3-year period on plots previously managed with each of the same three practices for 14 years. A change from DD to CC or SI practice resulted in a significant decline, in the top 0-5 cm of soil, in organic C, total N, electrical conductivity, NH4-N, NO3-N, soil moisture holding capacity, microbial biomass and CO2 respiration as well as a decline in the microbial quotient (the ratio of microbial biomass C to organic C; P <0.05). In contrast, a change from SI to DD or CC practice or a change from CC to DD or SI practice had only negligible impact on soil chemical properties (P >0.05). However, there was a significant increase in microbial biomass and the microbial quotient in the top 0-5 cm of soil following the change from CC to DD or SI practice and with the change from SI to DD practice (P <0.05). Analysis of ester-linked fatty acid methyl esters (EL-FAMEs) extracted from the 0- to 5-cm and 5- to 10-cm layers of the soils of the various treatments detected changes in the FAME profiles following a change in tillage practice. A change from DD practice to SI or CC practice was associated with a significant decline in the ratio of fungal to bacterial fatty acids in the 0- to 5-cm soil (P <0.05). The results show that a change in tillage practice, particularly the cultivation of a previously minimum-tilled (direct-drilled) soil, will result in significant changes in soil chemical and microbiological properties within a 3-year period. They also show that soil microbiological properties are sensitive indicators of a change
