The health risks associated with exposure to radon are variously described across different jurisdictions but the exposure-risk scenario may change as buildings are better sealed to make them more energy efficient as a climate change mitigation strategy. Radon which is ubiquitous in the air and often concentrated inside buildings may become more concentrated with better sealed buildings. Some countries such as the UK and the US currently have radon mitigation design standards and building codes which they use for the design and construction of buildings, whereas some other countries such as Australia and New Zealand presently take little or no cognisance of the potential problem of radon exposure in buildings. The increasing awareness of constructing well sealed and insulated buildings that are more energy-efficient is likely to result in increased radon concentrations in buildings. Radon is a known carcinogen that is a significant cause of lung cancer, and greater concentrations of radon in buildings are likely to result in increased levels of lung cancer. Better sealed energy-efficient buildings are undoubtedly required as a climate change mitigation strategy but there needs to be a greater awareness of the potential problem of increased radon concentrations so that buildings are also designed and constructed to address this. Land-use regression (LUR) modelling is widely used in many parts of the world, especially in Europe and North America. Only one published study has previously been carried out nationally in New Zealand for PM10; none for NO2. One previously identified limitation of LUR has been its limited effectiveness in small areas at fine spatial scales. In this study, LUR was used in two small areas of Auckland: Otahuhu and Mangere. Adjusted R² figures of 0.72 (Mangere) and 0.44 (Otahuhu) were comparable with other studies. The low R² found for Otahuhu may relate to the size of the monitoring area (3.5 km²), whereas the Mangere area was ten times larger (35 km²), while the extent of the modelled area was larger again: 10 km² at Otahuhu and 70 km² at Mangere. From these findings, we conclude there may be a lower limit on the size of study area used in LUR modelling, and that this area is somewhere in the range of 4-30 km². It is hypothesised that in smaller areas there is not enough variability in the local geographic features used to as accurately predict the monitored NO2 values. [ABSTRACT FROM AUTHOR]

Abstract Background There is mounting international evidence that exposure to green environments is associated with health benefits, including lower mortality rates. Consequently, it has been suggested that the uneven distribution of such environments may contribute to health inequalities. Possible causative mechanisms behind the green space and health relationship include the provision of physical activity opportunities, facilitation of social contact and the restorative effects of nature. In the New Zealand context we investigated whether there was a socioeconomic gradient in green space exposure and whether green space exposure was associated with cause-specific mortality (cardiovascular disease and lung cancer). We subsequently asked what is the mechanism(s) by which green space availability may influence mortality outcomes, by contrasting health associations for different types of green space. Methods This was an observational study on a population of 1,546,405 living in 1009 small urban areas in New Zealand. A neighbourhood-level classification was developed to distinguish between usable (i.e., visitable) and non-usable green space (i.e., visible but not visitable) in the urban areas. Negative binomial regression models were fitted to examine the association between quartiles of area-level green space availability and risk of mortality from cardiovascular disease (n = 9,484; 1996 - 2005) and from lung cancer (n = 2,603; 1996 - 2005), after control for age, sex, socio-economic deprivation, smoking, air pollution and population density. Results Deprived neighbourhoods were relatively disadvantaged in total green space availability (11% less total green space for a one standard deviation increase in NZDep2001 deprivation score, p < 0.001), but had marginally more usable green space (2% more for a one standard deviation increase in deprivation score, p = 0.002). No significant associations between usable or total green space and mortality were observed after adjustment for confounders. Conclusion Contrary to expectations we found no evidence that green space influenced cardiovascular disease mortality in New Zealand, suggesting that green space and health relationships may vary according to national, societal or environmental context. Hence we were unable to infer the mechanism in the relationship. Our inability to adjust for individual-level factors with a significant influence on cardiovascular disease and lung cancer mortality risk (e.g., diet and alcohol consumption) will have limited the ability of the analyses to detect green space effects, if present. Additionally, green space variation may have lesser relevance for health in New Zealand because green space is generally more abundant and there is less social and spatial variation in its availability than found in other contexts.

We aimed to examine the health effects of ambient particulate air pollution from wood burning on school-age students in Christchurch, New Zealand, and to explore the utility of urine and exhaled breath condensate biomarkers of exposure in this population. Methods A panel study of 93 male students (26 with asthma) living in the boarding house of a metropolitan school was undertaken in the winter of 2004. Indoor and outdoor pollution data was continuously monitored. Longitudinal assessment of lung function (FEV1 and peak flow) and symptoms were undertaken, with event studies of high pollution on biomarkers of exposure (urinary 1-hydroxypyrene) and effect (exhaled breath condensate (EBC) pH and hydrogen peroxide concentration). Results Peak levels of air pollution were associated with small but statistically significant effects on lung function in the asthmatic students, but not healthy students. No significant effect of pollution could be seen either on airway inflammation and oxidative stress either in healthy students or students with asthma. Minor increases in respiratory symptoms were associated with high pollution exposure. Urinary 1-hydroxypyrene levels were raised in association with pollution events by comparison with low pollution control days. Conclusion There is no significant effect of ambient wood-smoke particulate air pollution on lung function of healthy school-aged students, but a small effect on respiratory symptoms. Asthmatic students show small effects of peak pollution levels on lung function. Urinary 1-hydroxypyrene shows potential as a biomarker of exposure to wood smoke in this population; however measurement of EBC pH and hydrogen peroxide appears not to be useful for assessment of population health effects of air pollution. Numerous studies have noted a strong social gradient in many types of ill health. In particular, people in more deprived areas tend to be less healthy than those in more affluent communities, even once the demographic and socio-economic differences of the people in those areas have been taken into account. The social gradient is evident for many types of health outcomes, including diseases such as cancer. However, this positive relationship is not evident for rates of melanoma incidence and mortality, with rates of the disease tending to decrease with measures of disadvantage. In this study, we assess the relationship between the incidence of melanoma and deprivation in New Zealand, a country with particularly high rates of the disease. In the light of greater public awareness of the risk factors associated with melanoma, through public awareness campaigns such as 'Slip! Slap! Slop' and 'No Suntan is Safe', we analyse small-area data on standardised rates of melanoma for the period 1995-2000. We found that melanoma rates increase with social status, even once other confounding factors are controlled for, but that the relationship is very small. Furthermore, the relationship between melanoma incidence and deprivation is context-dependent. Possible explanations for the relationship between melanoma and deprivation are discussed, including more frequent exposure to intermittent sunshine among less disadvantaged groups and the underreporting of melanoma incidence in the New Zealand cancer registry among individuals in lower social groups.
