4. Dysphagia severity is associated with worse sleep-disordered breathing in infants with Down syndrome.

Abstract

STUDY OBJECTIVES: Hypotonia, commonly seen in infants with Down syndrome (I-DS), can contribute to masticatory and oropharyngeal muscle weakness, increasing the risk for dysphagia and sleep-disordered breathing. Data describing the occurrence of dysphagia and sleep-disordered breathing in I-DS are limited. This study aims to determine the frequency and severity of dysphagia and its relationship to polysomnogram parameters in I-DS. METHODS: We included I-DS who underwent polysomnography at a single academic center over a 6-year period. Data collected included sex, age, presence of dysphagia (low suspicion of dysphagia vs dysphagia vs feeding tube), and polysomnographic data. Dysphagia was determined by a video fluoroscopic swallow study in the presence of clinical suspicion. RESULTS: A total of 40 I-DS were identified (mean age 6.6 months ± 3; male 65%). There were 11, 13, and 16 I-DS with low suspicion of dysphagia, dysphagia, and feeding tube, respectively. Obstructive sleep apnea was more severe in I-DS in the feeding tube group when compared with the group with a low suspicion of dysphagia and (apnea-hypopnea index mean [standard error] = 49.3 [7.6] vs 19.2 [9.2] events/h; P = .016). Dysphagia severity was positively correlated with a higher obstructive apnea-hypopnea index (r = .43, P = .006). CONCLUSIONS: There is a high incidence of dysphagia and sleep-disordered breathing in I-DS. Dysphagia severity correlated with obstructive apnea-hypopnea index severity. Our results suggest that I-DS need early evaluation of both sleep-disordered breathing and dysphagia. CITATION: Cho Y, Kwon Y, DelRosso L, Sobremonte-King M. 8. Teachers' Review of Tasks as a Tool for Examining Secondary Teachers' Mathematical Knowledge for Teaching

Abstract

One important aspect of teaching is reviewing tasks in preparation for instruction. The goal of the multicase study of four secondary teachers was to examine the interplay between their mathematical knowledge for teaching [MKT] and what they attend to when reviewing a mathematics task. We engaged secondary mathematics teachers in a semi-structured, clinical interview focused on a nonroutine mathematical task involving exponential growth. The results suggest experienced teachers may not explicitly attend to learning opportunities in their review of a task, and their own mathematical work contributes to their anticipation of student work and thinking. This work highlights how researchers focused on MKT can use clinical interviews as a tool for extracting and describing a teacher's MKT. [For the complete proceedings, see ED629884.]

29. Dedifferentiation and Difference: People with Profound Intellectual and Multiple Disabilities and the National Disability Insurance Scheme (NDIS)

Abstract

"Dedifferentiation" represents the merging of the wider "intellectual disability" category with the broader one of "disability" generally; a product of the ascendance of the social model of disability. The recent re-emergence of the significance of impairment and embodiment in disability theory re-establishes the importance of situated experience and reinserts realities of difference in the dedifferentiation/difference debate. This paper highlights these issues by exploring the experiences of people with Profound Intellectual and Multiple Disabilities (PIMD) and their decision-making supporters in the context of the National Disability Insurance Scheme (NDIS) in Australia. Lived experiences accessing, planning for, and implementing NDIS supports illustrate some limitations of dedifferentiation in social services, highlighting the need for recognition of realities of embodied difference for people with PIMD. Here, our systems and theories are most uncomfortable, our ideals of inclusion are most challenged in practice, and some balance between realities of difference and ideals of dedifferentiation is most needed.

36. Living and Learning at the Intersections of Identity and Community: Facilitating Theatre of the Oppressed Workshops with Educators

Abstract

For the past year, we have been co-facilitating Theatre of the Oppressed (TO) workshops across Pittsburgh, tailored specifically toward local educators and educational activists. The overarching intentions of these workshops were twofold: (a) to cultivate educators' understanding of and response to how power and privilege operate in educational systems; and (b) to provide useful tools for educators to continue these conversations and dismantle systems of oppression in their places of practice. As a collective of facilitators and in our workshops, we attempt to create, even if briefly, a beloved community where participants are invited to bring their full, human selves and engage in nuanced reflection in the hopes of creating a more compassionate, just, and equitable society. Like our workshops, the purpose of this article is both practical and theoretical: (a) to provide tangible tools to those readers interested in facilitating TO workshops for educators; and (b) to contribute to our understanding of how TO can serve to cultivate educators' analysis of oppression.

Abstract

This thesis introduces Pulse Project (2011-2016), a creative research series exploring an ecology of complex relations between art, humanities, medicine, and technology. In this series, I embody transdisciplinary research practice through my performing as an instrument or medium between others and myself, and between cultural traditions for understanding and mediating the body. Drawing upon my expertise as a clinical acupuncturist with training in biomedicine, I use Chinese medicine and music theories to inform my algorithmic soundscape compositions. These soundscapes are not sonifications of modern Euro-American principles of circulation or embodiment but offer another perspective to conceive of/listen to the interior spaces of the body-in-being. In this transdisciplinary study, practice-based research is used to give form to the transdisciplinary research process by producing performances, personalised sound works, graphic notations and interpersonal correspondences that interweave artistic, medical and technological ways knowing together into new material configurations. This project also questions the distinctions between premodern and modern, East and West and self and other. To investigate these distinctions, Pulse Project interrogates the aesthetic and philosophical axioms underpinning modern and contemporary art, medicine and technology through using premodern Chinese medicine and music theories in tandem with cutting edge technology. Thus, this research travels laterally between cultures, practices and epochs and calls for a radical change in conceiving of the body in ‘oriental’ and ‘occidental’ terms in order to both reduce ethnocentrism and also to travel beyond the tired bifurcations between mind and body, self and others and Western and othered cultures. In combining art, technology and diverse medicines together with contemporary digital culture, this project opens transverse lines of inquiry that create new interconnections between disciplinary practices, whilst at the same time, it generates new forms of cultural engagement through performance and sound works.

Abstract

IntroductionLichen sclerosus (LS) is a chronic inflammatory dermatosis, usually affecting the anogenital skin in women. This chronic inflammation can cause scarring of genitalia including narrowing of the introitus and phimosis of the clitoris. These architectural changes can lead to recurrent tearing during intercourse (vulvar granuloma fissuratum) and decreased clitoral sensation. Surgical correction of vulvar granuloma fissuratum (VGF) and clitoral phimosis can be performed, but there is little data on the patient satisfaction and complications following these surgical procedures.AimTo evaluate patient experience and outcomes in women undergoing surgical correction of scarring caused by anogenital LS.MethodsA retrospective chart review of patients at a vulvar disorders clinic was performed to identify women who had undergone surgical correction of clitoral phimosis or lysis of vulvar adhesions for VGF due to LS. Twenty-eight women were contacted via telephone between 4 and 130 months postoperatively. An eight-question survey was used to determine patient experience and outcomes.Main outcome measuresAll participants completed an eight-question survey to evaluate patient satisfaction with the surgery, effects on clitoral sensation, orgasm and pain with intercourse, postoperative symptoms or complications, and the presence of recurrent vulvar scarring.ResultsParticipants reported that they were either very satisfied (44%) or satisfied (40%) with the procedure. Of the women who experienced decreased clitoral sensation prior to surgery, 75% endorsed increased clitoral sensitivity postoperatively. Of the women who had dyspareunia prior to surgery, the majority of women reported having pain-free sex (33%) or improved but not completely pain-free sex (58%) after surgery. There were no complications or symptoms made worse by the surgical procedures.ConclusionsThis study shows high patient satisfaction and low complication risk associated with surgical correction of clitoral phimosis and lysis of vulvar adhesions for VGF caused by LS. Patients reported improvement in clitoral sensation and ability to achieve orgasm, as well as decreased dyspareunia. Surgical correction of vulvar scarring is a viable option to restore vulvar anatomy and sexual function in appropriate candidates with anogenital LS. 50. Form over Substance: Learning Objectives in the Business Core

Abstract

While members of the business faculty community have been advocating active learning in the classroom, it appears that textbooks encourage learning from a passive perspective. A review of learning objectives from 16 textbooks used in Financial Accounting, Managerial Accounting, Finance, and Marketing demonstrates a focus on basically the same set of primary verbs at a low cognitive level. These low cognitive level verbs differ in substance from the expectations contained in the end-of-the-chapter materials. In a world of assessment, the authors are concerned that the textbook learning objectives seem to focus on the form of technical content and not the substance of student learning.
