The perinatal period is a vulnerable time for the development of psychopathology, particularly mood and anxiety disorders. In the study of maternal anxiety, important questions remain regarding the association between maternal anxiety symptoms and subsequent child outcomes. This study examined the association between depressive and anxiety symptoms, namely social anxiety, panic, and agoraphobia disorder symptoms during the perinatal period and maternal perception of child behavior, specifically different facets of development and temperament. Participants (N = 104) were recruited during pregnancy from a community sample. Participants completed clinician-administered and self-report measures of depressive and anxiety symptoms during the third trimester of pregnancy and at 16 months postpartum; child behavior and temperament outcomes were assessed at 16 months postpartum. Child development areas included gross and fine motor skills, language and problem-solving abilities, and personal/social skills. Child temperament domains included surgency, negative affectivity, and effortful control. Hierarchical multiple regression analyses demonstrated that elevated prenatal social anxiety symptoms significantly predicted more negative maternal report of child behavior across most measured domains. Elevated prenatal social anxiety and panic symptoms predicted more negative maternal report of child effortful control. Depressive and agoraphobia symptoms were not significant predictors of child outcomes. Elevated anxiety symptoms appear to have a distinct association with maternal report of child development and temperament. Considering the relative influence of anxiety symptoms, particularly social anxiety, on maternal report of child behavior and temperament can help to identify potential difficulties early on in mother–child interactions as well as inform interventions for women and their families.

Objective : To identify the most prominent sources of stress, examine correlates of stress, and assess support preferences to inform future interventions among women pregnant with twins who are on bed rest. Materials and Methods : A cross-sectional online survey was administered to pregnant and postpartum women who were recommended bed rest during their twin pregnancy. Total stress while on bedrest was assessed using an adapted version of the Antepartum Hospital Stressors Inventory (AHSI). Bivariate analyses were used to compare total stress scores across participant characteristics using linear regression, Spearman's correlation, and one-way ANOVA. Qualitative content regarding women's bed rest experiences and future intervention preferences was analyzed into themes. Findings : Women's (N=104, mean age 30.6 years ± 4.6) most prominent sources of stress while on bed rest included thinking about babies’ health, feeling dependent on others, and feeling uncertain about the outcome. Women who were recommended bed rest earlier in their pregnancy or were recommended both home and hospital bedrest had higher stress scores, while women with monochorionic/monoamniotic pregnancies had lower stress scores. Approximately 64% of women indicated interest in a bed rest support program, with 88% indicating a preference for online support and 70% wanting support from family. Key Conclusions : Women pregnant with twins who are recommended bed rest identify many sources of stress. Our findings suggest a need for stress-management interventions targeting this population.

Disaster exposure during pregnancy has received limited attention. This study examined the impact of the 2008 Iowa Floods on perinatal maternal depression and well-being, and the role of peritraumatic distress as a possible mechanism explaining this link. Perinatal women (N = 171) completed measures of depressive symptoms and general well-being at 5 timepoints from pregnancy to 30 months postpartum. Objectively assessed prenatal flood exposure was associated with greater depression (r = .15). Further, flood-related peritraumatic distress was uniquely associated with greater depression (r = .23), and was a key mechanism through which flood exposure led to depression. Prenatal flood exposure was also associated with general well-being (r = .18); however, a mechanism other than peritraumatic distress appears to have been responsible for the effect of flood exposure on well-being. We discuss the implications of these findings for informing etiological models and enhancing the efficacy of interventions for maternal psychopathology.

The current study examined the moderating role of social support in the association between prenatal maternal stress (PNMS) and childhood body mass index (BMI) in the context of the Iowa floods of 2008. In addition, the mediating role of offspring birthweight was examined in the association between PNMS and childhood BMI. We recruited women from eastern Iowa who were pregnant in 2008 when disastrous floods occurred. Self-report measures of PNMS and cognitive appraisal of the flood's consequences were obtained shortly after the disaster. Social support was assessed during pregnancy. Offspring anthropometric measures were collected at birth and 30 months. Moderated mediation results indicated that greater PNMS predicted greater BMI at age 30 months through effects on higher birthweight as a mediator, but only for participants with low social support. High social support (satisfaction or number) buffered the effect of PNMS or a negative appraisal of the flood on birthweight. The combination of high PNMS or a negative appraisal of the flood's consequences and low social support resulted in higher offspring birthweight, which predicted greater BMI at 30 months. Providing strong social support to pregnant women following a stressor might buffer the effects of PNMS on offspring birthweight and later obesity.

Assessing postpartum depressive symptoms is complicated by the fact that irritability, fatigue, insomnia, and appetite disruptions are also related to normative aspects of the childbearing process. We used multigroup confirmatory factor analysis to compare symptoms in 271 postpartum women with those of 820 non-postpartum women. We found that (a) irritability, insomnia, fatigue, and appetite loss were substantially associated with depressed mood among postpartum women whereas increased appetite was not; (b) irritability, insomnia, fatigue, and appetite changes were as strongly related to depression among postpartum women as they were among non-postpartum women; and (c) after controlling for overall depressed mood, postpartum women reported more irritability, insomnia, and appetite loss than the non-postpartum women. Irritability, fatigue, insomnia, and appetite loss are valid indicators and should be used to measure postpartum depressive symptoms. However, scores on these symptoms should be adjusted downward to account for their higher baseline rates in the postpartum population.

A systematic investigation of the role of prenatal partner support in perinatal maternal depression was conducted. Separate facets of partner support were examined (i.e., received support and support adequacy) and a multidimensional model of support was applied to investigate the effects of distinct types of support (i.e., informational, physical comfort, emotional/esteem, and tangible support). Both main and stress-buffering models of partner support were tested in the context of prenatal maternal stress resulting from exposure to a natural disaster. Questionnaire data were analyzed from N=145 partnered women using growth curve analytic techniques. Results indicate that received support interacts with maternal flood stress during pregnancy to weaken the association between stress and trajectories of maternal depression from pregnancy to 30 months postpartum. Support adequacy did not interact with stress, but was associated with levels of depressive symptoms controlling for maternal stress and received support. Results demonstrate the distinct roles of various facets and types of support for a more refined explanatory model of prenatal partner support and perinatal maternal depression. Results inform both main effect and stress buffering models of partner support as they apply to the etiology of perinatal maternal depression, and highlight the importance of promoting partner support during pregnancy that matches support preferences.

Lynch syndrome is characterized by mutations in the mismatch repair genes MLH1, MSH2, MSH6, and PMS2. In PMS2, detection of mutations is confounded by numerous pseudogenes. Detection of 3' deletions is particularly complicated by the pseudogene PMS2CL, which has strong similarity to PMS2 exons 9 and 11-15, due to extensive gene conversion. A newly designed multiplex ligation-dependent probe amplification (MLPA) kit incorporates probes for variants found in both PMS2 and PMS2CL. This provides detection of deletions, but does not allow localization of deletions to the gene or pseudogene. To address this, we have developed a methodology incorporating reference samples with known copy numbers of variants, and paired MLPA results with sequencing of PMS2 and PMS2CL. We tested a subset of clinically indicated samples for which mutations were either unidentified or not fully characterized using existing methods. We identified eight unrelated patients with deletions encompassing exons 9-15, 11-15, 13-15, 14-15, and 15. By incorporating specific, characterized reference samples and sequencing the gene and pseudogene it is possible to identify deletions in this region of PMS2 and provide clinically relevant results. This methodology represents a significant advance in the diagnosis of patients with Lynch syndrome caused by PMS2 mutations.

Mood and anxiety disorders are highly prevalent in perinatal samples, affecting as many as 20% of childbearing women (Gavin et al., 2005). In an effort to prevent adverse outcomes associated with perinatal mood and anxiety disorders, researchers and clinicians have advocated routine screening during the perinatal period (NRC, 2009). Although, there are several screening measures for depression, many of which have been used or validated in perinatal populations, few screening tools have been developed specifically for or validated in perinatal samples for bipolar disorder or anxiety disorders. Despite the ongoing need for brief, accurate, and easily administered screening measures, it seems clear that perinatal mood and anxiety screening is associated with substantial improvement in rate of detection (Georgiopoulous et al., 1999; Georgiopoulos, Bryan, Wollan, and Yawn, 2001; Gilbody, Sheldon, and House, 2008). However, in the absence of systematic protocols to ensure further assessment, treatment, and follow-up, screening is unlikely to have a positive impact on depression-associated morbidity (Gjerdingen, Katon, and Rich, 2008; Gilbody et al., 2008; Miller et al., 2012; NRC, 2009). Preliminary evidence suggests that screening for perinatal mood and anxiety disorders, when embedded within larger systems to ensure comprehensive assessment, connection to treatment, and regular monitoring, has the potential to improve outcomes for women and their families. The question of whether screening programs can ultimately decrease depression-associated morbidity and prevent adverse outcomes cannot be answered given the existing research base (Myers et al., 2013). Although much is left to be understood about perinatal screening for mood and anxiety disorders, the impact of this research lies in potential for reducing negative maternal outcomes as well as for prevention of the negative impact of perinatal depression on the health and well-being of babies born to depressed or anxious mothers.

Genetic counseling is a relatively new discipline in medicine that emerged with the advancement of genetic diagnostics and molecular medicine. Genetic counselors are experts at translating the complex language of genomic medicine into terms that are easy to understand for patients, their relatives, and the medical support team. In general, the genetic counseling protocol involves diagnosis, management, and support for individuals and their families at risk for a genetic disorder.

Pulmonary hypertension causes right ventricular ischemia and failure as a result of increased afterload combined with reduced coronary blood flow. Increasing coronary driving pressure by raising aortic pressure with phenylephrine has been shown to reverse right ventricular ischemia from pulmonary hypertension in animals. Since vasodilators often fail to reduce afterload, we tested whether raising the coronary driving pressure would improve right ventricular function in man. Ten patients with pulmonary hypertension had hemodynamics and right ventricular coronary driving pressure measured before and 10 minutes after a steady state was reached with a phenylephrine infusion titrated to raise aortic pressure by 25 percent. Phenylephrine caused a significant (p less than .01) increase in mean aortic pressure (84 to 108 mm Hg) and right ventricular coronary driving pressure (46 to 69 mm Hg). In response, there was a significant (p less than .01) rise in mean pulmonary artery pressure (58 to 67 mm Hg), right ventricular end-diastolic pressure (10 to 16 mm Hg) and wedge pressure (5 to 9 mm Hg), and an insignificant fall in cardiac output (3.26 to 3.09 L/min) and pulmonary artery O2 saturation (57 to 49 percent). Although phenylephrine increased right ventricular coronary driving pressure, it worsened right ventricular function as manifest by a rise in end-diastolic pressure and fall in cardiac output. Any benefit of raising right ventricular coronary driving pressure may have been offset by alpha vasoconstriction of right ventricular coronary blood flow and/or pulmonary arterial vasoconstriction. Phenylephrine does not appear to be a useful therapy of right ventricular failure from pulmonary hypertension in patients who fail vasodilators.

Mothers facing stressors or lacking adequate support often find parenting more challenging and less enjoyable. This study examined the mechanisms through which contextual variables might influence perceptions of parenting. Participants were 153 mothers of toddlers who completed interviews and questionnaires on life adversity, social support, negative affect, and parenting stress. Life adversity was positively associated with parenting stress and this association was mediated through negative affect. Emotional support moderated the association between adversity and negative affect. Life adversity appeared to promote negative affect, which in turn led mothers to regard their child as more obstinate and demanding and their interactions with their children as less enjoyable. Adversity had little effect on parental perceptions among mothers with adequate emotional support.

Because a definitive diagnosis of pulmonary hypertension requires cardiac catheterization, there have been no data on the prevalence of pulmonary hypertension in the general population. The diameter of the right descending pulmonary artery, as measured from chest roentgenograms, has been used by clinicians as a noninvasive indicator of pulmonary hypertension. Chest roentgenograms from the Second National Health and Nutrition Examination Survey, a sample survey of the US civilian noninstitutional population, were used in conjunction with estimates of sensitivity and specificity of this technique determined from patients who underwent right heart catheterization to estimate the prevalence of pulmonary hypertension (mean pulmonary artery pressure > 20 mm Hg at rest) in the US population aged 25 years and older. The prevalence is estimated to be very low in the population at large, primarily because of the low prevalence in women of all ages. The prevalence in men is 13.4 percent above age 34, and increases to 28.2 percent above age 64. Thus, in elderly men, pulmonary hypertension is relatively common and may have an important impact in the management of this age group. (Chest 1989; 96:236–41)
