Abstract

El padecimiento físico representa uno de los medios de propulsión intelectual femenina para romper los límites impuestos por la sociedad misógina de la Edad Media. A través de la Arboleda se ratifica que la tendencia ideológica y teórica de Teresa de Cartagena conserva fuerzas positivas hacia el sufrimiento por la discapacidad de la sordera, mientras que en la práctica no pasaba lo mismo. Por eso, con diferentes discursos religiosos y médicos se manifiestan, explícita e implícitamente, la discriminación y marginalización con que convivía la autora a partir de su discapacidad física. Al combinar las connotaciones negativas de la sordera con los preceptos de la doctrina cristiana, la monja castellana logra imponer su escritura frente a la sociedad misógina. Physical suffering represents nne of the means to launch an intellectual feminine discourse in order to overcome the limits imposed by misogynist medieval society. The Arboleda confirais Teresa de Cartagena's ideological and theoretieal tendency to view suffering as holding a positive value while in practice the same did not occur. The discrimination and marginalization the author lived with because of her physical disability are explicitly and implicitly manifestad through the religious and medical discourses of the time. By combining the negative connotations of deafness with the precepts of Christian doctrine, the Castilian nun succeeds in imposing her writing against misogynist society.

Abstract

Background: Concurrent chemoradiation therapy (CCRT) followed by hepatic arterial infusional chemotherapy (HAIC) was reported to be effective for advanced hepatocellular carcinoma (HCC) with portal vein thrombosis. However, transarterial chemoembolization (TACE) is not preferred in this setting. The aim of this study was to assess the factors affecting survival after CCRT, including additional TACE during repeated HAIC.Methods: Thirty-eight patients who underwent CCRT as the initial treatment for Barcelona Clinic Liver Cancer stage C HCC with vascular invasion between 2009 and 2016 were reviewed retrospectively. During CCRT, 5-fluorouracil (5-FU) was infused via chemoport during the first and last five days of five weeks of external beam radiation therapy. After CCRT, repeated HAIC with cisplatin and 5-FU was performed monthly. Nineteen patients (50%) underwent additional TACE between repeated HAICs. Factors related to overall survival and progression free survival (PFS) were analyzed.Results: The mean age of patients was 55 years (male:female, 33:5). Underlying liver diseases were hepatitis B, hepatitis C and non-B/C in 29, 1 and 8 patients, respectively. The median radiation dose was 4500 cGy. The objective response (OR) rate at one months after CCRT was 36.8%. The median PFS was 7.4 (range, 1.8 - 32.1) months. The median overall survival was 11.6 (range 2.8-65.7) months. Achieving an OR after CCRT (hazard ratio [HR], 0.028; P < 0.001), additional TACE (HR, 0.134, P < 0.001), and further rounds of HAIC (HR, 0.742, P = 0.001) were independent significant factors related to overall survival. The overall survival duration of patients with an OR after CCRT (median 44.2 vs. 6.6 months, P < 0.001) and additional TACE (median 19.8 vs. 9.1 months, P = 0.001) were significantly greater than those without an OR after CCRT or additional TACE.Conclusion: Patients who achieved an OR after CCRT, underwent additional TACE, and were subjected to repeated rounds of HAIC following CCRT showed better survival after CCRT for advanced stage of HCC with vascular invasion. Abstract

ObjectiveHepatologists and colonoscopists often hesitate to perform a colonoscopic polypectomy in patients with chronic liver disease (CLD), especially those with cirrhosis, because of the risk of postpolypectomy bleeding (PPB). We aimed to investigate the incidence and risk factors of delayed PPB after a colonoscopic polypectomy in patients with CLD.Materials and methodsIn total, 152 patients with CLD who underwent colonoscopic polypectomy from December 2005 to December 2012 were retrospectively reviewed.ResultsCirrhosis was identified in 80 (52.6%) patients. During the study period, 442 polyps were removed and delayed PPB developed in 14 (9.2%) patients. The incidence of delayed PPB was significantly higher in patients with cirrhosis than in those without the disease (13.8% [n = 11] vs. 4.2% [n = 3],p = 0.041). The polyp size (odds ratio, 1.087; 95% confidence interval, 1.009–1.172) and cirrhosis (odds ratio, 8.535; 95% confidence interval, 2.417–30.140) were independent risk factors for delayed PPB. In patients with cirrhosis, the optimal cut-off size to identify high-risk polyps for delayed PPB was 10 mm (area under the receiver operating characteristics curve, 0.737; sensitivity, 52%; specificity, 88%).ConclusionCaution is needed when colonoscopic polypectomy is planned in patients with CLD who have larger polyps and cirrhosis.
