11. Reduced antibiotic prescription rates following physician-targeted interventions in a dental practice.

Abstract

Background: The prescription rate for antibiotics in dental clinics is not declining despite the increase in the antibiotic resistance problem. In this study, we observed the change in antibiotic prescription rates by dentists in a Korean dental hospital for various treatments after conducting interventions targeting dentists.Methods: The first intervention was to distribute guidelines. The second intervention was to remove the bundled prescription button containing antibiotics from the Electronic Medical Record system. A total of 22,098 treatment records were divided into 12 main treatment categories, and Chi-square tests and logistic regression analyses were performed.Results: After the interventions were applied, the overall prescription rate for antibiotics dropped. The antibiotic prescription rate decreased by an odds ratio of 0.774 (95% CI: 0.686-0.873) after intervention 1 and by an odds ratio of 0.574 (95% CI: 0.501-0.658) after intervention 2. The treatments with significantly reduced antibiotic prescription rates were extraction for orthodontic treatment, dental implant surgery, extraction of an impacted tooth and general extraction. These treatments are typically performed in patients without an active infection. The prescription rate did not change for periodontal treatments or endodontic treatments, which are usually performed in patients with an infection. The prescription rate also remained constant for minor operations and other basic treatments.Conclusion: The interventions induced behavioural changes in the dentists and were effective in lowering the antibiotic prescription rates in a dental hospital. In particular, there was a significant reduction in the prescription rates for implant surgery and tooth extraction in the absence of infection. [ABSTRACT FROM AUTHOR]

Abstract

An ontology is a formal, explicit specification of a domain. An important benefit of using an ontology during software development is that it enables the developer to reuse and share application domain knowledge using a common vocabulary across heterogeneous software platforms and programming languages. One of the most important components of ontologies is concept hierarchy, which models the information on the domain of interest in terms of concepts and subsumption relationships between them. However, it is extremely difficult and time-consuming for human experts to discover concepts and construct concept hierarchies from the domain. In this paper we introduce Formal Concept Analysis(FCA) as the basis for a practical and well founded methodological approach to the construction of concept hierarchy. We present a semi-automatic tool, FCAwizard, to support the concept hierarchy construction. Based on the FCAwizard, we are now exploring a data-driven approach to construct medical ontologies from some medical data contained in clinical documents. 24. Staffing and scheduling methodologies for hospital ancillary units.

Abstract

This paper presents staffing and scheduling methodologies, which attempt to determine staffing levels and their allocations for hospital's ancillary units, given a set of dem and data. The staffing methodologies are mostly based on the underst and ing of hospital's systems characteristics of concern, which may be expressed as assumptions underlying and constraints imposed on the methodologies. Depending on the requirements of the hospital's system, a different set of methodologies may be employed. As determined a set of staffing levels, a method of scheduling staff is applied over a 4-week period ahead; and a guideline on yearly vacations and holidays assignments. All in all, the methodologies are in practice complementary, supplementary, and sequential with one another; and thus when well combined, enable themselves to apply for various hospitals' staffing environments. These methodologies were working effectively, when applied to a study hospital. Resultant outputs include: staffing cost, expected productivity and its control, and labor information system.
