1. Forecasting adverse surgical events using self-supervised transfer learning for physiological signals

Abstract

Hundreds of millions of surgical procedures take place annually across the world, which generate a prevalent type of electronic health record (EHR) data comprising time series physiological signals. Here, we present a transferable embedding method (i.e., a method to transform time series signals into input features for predictive machine learning models) named PHASE (PHysiologicAl Signal Embeddings) that enables us to more accurately forecast adverse surgical outcomes based on physiological signals. We evaluate PHASE on minute-by-minute EHR data of more than 50,000 surgeries from two operating room (OR) datasets and patient stays in an intensive care unit (ICU) dataset. PHASE outperforms other state-of-the-art approaches, such as long-short term memory networks trained on raw data and gradient boosted trees trained on handcrafted features, in predicting five distinct outcomes: hypoxemia, hypocapnia, hypotension, hypertension, and phenylephrine administration. In a transfer learning setting where we train embedding models in one dataset then embed signals and predict adverse events in unseen data, PHASE achieves significantly higher prediction accuracy at lower computational cost compared to conventional approaches. Finally, given the importance of understanding models in clinical applications we demonstrate that PHASE is explainable and validate our predictive models using local feature attribution methods.

2. Optimal Time-Reversed Wideband Signals for Distributed Sensing

Abstract

This paper considers a distributed wave-based sensing system that probes a scene consisting of multiple interacting idealized targets. Each sensor is a collocated transmit-receive pair that is capable of transmitting arbitrary wideband waveforms. We address the problem of finding the space-time transmit waveform that provides the best target detection performance in the sense of maximizing the energy scattered back into the receivers. Our approach is based on earlier work that constructed the solution by an iterative time-reversal (TR) process. In particular, for the case of idealized point-like scatterers in free space, we examine the frequency dependence of the eigenvalues of the TR operator, and we show that their behavior depends on constructive and destructive interference of the waves traveling along different paths. In addition, we show how these eigenvalues are connected to the poles of the Singularity Expansion Method. Our study of the frequency behavior distinguishes this work from most previous TR work, which focused on single-frequency waveforms and noninteracting targets. Abstract

Recommendations for laboratories to report incidental findings from genomic tests have stimulated interest in such results. In order to investigate the criteria and processes for assigning the pathogenicity of specific variants and to estimate the frequency of such incidental findings in patients of European and African ancestry, we classified potentially actionable pathogenic single-nucleotide variants (SNVs) in all 4300 European- and 2203 African-ancestry participants sequenced by the NHLBI Exome Sequencing Project (ESP). We considered 112 gene-disease pairs selected by an expert panel as associated with medically actionable genetic disorders that may be undiagnosed in adults. The resulting classifications were compared to classifications from other clinical and research genetic testing laboratories, as well as with in silico pathogenicity scores. Among European-ancestry participants, 30 of 4300 (0.7%) had a pathogenic SNV and six (0.1%) had a disruptive variant that was expected to be pathogenic, whereas 52 (1.2%) had likely pathogenic SNVs. For African-ancestry participants, six of 2203 (0.3%) had a pathogenic SNV and six (0.3%) had an expected pathogenic disruptive variant, whereas 13 (0.6%) had likely pathogenic SNVs. Genomic Evolutionary Rate Profiling mammalian conservation score and the Combined Annotation Dependent Depletion summary score of conservation, substitution, regulation, and other evidence were compared across pathogenicity assignments and appear to have utility in variant classification. This work provides a refined estimate of the burden of adult onset, medically actionable incidental findings expected from exome sequencing, highlights challenges in variant classification, and demonstrates the need for a better curated variant interpretation knowledge base.

Abstract

Experts play a crucial role in modern organizations, but evidence regarding the soundness and reliability of their decision-making is mixed and often contradictory. We develop and test a moderated-mediation model of expert decision-making linking expertise, identity threat, and overprecision to understand when and why experts offer overly precise judgments, and how they can cope with disconfirming feedback. We find support for this model in a series of lab experiments which show that (a) experts are more likely than novices to double down and produce overly precise predictions following disconfirming feedback, (b) this feedback-induced overprecision by experts is mediated by perceived level of expert identity threat, (c) the source of the feedback matters for identity threat and overprecision, and (d) self-affirmation attenuates identity threat and reduces overprecision. We supplement these experimental findings by investigating experts' response to disconfirming feedback in two real-world settings: Major League Baseball umpiring and chief financial officer predictions of stock market returns. Our model and results show that feedback can harm expert decision-making by leading experts to be overly precise in their judgment, challenging existing notions on the ability of expert decision-makers, and providing insight into when and why experts should be relied upon in organizational decisions. 8. Caught in the Revolving Door: Firm-Government Employee Mobility as a Fleeting Regulatory Advantage.

Abstract

How does the exchange of employees between regulatory agencies and regulated firms (i.e., the firm-government revolving door) affect firm regulatory outcomes? Existing work has mostly found a positive impact of revolving door hiring on firm outcomes, but it has overlooked potential limitations of this corporate political activity (CPA) tactic. We argue that the advantages firms can gain from hiring former regulators are bound by the timing of revolving door employment relative to the regulatory process. Within the context of agribiotechnology and its main regulator, the U.S. Department of Agriculture, we study regulators who move to in-house and contract lobbying positions (i.e., exit revolving door). We find that firms receive better regulatory outcomes (i.e., faster regulatory approval for new crops) only prior to the regulators' move to in-house lobbying, consistent with the regulatory capture perspective. Moreover, this revolving door was only valuable in the time period immediately before the mobility event. Additionally, contrary to the belief that former regulators provide firms with expertise and social capital as lobbyists, we find that firms did not gain any advantage after regulators became lobbyists. Taken together, our results suggest that revolving doors can be an effective business political mobilization strategy, albeit one that has limited success in shaping firm government outcomes, much like other types of CPA. Abstract

This document compares two approaches to finding resonance peaks from asingle sensor: 1) averaging multiple measurements of the transfer function orfrequency-domain scattering matrix, versus 2) using the iterative time-reversalprocess, which involves iteratively re-transmitting a time-reversed version ofthe scattered field at the previous iterations. The averaging method has theadvantage of handling arbitrarily much noise if sufficiently many averages areused. On the other hand, up to a certain level of noise, the time-reversalmethod has dramatic advantages over the averaging method; but it also requires more complex equipment. This document discusses the tradeoffs involved, with the goal of providing information that may be useful in the system design process.

BACKGROUND The global pandemic of COVID-19 has continued to spread across the world due to its highly infectious nature. Its control as of present is dependent on measures that involve banning of large gatherings, physical distancing, hand hygiene, cough hygiene and restrictions of movement. This requires that the general public is made aware of these strategies and adheres to them strictly. OBJECTIVE This study aimed to generate information on the knowledge base, perception, and prevalent practices in Plateau State of Nigeria. METHODS It was a cross-sectional survey in which responses were obtained through an online platform from adult residents of the State. Knowledge, perceptions and prevention practices of COVID-19 were assessed and analysed using Microsoft Excel and Epi-Info 7.2.3.1 at a significance level of p < 0.05 RESULTS Responses were obtained from 732 participants. Sixteen out of the 17 local government areas of the state were captured. The age group 21-50 year made up 85% of the respondents and all had some level of education. The mean level of knowledge was 24.5 ± 3.9 and was significantly associated with age and level of education. Perceptions and practices were however not commensurate with the high level of knowledge. CONCLUSIONS There is a need to increase awareness efforts to fill knowledge gaps and dispel some misconceptions and there is a need for a reciprocal improvement by the residents to adapt the practices that are needed to ensure that the disease is brought under control.

Abstract

Background: COVID-19 is a disease that has resulted in a worldwide pandemic. Health workers have been identified to be at a higher risk compared to the general population due to increased exposure primarily at the work place and having to deal with a novel disease whose epidemiology is still evolving. As health workers are vital to control efforts, their response will be influenced by what they know, their perception of the disease and their practices. This study therefore sought to assess the knowledge, perceptions and practices of health workers on COVID-19 in Plateau State, Nigeria. Method: It is a descriptive study that used a self-administered questionnaire to collect data on 105 health workers of Plateau State who were purposively selected. Data was collected on their knowledge, perceptions and practice of COVID-19 and analyzed using SPSS version 23 at a confidence interval of 95%. Results: Mean age of health workers was 42.5 ± 9.2 years.  Females made up 58% of the respondents, males were 42%.  Nurses made up 69% of the respondents.  More than 70% had worked for 10 – 29 years. Knowledge of COVID-19 was found to be inadequate in 57% of the health workers with social media being most frequent (69.5%) source of information. Most (86.6%) respondents had a good perception towards COVID-19 and 62.8% were willing to attend to a COVID-19 patient. There were consistent practices of hand hygiene and cough etiquette. Sixty percent had access to at least one form of personal protective equipment though 72.4% had never had training on putting on the full personal protective gear required for COVID-19. Conclusion: There is an urgent need for training of health care workers in Plateau State and provision of full personal protective gear to ensure their safety at work.

Abstract

This paper develops theory for an iterative experimental approach to use one or more antennas to identify resonances of a scattering system, which could consist of the antennas and one or more scattering objects. We include realistic mathematical models for the antennas and for target(s), and we show that the resonances include effects from the antennas and wave propagation as well as the target scattering operator and its poles. We show how the effects of the antennas and wave propagation paths can be removed, to leave only the effect of the target scattering operator. We include simulations for the case of one and two cylindrical dipole antennas probing a dielectric sphere, and we show the effect of antenna resonances on the iterative experimental process.
