3. Perioperative outcomes of laparoscopic low anterior resection using ArtiSential® versus robotic approach in patients with rectal cancer: a propensity score matching analysis.

Abstract

Background: Total mesorectal excision using conventional straight fixed devices may be technically difficult because of the narrow and concave pelvis. Several laparoscopic articulating tools have been introduced as an alternative to robotic systems. The aim of this study was to compare perioperative outcomes between laparoscopic low anterior resection using ArtiSential® and robot-assisted surgery for rectal cancer. Methods: This retrospective study included 682 patients who underwent laparoscopic or robotic low anterior resection for rectal cancer from September 2018 to December 2021. Among them, 82 underwent laparoscopic surgery using ArtiSential® (group A) and 201 underwent robotic surgery (group B). A total of 73 [group A; 66.37 ± 11.62; group B 65.79 ± 11.34] patients were selected for each group using a propensity score matching analysis. Results: There was no significant difference in the baseline characteristics between group A and B. Mean operative time was longer in group B than A (163.5 ± 61.9 vs 250.1 ± 77.6 min, p < 0.001). Mean length of hospital stay was not significantly different between the two groups (6.2 ± 4.7 vs 6.7 ± 6.1 days, p = 0.617). Postoperative complications, reoperation, and readmission within 30 days after surgery were similar between the two groups. Pathological findings revealed that the circumferential resection margins were above 10 mm in both groups (11.00 ± 7.47 vs 10.17 ± 6.25 mm, p = 0.960). At least 12 lymph nodes were sufficiently harvested, with no significant difference in the number harvested between the groups (20.5 ± 9.9 vs 19.7 ± 7.3, p = 0.753). Conclusions: Laparoscopic low anterior resection using ArtiSential® can achieve acceptable clinical and oncologic outcomes. ArtiSential®, a multi-joint and articulating device, may serve a feasible alternative approach to robotic surgery in rectal cancer. [ABSTRACT FROM AUTHOR]

Abstract

International audience; OBJECTIVE: An independent Safety Review Committee (SRC; supported by Novartis Pharma AG [Basel, Switzerland]) analyzed investigator-reported cases of intraocular inflammation (IOI), endophthalmitis and retinal arterial occlusion in the phase 3 HAWK and HARRIER trials of brolucizumab versus aflibercept in neovascular age-related macular degeneration (nAMD). DESIGN: A post-hoc analysis of a subset of data from two 2-year, double-masked, multicenter, active-controlled randomized phase 3 trials (NCT02307682, NCT02434328). PARTICIPANTS: Patients (N=1817) with untreated, active choroidal neovascularization due to AMD in the study eye were randomized and treated in HAWK/HARRIER. The SRC reviewed data from cases of investigator-reported IOI (60/1088 brolucizumab-treated eyes; 8/729 aflibercept-treated eyes).METHODS: The SRC received details and images (color fundus photography, fluorescein angiography and optical coherence tomography) for all investigator-determined cases of IOI, retinal arterial occlusion and endophthalmitis. Cases were reviewed in detail by ≥2 readers, then adjudicated by the SRC as a group. MAIN OUTCOME MEASURES: Within this subset of patients: incidence of IOI, signs and incidence of retinal vasculitis and/or retinal vascular occlusion, and visual acuity loss; time since first brolucizumab injection to IOI event onset; frequency of visual acuity loss following brolucizumab injection by time of first IOI event onset.RESULTS: Fifty brolucizumab-treated eyes were considered to have definite/probable drug-related events within the spectrum of IOI, retinal vasculitis and/or vascular occlusion. Based on these cases, incidence of definite/probable IOI was 4.6% (IOI + vasculitis, 3.3%; IOI + vasculitis + occlusion, 2.1%). There were 8 cases (incidence 0.74%) of at least moderate visual acuity loss (≥15 ETDRS letters) in eyes with IOI; 7 were in eyes with IOI + vasculitis + occlusion. Of the 8 cases, 5 experienced their first IOI-related event within 3 months of the first brolucizumab injection (increasing to 7/8 within 6 months). Incidence of IOI in aflibercept-treated eyes was 1.1%, with at least moderate visual acuity loss in 0.14%.CONCLUSIONS: This analysis of IOI cases following brolucizumab injection identified signs of retinal vasculitis with or without retinal vascular occlusion, and an associated risk of visual acuity loss. The findings will help physicians to evaluate the risks and benefits of brolucizumab treatment for nAMD.

21. Theoretical investigation of nonlinear resonances in a carbon nanotube cantilever with a tip-mass under electrostatic excitation.

Abstract

The nonlinear dynamics of a resonating carbon nanotube (CNT) cantilever having an attached mass at the tip ('tip mass') were investigated by incorporating electrostatic forces and intermolecular interactions between the CNT and a conducting plane surface. This work enables applications of CNT resonating sensors for tiny mass detection and provides a better understanding of the dynamics of CNT cantilevers. The effect of tip mass on a resonating CNT cantilever is normally characterized by the fundamental frequency shift in the linear resonance regime. However, there are more complex dynamics in the nonlinear resonance regime, such as secondary resonances with parametric excitation. The latter have been limited to nano-cantilevers without tip mass or to axially excited micro-beams. To analyze the nonlinear dynamics, we developed a differential equation model that includes both geometric and inertial nonlinear terms for the large vibration amplitudes at increasing drive forces. In our approach, we used Galerkin discretization techniques and numerical integration methods. The CNT cantilever exhibited complex nonlinear responses due to the applied AC and DC voltages and various tip masses. The nonlinear model had a softer response for increasing tip mass than those of the linear model with the same driving conditions. At low applied voltages, the cantilever had linear amplitude and phase responses at primary and secondary superharmonic resonance frequencies. The response branches were softened at the primary resonance through saddle-node (SN) bifurcation from harmonic electrostatic excitation at higher applied voltages. After SN bifurcation, the lower branch of the solution near resonance became unstable. In addition, theoretical analyses were performed on more complex nonlinear responses and stability changes with tip mass variations, such as period-doubling (PD) bifurcation at subharmonic resonance frequencies. Abstract

We obtain a solution of the three-phase model in the limit d/λ<1, where the complex refractive index ñ=n+iκ and thickness d of an isotropic film on an isotropic substrate are given analytically at any single wavelength λ from polarimetric data Δρ/ρ and ΔR/R, where ρ is the complex reflectance ratio and R is either the p- or s-polarized reflectance. We describe several procedures for extending the range of validity of the solution. Analysis of correlations shows that the uncertainty δ(ΔR/R) of ΔR/R is significantly more important than the δ(Δρ/ρ) of Δρ/ρ, which allows us to obtain an expression for the uncertainties δn, δκ, and δd of n, κ, and d, and to identify conditions that optimize the determination of the layer parameters. We find that the relative uncertainties δn/n and δd/d are not equal, as would be expected if they were determined by the optical thickness nd measured by ellipsometry, but that ΔR/R breaks the connection. We verify our results by measurements of H2O reversibly physisorbed on oxidized GaAs, finding, for example, that for our conditions δκ is determined more accurately than δn, and δn more accurately than δd. These data and model calculations show that fluctuations in parameters, particularly d, are asymmetric, leading in principle to inaccurate average values. However, we show that the importance of the ΔR/R data together with the remaining high correlation between n and d allows us to define a characteristic curve that can be used to correct the results for this nonlinearity. Finally, we extend our analysis to determine the orthogonal linear combinations of n, κ, and d that the data actually determine, which explains why the data fit the characteristic curve so well. Our results will be useful in various contexts for the analysis of films less than 1 nm thick, for example, in applications involving preparation of next-generation electronic and optoelectronic devices with complicated multilayer structures, real-time control of deposition, and the identification of physisorbed and chemisorbed layers on the monolayer scale. Abstract

TNF-related apoptosis-inducing ligand (TRAIL) is known to selectively induce apoptosis in various tumour cells. However, downstream-signalling of TRAIL-receptor is not well defined. A functional genetic screening was performed to isolate genes interfering with TRAIL-induced apoptosis using cDNA retroviral library. Bcl-X(L) and FLIP were identified after DNA sequencing analysis of cDNA rescued from TRAIL-resistant clones. We found that increased expression of Bcl-X(L), but not Bcl-2, suppressed TRAIL-induced apoptosis in tumour cells. Western blot and immunohistochemical analyses showed that expression of Bcl-X(L), but not Bcl-2, was highly increased in human breast cancer tissues. Exposure of MDA-MB-231 breast tumour cells to TRAIL induced apoptosis accompanied by dissipation of mitochondrial membrane potential and enzymatic activation of caspase-3, -8, and -9. However, SK-BR-3 breast tumour cells exhibiting increased expression level of Bcl-X(L) were resistant to TRAIL, though upon exposure to TRAIL, caspase-8 and Bid were activated. Forced expression of Bcl-X(L), but not Bcl-2, desensitised TRAIL-sensitive MDA-MB-231 cells to TRAIL. Similar inhibitory effects were also observed in other tumour cells such as HeLa and Jurkat cells stably expressing Bcl-X(L), but not Bcl-2. These results are indicative of the crucial and distinct function of Bcl-X(L) and Bcl-2 in the modulation of TRAIL-induced apoptosis.
