22. Accuracy and feasibility of continuous glucose monitoring system in pancreatectomy patients.

Abstract

Purpose: Pancreatectomy patients often experience challenging fluctuations in blood glucose levels; therefore, they require a reliable monitoring system. This study aimed to determine the accuracy and acceptability of a continuous glucose monitoring (CGM) system compared with the intermittent capillary glucose test in patients who have undergone pancreatectomy. Methods: Thirty non-diabetic pancreatectomy patients participated. We used the FreeStyle Libre Flash Glucose Monitoring System (Abbott Diabetes Care) for continuous interstitial glucose monitoring. Capillary reference glucose levels were checked four times daily. Accuracy was checked using the Clarke Error Grid. Results: The mean age of the participants was 56.8 ± 12.0 years, of whom 61.3% underwent pancreaticoduodenectomy and 38.7% underwent distal pancreatectomy. Three patients developed pancreatogenic diabetes after pancreatectomy. The clinical accuracy of continuous glucose monitoring compared with capillary glucose was 43.9% in Zone A (clinically accurate zone) and 99.8% in Zone A + B (low risk of error) of the Clarke Error Grid. No device-related adverse events were reported. Patients rated favorable user acceptability on the questionnaire. Conclusion: This pilot study demonstrated that the CGM device is accurate and safe for patients who underwent pancreatectomy, with favorable user acceptability. Despite these challenges, the study proposes that the CGM device is beneficial for monitoring glucose levels after discharge in patients with impaired glucose levels following pancreatectomy. 23. Two-Step Image Registration for Dual-Layer Flat-Panel Detectors.

Abstract

Background: For a single exposure in radiography, a dual-layer flat-panel detector (DFD) can provide spectral images and efficiently utilize the transmitted X-ray photons to improve the detective quantum efficiency (DQE) performance. In this paper, to acquire high DQE performance, we present a registration method for X-ray images acquired from a DFD, considering only spatial translations and scale factors. The conventional registration methods have inconsistent estimate accuracies depending on the captured object scene, even when using entire pixels, and have deteriorated frequency performance because of the interpolation method employed. Methods: The proposed method consists of two steps; the first step is conducting a spatial translation according to the Fourier shift theorem with a subpixel registration, and the second step is conducting a scale transformation using cubic interpolation to process the X-ray projections. To estimate the subpixel spatial translation, a maximum-amplitude method using a small portion of the slant-edge phantom is used. Results: The performance of the proposed two-step method is first theoretically analyzed and then observed by conducting extensive experiments and measuring the noise power spectrum and DQE. An example for registering chest images is also shown. For a DFD, the proposed method shows a better registration result than the conventional one-step registration. The DQE improvement was more than 56% under RQA 9 compared to the single flat-panel detector case. Conclusions: The proposed two-step registration method can efficiently provide aligned image pairs from the DFD to improve the DQE performance at low doses and, thus, increase the accuracy of clinical diagnosis. 24. Electricity Bill Savings from Reduced Household Energy Consumption in Apartment Complexes.

Abstract

Apartments account for 64.6% of all housing units in the Republic of Korea, and most of them receive electricity under a contract, which includes a progressive rate plan. Recently, due to the electrification of energy used in homes and the growing adoption of electric vehicles, electricity consumption in apartment complexes has been gradually increasing. Given the characteristics of the progressive rate system, an increase in electricity usage results in a significant higher rise in electricity bills. Thus, an effective alternative is required to reduce electricity bills for each household. In this paper, the savings in electricity bills achieved by reducing household electricity usage are analyzed from both apartment complex and individual household perspectives, using metering data from 13,332 households. When households are sorted by the amount of savings in descending order, the resulting values are found to follow a negative exponential curve. This indicates that the benefits from reducing electricity usage in households with higher saving are significantly larger compared to other ones. We analyzed bill savings when electricity usage reductions were selectively applied to the top 10%, 20%, and 30% of households with the largest savings. From the results, it is found that the largest savings in electricity bills for households are achieved when usage reductions are applied to the top 10% of households. It is expected that this amount of savings would encourage these households to reduce their electricity consumption. Additionally, it is found that the savings for apartment complexes and the total savings for selected households are not the same, resulting in changes in the bills for households that do not reduce their usage. From the results, it was observed that when the usage reduction of selected households is small or the proportion of households reducing usage is low, the common area charges for non-reducing households tend to increase, leading to higher electricity bills. On the contrary, when the usage reduction of selected households is large or the proportion of households reducing usage is high, the common area charges for non-reducing households tend to decrease, resulting in lower electricity bills. Abstract

Background: Although advancements in desensitization protocols have led to increased ABO-incompatible (ABOi) living-donor liver transplantation (LDLT), a higher biliary complication rate remains a problem. This study evaluated the effect of baseline anti-ABO antibody titers before desensitization on biliary complications after ABOi LDLT. Methods: The study cohort comprised 116 patients in the ABO-compatible group (ABOc), 29 in the ABOi with the low titer (<1:128) group (ABOi-L), and 14 in the high titer (≥1:128) group (ABOi-H). Results: Biliary complications occurred more frequently in the ABOi-H group than in the ABOi-L and ABOc groups (7 [50.0%] vs. 8 [27.6%] and 24 [20.7%], respectively, p = 0.041). Biliary complication-free survival was significantly worse in the ABOi-H group than in the other groups (p = 0.043). Diffuse intrahepatic biliary strictures occurred more frequently in the ABOi-H group than in the other groups (p = 0.005). Multivariable analysis revealed that the high anti-ABO antibody titer (≥1:128) is an independent risk factor for biliary complications (hazard ratio 3.943 [1.635–9.506]; p = 0.002). Conclusions: A high baseline anti-ABO antibody titer (≥1:128), female sex, and hepatic artery complications are significant risk factors for biliary complications. [ABSTRACT FROM AUTHOR]
