To assess the effect of respiratory motion and cardiac driven pulsation in renal DWI and to examine asymmetrical velocity-compensated diffusion encoding waveforms for robust ADC mapping in the kidneys.The standard monopolar Stejskal-Tanner pulsed gradient spin echo (pgse) and the asymmetric bipolar velocity-compensated (asym-vc) diffusion encoding waveforms were used for coronal renal DWI at 3T. The robustness of the ADC quantification in the kidneys was tested with the aforementioned waveforms in respiratory-triggered and breath-held cardiac-triggered scans at different trigger delays in 10 healthy subjects.The pgse waveform showed higher ADC values in the right kidney at short trigger delays in comparison to longer trigger delays in the respiratory triggered scans when the diffusion gradient was applied in the feet-head (FH) direction. The coefficient of variation over all respiratory trigger delays, averaged over all subjects was 0.15 for the pgse waveform in the right kidney when diffusion was measured in the FH direction; the corresponding coefficient of variation for the asym-vc waveform was 0.06. The effect of cardiac driven pulsation was found to be small in comparison to the effect of respiratory motion.Short trigger delays in respiratory-triggered scans can cause higher ADC values in comparison to longer trigger delays in renal DWI, especially in the right kidney when diffusion is measured in the FH direction. The asym-vc waveform can reduce ADC variation due to respiratory motion in respiratory-triggered scans at the cost of reduced SNR compared to the pgse waveform.

Objectives To evaluate the performance of single-echo Dixon water-fat imaging and computed tomography (CT)–like imaging based on a single ultrashort echo time (sUTE) MR sequence for imaging of vertebral fractures as well as degenerative bone changes of the spine in comparison to conventional CT and MR sequences. Methods Thirty patients with suspected acute vertebral fractures were examined using a 3-T MRI, including an sUTE sequence as well as short-tau inversion recovery (STIR) and T1-weighted sequences. During postprocessing, water-fat separation was performed by solving the smoothness-constrained inverse water-fat problem based on a single-complex UTE image. By removing the unwanted low-frequency phase terms, additional MR-based susceptibility-weighted-like (SW-like) images with CT-like contrast were created. Two radiologists evaluated semi-quantitative and quantitative features of fractures and degenerative changes independently and separately on CT and MR images. Results In total, all 58 fractures were accurately detected of whom 24 were correctly classified as acute fractures with an edema detected on the water-fat-separated UTE images, using STIR and T1w sequences as standard of reference. For the morphological assessment of fractures and degenerative changes, the overall agreement between SW-like images and CT was substantial to excellent (e.g., Genant: κ 0.90 (95% confidence interval 0.54–1.00); AO/Magerl: κ 0.75 (95% confidence interval 0.43–1.00)). Overall inter-reader agreement for water-fat-separated UTE images and SW-like images was substantial to almost perfect. Conclusion Detection and assessment of vertebral fractures and degenerative bone changes of the spine were feasible and accurate using water-fat-separated images as well as SW-like images, both derived from the same sUTE-Dixon sequence. Key Points • The detection of acute vertebral fractures was feasible using water-fat-separated images and CT-like images reconstructed from one sUTE sequence. • Assessment of the vertebral fractures using SW-like images with CT-like contrast was found to be comparable to conventional CT. • sUTE imaging of the spine can help reduce examination times and radiation exposure.

Objectives To examine the effect of high-b-value computed diffusion-weighted imaging (cDWI) on solid lesion detection and classification in pancreatic intraductal papillary mucinous neoplasm (IPMN), using endoscopic ultrasound (EUS) and histopathology as a standard of reference. Methods Eighty-two patients with known or suspected IPMN were retrospectively enrolled. Computed high-b-value images at b = 1000 s/mm2 were calculated from standard (b = 0, 50, 300, and 600 s/mm2) DWI images for conventional full field-of-view (fFOV, 3 × 3 × 4 mm3 voxel size) DWI. A subset of 39 patients received additional high-resolution reduced-field-of-view (rFOV, 2.5 × 2.5 × 3 mm3 voxel size) DWI. In this cohort, rFOV cDWI was compared against fFOV cDWI additionally. Two experienced radiologists evaluated (Likert scale 1–4) image quality (overall image quality, lesion detection and delineation, fluid suppression within the lesion). In addition, quantitative image parameters (apparent signal-to-noise ratio (aSNR), apparent contrast-to-noise ratio (aCNR), contrast ratio (CR)) were assessed. Diagnostic confidence regarding the presence/absence of diffusion-restricted solid nodules was assessed in an additional reader study. Results High-b-value cDWI at b = 1000 s/mm2 outperformed acquired DWI at b = 600 s/mm2 regarding lesion detection, fluid suppression, aCNR, CR, and lesion classification (p = -resolution rFOV-DWI compared to conventional fFOV-DWI (p ≤ .001–.018). High-b-value cDWI images were rated non-inferior to directly acquired high-b-value DWI images (p = .095–.655). Conclusions High-b-value cDWI may improve the detection and classification of solid lesions in IPMN. Combining high-resolution imaging and high-b-value cDWI may further increase diagnostic precision. Clinical relevance statement This study shows the potential of computed high-resolution high-sensitivity diffusion-weighted magnetic resonance imaging for solid lesion detection in pancreatic intraductal papillary mucinous neoplasia (IPMN). The technique may enable early cancer detection in patients under surveillance. Key Points • Computed high-b-value diffusion-weighted imaging (cDWI) may improve the detection and classification of intraductal papillary mucinous neoplasms (IPMN) of the pancreas. • cDWI calculated from high-resolution imaging increases diagnostic precision compared to cDWI calculated from conventional-resolution imaging. • cDWI has the potential to strengthen the role of MRI for screening and surveillance of IPMN, particularly in view of the rising incidence of IPMNs combined with now more conservative therapeutic approaches.

Objectives To evaluate the diagnostic performance of an automated reconstruction algorithm combining MR imaging acquired using compressed SENSE (CS) with deep learning (DL) in order to reconstruct denoised high-quality images from undersampled MR images in patients with shoulder pain. Methods Prospectively, thirty-eight patients (14 women, mean age 40.0 ± 15.2 years) with shoulder pain underwent morphological MRI using a pseudo-random, density-weighted k-space scheme with an acceleration factor of 2.5 using CS only. An automated DL-based algorithm (CS DL) was used to create reconstructions of the same k-space data as used for CS reconstructions. Images were analyzed by two radiologists and assessed for pathologies, image quality, and visibility of anatomical landmarks using a 4-point Likert scale. Results Overall agreement for the detection of pathologies between the CS DL reconstructions and CS images was substantial to almost perfect (κ 0.95 (95% confidence interval 0.82–1.00)). Image quality and the visibility of the rotator cuff, articular cartilage, and axillary recess were overall rated significantly higher for CS DL images compared to CS (p p = 0.02) and ligament/fluid (CS DL 184 ± 17.3, CS 141 ± 23.5, p = 0.03) and SNR values were significantly higher for ligaments and muscle of the CS DL reconstructions (p Conclusion Evaluation of shoulder pathologies was feasible using a DL-based algorithm for MRI reconstruction and denoising. In clinical routine, CS DL may be beneficial in particular for reducing image noise and may be useful for the detection and better discrimination of discrete pathologies. Summary statement Assessment of shoulder pathologies was feasible with improved image quality as well as higher SNR using a compressed sensing deep learning–based framework for image reconstructions and denoising. Background Magnetic resonance imaging (MRI) is currently explored as supplemental tool to monitor disease progression and treatment response in various neuromuscular disorders. We here assessed the utility of a multi-parametric magnetic resonance imaging (MRI) protocol including quantitative water T2 mapping, Dixon-based proton density fat fraction (PDFF) estimation and diffusion tensor imaging (DTI) to detect loss of spinal motor neurons and subsequent muscle damage in adult SMA patients. Methods Sixteen SMA patients and 13 age-matched controls were enrolled in this prospective, longitudinal study. All participants underwent MRI imaging including measurements of Dixon-based PDFF and DTI of the sciatic nerve. SMA patients furthermore underwent measurements of muscle water T2 (T2w) of the biceps femoris muscle (BFM) and quadriceps femoris muscle (QFM). Ten participants returned for a second scan six months later. MRI parameter were correlated with clinical data. All patients were on nusinersen treatment. Results There were significantly higher intramuscular fat fractions in the BFM and QFM of SMA patients compared to healthy controls at baseline and after 6 months. Furthermore, T2 values significantly correlated positively with intramuscular fat fractions. The Hammersmith functional motor scale significantly correlated with the QFM’s intramuscular fat fractions. DTI scans of the sciatic nerve were not significantly different between the two groups. Conclusion This study demonstrates that, water T2 mapping and Dixon-based PDFF estimation may distinguish between adult SMA patients and controls, due to massive intramuscular fat accumulation in SMA. More extensive long-term studies are warranted to further evaluate these two modalities as surrogate markers in SMA patients during treatment.

Objectives We examined the effects of aging and of gadolinium-based contrast agent (GBCA) exposure on MRI measurements in brain nuclei of healthy women. Methods This prospective, IRB-approved single-center case-control study enrolled 100 healthy participants of our high-risk screening center for hereditary breast cancer, who had received at least six doses of macrocyclic GBCA (exposed group) or were newly entering the program (GBCA-naïve group). The cutoff “at least six doses” was chosen to be able to include a sufficient number of highly exposed participants. All participants underwent unenhanced 3.0-T brain MRI including quantitative T1, T2, and R2* mapping and T1- and T2-weighted imaging. The relaxation times/signal intensities were derived from region of interest measurements in the brain nuclei performed by a radiologist and a neuroradiologist, both board certified. Statistical analysis was based on descriptive evaluations and uni-/multivariable analyses. Results The participants (exposed group: 49, control group: 51) were aged 42 ± 9 years. In a multivariable model, age had a clear impact on R2* (p < 0.001–0.012), T2 (p = 0.003–0.048), and T1 relaxation times/signal intensities (p < 0.004–0.046) for the majority of deep brain nuclei, mostly affecting the substantia nigra, globus pallidus (GP), nucleus ruber, thalamus, and dentate nucleus (DN). The effect of prior GBCA administration on T1 relaxation times was statistically significant for the DN, GP, and pons (p = 0.019–0.037). Conclusions In a homogeneous group of young to middle-aged healthy females aging had an effect on T2 and R2* relaxation times and former GBCA applications influenced the measured T1 relaxation times. Key Points The quantitative T1, T2, and R2* relaxation times measured in women at high risk of developing breast cancer showed characteristic bandwidth for all brain nuclei examined at 3.0-T MRI. The effect of participant age had a comparatively strong impact on R2*, T2, and T1 relaxation times for the majority of brain nuclei examined. The effect of prior GBCA administrations on T1 relaxation times rates was comparatively less pronounced, yielding statistically significant results for the dentate nucleus, globus pallidus, and pons. Summary statement Healthy women with and without previous GBCA-enhanced breast MRI exhibited age-related T2* and T2 relaxation alterations at 3.0 T-brain MRI. T1 relaxation alterations due to prior GBCA administration were comparatively less pronounced.

BACKGROUND: In stroke magnetic resonance imaging (MRI), contrast-enhanced magnetic resonance angiography (CE-MRA) is the clinical standard to depict extracranial arteries but native MRA techniques are of increased interest to facilitate clinical practice. The purpose of this study was to assess the detection of extracranial internal carotid artery (ICA) stenosis and plaques as well as the image quality of cervical carotid arteries between a novel flow-independent relaxation-enhanced angiography without contrast and triggering (REACT) sequence and CE-MRA in acute ischemic stroke (AIS). METHODS: In this retrospective, single-center study, 105 consecutive patients (65.27±18.74 years, 63 males) were included, who received a standard stroke protocol at 3T in clinical routine including Compressed SENSE (CS) accelerated (factor 4) 3D isotropic REACT (fixed scan time: 02:46 min) and CS accelerated (factor 6) 3D isotropic CE-MRA. Three radiologists independently assessed scans for the presence of extracranial ICA stenosis and plaques (including hyper-/hypointense signal) with concomitant diagnostic confidence using 3-point scales (3= excellent). Vessel quality, artifacts, and image noise of extracranial carotid arteries were subjectively scored on 5-point scales (5= excellent/none). Wilcoxon tests were used for statistical comparison. RESULTS: Considering CE-MRA as the standard of reference, REACT provided a sensitivity of 89.8% and specificity of 95.2% for any and of 93.5% and 95.8% for clinically relevant (≥50%) extracranial ICA stenosis and yielded a to CE-MRA comparable diagnostic confidence [mean ± standard deviation (SD), median (interquartile range): 2.8±0.5, 3 (3–3) vs. 2.7±0.5, 3 (2–3), P=0.03]. Objectives There is a clinical need for a non-ionizing, quantitative assessment of breast density, as one of the strongest independent risk factors for breast cancer. This study aims to establish proton density fat fraction (PDFF) as a quantitative biomarker for fat tissue concentration in breast MRI and correlate mean breast PDFF to mammography. Methods In this retrospective study, 193 women were routinely subjected to 3-T MRI using a six-echo chemical shift encoding-based water-fat sequence. Water-fat separation was based on a signal model accounting for a single T2* decay and a pre-calibrated 7-peak fat spectrum resulting in volumetric fat-only, water-only images, PDFF- and T2*-values. After semi-automated breast segmentation, PDFF and T2* values were determined for the entire breast and fibroglandular tissue. The mammographic and MRI-based breast density was classified by visual estimation using the American College of Radiology Breast Imaging Reporting and Data System categories (ACR A-D). Results The PDFF negatively correlated with mammographic and MRI breast density measurements (Spearman rho: −0.74, p < .001) and revealed a significant distinction between all four ACR categories. Mean T2* of the fibroglandular tissue correlated with increasing ACR categories (Spearman rho: 0.34, p < .001). The PDFF of the fibroglandular tissue showed a correlation with age (Pearson rho: 0.56, p = .03). Conclusion The proposed breast PDFF as an automated tissue fat concentration measurement is comparable with mammographic breast density estimations. Therefore, it is a promising approach to an accurate, user-independent, and non-ionizing breast density assessment that could be easily incorporated into clinical routine breast MRI exams. Key Points • The proposed PDFF strongly negatively correlates with visually determined mammographic and MRI-based breast density estimations and therefore allows for an accurate, non-ionizing, and user-independent breast density measurement. • In combination with T2*, the PDFF can be used to track structural alterations in the composition of breast tissue for an individualized risk assessment for breast cancer.

Purpose To evaluate a novel flow-independent 3D isotropic REACT sequence compared with CE-MRA for the imaging of extracranial arteries in acute ischemic stroke (AIS). Methods This was a retrospective study of 35 patients who underwent a stroke protocol at 3 T including REACT (fixed scan time: 2:46 min) and CE-MRA of the extracranial arteries. Three radiologists evaluated scans regarding vessel delineation, signal, and contrast and assessed overall image noise and artifacts using 5-point scales (5: excellent delineation/no artifacts). Apparent signal- and contrast-to-noise ratios (aSNR/aCNR) were measured for the common carotid artery (CCA), internal carotid artery (ICA, C1 segment), and vertebral artery (V2 segment). Two radiologists graded the degree of proximal ICA stenosis. Results Compared to REACT, CE-MRA showed better delineation for the CCA and ICA (C1 and C2 segments) (median 5, range 2–5 vs. 4, range 3–5; P P P > 0.05 vs. 4, range 2–5) and contrast (5, range 3–5 vs. 4, range 2–5; P > 0.05) than CE-MRA. The remaining segments of the blood-supplying vessels showed equal medians. There was no significant difference regarding artifacts, whereas REACT provided significantly lower image noise (4, range 3–5 vs. 4 range 2–5; P P P Conclusion Given its fast acquisition, comparable image quality to CE-MRA and high sensitivity and specificity for the detection of ICA stenosis, REACT was proven to be a clinically applicable method to assess extracranial arteries in AIS.

The aim of this study was to determine the influence of virtual monoenergetic images (vMEIs) on renal cortex volumetry (RCV) and estimation of split-renal function.Twenty-five patients (mean ± SD, 64.7 ± 9.9 years) underwent a contrast-enhanced dual-layer spectral detector computed tomography. Images were reconstructed with a reference standard (iterative model reconstruction, IMRRef), a newly spectral detector computed tomography algorithm (SPcon) and vMEI at 40, 60, 80, 100, and 120 keV. Two blinded independent readers performed RCV on all data sets with a semiautomated tool.Total kidney volume was up to 15% higher in vMEI at 40/60 keV compared with IMRRef (P0.001). Total kidney volume with vMEI at 80/100 keV was similar to IMRRef (P0.001). Split-renal function was similar in all reconstructions at approximately 50% ± 3%. Bland-Altman analysis showed no significant differences (P0.05), except for 40 keV versus SPcon (P0.05). The time required to perform RCV was reasonable, approximately 4 minutes, and showed no significant differences among reconstructions. Interreader agreement was greatest with vMEI at 80 keV (r = 0.68; 95% confidence interval, 0.39-0.85; P0.0002) followed by IMRRef images (r = 0.67; 95% confidence interval, 0.37-0.84; P0.0003). IMRRef showed the highest mean Hounsfield unit for cortex/medulla of 223.4 ± 73.7/62.5 ± 19.7 and a ratio of 3.7.Semiautomated RCV performed with vMEI and IMRRef/SPcon is feasible and showed no clinically relevant differences with regard to split-renal function. Low-kiloelectron volt vMEI showed greater tissue contrast and total kidney volume but no benefit for RCV. Moderate-kiloelectron volt vMEI (80 keV) results were similar to IMRRef with a faster postprocessing time.

Background Functional magnetic resonance imaging (fMRI) is a useful method for noninvasive presurgical functional mapping. However, the scanner environment is inherently unsuitable for the examination of auditory and language functions, due to the loud acoustic noise produced by the scanner. Interleaved acquisition methods alleviate this problem by providing a silent period for stimulus presentation and/or response control (sparse sampling) but at the expense of a diminished amount of data collected. There are possible improvements to these sparse acquisition methods that increase the amount of data by acquiring several images per event (clustered sampling). We tested accelerated clustered fMRI acquisition in comparison with conventional sparse sampling in a pilot study. Methods The clustered and sparse acquisition techniques (7.4 minutes scanning time per protocol) were directly compared in 15 healthy subjects (8 men; mean age: 24 ± 3 years) using both a motor (tongue movement) and a language (overt picture-naming) task. Functional imaging data were analyzed using Statistical Parametric Mapping software (SPM12 Wellcome Department of Imaging Neuroscience, London, UK). For both tasks, activation levels were compared and Euclidean distances (EDs) between cluster centers (i.e., local activation maxima and centers of gravity) were calculated. Overlaps and laterality indices were computed for the picture-naming task. In addition, the feasibility of the clustered acquisition protocol in a clinical setting was assessed in one pilot patient. Results For both tasks, activation levels were higher using the clustered acquisition protocol, reflected by bigger cluster sizes (p Conclusion Despite some drawbacks inherent to the acquisition technique, the clustered sparse sampling protocol showed increased sensitivity for activation in language-related cortical regions with short scanning times. Such scanning techniques may be particularly advantageous for investigating patients with contraindications for long scans (e.g., reduced attention span).

To (a) define multi-peak fat model-based effective in-phase echo times for quantitative susceptibility mapping (QSM) in water-fat regions, (b) analyze the relationship between fat fraction, field map quantification bias and susceptibility bias, and (c) evaluate the susceptibility mapping performance of the proposed effective in-phase echoes in comparison to single-peak in-phase echoes and water-fat separation for regions where both water and fat are present.Effective multipeak in-phase echo times for a bone marrow and a liver fat spectral model were derived from a single voxel simulation. A Monte Carlo simulation was performed to assess the field map estimation error as a function of fat fraction for the different in-phase echoes. Additionally, a phantom scan and in vivo scans in the liver, spine, and breast were performed and evaluated with respect to quantification accuracy.The use of single-peak in-phase echoes can introduce a worst-case susceptibility bias ofQSM based on the proposed effective multipeak in-phase echoes can alleviate the quantification bias present in QSM based on single-peak in-phase echoes. When compared to water-fat separation-based QSM the proposed effective in-phase echo times achieve a similar quantitative performance while drastically reducing the computational expense for field map estimation.

Background: Our purpose was to investigate the potential of high-resolution, high b-value computed DWI (cDWI) in pancreatic ductal adenocarcinoma (PDAC) detection. Materials and Methods: We retrospectively enrolled 44 patients with confirmed PDAC. Respiratory-triggered, diffusion-weighted, single-shot echo-planar imaging (ss-EPI) with both conventional (i.e., full field-of-view, 3 × 3 × 4 mm voxel size, b = 0, 50, 300, 600 s/mm2) and high-resolution (i.e., reduced field-of-view, 2.5 × 2.5 × 3 mm voxel size, b = 0, 50, 300, 600, 1000 s/mm2) imaging was performed for suspected PDAC. cDWI datasets at b = 1000 s/mm2 were generated for the conventional and high-resolution datasets. Three radiologists were asked to subjectively rate (on a Likert scale of 1–4) the following metrics: image quality, lesion detection and delineation, and lesion-to-pancreas intensity relation. Furthermore, the following quantitative image parameters were assessed: apparent signal-to-noise ratio (aSNR), contrast-to-noise ratio (aCNR), and lesion-to-pancreas contrast ratio (CR). Results: High-resolution, high b-value computed DWI (r-cDWI1000) enabled significant improvement in lesion detection and a higher incidence of a high lesion-to-pancreas intensity relation (type 1, clear hyperintense) compared to conventional high b-value computed and high-resolution high b-value acquired DWI (f-cDWI1000 and r-aDWI1000, respectively). Image quality was rated inferior in the r-cDWI1000 datasets compared to r-aDWI1000. Furthermore, the aCNR and CR were higher in the r-cDWI1000 datasets than in f-cDWI1000 and r-aDWI1000. Conclusion: High-resolution, high b-value computed DWI provides significantly better visualization of PDAC compared to the conventional high b-value computed and high-resolution high b-value images acquired by DWI.

Objectives To evaluate a compressed sensing artificial intelligence framework (CSAI) to accelerate MRI acquisition of the ankle. Methods Thirty patients were scanned at 3T. Axial T2-w, coronal T1-w, and coronal/sagittal intermediate-w scans with fat saturation were acquired using compressed sensing only (12:44 min, CS), CSAI with an acceleration factor of 4.6–5.3 (6:45 min, CSAI2x), and CSAI with an acceleration factor of 6.9–7.7 (4:46 min, CSAI3x). Moreover, a high-resolution axial T2-w scan was obtained using CSAI with a similar scan duration compared to CS. Depiction and presence of abnormalities were graded. Signal-to-noise and contrast-to-noise were calculated. Wilcoxon signed-rank test and Cohen’s kappa were used to compare CSAI with CS sequences. Results The correlation was perfect between CS and CSAI2x (κ = 1.0) and excellent for CS and CSAI3x (κ = 0.86–1.0). No significant differences were found for the depiction of structures between CS and CSAI2x and the same abnormalities were detected in both protocols. For CSAI3x the depiction was graded lower (p ≤ 0.001), though most abnormalities were also detected. For CSAI2x contrast-to-noise fluid/muscle was higher compared to CS (p ≤ 0.05), while no differences were found for other tissues. Signal-to-noise and contrast-to-noise were higher for CSAI3x compared to CS (p ≤ 0.05). The high - resolution axial T2-w sequence specifically improved the depiction of tendons and the tibial nerve (p ≤ 0.005). Conclusions Acquisition times can be reduced by 47% using CSAI compared to CS without decreasing diagnostic image quality. Reducing acquisition times by 63% is feasible but should be reserved for specific patients. The depiction of specific structures is improved using a high-resolution axial T2-w CSAI scan. Key Points • Prospective study showed that CSAI enables reduction in acquisition times by 47% without decreasing diagnostic image quality. • Reducing acquisition times by 63% still produces images with an acceptable diagnostic accuracy but should be reserved for specific patients. • CSAI may be implemented to scan at a higher resolution compared to standard CS images without increasing acquisition times.

Purpose To develop a methodology to simultaneously perform single echo Dixon water-fat imaging and susceptibility-weighted imaging (SWI) based on a single echo time (TE) ultra-short echo time (UTE) (sUTE) scan to assess vertebral fractures and degenerative bone changes in the thoracolumbar spine. Methods A methodology was developed to solve the smoothness-constrained inverse water-fat problem to separate water and fat while removing unwanted low-frequency phase terms. Additionally, the corrected UTE phase was used for SWI. UTE imaging (TE: 0.14 ms, 3T MRI) was performed in the lumbar spine of nine patients with vertebral fractures and bone marrow edema (BME). All images were reviewed by two radiologists. Water- and fat-separated images were analyzed in comparison with short-tau inversion recovery (STIR) and with respect to BME visibility. The visibility of fracture lines and cortical outlining of the UTE magnitude images were analyzed in comparison with computed tomography. Results Unwanted phase components, dominated by the B1 phase, were removed from the UTE phase images. The rating of the diagnostic quality of BME visualization showed a high preference for the sUTE-Dixon water- and fat-separated images in comparison with STIR. The UTE magnitude images enabled better visualizing fracture lines compared with STIR and slightly better visibility of cortical outlining. With increasing SWI weighting osseous structures and fatty tissues were enhanced. Conclusion The proposed sUTE-Dixon-SWI methodology allows the removal of unwanted low-frequency phases and enables water-fat separation and SWI processing from a single complex UTE image. The methodology can be used for the simultaneous assessment of vertebral fractures and BME of the thoracolumbar spine.

ObjectiveTo evaluate the utility of nerve magnetic resonance imaging (MRI), diffusion tensor imaging (DTI), and muscle MRI multi-echo Dixon for assessing lower motor neuron (LMN) degeneration in amyotrophic lateral sclerosis (ALS).MethodsIn this prospective observational cohort study, 14 patients with ALS and 13 healthy controls underwent a multiparametric MRI protocol, including DTI of the sciatic nerve and assessment of muscle proton density fat fraction of the biceps femoris and the quadriceps femoris muscles by a multi-echo Dixon sequence.ResultsIn ALS patients, mean fractional anisotropy values of the sciatic nerve were significantly lower than those of healthy controls. The quadriceps femoris, but not the biceps femoris muscle, showed significantly higher intramuscular fat fractions in ALS.InterpretationOur study provides evidence that multiparametric MRI protocols might help estimate structural nerve damage and neurogenic muscle changes in ALS.

Background and purpose For the assessment of cardiovascular blood flow, 2D flow (2D) and 4D flow with a single venc (4D Mono) are established techniques. The objective of this study was to validate a multi-venc 4D flow (4D Multi) sequence for the improved simultaneous assessment of arterial and venous flow in high and low flow conditions and to investigate the scan-rescan reproducibility and inter-observer variability of the novel sequence. Methods Eleven volunteers with no known heart condition (female: 6, mean age: 25.8 ± 9.1 years) and two patients with a Fontan circulation were examined using phase-contrast 2D and 4D flow MRI. Stroke volumes, maximum velocities, net flow curves and internal consistency were measured and compared between 2D, 4D Mono and 4D Multi. Additionally, scan-rescan and inter-observer variabilities were analyzed. Finally, qualitative visualization comparisons were performed. Results Bland-Altman analysis show a higher agreement in stroke volumes between 4D Multi and 2D (7 ± 11%) than 4D Mono and 2D (11 ± 24%). 4D Multi is more accurate than 4D Mono in measuring time-resolved net flow throughout the cardiac cycle and qualitative blood flow visualization of 4D Multi is more accurate in visualizing flow patterns revealing more details and less artifacts than 4D Mono. Scan-rescan reproducibility is higher in 4D Multi (−0.04 ± 4.5 ml) than 2D (2.1 ± 7.3 ml) and inter-observer variability is low in both techniques (2D: −0.4 ± 3.4 ml and 4D Multi: 0.4 ± 3.5 ml). Internal consistency was improved in volunteers and patients when using 4D Multi as compared to 4D Mono. Conclusion 4D Multi offers a comprehensive way to accurately quantify flow in arteries and veins both in high and low flow situations and to visualize detailed flow patterns. This technique is readily applicable in the clinical setting and has the potential to be beneficial in the clinical assessment of valvular and congenital heart diseases.

PURPOSE Respiration induces temporal variations of the main magnetic field B0 along the spinal cord. These variations are typically not compensated for in velocity quantifications using phase-contrast MRI. The goal of this study was to analyze errors caused by respiration-induced B0 variations in real-time phase-contrast echo planar imaging (PCEPI) of cervical cerebrospinal fluid (CSF) velocity measurements and to evaluate this effect for various sequence parameters using numerical simulations. METHODS Real-time B0 measurements with double gradient echo sequence and PCEPI measurements were acquired in the cervical CSF of 10 healthy subjects. Dynamic phase offsets attributed to respiration-induced B0 variations were analyzed by quantifying amplitudes and comparing the temporal behavior with respiratory signals. In experiments and simulations, the influence of the echo time (TE) and the delay between PCEPI images (Δt) with respect to respiration on the dynamic phase offsets were investigated. RESULTS A good agreement was found between phase offsets extracted from both acquisition types. Furthermore, respiratory signals qualitatively matched the temporal behavior of the measured phase offsets showing a dependency on subject-dependent local B0 distribution and respiration physiology. Simulations revealed residual background phases in PCEPI velocity quantification varying with TE and Δt. CONCLUSION Respiration-induced B0 variations result in dynamic background phases in real-time PCEPI velocity quantifications of the CSF in the cervical spine. The current work underlines that these background phases need to be corrected to avoid confounding effects.

The aim of this study was to investigate the robustness and reproducibility of radiomic features in different magnetic resonance imaging sequences.A phantom was scanned on a clinical 3 T system using fluid-attenuated inversion recovery (FLAIR), T1-weighted (T1w), and T2-weighted (T2w) sequences with low and high matrix size. For retest data, scans were repeated after repositioning of the phantom. Test and retest datasets were segmented using a semiautomated approach. Intraobserver and interobserver comparison was performed. Radiomic features were extracted after standardized preprocessing of images. Test-retest robustness was assessed using concordance correlation coefficients, dynamic range, and Bland-Altman analyses. Reproducibility was assessed by intraclass correlation coefficients.The number of robust features (concordance correlation coefficient and dynamic range ≥ 0.90) was higher for features calculated from FLAIR than from T1w and T2w images. High-resolution FLAIR images provided the highest percentage of robust features (n = 37/45, 81%). No considerable difference in the number of robust features was observed between low- and high-resolution T1w and T2w images (T1w low: n = 26/45, 56%; T1w high: n = 25/45, 54%; T2 low: n = 21/45, 46%; T2 high: n = 24/45, 52%). A total of 15 (33%) of 45 features showed excellent robustness across all sequences and demonstrated excellent intraobserver and interobserver reproducibility (intraclass correlation coefficient ≥ 0.75).FLAIR delivers the most robust substrate for radiomic analyses. Only 15 of 45 features showed excellent robustness and reproducibility across all sequences. Care must be taken in the interpretation of clinical studies using nonrobust features.

Fluorine-19 (19F) MRI with intravenously applied perfluorocarbons allows the in vivo monitoring of infiltrating immune cells as demonstrated in small animal models at high field. Here, we aimed to transfer this approach to a clinical scanner for detection of inflammatory processes in the heart after acute myocardial infarction (AMI) in a large animal model. Optimization of coil and sequence performance was carried out on phantoms and in vivo at a 3 T Philips Achieva. AMI was induced in Munich mini pigs by 90-min occlusion of the left anterior descending artery. At day 3 after AMI, pigs received a body weight-adjusted intravenous dose of a perfluorooctyl bromide nanoemulsion followed by 1H/19F MRI at day 6 after AMI. A balanced steady-state free precession turbo gradient echo sequence using an ellipsoidal 19F/1H surface coil provided the best signal-to-noise ratio and a superior localization of 19F patterns in vivo. This approach allowed the reliable detection of 19F signals in the injured myocardium within less than 20 min. The 19F signal magnitude correlated significantly with the functional impairment after AMI. This study demonstrates the feasibility of in vivo 19F MR inflammation imaging after AMI at 3 T within a clinically acceptable acquisition time.

Severe aortic stenosis (AS) is known to be associated with substantial myocardial remodelling, leading to diffuse myocardial fibrosis (DMF). Native myocardial T1 is emerging as a novel imaging biomarker for the non-invasive assessment of DMF. In contrast, no studies exist elucidating changes of myocardial T2 reflecting myocardial oedema in the presence of AS. The purpose of the present study was to combine native T1 and T2 mapping in order to characterize myocardial tissue changes in the setting of severe AS.After obtaining ethical approval and informed consent, a total of 26 prospectively selected patients with severe AS (13 women, mean age 81 ± 7 years) and 17 healthy controls (12 women, mean age 63 ± 6 years) underwent cardiac magnetic resonance (CMR) imaging on a clinical 3 T scanner. The CMR protocol included a native Modified Look-Locker (MOLLI) T1 mapping and a Gradient Spin Echo (GraSE) T2-mapping sequence in three short-axis slices and one long-axis view. After segmentation, myocardial T1 and T2 values were averaged over the entire myocardium. Statistical analysis was performed using Wilcoxon sum-rank test, Welch's independent t-test and Pearson's correlation coefficient.Global native myocardial T1 was significantly increased in AS patients when compared to controls (1305 ± 39 vs. 1272 ± 21 ms, p = .005). Similarly, mean myocardial T2 was significantly elevated in AS patients (51 ± 4 vs. 46 ± 2 ms, p .001) and showed a strong correlation with native T1 (r = .60, p .001). An overlap was observed between T1 of both groups, whereas T2 discriminated nearly perfectly between the two groups (area under the curve in ROC analyses: 0.76 for T1, 0.87 for T2).Patients with severe AS exhibit significantly elevated native myocardial T1, which has previously been shown to correlate with the amount of myocardial collagen. Adding to this evidence, the present study is the first to show that native T1 and T2 are both significantly elevated and correlated in AS patients, pointing towards a potential role of oedematous/inflammatory processes in the pathophysiology of myocardial remodelling associated with AS.

OBJECTIVES To prospectively evaluate an L1 regularized iterative SENSE reconstruction (L1-R SENSE) to eliminate band-like artifacts frequently seen with parallel imaging (SENSE) at high acceleration factors in high resolution diffusion weighted magnetic resonance imaging of the pancreas. METHODS Fourteen patients with pancreatic ductal adenocarcinoma (PDAC) underwent respiratory triggered DWI ss-EPI at a resolution of 2.5 × 2.5 × 3 mm3 with uniform undersampling in the phase encoding direction (AP axis) with an acceleration factor of 4. Data were reconstructed using the standard SENSE reconstruction routine of the vendor and an iterative SENSE reconstruction employing L1 regularization after a wavelet sparsifying transformation (L1-R SENSE). Retrospective reconstruction of the data with a lower number of averages was performed using both reconstruction methods. Two radiologists independently assessed noise artifacts, anatomical details and image quality (IQ) subjectively with a 4-point scale. Apparent diffusion coefficient (ADC) and covariance (CV) of ADC estimated from images reconstructed at a different number of averages for PDAC and the normal pancreas were assessed. RESULTS L1-R SENSE resulted in higher IQ and less noise artifacts than SENSE. Anatomical details were significantly higher for SENSE in one reader. Mean ADC of PDAC and normal pancreas were significantly higher for L1-R SENSE than SENSE. L1-R SENSE revealed lower CV of ADC for normal pancreas compared to SENSE, whereas no difference was noted for PDAC. CONCLUSION Compared with traditional SENSE reconstruction, L1-R SENSE effectively reduces band-like noise and improves the robustness of the ADC estimation from acquisitions using single-shot DW-EPI of the pancreas.

Purpose Increasing economic pressure and patient demands for comfort require an ever-increasing acceleration of scan times without compromising diagnostic certainty. This study tested the new acceleration technique Compressed SENSE (CS-SENSE) as well as different reconstruction methods for the lumbar spine. Methods In this prospective study, 10 volunteers and 14 patients with lumbar disc herniation were scanned using a sagittal 2D T2 turbo spin echo (TSE) sequence applying different acceleration factors of SENSE and CS-SENSE. Gradient echo (GRE), autocalibration (CS-Auto) and TSE prescans were tested for reconstruction. Images were analysed by two readers regarding anatomical delineation, diagnostic certainty (for patients only) and image quality as well as objectively calculating the root mean square error (RMSE), structural similarity index (SSIM), SNR and CNR. The Friedman test and Chi-squared were used for ordinal, ANOVA for repeated measurements and Tukey Kramer test for continuous data. Cohen’s kappawas calculated for interreader reliability. Results CS-SENSE outperformed SENSE and CS-Auto regarding RMSE (e.g. CS-SENSE 1.5: 43.03 ± 11.64 versus SENSE 1.5: 80.41 ± 17.66; p = 0.0038) and SSIM as well as in the subjective rating for CS-SENSE 3 TSE. In the patient setting image quality was unchanged in all subjective criteria up to CS-SENSE 3 TSE (all p > 0.05) compared to standard T2 with 43 % less scan time while the GRE prescan only allowed a reduction of 32 %. Conclusion Combining a TSE prescan with CS-SENSE enables significant scan time reductions with unchanged ratings for lumbar spine disc herniation making this superior to the currently used SENSE acceleration or GRE reconstructions.

Background Late gadolinium enhancement (LGE) cardiovascular magnetic resonance (CMR) represents the gold standard for assessment of myocardial viability. The purpose of this study was to investigate the clinical potential of Compressed SENSE (factor 5) accelerated free-breathing three-dimensional (3D) whole heart LGE with high isotropic spatial resolution (1.4 mm3 acquired voxel size) compared to standard breath-hold LGE imaging. Methods This was a retrospective, single-center study of 70 consecutive patients (45.8 ± 18.1 years, 27 females; February–November 2019), who were referred for assessment of left ventricular myocardial viability and received free-breathing and breath-hold LGE sequences at 1.5 T in clinical routine. Two radiologists independently evaluated global and segmental LGE in terms of localization and transmural extent. Readers scored scans regarding image quality (IQ), artifacts, and diagnostic confidence (DC) using 5-point scales (1 non-diagnostic—5 excellent/none). Effects of heart rate and body mass index (BMI) on IQ, artifacts, and DC were evaluated with ordinal logistic regression analysis. Results Global LGE (n = 33) was identical for both techniques. Using free-breathing LGE (average scan time: 04:33 ± 01:17 min), readers detected more hyperenhanced lesions (28.2% vs. 23.5%, P Conclusions In a clinical setting, free-breathing Compressed SENSE accelerated 3D high isotropic spatial resolution whole heart LGE provides good to excellent image quality in 80% of scans independent of heart rate while enabling improved depiction of small and particularly non-ischemic hyperenhanced lesions in a shorter scan time than standard breath-hold LGE.

To evaluate the feasibility of a k-t accelerated multi-VENC 4D phase contrast flow MRI acquisition of the main heart-surrounding vessels, its benefits over a traditional single-VENC acquisition and to present reference flow and velocity values in a large cohort of volunteers.44 healthy volunteers were examined on a 3 T MRI scanner (Ingenia, Philips, Best, The Netherlands). 4D flow measurements were obtained with a FOV including the aorta and the pulmonary arteries. VENC values were set to 40, 100 and 200 cm/s and unfolded based on an MRI signal model. Unfolded multi-VENC data was compared to the single-VENC with VENC 200 cm/s. Flow and velocity quantification was performed in several regions of interest (ROI) placed in the ascending aorta and in the main pulmonary artery. Conservation of mass analysis was performed for single- and multi-VENC datasets. Values for mean and maximal flow velocity and stroke volume were calculated and compared to the literature.Mean scan time was 13.8 ± 4 min. Differences between stroke volumes between the ascending aorta and the main pulmonary artery were significantly lower in multi-VENC datasets compared to single-VENC datasets (9.6 ± 7.8 mL vs. 25.4 ± 26.4 mL, p0.001). This was also true for differences in stroke volume between up- and downstream ROIs in the ascending aorta and pulmonary artery. Values for mean and maximal velocities and stroke volume were in-line with previous studies. To highlight potential clinical applications two exemplary 4D flow measurements in patients with different pathologies are shown and compared to single-VENC datasets.k-t accelerated multi-VENC 4D phase contrast flow MRI acquisition of the great vessels is feasible in a clinically acceptable scan duration. It offers improvements over traditional single-VENC 4D flow, expectedly being valuable when vessels with different flow velocities or complex flow phenomena are evaluated.

BACKGROUND Physical frailty in older individuals is characterized by subjective symptoms of fatigue and exercise intolerance (EI). Objective abnormalities in skeletal muscle (SM) mitochondrial high-energy phosphate (HEP) metabolism contribute to EI in inherited myopathies; however, their presence or link to EI in the frail older adult is unknown. METHODS Here, we studied 3 groups of ambulatory, community-dwelling adults with no history of significant coronary disease: frail older (FO) individuals (81 ± 2.7 years, mean ± SEM), nonfrail older (NFO) individuals (79 ± 2.0 years), and healthy middle-aged individuals, who served as controls (CONT, 51 ± 2.1 years). Lower extremity SM HEP levels and mitochondrial function were measured with 31P magnetic resonance (MR) techniques during graded multistage plantar flexion exercise (PFE). EI was quantified by a 6-minute walk (6MW) and peak oxygen consumption during cardiopulmonary testing (peak VO2). RESULTS During graded exercise, FO, NFO, and CONT individuals all fatigued at similar SM HEP levels, as measured by 31P-MR. However, FO individuals fatigued fastest, with several-fold higher rates of PFE-induced HEP decline that correlated closely with shorter exercise duration in the MR scanner and with 6MW distance and lower peak oxygen consumption on cardiopulmonary testing (P < 0.001 for all). SM mitochondrial oxidative capacity was lower in older individuals and correlated with rapid HEP decline but less closely with EI. CONCLUSION Several-fold faster SM energetic decline during exercise occurs in FO individuals and correlates closely with multiple measures of EI. Rapid energetic decline represents an objective, functional measure of SM metabolic changes and a potential new target for mitigating frailty-associated physical limitations. To evaluate potential clinical acceleration factors of Compressed SENSE (CS)Twenty healthy volunteers were scanned with a 3 T scanner, all receiving a standard, fs 2D PD, three CS (CS 2, CS 3, CS 5) as well as time-equivalent SENSE accelerations (S 2, S 3, S 5). The fs 3D PD sequence was acquired with four CS (CS 6, CS 8, CS 10, CS 15) and equivalent SENSE (S 5.72, S 7.69, S 9.57, S 14) factors. Three independent readers rated the images. Signal-to-noise, contrast-to-noise, root-mean-square error and structural similarity index were analyzed for objective evaluation.Scan time decreased with increasing CS factor (2D CS 2: 145 s, 2D CS 3: 95 s, 2D CS 5: 57 s, 3D CS 6: 293 s, 3D CS 8: 220 s, 3D CS 10: 176 s, 3D CS 15: 119 s). The 2D standard sequence was rated best for diagnostic certainty and overall image impression with an average of 4.97 ± 0.10 and 4.80 ± 0.24 (all p 0.05), except for 2D CS 2 and 2D S 2. For the 3D sequences, the standard sequence performed better for both parameters for CS 15, S 9.57 and S 4, as well as S 7.69 for overall image impression while CS 8 was non-inferior for all tested criteria and CS 10 only inferior for delineation of the anterior cruciate ligament, both outperforming the time-equivalent SENSE accelerations.Compressed SENSE can significantly decrease (34.39 % for 2D CS 2 and 54.17 % for 3D CS 10) scan time in knee imaging with unchanged diagnostic certainty and overall image impression compared to the clinical reference.

To compare a novel Compressed SENSE accelerated ECG- and respiratory-triggered flow-independent 3D isotropic Relaxation-Enhanced Angiography without Contrast and Triggering (modified REACT) with standard non-ECG-triggered 3D contrast-enhanced magnetic resonance angiography (CE-MRA) for imaging of the thoracic aorta in patients with connective tissue diseases (CTD) or other aortic diseases using manual and semiautomatic measurement approaches. This retrospective, single-center analysis of 30 patients (June–December 2018) was conducted by two radiologists, who independently measured aortic diameters on modified REACT and CE-MRA using manual (Multiplanar-Reconstruction) and semiautomatic (Advanced Vessel Analysis) measurement tools on seven levels (inner edge): Aortic annulus and sinus, sinotubular junction, mid- and high-ascending aorta, aortic isthmus, and descending aorta. Bland–Altman analysis was conducted to evaluate differences between the mean values of aortic width and ICCs were calculated to assess interobserver agreement. For each level, image quality was evaluated on a four-point scale in consensus with Wilcoxon matched-pair test used to evaluate for differences between both MRA techniques. Additionally, evaluation time for each measurement technique was noted, which was compared applying one-way ANOVA. When comparing both imaging and measurement methods, CE-MRA (mean difference 0.24 ± 0.27 mm) and the AVA-tool (− 0.21 ± 0.15 mm) yielded higher differences compared to modified REACT (− 0.11 ± 0.11 mm) and the MPR-tool (0.07 ± 0.21 mm) for all measurement levels combined without yielding clinical significance. There was an excellent interobserver agreement between modified REACT and CE-MRA using both tools of measurement (ICC > 0.9).
