However, the duration of macroscopic improvement of melanosis coli after discontinuation of anthracene laxatives is not well understood. We describe the case of an 81-year-old female diagnosed with melanosis coli via colonoscopy who had been taking senna laxatives for 5 years. Seven months after cessation of senna laxatives, colonoscopy showed no melanosis coli in the colon. This study aimed to analyze the reliability and validity of the ASBQ translated into Japanese (ASBQ-J).Patients and Methods: 111 female collegiate athletes completed both the initial test and retest. Independent variables were what kind of sports they perform, level as an athlete, grade (freshman, sophomore, junior, senior), and age. The reliability of the survey was assessed using Cronbach’s alpha and intraclass correlation, and Pearson’s correlation coefficient was calculated to determine the correlation between the total ASBQ scores and three components which were routine/environmental, behavioral, and sports factors.Results: A total of 37.8% of athletes had ASBQ scores equal or less than 36 indicating good sleep. However, 19.8% of athletes scored more than 42, indicating poor sleep. Cronbach alpha was 0.62 and 0.65 for test and retest, respectively. Intraclass correlation was 0.78 (P < 0.01), indicating sufficient internal consistency and test–retest validity. Mean ASBQ scores were 38.5 ± 5.0 and 37.0 ± 6.5 for test and retest, respectively, and it was not significantly different between the two groups (P = 0.37). The correlation coefficients between the ASBQ-J and component scores were all statistically significant (all P < 0.01).Conclusion: ASBQ-J is a reliable questionnaire for assessing sleep disorders in athletes whose primary language is Japanese.Keywords: collegiate athlete, female athlete, performance level, survey

Abstract

Volatile compositions of asteroids provide information on the Solar System history and the origins of Earth's volatiles. Visible to near-infrared observations at wavelengths of $<2.5\ {\rm \mu m}$ have suggested a genetic link between outer main belt asteroids located at $2.5$-$4\ {\rm au}$ and carbonaceous chondrite meteorites (CCs) that show isotopic similarities to volatile elements on Earth. However, recent longer wavelength data for large outer main belt asteroids show $3.1\ {\rm \mu m}$ absorption features of ammoniated phyllosilicates that are absent in CCs and cannot easily form from materials stable at those present distances. Here, by combining data collected by the AKARI space telescope and hydrological, geochemical, and spectral models of water-rock reactions, we show that the surface materials of asteroids having $3.1\ {\rm \mu m}$ absorption features and CCs can originate from different regions of a single, water-rock-differentiated parent body. Ammoniated phyllosilicates form within the water-rich mantles of the differentiated bodies containing NH$_3$ and CO$_2$ under high water-rock ratios ($>4$) and low temperatures ($<70^\circ$C). CCs can originate from the rock-dominated cores, that are likely to be preferentially sampled as meteorites by disruption and transport processes. Our results suggest that multiple large main belt asteroids formed beyond the NH$_3$ and CO$_2$ snow lines (currently $>10$ au) and could be transported to their current locations. Abstract

Hayabusa2 is the Japanese Asteroid Return Mission and targeted the carbonaceous asteroid Ryugu, conducted by the Japan Aerospace Exploration Agency (JAXA). The goal of this mission was to conduct proximity operations including remote sensing observations, material sampling, and a Small Carry-On Impact experiment, as well as sample analyses. As of September 2020, the spacecraft is on the way back to Earth with samples from Ryugu with no critical issues after the successful departure in November 2019. Here, we propose an extended mission in which the spacecraft will rendezvous with a small asteroid with ~30 m - ~40 m in diameter that is rotating at a spin period of ~10 min after an additional ~10-year cruise phase. We introduce that two scenarios are suitable for the extended mission. In the first scenario, the spacecraft will perform swing-by maneuvers at Venus once and Earth twice to arrive at asteroid 2001 AV43. In the second scenario, it will perform swing-by maneuvers at Earth twice to reach asteroid 1998 KY26. In both scenarios, the mission will continue until the early 2030s. JAXA recently released the decision that the spacecraft will rendezvous with 1998 KY26. This paper focuses on our scientific assessments of the two scenarios but leaves the decision process to go to 1998 KY26 for future reports. Rendezvous operations will be planned to detail the physical properties and surrounding environments of the target, one of the smallest elements of small planetary bodies. By achieving the planned operations, the mission will provide critical hints on the violent histories of collisions and accumulations of small bodies in the solar system. Our smartphone application presenting positive word stimulation in video (ie, Subliminal Priming with Supraliminal Reward Stimulation, SPSRS) was suggested to improve depressive symptoms in people with StD, although it is unclear whether it can immediately improve depressed mood. This pilot randomized controlled trial (RCT) aimed to investigate the preliminary efficacy of SPSRS application intervention on depressive mood in people with StD.Methods: Thirty-two participants with StD were randomly assigned to the experimental (n = 16) or control group (n = 16). The experimental group received SPSRS application intervention (10-minute video with positive word stimulation) and the control group received YouTube application intervention (10-minute video without positive word stimulation). Both groups used identical iPhones managed by the research team. The primary outcome was the change from baseline in depression-dejection on the Profile of Mood States 2nd Edition-Adult Short (POMS 2-A Short) after the intervention.Results: No participants dropped out of the study. The experimental group showed a small improvement in depression-dejection on the POMS 2-A Short score (adjusted Hedges’s g = − 0.32) compared to the control group. Post-hoc power analyses estimated a sample size of 56 per group (112 total) to evaluate depression-dejection on the POMS 2-A Short in a future full-scale RCT.Conclusion: SPSRS application intervention may be effective in immediately improving depressive mood in people with StD. It is essential to assess the severity of patient-reported outcomes to provide effective treatment for psychological distress. The Kessler Psychological Distress Scale (K6) is one of the most widely used psychological distress scales. However, the smallest detectable change (SDC) of the K6 score has not been elucidated. Therefore, the current study aimed to determine the SDC of the K6 score in a Japanese adult population.Methods: Participants aged 20– 59 years who are native Japanese speakers were recruited from the panel list of a web research firm. The K6 score was assessed at baseline (T1) and at 2-week follow-up (T2). SDCs were calculated at the individual (SDCind) and group (SDCgroup) levels. Intraclass correlation coefficient agreement (ICCagreement) was calculated to assess test–retest reliability and Cronbach’s alpha to evaluate internal consistency.Results: A total of 3254 (1627 [50%] female) responded at T1 and T2. The mean (standard deviation) K6 scores were 5.71 (5.84) at T1 and 5.65 (5.83) at T2. The SDCind and SDCgroup of the K6 score were 8.47 (35.31%) and 0.15 (0.63%), respectively. The ICCagreement was 0.73, and the Cronbach’s alpha was 0.94.Conclusion: Our study provided evidence on the reliability and interpretation of the K6 score. Calculating the SDC of the K6 score can help identify the significance of changes in psychological distress over time and can determine the efficacy of interventions for psychological distress.Keywords: psychological distress, reliability, measurement error, mental health

Abstract

We report a measurement of the E-mode polarization power spectrum of the cosmic microwave background (CMB) using 150 GHz data taken from July 2014 to December 2016 with the POLARBEAR experiment. We reach an effective polarization map noise level of $32\,\mu\mathrm{K}$-$\mathrm{arcmin}$ across an observation area of 670 square degrees. We measure the EE power spectrum over the angular multipole range $500 \leq \ell <3000$, tracing the third to seventh acoustic peaks with high sensitivity. The statistical uncertainty on E-mode bandpowers is $\sim 2.3 \mu {\rm K}^2$ at $\ell \sim 1000$ with a systematic uncertainty of 0.5$\mu {\rm K}^2$. The data are consistent with the standard $\Lambda$CDM cosmological model with a probability-to-exceed of 0.38. We combine recent CMB E-mode measurements and make inferences about cosmological parameters in $\Lambda$CDM as well as in extensions to $\Lambda$CDM. Adding the ground-based CMB polarization measurements to the Planck dataset reduces the uncertainty on the Hubble constant by a factor of 1.2 to $H_0 = 67.20 \pm 0.57 {\rm km\,s^{-1} \,Mpc^{-1}}$. When allowing the number of relativistic species ($N_{eff}$) to vary, we find $N_{eff} = 2.94 \pm 0.16$, which is in good agreement with the standard value of 3.046. Instead allowing the primordial helium abundance ($Y_{He}$) to vary, the data favor $Y_{He} = 0.248 \pm 0.012$. This is very close to the expectation of 0.2467 from Big Bang Nucleosynthesis. Abstract

We present a measurement of the $B$-mode polarization power spectrum of the cosmic microwave background (CMB) using taken from July 2014 to December 2016 with the POLARBEAR experiment. The CMB power spectra are measured using observations at 150 GHz with an instantaneous array sensitivity of $\mathrm{NET}_\mathrm{array}=23\, \mu \mathrm{K} \sqrt{\mathrm{s}}$ on a 670 square degree patch of sky centered at (RA, Dec)=($+0^\mathrm{h}12^\mathrm{m}0^\mathrm{s},-59^\circ18^\prime$). A continuously rotating half-wave plate is used to modulate polarization and to suppress low-frequency noise. We achieve $32\,\mu\mathrm{K}$-$\mathrm{arcmin}$ effective polarization map noise with a knee in sensitivity of $\ell = 90$, where the inflationary gravitational wave signal is expected to peak. The measured $B$-mode power spectrum is consistent with a $\Lambda$CDM lensing and single dust component foreground model over a range of multipoles $50 \leq \ell \leq 600$. The data disfavor zero $C_\ell^{BB}$ at $2.2\sigma$ using this $\ell$ range of POLARBEAR data alone. We cross-correlate our data with Planck high frequency maps and find the low-$\ell$ $B$-mode power in the combined dataset to be consistent with thermal dust emission. We place an upper limit on the tensor-to-scalar ratio $r < 0.90$ at 95% confidence level after marginalizing over foregrounds.

Abstract

Using only cosmic microwave background polarization data from the POLARBEAR experiment, we measure $B$-mode polarization delensing on subdegree scales at more than $5\sigma$ significance. We achieve a 14% $B$-mode power variance reduction, the highest to date for internal delensing, and improve this result to 2% by applying for the first time an iterative maximum a posteriori delensing method. Our analysis demonstrates the capability of internal delensing as a means of improving constraints on inflationary models, paving the way for the optimal analysis of next-generation primordial $B$-mode experiments., Comment: Matches version published in Physical Review Letters

Abstract

We present a measurement of the B-mode polarization power spectrum of the cosmic microwave background (CMB) using data taken from 2014 July to 2016 December with the Polarbear experiment. The CMB power spectra are measured using observations at 150 GHz with an instantaneous array sensitivity of NETarray=23μ K√s on a 670 square degree patch of sky centered at (R.A., decl.) = (+0h12m0s, -59°18′). A continuously rotating half-wave plate is used to modulate polarization and to suppress low-frequency noise. We achieve 32 μK arcmin effective polarization map noise with a knee in sensitivity of ℓ = 90, where the inflationary gravitational-wave signal is expected to peak. The measured B-mode power spectrum is consistent with a ΛCDM lensing and single dust component foreground model over a range of multipoles 50 ≤ ℓ ≤ 600. The data disfavor zero CℓBB at 2.2σ using this ℓ range of Polarbear data alone. We cross-correlate our data with Planck full mission 143, 217, and 353 GHz frequency maps and find the low-ℓ B-mode power in the combined data set to be consistent with thermal dust emission. We place an upper limit on the tensor-to-scalar ratio r < 0.90 at the 95% confidence level after marginalizing over foregrounds.

Abstract

We present a measurement of the gravitational lensing deflection power spectrum reconstructed with two seasons of cosmic microwave background polarization data from the Polarbear experiment. Observations were taken at 150 GHz from 2012 to 2014 and surveyed three patches of sky totaling 30 square degrees. We test the consistency of the lensing spectrum with a cold dark matter cosmology and reject the no-lensing hypothesis at a confidence of 10.9σ, including statistical and systematic uncertainties. We observe a value of A L = 1.33 ± 0.32 (statistical) ±0.02 (systematic) ±0.07 (foreground) using all polarization lensing estimators, which corresponds to a 24% accurate measurement of the lensing amplitude. Compared to the analysis of the first-year data, we have improved the breadth of both the suite of null tests and the error terms included in the estimation of systematic contamination.

However, the relationship between LSS and nocturnal leg cramps in the general population remains unclear. The purpose of this study was to investigate the prevalence and characteristics of nocturnal leg cramps in LSS in the community.Patients and Methods: 328 voluntary participants were enrolled in this study. The presence of LSS was assessed by a validated and self-administered diagnostic support tool. The presence of nocturnal leg cramps and neurological findings were evaluated by one experienced spine surgeon. To investigate the relationship between leg cramps and anatomical factors, the participants underwent an MRI scan, and the dural sac cross-sectional area (DCSA) at each lumbar intervertebral disc level was measured.Results: A total of 214 participants (65.2%) had nocturnal leg cramps, and 94 of 328 participants (28.7%) showed typical LSS symptoms. In the typical LSS symptom group, 31 participants (33.0%) had nocturnal leg cramps. In the atypical LSS symptom group, 83 participants (35.5%) had nocturnal leg cramps. There was no statistically significant difference in the prevalence of nocturnal leg cramps between the two groups. The narrowest DCSA (< 25 mm2 and 25– 49.4mm2) was statistically related to the presence of nocturnal leg cramp. Statistically significant differences in sensory disturbance and motor weakness were not observed between the subjects with and those without nocturnal leg cramps. Moreover, impaired PTR was statistically related to the presence of nocturnal leg cramp.Conclusion: The prevalence of nocturnal leg cramps did not differ with or without typical LSS symptoms in the community. The degree of dural tube compression that is determined by DCSA had a direct effect on the presence of nocturnal leg cramps. Thus, preventive interventions against subthreshold depression (StD), which is one of the key risk factors for the development of MDD, are important. The study developed a smartphone application (SPSRS) that improves depressive symptoms in people with StD by automatically presenting positive word stimuli during videos. The SPSRS application has the potential to improve depressive symptoms in people with StDs. However, whether it can immediately improve depressed moods in people with StD is unclear. The study presents a protocol for investigating the immediate effects of the SPSRS application intervention on depressed mood in people with StD.Methods: The study is designed as an open-label, parallel-group, and pilot randomized controlled trial (RCT). Thirty-two people who meet the criteria for StD (Beck Depression Inventory-II score of 10 points or more and fails to meet the diagnostic criteria for MDD) will be recruited and studied. It compares the immediate effects of the SPSRS application intervention (with positive word stimulus in a 10-minute video; n = 16) and YouTube application intervention (without positive word stimulus in 10-minute video; n = 16). The primary outcome is the change in the score for the Profile of Mood States 2nd Edition-Adult Short after 10 minutes of intervention. The secondary outcome is the change in the State–Trait Anxiety Inventory State after 10 min of intervention.Discussion: The pilot RCT is the first to evaluate the immediate effects of the SPSRS application intervention on depressed mood in people with StD. The primary endpoint was proportion of patients with successful maintenance of pain control in part 1 and long-term safety in part 2. Secondary endpoints included time to inadequate analgesia, rate of transition to the maintenance period, and discontinuation due to inadequate analgesia/adverse events (AEs), Brief Pain Inventory (BPI) pain severity, BPI pain interference, 36-item Short Form Health Survey (SF-36) score, and Western Ontario and McMaster Universities (WOMAC) index, Subjective Opioid Withdrawal Scale (SOWS), Clinical Opioid Withdrawal Scale (COWS), Dependency-2-A (D-2-A), and Dependency-2-B (D-2-B) questionnaires.Results: Of 130 patients (mean age, 63.6 years; women, 62.3%) in the dose-titration period, 95 entered the maintenance period; 60 of 83 who entered the long-term administration period completed it. The proportion of patients (95% confidence interval) with successful maintenance of pain control, transition to maintenance period, and discontinuation due to inadequate analgesia/AEs was 78.9% (69.4–86.6), 73.1% (64.6–80.5), and 21.1% (13.4–30.6), respectively. Time to inadequate analgesia could not be estimated. Changes from baseline in BPI, SF-36, and WOMAC index scores suggested improvements in pain relief and quality of life. Based on the SOWS, COWS, D-2-A and D-2-B questionnaires, no patient developed clinically relevant withdrawal syndrome or was ascertained to have developed drug dependence. Overall, the incidence of treatment-emergent AEs (TEAEs) was 93.8%; most common TEAEs were constipation (49.2%), nausea (42.3%), nasopharyngitis (34.6%), and somnolence (32.3%).Conclusion: These results demonstrate the efficacy and safety of S-8117 in Japanese patients with CNCP.Keywords: analgesics, chronic pain, Japan, opioids

Ltd., Osaka, Japan Background: Oxycodone is one of the options for the management of CLBP in patients with an inadequate response to other analgesics. However, oxycodone is not yet approved for noncancer pain in Japan. Here, we assessed the efficacy and long-term safety of S-8117, a controlled-release oxycodone formulation, for the management of Japanese CLBP patients. Patients and methods: An initial enriched enrollment randomized withdrawal, double-blind, placebo-controlled, 5-week phase III trial was conducted across 54 centers in Japan to assess the efficacy of S-8117 vs placebo in moderate-to-severe CLBP patients. Subsequently, a 52-week, open-label, single-arm study was conducted across 53 centers in Japan to evaluate the long-term safety of S-8117. The primary endpoint was the time to inadequate analgesic response during 35 days of the double-blind period. Secondary endpoints were the percentages of patients with inadequate analgesic response, discontinuation rate due to inadequate analgesic effects or AEs, and changes in scores of BPI severity, BPI pain interference, SF-36, and Roland-Morris Disability Questionnaire. Safety was assessed as the incidence of AEs and ADRs. Results: Of the 189 patients enrolled in the double-blind study, 130 patients who completed the initial titration period were randomized 1:1 to receive either S-8117 (n=62) or placebo (n=68). Baseline characteristics were comparable across the study groups. The time to inadequate analgesic response was significantly longer in patients treated with S-8117 than placebo (P=0.0095). Secondary endpoints corroborated the efficacy of S-8117 vs placebo. Overall, 478 AEs were reported in 73/75 patients in the long-term study. The most frequent ADRs were somnolence, constipation, and nausea. No case of drug dependence was reported in the long-term study. Conclusion: Short-term efficacy vs placebo and long-term safety of S-8117 were demonstrated for the management of Japanese patients with moderate-to-severe CLBP. The main purpose of this study was to assess the influence of the magnitude of dural tube compression on MRI on LSS symptoms at the cross-sectional and 1-year follow-up. Methods: This was a prospective cohort study of 459 participants who were assessed for LSS using a questionnaire and conventional MRI of the lumbar spine. After 1 year, 335 subjects (follow-up rate 73.0%) were assessed for LSS using the same questionnaire. The time course of the clinical subjective symptoms of LSS and the relationship between the said symptoms of LSS and magnitude of dural tube compression on MRI were analyzed in a cross-sectional and longitudinal fashion. Results: 1) The dural sac cross-sectional area (DCSA) decreased with age. 2) Severe dural tube compression had a strong influence on the presence of symptomatic LSS; however, 40%–70% of participants with severe dural tube compression did not show clinical symptoms of LSS. 3) At the 1-year follow-up, >50% of the LSS-positive participants in the initial year were reclassified as LSS negative, and 10% of the LSS-negative participants were reclassified as LSS positive. 4) The magnitude of the DCSA on MRI did not directly affect the presence of LSS at the 1-year follow-up. Conclusion: LSS symptoms were changeable. Anatomical dural tube compression on MRI did not predict the presence of clinical LSS symptoms at the 1-year follow-up. However, limited information exists on discrepancies between LSS symptoms and imaging findings and/or prognostic factors of LSS, as well as the relationship between changes in LSS symptoms and quality of life (QoL) during the natural course of LSS. The purpose of the current study was to clarify any changes in clinically diagnosed LSS at a one-year follow-up, and identify its prognostic factors, using a community-dwelling cohort.Participants and methods: In this study, the presence of LSS, its associated comorbidities, and the status of QoL pertaining to general health and low-back pain were assessed in 1,080 community-dwelling volunteers. The same assessment was carried out a year after the initial survey. Clinically diagnosis as LSS (LSS-positive) was determined by a validated diagnostic support tool in the form of a self-administered, self-reported history questionnaire. QoL was assessed using the 36-Item Short Form Health Survey and the Roland–Morris Disability Questionnaire.Results: Among subjects who were LSS-positive in the initial assessment, 54% were clinically diagnosed as negative for LSS (LSS-negative) after the one-year period, whereas 10% of those who were initially diagnosed as LSS-negative changed to LSS-positive during the same period. With the improvement or deterioration of LSS-positive/negative status, low-back pain-related QoL and some components of the 36-Item Short Form Health Survey similarly improved or deteriorated. Decisive prognostic factors of LSS-positive status were not determined at the one-year follow-up.Conclusion: Approximately half of the subjects who had initially been diagnosed as LSS-positive converted to LSS-negative after one year. Abstract

Purpose: High mortality and a limited performance of valvular surgery are typical features of infective endocarditis (IE) in octogenarians, even though surgical treatment is a major determinant of a successful outcome in IE. Methods: Data from the prospective multicentre ESC EORP EURO-ENDO registry were used to assess the prognostic role of valvular surgery depending on age. Results: As compared to < 80yo patients, ≥ 80yo had lower rates of theoretical indication for valvular surgery (49.1% vs. 60.3%, p < 0.001), of surgery performed (37.0% vs. 75.5%, p < 0.001), and a higher in-hospital (25.9% vs. 15.8%, p < 0.001) and 1-year mortality (41.3% vs. 22.2%, p < 0.001). By multivariable analysis, age per se was not predictive of 1-year mortality, but lack of surgical procedures when indicated was strongly predictive (HR 2.98 [2.43–3.66]). By propensity analysis, 304 ≥ 80yo were matched to 608 < 80yo patients. Propensity analysis confirmed the lower rate of indication for valvular surgery (51.3% vs. 57.2%, p = 0.031) and of surgery performed (35.3% vs. 68.4%, p < 0.0001) in ≥ 80yo. Overall mortality remained higher in ≥ 80yo (in-hospital: HR 1.50[1.06–2.13], p = 0.0210; 1-yr: HR 1.58[1.21–2.05], p = 0.0006), but was not different from that of < 80yo among those who had surgery (in-hospital: 19.7% vs. 20.0%, p = 0.4236; 1-year: 27.3% vs. 25.5%, p = 0.7176). Conclusion: Although mortality rates are consistently higher in ≥ 80yo patients than in < 80yo patients in the general population, mortality of surgery in ≥ 80yo is similar to < 80yo after matching patients. These results confirm the importance of a better recognition of surgical indication and of an increased performance of surgery in ≥ 80yo patients.

Abstract

International audience; AIMS: To evaluate the current management and survival of patients with left-sided infective endocarditis (IE) complicated by congestive heart failure (CHF) in the ESC-EORP European Endocarditis (EURO-ENDO) registry. METHODS AND RESULTS: Among the 3116 patients enrolled in this prospective registry, 2449 (mean age: 60 years, 69% male) with left-sided (native or prosthetic) IE were included in this study. Patients with CHF (n~=~698, 28.5%) were older, with more comorbidity and more severe valvular damage (mitro-aortic involvement, vegetations >10\,mm and severe regurgitation/new prosthesis dehiscence) than those without CHF (all p\,≤q\,0.019). Patients with CHF experienced higher 30-day and 1-year mortality than those without (20.5% vs. 9.0% and 36.1% vs. 19.3%, respectively) and CHF remained strongly associated with 30-day (odds ratio[OR] 2.37, 95% confidence interval [CI] [1.73-3.24; p\,10\,mm, severe valvular regurgitation and/or new prosthetic dehiscence, perivalvular complication, and prosthetic IE (OR 0.22, 95% CI 0.12-0.38; p\

Abstract

Aims Infective endocarditis (IE) is a life-threatening disease associated with high mortality and morbidity worldwide. We sought to determine how socioeconomic factors might influence its epidemiology, clinical presentation, investigation and management, and outcome, in a large international multicentre registry. Methods and results The EurObservational Programme (EORP) of the European Society of Cardiology EURO-ENDO (European Infective Endocarditis) registry comprises a prospective cohort of 3113 adult patients admitted for IE in 156 hospitals in 40 countries between January 2016 and March 2018. Patients were separated in three groups, according to World Bank economic stratification [group 1: high income (75.6%); group 2: upper-middle income (15.4%); group 3: lower-middle income (9.1%)]. Group 3 patients were younger [median age (interquartile range, IQR): group 1, 66 (53–75) years; group 2, 57 (41–68) years; group 3, 33 (26–43) years; P < 0.001] with a higher frequency of smokers, intravenous drug use, and human immunodeficiency virus infection (all P < 0.001) and presented later [median (IQR) days since symptom onset: group 1, 12 (3–35); group 2, 19 (6–54); group 3, 31 (12–62); P < 0.001] with a higher likelihood of developing congestive heart failure (13.6%, 11.1%, and 22.6%, respectively; P < 0.001) and persistent fever (9.8%, 14.2%, and 27.9%, respectively; P < 0.001). Among 2157 (69.3%) patients with theoretical indication for cardiac surgery, surgery was performed less frequently in group 3 patients (75.5%, 76.8%, and 51.3%, respectively; P < 0.001), who also demonstrated the highest mortality (15.0%, 23.0%, and 23.7%, respectively; P < 0.001). Conclusion Socioeconomic factors influence the clinical profile of patients presenting with IE across the world. Despite younger age, patients from the poorest countries presented with more frequent complications and higher mortality associated with delayed diagnosis and lower use of surgery.
