3. Women and Men: Sex Differences in Cardiovascular Risk Prevention

Abstract

Cardiovascular disease (CVD) is the leading cause of death in women, and knowledge of the clinical consequences of atherosclerosis and CVD in women has grown tremendously over the past 20 years. Research efforts have increased and many reports on various aspects of ischaemic heart disease (IHD) in women have been published highlighting sex differences in pathophysiology, presentation, and treatment of IHD. For the past three decades, dramatic declines in heart disease mortality for both men and women have been observed, especially in the > 65 years’ age group. But despite it according to the Global Burden of Disease, in 2004, CVD caused almost 32 % of deaths in women worldwide vs. 27 % in men. In Europe, 54 % of all females’ death are from CVD vs. 43 % in men. Recent evidence has emerged that recognizes new, potentially independent, CVD risk factors exclusive to women. In particular, common disorders of pregnancy, such as gestational hypertension and diabetes, as well as frequently occurring endocrine disorders in women of reproductive age are associated with accelerated development of CVD and impaired CVD free survival. With the recent availability of prospective studies comprising men and women, the equivalency of major risk factors prevalence and effects on CVD between men and women can be examined. Furthermore, female specific risk factors might be identified enabling early detection of apparently healthy women with a high lifetime risk of CVD. Therefore, we examined the available literature regarding the prevalence and effects of the traditional major risk factors for CVD in men and women.

Abstract

Aim The objective behind the Euro Heart Survey on diabetes and the heart was to study the prevalence of abnormal glucose regulation in adult patients with coronary artery disease (CAD). Methods and results The survey engaged 110 centres in 25 countries recruiting 4196 patients referred to a cardiologist due to CAD out of whom 2107 were admitted on an acute basis and 2854 had an elective consultation. Patient data were collected via a web-based case record form. An oral glucose tolerance test (OGTT) was used for the characterisation of the glucose metabolism. Thirty-one per cent of the patients had diabetes. An OGTT was performed on the 1920 patients without known diabetes, of whom 923 had acute and 997 had a stable manifestation of CAD, respectively. In patients with acute CAD, 36% had impaired glucose regulation and 22% newly detected diabetes. In the stable group these proportions were 37% and 14%. Conclusion This survey demonstrates that normal glucose regulation is less common than abnormal glucose regulation in patients with CAD. OGTT easily discloses the glucometabolic state and should be a routine procedure. The knowledge of glucometabolic state among these patients should influence their future management because it has great potential to improve the outcome.

Abstract

In developed countries chronic heart failure (CHF) is one of the main reasons of death and physical disability. CHF affects approximately 1-2% of the population and its prevalence is still increasing. Today ACE inhibitors, beta-blockers, diuretics are the first line drugs in treatment of CHF. Mildronate is a new drug optimizing cardiac energy metabolism and could be an effective approach in CHF patients' management. The aim of the study was to investigate the efficacy and safety of mildronate in treatment of CHF (NYHA III-IV funct. class) patients. 110 patients with CHF, stable on conventional treatment (diuretics, ACE inhibitors, beta-blokers, digoxin), were treated additionally with mildronate. Assessment was performed with clinical data, FC change, ECO-cardiography, 6-minute walk test. Mildronate showed to be an effective drug in complex treatment of chronic heart failure.
