30. Prenatal factors associated with neonatal survival of infants with congenital chylothorax

Abstract

Objectives:Congenital chylothorax is a rare disease and prognostic factors are key element in properly informing parents. This study aimed at determining the prenatal factors associated with neonatal survival in a cohort of liveborn infants with congenital chylothorax.Study design:Observational monocentric cohort study including all liveborn neonates consecutively admitted for congenital chylothorax.Results:Neonatal mortality was 32% (16/50). Prematurity (or birth weight), persistence of hydrops at birth and the absence of thoracoamniotic shunt procedure were significantly associated with mortality, whereas prenatal diagnosis of pleural effusion, side of pleural effusion, hydrops fetalis and amniodrainage were not. In case of prenatal diagnosis of hydrops fetalis, the reversal in utero of hydrops fetalis was significantly associated with survival (P=0.001). In case of thoracoamniotic shunting, the interval between thoracoamniotic shunting intervention and delivery was significantly longer for patients who survived (P=0.03).Conclusions:Thoracoamniotic shunting and reversal of hydrops significantly improves survival, whereas prematurity worsened outcome of liveborn infants with congenital chylothorax. Our data also suggest that the interval between thoracoamniotic shunting and birth appears to be crucial; the longer the interval, the more likely is the reversal of antenatal hydrops and neonatal survival.

Abstract

Introduction: l’incidence de l’atrésie de l’œsophage (AO) est mal connue et varie entre 1/2500 à 1/4600 naissances. Le but de ce travail est de présenter les données 2008 du registre national de l’AO (métropole plus DOM TOM). Méthodes: des fiches d’inclusion ont été remplies par tous les centres prenant en charge une AO et saisies et analysées par le centre de référence. Les DIM de chaque centre ont été contactés ainsi que 4 registres régionaux de malformations pour augmenter le nombre et l’exhaustivité des sources. Résultats: 142 nouveaux cas ont été déclarés pour l’année 2008 (incidence = 1/5824 naissances). Il existait une prédominance de garçons (n = 83), le poids de naissance moyen était de 2527 g avec un terme de 37 SA. Un diagnostic anténatal était possible dans 17 % des cas. Il y avait 15 atrésies de type I et 123 de type III. Le diagnostic était posé dans les 24h dans 89 % des cas et la chirurgie initiale réalisée avant 48h dans 94 % pour le type III. Le taux des malformations associées était de 53 % et la mortalité globale de 5 %. Conclusions: Ces premiers résultats montrent une incidence de l’AO similaire à celle observée par le registre Européen des malformations. Le diagnostic anténatal reste rare mais la prise en charge dans notre pays est précoce et la mortalité faible. [ABSTRACT FROM AUTHOR]

49. Robotic thoracic surgery: lessons learned from the first 1,000 procedures.

Abstract

Introduction: The aim of this study was to evaluate the impact of the thoracic robotic approach in a high-volume center regarding procedures and clinical outcomes after 1,000 procedures., Methods: In a single-center subset of the Epithor® database, a prospective cohort database of French thoracic surgery, we analyzed procedural characteristics and clinical outcomes from February 2014 to April 2023. A surgical technique for lung surgery was conducted with a four-arm closed chest with the port access approach and vascular sewing and knotting were preferred over stapling. Statistical analysis was performed using the Chi-2 test for discontinuous variables and the Mann-Whitney-Wilcoxon test for continuous variables. Tests were considered significant for a p -value <0.05., Results: Robotic thoracic surgery was used in anatomical lung resection in 85% of the cases. Over the study period, 1,067 patients underwent robotic surgery, of which 509 had lobectomies and 391 segmentectomies. In the segmentectomy group vs. lobectomy group we observed a shorter length of stay (9 ± 7 vs. 7 ± 5.6 days, p  < 0.001), a shorter surgery time (99 ± 24 vs. 116 ± 38 min, p  < 0.001) a lower conversion rate ( n  = 2 vs. n  = 17, p  = 0.004), and a lower complication rate (28% vs. 40%, p  = 0.009, mainly Clavien-Dindo II, 18% and 28%, respectively). For cancer treatment surgery, we found more previous cancer in the segmentectomy group (48% vs. 26%, p  < 0.001). We also observed a progressive change of lobectomy vs. segmentectomy from 80%/20% to 30%/70% over the 9 years., Discussion: A robotic platform is an appropriate tool to perform anatomical lung resection and especially to develop a safe and systematic approach to lung-sparing sub-lobar resection., Competing Interests: MD is official proctor for Intuitive Surgical. Abstract

We report the case of a 5-year-old girl who underwent left pneumonectomy for Ewing sarcoma of the lung. Two expandable prostheses were placed in the left hemi-thorax to prevent post-pneumonectomy syndrome and to protect the heart from radiotherapy. With a follow-up of 10 years, the procedure proved to be effective both on post-pneumonectomy syndrome and on cardiac protection., (© The Author(s) 2024.
