18. Associations between obstructive sleep apnea and cardiac troponin T levels: a meta-analysis

Abstract

Cardiac troponin T (cTnT) is a sensitive indicator of heart damage and an important clinical marker for acute myocardial infarction (MI). The recent development of a high-sensitivity cardiac troponin T (hs-cTnT) assay allows a more accurate diagnosis of serum cTnT level. There appears to be an association between Obstructive Sleep Apnea (OSA) and changes in serum cTnT level. However, existing research shows mixed results. Therefore, this meta-analysis was performed to evaluate the relationship between cTnT level and OSA. The Scopus, EMBASE, Web of Science, PubMed and Science Direct databases were searched without a lower time limit and until April 2020. In order to perform the meta-analysis, the heterogeneity of articles was examined using the I.sup.2 test, and subsequently a random effects model was applied. Data analysis was performed within the Comprehensive Meta-Analysis software (version 2). Six studies comprising of 1689 cases and 2171 controls were included in this meta-analysis. The odds ratio of cTnT level in patients with OSA is 1.25 (95% confidence interval: 0.97-1.61), indicating that OSA patients have 25% higher odds to develop cTnT level than people without OSA. Our study showed that OSA is associated with higher serum hs-cTnT level compared to individuals with no OSA. Given that elevated cTnT level can indicate cardiovascular disease, these individuals should be prioritized for screening, early detection and swift interventions. Abstract

Depression and anxiety are common problems in patients with cancer which may interfere with the daily activities of these patients. Acceptance and Commitment Therapy (ACT) is a type of third-wave behavioral that increases psychological flexibility in relation to personal events. The aim of this study was to investigate the effect of ACT on the psychological distress of patients with cancer. In this study, a systematic review was conducted by searching the PubMed, Embase, Science Direct, Cochrane, CINHAL, Google Scholar, Scopus, SID, Mag Iran databases with no lower time limit and until May 2020. A total of 15 studies entered the final review with the total sample size of 965 of which 470, and 495 were in the ACT, and the comparison groups respectively. The results of the study showed that ACT in all studies reduced the mean score of anxiety and depression after treatment, as well as during follow-up. This reduction was significant in most studies. The present systematic review study showed that ACT can be used as an appropriate intervention in improving the psychological state of patients with cancer. Abstract

Hierarchical psychopathology contributes to providing a broader picture of the links between emerging personality structures such as the DSM‐5/ICD‐11 trait models and clinical disorders. The present study aimed to predict the specific and general clinical symptoms by the less studied constructs of the ICD‐11 model (negative affectivity, detachment, dissociality, disinhibition, and anankastia). Data from 642 young adults from Iran (63% female, 18–34 years) were collected by three mental symptom scales and the Personality Inventory for DSM‐5 (PID‐5), which was recently used to harmonize the constructs of the DSM‐5 and ICD‐11 trait models. Multiple linear regressions showed that the ICD‐11 model significantly predicted both the specific clinical symptoms (ranging from R2 = 0.15 to 0.40) and the general factor of clinical symptoms extracted by exploratory factor analysis (R2 = 0.40, all p < 0.001). Negative affectivity was the strongest construct correlated with both the specific symptoms (ranging from β = 0.36 to 0.69) and the general symptom factor (β = 0.59, all p < 0.001). Because the ICD‐11 trait model is a practical structure related to the clinical psychopathology in young adults, screening for maladaptive traits can help clinicians in case formulation for diagnosis and treatment. Abstract

Background: The prevalence of sexual distress in women with a history of breast surgery is a critical issue that warrants investigation. Objectives: This study aimed to predict sexual distress in women with a history of breast surgery based on their self-compassion, considering the mediating roles of body image and emotion regulation abilities. Methods: This correlational descriptive study was conducted in 2022 - 2023. The participants comprised 160 patients with a history of breast surgery from Hazrat Rasool Akram, Bazarganan, and Tarbiat Modares hospitals, and Sepid Psychotherapy Clinic in Tehran. A purposive sampling method was employed. Questionnaires were distributed and completed both in-person and online. The instruments used included the Female Sexual Distress Scale-Revised (FSDS-R), Self-Compassion Scale (SCS), Difficulties in Emotion Regulation Scale (DERS), and Body Image Scale (BIS). Data were analyzed using Pearson correlation coefficients and structural equation modeling in SPSS 25 and R software. Results: The research results demonstrated satisfactory model fit. A positive and significant relationship was found between self-compassion and sexual distress, mediated by body image, in participants with a negative history of breast surgery (P < 0.01). Additionally, difficulties in emotion regulation significantly and negatively mediated the relationship between self-compassion and sexual distress (P < 0.01). Conclusions: The findings suggest that self-compassion and difficulties in emotion regulation, with the mediating role of body image, can predict sexual distress in women post-mastectomy. Self-compassion may shield against the impacts of mastectomy by fostering an empathetic and accepting view of oneself. This perspective helps patients adapt more easily to appearance changes, experience fewer emotional difficulties, and endure less sexual distress. Thus, to mitigate sexual problems, psychotherapists are encouraged to employ approaches that emphasize self-compassion, such as acceptance and commitment therapy. [ABSTRACT FROM AUTHOR]

45. How are poor sleepers with other clinical conditions affected by maladaptive personality traits? A neural network-based analysis.

Abstract

Background: Psychopathology research mainly focused on the cross-sectional and longitudinal associations between personality and psychiatric disorders without considering the moment-to-moment dynamics of personality in response to environmental situations. The present study aimed to both cluster a young sample according to three mixed clinical conditions (poor sleep quality, depression, and somatization) and to predict the derived clusters by maladaptive personality traits and sex differences using a deep machine learning approach. Methods: A sample of 839 adults aged 18-40 years (64% female) from the west of Iran were clustered according to the mixed clinical conditions using the cluster analysis techniques. An Artificial Neural Network (ANN) modeling is used to predict the derived clusters by maladaptive personality traits and biological gender. A receiver operating characteristic (ROC) curve was used to identify independent variables with high sensitivity specific to the derived clusters. Results: The cluster analysis techniques suggested a fully stable and acceptable four-cluster solution for Depressed Poor Sleepers, Nonclinical Good Sleepers, Subclinical Poor Sleepers, and Clinical Poor Sleepers. The ANN model led to the identification of one hidden layer with two hidden units. The results of Area under the ROC Curve were relatively to completely acceptable, ranging from.726 to.855. Anhedonia, perceptual dysregulation, depressivity, anxiousness, and unusual beliefs are the most valuable traits with importance higher than 70%. Conclusion: The machine learning approach can be well used to predict mixed clinical conditions by maladaptive personality traits.
