Abstract

To determine the diagnostic accuracy of lung ultrasound and chest X-ray in diagnosing Acute Pulmonary Edema (APE) in patients presenting with acute dyspnea in emergency department. This study was conducted at the emergency department of Shifa International Hospital, Islamabad from 31st July 2018 to 31st January 2019. A total of 225 patients presenting with dyspnoea and satisfying inclusion criteria were assessed by clinical examination, lung USG and chest X-ray. The results of chest X-ray and lung USG were compared with clinical diagnosis. The current study revealed sensitivity of chest X-ray and LUS ( 60.16% versus 91.05%), specificity (66.67% versus 91.18%), positive predictive value (68.52% versus 92.56%) and the negative predictive values (58.12% versus 89.42%) respectively. 8. Neuronal N-acetyl-β-D-glucosaminidase. Evidence for its biosynthesis in vitro

Abstract

Neuronal cell bodies, isolated in bulk from 8-day-old rat cerebral cortices, were incubated in the presence of a 3H-labelled amino acid mixture, and subcellular fractions isolated by differential centrifugation. The particulate fractions were frozen/thawed in 0.20 M-sucrose/0.1 M-KCl [Selling et al. (1973) Biochim. Biophys. Acta 315, 128-146] and the profiles of acid-insoluble radioactivity and N-acetyl-beta-D-glucosaminidase (glucosaminidase) activity compared in the resulting non-sedimentable fractions by DEAE-cellulose chromatography and cellulose acetate electrophoresis. Radioactivity and glucosaminidase activity co-migrated to a significant extent. Electrophoresis revealed that after 1 min of incubation 42% of the radioactivity of the non-sedimentable microsomal fraction after freezing and thawing co-migrated with an intensely fluorescent band of glucosaminidase activity. Since the pellet fraction obtained on freezing/thawing the microsomal fraction contained up to 75% of the RNA, 95% of the radioactivity and 45% of the glucosaminidase, a detailed study of the association between its radioactivity and nascent glucosaminidase activity was undertaken. After 1 and 2 min of incubation, followed by centrifugation of the microsomal pellet on 35-60% (w/v) sucrose density gradients, radioactivity and glucosaminidase activity exhibited parallel profiles in the region of heavy polyribosomes and at the top of the gradient which contains spontaneously released nascent polypeptide chains. DEAE-cellulose chromatography of these chains revealed glucosaminidase A to be the principal nascent glucosaminidase component, with glucosaminidases B and C as minor peaks. After 2 min of incubation, all of the glucosaminidase components appeared labelled, and glucosaminidase A exhibited two distinct sub-components. The pattern of glucosaminidase labelling in the soluble and microsomal fractions suggested that newly formed glucosaminidase molecules traverse both the cellular sap and the lumen of the endoplasmic reticulum. Only glucosaminidase A reacted specifically with concanavalin A and radioactive glucosaminidase A could be successfully regenerated by treatment with alpha-methyl-D-glucoside. Glucosaminidase A and a substantial portion of the radioactivity associating with it could be readily converted into glucosaminidase B by re-chromatography on DEAE-cellulose and by reaction of the concanavalin A-glucosaminidase A complex with methyl glucosides.

Abstract

Background: The American Academy of Pediatrics advises that the nutrition of preterm infants should target a body composition similar to that of a fetus in utero. Still, reference charts for intrauterine body composition are missing. Moreover, data on sexual differences in intrauterine body composition during pregnancy are limited., Objectives: The objective of this study was to create reference charts for intrauterine body composition from 30 to 36+6 weeks postconception and to evaluate the differences between sexes., Methods: In this single-center retrospective study, data from 197 normal developing fetuses in late gestation was acquired at 3T magnetic resonance imaging (MRI) scans, including True Fast Imaging with Steady State Free Precession and T 1 -weighted 2-point Dixon sequences covering the entire fetus. Deep convolutional neural networks were utilized to automatically segment the fetal body and subcutaneous adipose tissue. The fetus's body mass (BM), fat signal fraction (FSF), fat mass (FM), FM percentage (FM%), fat-free mass (FFM), and FFM percentage (FFM%) were calculated. Using the Generalized Additive Models for Location, Scale, and Shape (GAMLSS) method, reference charts were created, and sexual dimorphism was examined using analysis of covariance (ANCOVA). A P value <0.05 was deemed significant., Results: Throughout late gestation, BM, FSF, FM, FM%, and FFM increased, while the FFM% decreased. Reference charts for gestational age and sex-specific percentiles are provided. Males exhibited significantly higher BM (7.2%; 95% confidence interval [95% CI]: 1.9, 12.4), FFM (8.8%; 95% CI: 5.8, 11.9), and FFM% (1.7%; 95% CI: 1, 2.4) and lower FSF (-3.6%; 95% CI: -5.6, -1.8) and FM% (-1.7%; 95% CI: -2.4, -1), (P < 0.001) compared with females, with no significant difference in FM between sexes (P = 0.876)., Conclusions: MRI-derived intrauterine body composition growth charts are valuable for tracking growth in preterm infants. Abstract

The gold standard to estimate muscle mass and quality is computed tomography (CT) scan. Lower mass and density (intramuscular fat infiltration) of skeletal muscles are markers of sarcopenia, associated with increased mortality risk, impaired physical function, and poorer prognosis across various populations and medical conditions. We aimed to describe standard reference values in healthy population, prospective kidney donors, and correlate clinical parameters to muscle mass and density. Included in the cohort 384 consecutive kidney donors. Mean age was 44.6 ± 11.5 (range 18.4-74.2), 46% were female and mean BMI was 25.6 ± 3.8 kg/m 2 . Our quantified reference values for psoas cross -sectional area (CSA) index at L3 level (males/females respectively) were 6.3 ± 1.8 and 4.8 ± 1.9 cm 2 /m 2 , and density was 46.1 ± 5 and 41 ± 5 HU at that level. Older age (standardized beta coefficient - 0.12, p = 0.04), sex (- 0.32, p < 0.001) and BMI (0.17, p = 0.002) were significantly associated with CSA index of psoas at L3. Density, however, was associated with triglycerides level (- 0.21, p < 0.001), in addition to age (- 0.22, p < 0.0001), sex (- 0.27, p < 0.001) and BMI (- 0.1, p = 0.05). Our study validates the normative values of psoas muscle mass and density in healthy individuals and suggests correlations with clinical parameters. We demonstrate the significance of measuring not only the mass of the muscle, but also its density, as it has a valid association with metabolic parameters, including BMI and lipid level, even in healthy individuals and in the normal range of the tests., (© 2024. 13. COVID-19 Infection Presenting as an Isolated Severe Acute Liver Failure.

Abstract

The clinical features of severe acute respiratory syndrome-coronavirus disease 2019 (SARS-COVID-19) infection range from mild upper respiratory symptoms to severe acute respiratory failure. Among other less common features are diarrhea, nausea, vomiting, elevated liver enzymes, and acute kidney injury. We present a case of a 49-year-old female with no preexisting liver disease who presented with weakness and dizziness for one week. Initial investigations revealed acute liver failure (ALF) and positive COVID-19 on polymerase chain reaction (PCR) testing. The patient did not have any upper or lower respiratory symptoms, and extensive workup to look for other etiologies of acute liver failure was unremarkable. She eventually deteriorated to decompensated liver failure and was transitioned to comfort measures only. Liver injury is a well-documented phenomenon associated with COVID-19 infection. Some of the common pathophysiological mechanisms include direct liver injury, immune-mediated liver damage due to the severe inflammatory response, ischemic injury, endothelial disruption, and coagulopathy. Our case uniquely highlights that SARS-COVID-19 infection may have the potential to solely affect hepatocytes without the classic severe acute respiratory distress syndrome. Abstract

B-cell-depleting agents are among the most commonly used drugs to treat haemato-oncological and autoimmune diseases. They rapidly induce a state of peripheral B-cell aplasia with the potential to interfere with nascent vaccine responses, particularly to novel antigens. We have examined the relationship between B-cell reconstitution and SARS-CoV-2 vaccine responses in two cohorts of patients previously exposed to B-cell-depleting agents: a cohort of patients treated for haematological B-cell malignancy and another treated for rheumatological disease. B-cell depletion severely impairs vaccine responsiveness in the first 6 months after administration: SARS-CoV-2 antibody seroprevalence was 42.2% and 33.3% in the haemato-oncological patients and rheumatology patients, respectively and 22.7% in patients vaccinated while actively receiving anti-lymphoma chemotherapy. After the first 6 months, vaccine responsiveness significantly improved during early B-cell reconstitution; however, the kinetics of reconstitution was significantly faster in haemato-oncology patients. The AstraZeneca ChAdOx1 nCoV-19 vaccine and the Pfizer BioNTech 162b vaccine induced equivalent vaccine responses; however, shorter intervals between vaccine doses (<1 m) improved the magnitude of the antibody response in haeamto-oncology patients. In a subgroup of haemato-oncology patients, with historic exposure to B-cell-depleting agents (>36 m previously), vaccine non-responsiveness was independent of peripheral B-cell reconstitution. Abstract

There is an increased interest in the role of magnesium ions in clinical medicine, nutrition and physiology. The characteristics of the binding of magnesium and calcium ions to various components, macromolecules and biological membranes are described. Magnesium affects many cellular functions, including transport of potassium and calcium ions, and modulates signal transduction, energy metabolism and cell proliferation. The mechanism of cellular uptake and efflux of magnesium, its intracellular transport, intestinal absorption, renal excretion and the effect of hormones on these are reviewed. Magnesium deficiency is not uncommon among the general population: its intake has decreased over the years especially in the western world. The magnesium supplementation or intravenous infusion may be beneficial in various diseased states. Of special interest is the magnesium status in alcoholism, eclampsia, hypertension, atherosclerosis, cardiac diseases, diabetes, and asthma. The development of instrumentation for the assay of ionized magnesium is reviewed, as are the analytical procedures for total magnesium in blood and free magnesium in the cytosol. The improved procedures for the assay of different magnesium states are useful in understanding the role of magnesium in health and disease.

Abstract

In this study we compare the uptake of spermine by mitochondria from Ehrlich ascites tumour cells with those from rat liver. The spermine uptake was membrane potential-dependent in both systems, but there were significant quantitative differences. The amount of spermine taken up by tumour mitochondria as a function of time was twice as large and a Lineweaver-Burk analysis showed that the Vmax was more than 6 times higher in tumour mitochondria. As in liver mitochondria, the uptake of spermine by ascites mitochondria was competitively inhibited by the polyamine spermidine and by putrescine. Mg2 was also inhibitory, as were the monovalent cations K+, Na+, Li+ and choline.

Abstract

Influence of prolonged ethanol ingestion on the nucleocytoplasmic transport of RNA has been examined in a cell-free system. The nucleocytoplasmic transport of RNA was energy- and temperature-dependent. Ethanol treatment of rats for 10 weeks led to a significant increase in the release of isotopically labelled nuclear RNA from the nucleus, suggesting a partial loss of nuclear restrictive control. Results from crossover experiments led to the conclusion that the observed effect of ethanol ingestion was mediated through factors present in the cytosol as well as in the nuclear fraction.

Abstract

The distribution and amino acid incorporative activities of free and membrane-bound ribosomes in neuronal perikarya of rat cerebral cortex at successive stages of postnatal development were investigated. The relative proportion of neuronal membrane-bound ribosomes increased significantly between 6 and 18 days of age, reaching 50% of the total ribosomal population around day 18. In contrast to the neuronal fraction, the membrane-bound ribosomes in unfractionated cerebral cortex decreased from 50% at birth to 35% in 18-day-old pups. When tested in a cell-free amino acid incorporation system the activities of both free and membrane-bound ribosomes increased up to day 10 post partum. However, whereas the activity of free ribosomes reached a constant level at this age, that of the membrane-bound fraction continued to rise until 18th day after birth. Crossover experiments employing homologous or heterologous cell sap fractions showed that the preparation from the 18-day-old animals was more efficient in supporting protein synthesis in vitro, if compared with similar preparation from the neonatal animals. This was attributed to an enhanced aminoacylation of transfer RNA in neuronal cell sap of older compared with younger rats. The observed developmental alterations in the distribution and activities of free and membrane-bound ribosomes are discussed in relation to changes that occur in morphology and function of brain during the early postnatal period.

Ethanol or 1,3-butanediol ingestion during pregnancy and lactation results in a marked inhibition of neuronal protein synthesis in the 18-day-old rat progeny, however the same treatment had a stimulatory effect on the amino acid incorporation by neuronal perikarya of 8-day-old pups. Liver protein synthesis in vitro, on the other hand, was enhanced in both 8-day and 18-day-old pups as a consequence of maternal ethanol consumption. This was attributable to increased amino-acylation in the liver pH 5 enzyme fraction.

Ethanol ingestion by weanling rats results in an inhibition of protein synthetic activity of cerebral membrane-bound ribosomes. Among various amino acids tested, the incorporation of serine was inhibited to a maximum degree. In contrast to membrane-bound ribosomes, the activity of free ribosomes was enhanced as a consequence of ethanol treatment. Aminoacylation of tRNA in pH 5 fraction was also reduced in the brain cortices of ethanol-fed animals.

Abstract

Rough endoplasmic reticulum (RER) prepared from bulk-isolated neuronal perikarya of rat brain cortex of different postnatal ages was found to be active in vectorial discharge of nascent proteins through the membrane; this activity increased with the increasing age of animals and reached maximal values in adults. RER isolated from whole cortical tissue (containing all cell types) exhibited vectorial release only up to 18 days of age; the preparation from adult animals was essentially devoid of secretory activity. Controlled proteolysis of various preparations suggested that in neuronal RER of 8-day-old rats the proportion of nascent proteins operationally retained in the intravesicular space was about twice that retained by cortical preparations. For the purpose of comparison, these parameters were studied also in liver RER.

Abstract

The influence of chronic ethanol ingestion on the protein synthetic activities of free and membrane-bound ribosomes was studied in vitro in the liver of young rat. Ethanol administration caused about 120% increase in the amino acid incorporative activity of free ribosomes. In contrast, membrane-bound ribosomes from the livers of ethanol-fed animals exhibited a 25% decline in their protein synthetic activity. This decline in the activity of membrane-bound ribosomes was abolished after dissolution of membranes by Triton X-100. Evidence was obtained suggesting that the observed alterations in protein synthesis due to ethanol intake are attributable to changes in pH5 enzyme fraction, altered amount of ribosome-associated mRNA and a damage to the endoplasmic reticulum membranes.

Abstract

Neuronal cell bodies, isolated in bulk from 8-day-old rat cerebral cortices, were incubated in the presence of a 3H-labelled amino acid mixture, and subcellular fractions isolated by differential centrifugation. The particulate fractions were frozen/thawed in 0.20 M-sucrose/0.1 M-KCl [Selling et al. (1973) Biochim. Biophys. Acta 315, 128-146] and the profiles of acid-insoluble radioactivity and N-acetyl-beta-D-glucosaminidase (glucosaminidase) activity compared in the resulting non-sedimentable fractions by DEAE-cellulose chromatography and cellulose acetate electrophoresis. Radioactivity and glucosaminidase activity co-migrated to a significant extent. Electrophoresis revealed that after 1 min of incubation 42% of the radioactivity of the non-sedimentable microsomal fraction after freezing and thawing co-migrated with an intensely fluorescent band of glucosaminidase activity. Since the pellet fraction obtained on freezing/thawing the microsomal fraction contained up to 75% of the RNA, 95% of the radioactivity and 45% of the glucosaminidase, a detailed study of the association between its radioactivity and nascent glucosaminidase activity was undertaken. After 1 and 2 min of incubation, followed by centrifugation of the microsomal pellet on 35-60% (w/v) sucrose density gradients, radioactivity and glucosaminidase activity exhibited parallel profiles in the region of heavy polyribosomes and at the top of the gradient which contains spontaneously released nascent polypeptide chains. DEAE-cellulose chromatography of these chains revealed glucosaminidase A to be the principal nascent glucosaminidase component, with glucosaminidases B and C as minor peaks. After 2 min of incubation, all of the glucosaminidase components appeared labelled, and glucosaminidase A exhibited two distinct sub-components. The pattern of glucosaminidase labelling in the soluble and microsomal fractions suggested that newly formed glucosaminidase molecules traverse both the cellular sap and the lumen of the endoplasmic reticulum. Only glucosaminidase A reacted specifically with concanavalin A and radioactive glucosaminidase A could be successfully regenerated by treatment with alpha-methyl-D-glucoside. Glucosaminidase A and a substantial portion of the radioactivity associating with it could be readily converted into glucosaminidase B by re-chromatography on DEAE-cellulose and by reaction of the concanavalin A-glucosaminidase A complex with methyl glucosides.
