Abstract

Objective To assess how the returns on investment from correcting refractive errors and cataracts in low- and middle- income countries compare with the returns from other global development interventions. Methods We adopted two complementary approaches to estimate benefit-cost ratios from eye health investment. First, we systematically searched PubMed[R] and Web of Science[TM] on 14 August 2023 for studies conducted in low-and-middle-income countries, which have measured welfare impacts associated with correcting refractive errors and cataracts. Using benefit-cost analysis, we compared these impacts to costs. Second, we employed an economic modelling analysis to estimate benefit-cost ratios from eye health investments in India. We compared the returns from eye health to returns in other domains across global health and development. Findings We identified 21 studies from 10 countries. Thirteen outcomes highlighted impacts from refractive error correction for school students. From the systematic review, we used 17 out of 33 outcomes for benefit- cost analyses, with the median benefit- cost ratio being 36. The economic modelling approach for India generated benefit-cost ratios ranging from 28 for vision centres to 42 for school eye screening, with an aggregate ratio of 31. Comparing our findings to the typical investment in global development shows that eye health investment returns six times more benefits (median benefit-cost ratio: 36 vs 6). Conclusion Eye health investments provide economic benefits with varying degrees based on the intervention type and location. Our findings underline the importance of incorporating eye health initiatives into broader development strategies for substantial societal returns. Objectif Comparer les retours sur investissement d'une correction des defauts de refraction et des cataractes dans les pays a revenu faible et intermediaire avec ceux d'autres interventions liees au developpement mondial. Methodes Nous avons adopte deux approches complementaires afin d'estimer les rapports couts/avantages d'un investissement dans la sante oculaire. Tout d'abord, nous avons passe au crible PubMed[R] et Web of Science[TM] le 14 aout 2023, a la recherche d'etudes menees dans des pays a revenu faible et intermediaire sur l'impact d'une correction des defauts de refraction et des cataractes, puis nous avons procede a une analyse couts/avantages dans le but de comparer cet impact aux depenses engendrees. Ensuite, nous avons effectue une analyse par modelisation economique afin de determiner les rapports couts/ avantages des investissements dans la sante oculaire en Inde. Enfin, nous avons compare les retours sur investissement dans ce domaine avec ceux d'autres domaines lies a la sante et au developpement a l'echelle mondiale. Resultats Nous avons identifie 21 etudes portant sur 10 pays. Parmi les resultats obtenus, 13 mettaient en evidence l'impact qu'une correction des defauts de refraction representait pour les etudiants. A l'issue de notre revue systematique, nous avons utilise 17 des 33 resultats pour nos analyses couts/avantages, le rapport median s'elevant a 36. Lapproche par modelisation economique pour l'Inde a debouche sur des rapports couts/avantages allant de 28 pour les centres de la vision a 42 pour le depistage des troubles de la vue en milieu scolaire, avec une proportion globale de 31. En comparant nos resultats avec les depenses classiques en matiere de developpement mondial, nous avons constate qu'investir dans la sante oculaire permettait de multiplier les avantages par six (rapport couts/avantages median: 36 contre 6). Conclusion Les investissements dans la sante oculaire s'accompagnent d'avantages economiques a differents niveaux, en fonction du lieu et du type d'intervention. Nos resultats montrent a quel point l'integration d'initiatives de sante oculaire au sein de strategies de developpement plus vastes est importante dans la realisation de benefices considerables pour la societe. Objetivo Evaluar la rentabilidad de la inversion en la correccion de los defectos de refraccion y las cataratas en los paises de ingresos bajos y medios en comparacion con la rentabilidad de otras intervenciones de desarrollo a escala mundial. Metodos Se adoptaron dos enfoques complementarios para estimar la relacion coste-beneficio de la inversion en salud ocular. En primer lugar, se realizo una busqueda sistematica en PubMed[R] y Web of Science[TM] el 14 de agosto de 2023 sobre estudios realizados en paises de ingresos bajos y medios, que han medido los efectos en el bienestar asociados a la correccion de los errores de refraccion y las cataratas. Mediante el analisis beneficio-coste, se compararon estos efectos con los costes. En segundo lugar, se empleo un analisis de modelos economicos para estimar la relacion beneficio-coste de las inversiones en salud ocular en la India. Asimismo, se compararon las rentabilidades de la salud ocular con las de otros sectores de la salud y el desarrollo mundiales. Resultados Se identificaron 21 estudios de 10 paises. Trece resultados destacaron los efectos de la correccion de los defectos de refraccion en estudiantes. A partir de la revision sistematica, se utilizaron 17 de los 33 resultados para los analisis de beneficio-coste, en los que la mediana de esta relacion fue de 36. El enfoque de modelacion economica para la India genero relaciones beneficio-coste que van de 28 para los centros oftalmologicos a 42 para el cribado ocular en las escuelas, con una relacion agregada de 31. Si se comparan los resultados obtenidos con la inversion tipica en desarrollo mundial, se observa que la inversion en salud ocular genera una rentabilidad seis veces mayor (relacion beneficio-coste mediana: 36 frente a 6). Conclusion Las inversiones en salud ocular aportan beneficios economicos de diversa magnitud segun el tipo de intervencion y la ubicacion. Abstract

Purpose: To evaluate rural community-based eye care models from the perspective of community ophthalmology experts and suggest sustainable technological solutions for enhancing rural eye care delivery. Methods: A semi-structured descriptive survey, using close-ended and open-ended questions, was administered to the experts in community ophthalmology sourced through purposive sampling. The survey was self-administered and was facilitated through online platforms or in-person meetings. Uniform questions were presented to all participants, irrespective of their roles. Results: Surveyed participants (n = 22 with 15 from India and 7 from Nepal) in high-volume tertiary eye hospitals faced challenges with resources and rural outreach. Participants had mixed satisfaction with pre-operative screening and theatre resources. Delayed presentations and inexperienced surgeons contributed to the surgery complications. Barriers to rural eye care included resource scarcity, funding disparities, and limited infrastructure. In rural/peri-urban areas 87% of participants agreed with providing primary eye care services, with more than 60% of the experts not in agreement with the makeshift center model of eye care delivery. Key components for an effective eye care model are sustainability, accessibility, affordability, and quality. These can be bolstered through a healthcare management platform and a human-chain supply distribution system. Conclusion: Tailored interventions are crucial for rural eye care, emphasizing the need for stronger human resources, optimized funding, and community awareness. Addressing challenges pertinent to delayed presentation and surgical training is vital to minimizing complications, especially with advanced cataracts. Enhancements in rural eye care demand a comprehensive approach prioritizing accessibility, affordability, and consistent quality. Abstract

Primary congenital glaucoma (PCG) occurs in children due to developmental abnormalities in the trabecular meshwork and anterior chamber angle. Previous studies have implicated rare variants in CYP1B1, LTBP2, and TEK and their interactions with MYOC, FOXC1, and PRSS56 in the genetic complexity and clinical heterogeneity of PCG. Given that some of the gene-encoded proteins are localized in the centrosomes (MYOC) and perform ciliary functions (TEK), we explored the involvement of a core centrosomal protein, CEP164, which is responsible for ocular development and regulation of intraocular pressure. Deep sequencing of CEP164 in a PCG cohort devoid of homozygous mutations in candidate genes (n = 298) and controls (n = 1757) revealed CEP164 rare pathogenic variants in 16 cases (5.36%). Co-occurrences of heterozygous alleles of CEP164 with other genes were seen in four cases (1.34%), and a physical interaction was noted for CEP164 and CYP1B1 in HEK293 cells. Cases of co-harboring alleles of the CEP164 and other genes had a poor prognosis compared with those with a single copy of the CEP164 allele. We also screened INPP5E, which synergistically interacts with CEP164, and observed a lower frequency of pathogenic variants (0.67%). Abstract

Purpose: To document the spectrum and magnitude of eye disorders and visual impairment in the Dongaria--a Particularly Vulnerable Tribal Group in the Rayagada district of Odisha, India. Methods: A door-to-door screening protocol included a record of basic health parameters, visual acuity for distance, and near and flashlight examination of the eyes. Spectacles were dispensed to those who improved; those who failed the screening were referred to fixed (primary and secondary) eye care centers. Results: We examined 89% (n = 9872/11,085) of people who consented for screening. The mean age was 25.5 ± 18.8 years; 55% (n = 5391) were female; 13.8% (n = 1361) were under-five children, and 39% (n = 3884) were 6 to 16 years. 86% (n = 8515) were illiterate. 12.4% (n = 1224) were visually impaired, of which 9.9% had early moderate VI, and 2.5% had severe VI and blindness. Uncorrected refractive error was detected in 7.5% (n = 744) and cataracts in 7.6% (n = 754); among the adults, 41.5% (n = 924/2227) had presbyopia. In children, 20% (n = 790) had vitamin A deficiency, 17% (n = 234) had global acute malnutrition, and 18% (n = 244) were stunted for their age. Almost two-thirds (62%, n = 6144) confirmed habitual intake of alcohol, and 4% (n = 389) of adults had essential hypertension. Following the screening, 43.5% (n = 837) of referred patients reported to the fixed centers, and 55% (134/243) of people advised underwent cataract surgery. Spectacles were dispensed to 1496 individuals. Conclusion: Visual impairment and malnutrition are high in Dongaria indigenous community. Abstract

BACKGROUND: To report the 15-year incidence rate of pseudo-exfoliation (PXF), PXF glaucoma and regional variation among rural participants in the Andhra Pradesh Eye Disease Study (APEDS) III. METHODS: This population-based longitudinal study was carried out at three rural study sites. Individuals of all ages who participated at baseline with a mean 15-year follow-up visit were included. Detailed Comprehensive ophthalmic examination was performed on all participants. The main outcome measure was development of PXF during the follow-up period in participants who were phakic in one or both eyes without PXF at baseline. RESULTS: Among 5395 participants, 5108 (94.6%) met the inclusion criteria. There were 93 (1.82%; 95% confidence interval (CI), 1.47-2.22) cases of incident PXF. 48. Fifteen-Year Incidence Rate of Primary Angle Closure Disease in the Andhra Pradesh Eye Disease Study

Abstract

PURPOSE: To report on the 15-year incidence of primary angle closure disease (PACD) among participants aged ≥40 years in rural southern India DESIGN: Population-based longitudinal incidence rate study METHODS: Setting: 3 rural study centres. STUDY POPULATION: Phakic participants aged ≥40 years who participated in both examination time points. OBSERVATION PROCEDURES: All participants at the baseline and at the mean 15-year follow-up visit underwent a detailed interview, anthropometry, blood pressure measurement, and comprehensive eye examination. Automated perimetry was attempted based on predefined criteria. Main outcome measures included development of any form of PACD, as defined by the International Society for Geographical and Epidemiological Ophthalmology (ISGEO), during the follow-up period in phakic participants, who did not have the disease at baseline. RESULTS: We analyzed data obtained from 1,197 (81.4% out of available 1,470) participants to calculate the incidence of the disease. The mean age (standard deviation) of the study participants at the baseline was 50.2 (8.1) years, with 670 male (45.5%) and 800 female (54.4%) participants. The incidence rate per 100 person-years (95% confidence interval) for primary angle closure suspect, primary angle closure, and primary angle closure glaucoma was 8.8 (8.4, 9.2), 6.2 (5.9, 6.6), and 1.6 (1.4, 1.8), respectively. Thus, the incidence of all forms of PACD was 16.4 (15.9, 17) per 100 person-years. On logistic regression analysis, female gender was a significant risk factor whereas presence of myopia was protective. CONCLUSIONS: This study reports long-term incidence of PACD from rural India. It has implications for eye health care policies, strategies, and planning.
