Abstract

Which patients will benefit most from hip arthroscopy? Careful patient selection and conservative indications, such as patients with an alpha angle of 60° or greater or a lateral center-edge angle of 40° or greater who fail a trial of conservative treatment, may benefit from hip arthroscopy for femoroacetabular impingement (FAI). In female patients in particular, a lower body mass index (BMI) will predict the most benefit from arthroscopic treatment. That said, patients with a higher BMI can also substantially improve after treatment of FAI. The true art of medicine is determining indications for an individual patient in addition to providing evidence-based counseling and education. We must not forget that sometimes "any improvement" can be a good outcome for a patient who is in pain. [ABSTRACT FROM AUTHOR]

Abstract

Introduction: Dislocations of the proximal tibiofibular joint (PTFJ) are a relatively rare orthopedic injury. They are often repaired with open reduction internal fixation utilizing a transfixation screw. Limited data are available concerning alternative repair methods. Case Report: This report details a 46-year-old male who presented with an open Type IIIA comminuted mid-shaft tibia and segmental fibula fractures after being struck by a car traveling 70mph. Computed tomography confirmed comminuted and displaced tibia and fibula fractures with PTFJ dislocation as well as air tracking through the anterior compartment of the leg communicating with the knee joint. He underwent open reduction internal fixation with intramedullary nailing of his tibial shaft fracture and PTFJ syndesmosis repair with two suture buttons. X-ray at post-operative follow-up 2 years later showed complete healing without evidence of failure or hardware loosening. Conclusion: This case illustrates a patient treated with suture buttons, which provides an alternative option in treating PTFJ dislocations with a reported decreased likelihood of hardware removal and faster return to work.

Abstract

Introduction: With the increasing number of children and adolescents participating in sports, pathologies once reserved for high-level athletes are now emerging in this younger population. Distal triceps tendon tears represent an injury infrequently seen even among older, skeletally mature athletes. We report a case of distal triceps tendon tear with concomitant ulnar collateral ligament (UCL) injury in a skeletally-immature football player. Case Report: This is a rare case of traumatic triceps tendon tear with UCL injury in a 13-year-old male football player during a fall and hyperextension of his elbow. Management included surgical treatment of the triceps tear with suture anchors in double row technique. The concomitant UCL injury was treated conservatively. Conclusion: This case suggests that this type of injury can occur in young athletes, but good prognosis can be expected with prompt management. Surgical repair of a functionally deficient triceps tendon tear and conservative management of associated UCL injury can result in returntoplay within 6 months.
