3. Carious lesion management in children and adolescents by Australian dentists

Abstract

BACKGROUND: The management of carious lesions in children and adolescents can have lifelong implications for the patient. The aim of this study was to assess the decision-making process of dentists when managing carious lesions in children and adolescents. METHODS: Approximately, 11 000 dentists listed as members of the Australian Dental Association Inc. (ADA) and Australian and New Zealand Society of Paediatric Dentistry (ANZSPD) were emailed a link in April 2017 to a 19-question survey delivered by SurveyMonkey™. RESULTS: In this study, 887 responses were received. In 'enamel-limited' carious lesions, dentists intervened most frequently in primary tooth approximal (365, 41.1%), followed by permanent tooth occlusal (295, 33.3%) and approximal (244, 27.5%), and primary tooth occlusal (203, 22.9%) surface carious lesions. Age, university of graduation, practicing state, decade of graduation and frequency of treatment of children between 6 and 15 years were significant demographic factors influencing the restorative threshold. CONCLUSIONS: Australian dentists reported significant variation in their management of approximal and occlusal carious lesions in both primary and permanent teeth. A substantial proportion of respondents would intervene surgically on non-cavitated enamel-limited lesions.

Abstract

The Tennessee Valley Authority (TVA) and the Department of Energy (DOE) have partnered to pursue the use of down-blended, off-specification highly-enriched uranium (HEU) in TVA reactors. Approximately 46 MTU of off-spec HEU are being dispositioned as BLEU fuel in TVA's reactors with another 15 MTU potentially available. Participation in this agreement required a significant up-front investment by TVA and the assumption of higher risk due to the different fabrication methods required for the BLEU fuel. The potential for significant cost savings that could be shared between TVA and the US Treasury also existed. This paper describes the BLEU program and the challenges TVA and DOE have overcome via implementation through TVA's contract with AREVA and AREVA's subcontractor, Nuclear Fuel Services. 21. Prosthetic valve endocarditis resulting from nosocomial bacteremia. A prospective, multicenter study.

Abstract

Objective: To determine the incidence of endocarditis in bacteremic patients with prosthetic heart valves and the risk factors for and the effect of duration of antibiotic therapy on development of endocarditis in such patients.Design: Multicenter, prospective observational study.Setting: Six university teaching hospitals with high-volume cardiothoracic surgery.Participants: One hundred seventy-one consecutive patients with prosthetic heart valves who developed bacteremia during hospitalization.Measurements and Main Results: Patients were evaluated when they were identified as having bacteremia and 1, 2, 6, and 12 months after its occurrence. Of 171 patients, 74 (43%) developed endocarditis: Fifty-six (33%) had prosthetic valve endocarditis at the time bacteremia was discovered ("endocarditis at outset"), whereas 18 (11%) developed endocarditis a mean of 45 days after bacteremia was discovered ("new endocarditis"). Mitral valve location and staphylococcal bacteremia (Staphylococcus aureus or S. epidermidis) were significantly associated with the development of "new" endocarditis. All 18 cases of new endocarditis were nosocomial, and in 6 of these cases (33%) bacteremia was acquired via intravascular devices. Twenty-one patients without evidence of endocarditis at the time of bacteremia received short-term antibiotic therapy (< 14 days); 1 patient (5%) developed endocarditis. Eleven of 70 patients (16%) who received long-term antibiotic therapy (> 14 days) developed endocarditis (P > 0.2).Conclusions: Bacteremic patients with prosthetic heart valves were at notable risk for developing endocarditis, even when they received antibiotic therapy before endocarditis developed and regardless of the duration of such therapy. 30. Collagen binding in clinical isolates of Staphylococcus aureus

Abstract

Collagen binding was examined in 90 strains of Staphylococcus aureus derived from patient samples. Slightly under one-half (39 of 90) of the S. aureus strains bound collagen. Collagen binding in S. aureus did not correlate with either immunoglobulin G or fibronectin binding by these organisms. Chi-square analysis of isolates obtained from patients with complicated bacteremia (bacteremia associated with deep tissue infection) compared with isolates from patients with uncomplicated bacteremia (bacteremia without other infection) showed that the former strains were significantly more likely to have collagen-binding ability. Subcloning of primary isolates from patients with bacteremia showed that all clones from individual patients were either all positive for collagen binding or all negative, suggesting a common clonal origin for this characteristic. The ability to bind collagen could not be induced in strains lacking collagen affinity by repeated subculture in media supplemented with collagen.

Abstract

This paper is an account of the accomplishments of some of the early past presidents of the Medical Library Association known personally to the author in his career as a medical librarian. It demonstrates the qualities that made these librarians leaders of our profession and also indicates their personal attributes that contributed to the advancement of medical librarianship. It is hoped that the historical presentation of some of the giants of our profession will inspire present and future presidents and other medical librarians with an understanding of some of the qualities necessary to the continuing success of our profession. Sir William Osler, who was a great believer in libraries and librarians and himself a past president of MLA, summed up four qualities in his advice to medical students equally applicable to past and present leadership in the library profession--(1) the art of detachment, (2) the virtue of method, (3) the quality of thoroughness, and (4) the grace of humility (Osler, Sir William. Teacher and Student. In his Aequanimitas: with Other Addresses to Medical Students. 3d ed. Philadelphia, Blakiston Company, 1904. p. 27-31). It is thought that our past presidents possessed all of these qualities.
