Abstract

Drinking alcohol increases the risk of death among boaters, even if they are not operating the boat. The risk of death is increased even at low blood alcohol levels and increases substantially at higher blood alcohol levels., Context Alcohol is increasingly recognized as a factor in many boating fatalities, but the association between alcohol consumption and mortality among boaters has not been well quantified. Objectives To determine the association of alcohol use with passengers' and operators' estimated relative risk (RR) of dying while boating. Design, Setting, and Participants Case-control study of recreational boating deaths among persons aged 18 years or older from 1990-1998 in Maryland and North Carolina (n=221), compared with control interviews obtained from a multistage probability sample of boaters in each state from 1997-1999 (n=3943). Main Outcome Measure Estimated RR of fatality associated with different levels of blood alcohol concentration (BAC) among boaters. Results Compared with the referent of a BAC of 0, the estimated RR of death increased even with a BAC of 10 mg/dL (odds ratio [OR], 1.3; 95% confidence interval [Cl], 1.2-1.4). The OR was 52.4 (95% Cl, 25.9-106.1) at a BAC of 250 mg/dL. The estimated RR associated with alcohol use was similar for passengers and operators and did not vary by boat type or whether the boat was moving or stationary. Conclusions Drinking increases the RR of dying while boating, which becomes apparent at low levels of BAC and increases as BAC increases. Prevention efforts targeted only at those operating a boat are ignoring many boaters at high risk. Countermeasures that reduce drinking by all boat occupants are therefore more likely to effectively reduce boating fatalities.

12. Characteristics of older pedestrians who have difficulty crossing the street

Abstract

Objectives. This study examined the sociodemographic and health characteristics and problems of older pedestrians. Methods. Interviews and assessments were conducted with 1249 enrollees aged 72 or older from the New Haven, Conn, community of the Established Populations for Epidemiologic Studies of the Elderly who agreed tO Participate in a seventh follow-up. Results. Approximately 11% of the New Haven residents reported difficulty crossing the street. Older pedestrians needing help in one or more activities of daily living were more than 10 times as likely as others, and those with the slowest walking speeds were nearly 3 times as likely as others, to report difficulty crossing the street. Fewer than 1% of these pedestrians aged 72 or older had a normal walking speed sufficient to cross the street in the time typically allotted at signalized intersections (1.22 m/sec). Conclusions. Crossing times at signalized intersections in areas with large Populations of elders should be extended, and the recommended walking speed for timing signalized crossings should be modified to reflect the range of abilities among older pedestrians. Abstract

The general population of one major city supports the use of needle exchange programs even though many think such a program might attract injection drug users (IDU's) to their neighborhoods. In Baltimore, Maryland, a group of 138 people demographically similar to the city's overall population responded to a survey on needle exchange. While 72% thought such a program might attract IDU's to the neighborhood, 65% favored such a program because it would decrease the number of discarded needles; would not increase drug use, and that it would decrease HIV transmission.

32. Disparities in the Emergency Department Evaluation of Chest Pain Patients

Abstract

Background: The existence of race and gender differences in the provision of cardiovascular health care has been increasingly recognized. However, few studies have examined whether these differences exist in the emergency department (ED) setting. Objectives: To evaluate race, gender, and insurance differences in the receipt of early, noninvasive diagnostic tests among persons presenting to an ED with a complaint of chest pain. Methods: Data were drawn from the U.S. National Hospital Ambulatory Health Care Survey of EDs. Visits made during 1995–2000 by persons aged 30 years or older with chest pain as a reason for the visit were included. Factors affecting the likelihood of ordering electrocardiography, cardiac monitoring, oxygen saturation measurement using pulse oximetry, and chest radiography were analyzed using multivariate probit analysis. Results: A total of 7,068 persons aged 30 years or older presented to an ED with a primary complaint of chest pain during the six-year period, corresponding to more than 32 million such visits nationally. The adjusted probability of ordering a test was highest for non–African American patients for all tests considered. African American men had the lowest probabilities (74.3% and 62% for electrocardiography and chest radiography, respectively), compared with 81.1% and 70.3%, respectively, among non African American men. Only 37.5% of African American women received cardiac monitoring, compared with 54.5% of non–African American men. Similarly, African American women were significantly less likely than non–African American men to have their oxygen saturation measured. Patients who were uninsured or self-pay, as well as patients with ‘‘other’’ insurance, also had a lower probability than insured persons of having these tests ordered. 50. Risk Factors for the Onset of Diagnostic Interview Schedule/DSM-III Agoraphobia in a Prospective, Population-Based Study

Abstract

• We analyzed the onset of agoraphobia in a large prospective study of the general population. The annual incidence of Diagnostic Interview Schedule/DSM-III agoraphobia is estimated at 22 per 1000 population per year. Latent class analysis of new cases in the at-risk population reveals the existence of two subtypes of agoraphobia. The relationship of the incidence of the two subtypes to sociodemographic and psychopathologic risk factors suggests different origins. The data also show that in two thirds of the 260 new cases of Diagnostic Interview Schedule/DSM-III agoraphobia, onset occurs without a history of panic attack. We analyzed the relationship of these epidemiologic results to earlier clinical findings in terms of possible measurement and sampling differences.
