1. A comparison of the nutrient intake of a community-dwelling first-episode psychosis cohort, aged 19–64 years, with data from the UK population

Abstract

Psychosis increases the risk of CVD, obesity and type 2 diabetes and reduces life expectancy. There are limited data comparing the dietary habits of community-dwelling first-episode psychosis sufferers – with autonomy over diet – and the general population. The data represent the retrospective evaluation of nutritional data collected between 2007 and 2013 from 143 individuals from the UK population receiving treatment for first-episode psychosis. Differences in mean nutrient intakes between the study cohort and the national sample were tested for statistical significance using independent t tests, incorporating Satterthwaite's correction where required. Mean total energy intake was lower for males (P = 0·049) and higher for females (P = 0·016) in the cohort than in the corresponding subgroups of the national sample. Females in the study cohort consumed 12·9 (95 % CI 4·3, 21·5) g more total fat per d, whilst males consumed 7·7 (95 % CI 0·5, 14·9) g less protein per d than the national sample. Males in the study also showed significantly lower mean intakes than nationally of folate, Fe, Se, vitamin D and Zn, but not vitamin C. The proportion of individuals not meeting the lower reference nutrient intakes, particularly for Se (males 54·0 % and females 57·1 %) and for Fe amongst females (29·6 %), is cause for concern regarding potentially severe deficiencies. Further exploration of dietary habits within first-episode psychosis is warranted to assess whether individuals make beneficial dietary changes for their physical and mental health and wellbeing following dietary change intervention. It would also be pertinent to assess any correlation between diet and mental health symptomology.

Abstract

A community of research practice (CRP) was established to increase research capacity and provide learning opportunities and networking for healthcare practitioners, working within a Children's Care Group. The CRP aimed to engage research-interested practitioners in research to develop their skills and confidence, encourage networking, and build research capacity.To report the results of a service evaluation that was undertaken to review the CRP's value in practice.Thematic analysis revealed four themes - 'positive environment', 'confidence', 'professional development' and 'networking' - highlighting benefits from the CRP, alongside the challenges encountered.This article highlights the significant contribution of CRP for practitioners in the context of an innovative organisation with a supportive culture.A CRP empowers healthcare practitioners to engage with research while in clinical practice, enabling increased research confidence, and the development of research skill and knowledge and enhanced networking. CRP can potentially influence recruitment and retention.

Abstract

Over 80% of malaria-attributable deaths are in children under five. However, the only malaria vaccine recommended by the World Health Organization (WHO) for paediatric use, Mosquirix™, has limited efficacy. Complementary strategies, like monoclonal antibodies (mAbs), will be required to eradicate malaria. To discover new anti-malaria mAbs, we evaluated >28,000 antibody sequences from circulating B cells obtained from 45 Mosquirix™ vaccinees and selected 369 for testing. Many antibodies bound the circumsporozoite protein (CSP), a main surface protein on malaria and the malaria antigen in Mosquirix™, and several were exceptionally protective in mouse models of malaria. Through this work, we identified surprising correlations that suggest certain CSP sequences in Mosquirix™ may induce immunodominant antibody responses that dilute protective immunity. Further, we selected the antibodies most protective in preclinical mouse models and engineered them for improved manufacturability and developability to meet WHO guidelines. An optimised clinical candidate, MAM01, suitable for paediatric populations living in low-to-middle-income countries, was selected for clinical development.

Abstract

Many salmonids have a male heterogametic (XX/XY) sex determination system, and they are supposed to have a conserved master sex-determining gene (sdY) that interacts at the protein level with Foxl2 leading to the blockage of the synergistic induction of Foxl2 and Nr5a1 of the cyp19a1a promoter. However, this hypothesis of a conserved master sex-determining role of sdY in salmonids is challenged by a few exceptions, one of them being the presence of naturally occurring “apparent” XY Chinook salmon, Oncorhynchus tshawytscha, females. Here, we show that some XY Chinook salmon females have a sdY gene (sdY-N183), with 1 missense mutation leading to a substitution of a conserved isoleucine to an asparagine (I183N). In contrast, Chinook salmon males have both a nonmutated sdY-I183 gene and the missense mutation sdY-N183 gene. The 3-dimensional model of SdY-I183N predicts that the I183N hydrophobic to hydrophilic amino acid change leads to a modification in the SdY β-sandwich structure. Using in vitro cell transfection assays, we found that SdY-I183N, like the wild-type SdY, is preferentially localized in the cytoplasm. However, compared to wild-type SdY, SdY-I183N is more prone to degradation, its nuclear translocation by Foxl2 is reduced, and SdY-I183N is unable to significantly repress the synergistic Foxl2/Nr5a1 induction of the cyp19a1a promoter. Altogether, our results suggest that the sdY-N183 gene of XY Chinook females is nonfunctional and that SdY-I183N is no longer able to promote testicular differentiation by impairing the synthesis of estrogens in the early differentiating gonads of wild Chinook salmon XY females.

Abstract

The Stokes-Brinkman equations model fluid flow in highly heterogeneous porous media. In this paper, we consider the numerical solution of the Stokes-Brinkman equations with stochastic permeabilities, where the permeabilities in subdomains are assumed to be independent and uniformly distributed within a known interval. We employ a truncated anchored ANOVA decomposition alongside stochastic collocation to estimate the moments of the velocity and pressure solutions. Through an adaptive procedure selecting only the most important ANOVA directions, we reduce the number of collocation points needed for accurate estimation of the statistical moments. However, for even modest stochastic dimensions, the number of collocation points remains too large to perform high-fidelity solves at each point. We use reduced basis methods to alleviate the computational burden by approximating the expensive high-fidelity solves with inexpensive approximate solutions on a low-dimensional space. We furthermore develop and analyze rigorous a posteriori error estimates for the reduced basis approximation. We apply these methods to 2D problems considering both isotropic and anisotropic permeabilities., Comment: 20 pages, 12 figures

Abstract

Sea angling has been shown to be a high value activity with significant expenditure by individuals on their sport. Deriving estimates of the economic contribution of recreational sea angling is important in a number of related policy contexts, from tourism management and economic development policy, to the sustainable management of inshore fish stocks. This paper reveals some of the challenges in understanding the economic effects associated with recreational sea angling, and provides estimates of the economic value of recreational sea angling in England. The results were derived from research undertaken in England in 2011- 13, which was conducted as part a wide ranging government-funded study, Sea Angling 2012, that estimated sea angler catches, spending and activity. Recreational sea angling made a significant contribution to the economy, supporting just over £2 billion of total spending, and 23,600 jobs in England in 2012-13. The implications of these results are discussed in the context of the management of recreational sea angling in England.

Abstract

The aim of this service evaluation was to understand the factors affecting meal provision on a dementia ward and assess whether meals-based dietary intake met estimated mean energy requirement and reference nutrient intake (RNI). A mixed-methods approach was adopted: 1. Mapping the dietary intake for a cohort ( n =6) of inpatients with advanced dementia over a period of four consecutive days. 2. Semi-structured interviews to explore the experiences of the professionals ( n =5) involved. Two of the participants' four-day mean intakes did not meet their estimated energy requirement based on meal provision. All participants apart from one met the target of 0.75g of protein per kg body weight per day for the general population. Several of the nutrients consumed were at or exceeded the RNI for adults of this age, although participants' mean dietary vitamin D intake was substantially below the RNI for the general population. The themes included communication, time pressure and the continuity of service provision. The meals provided were nutritionally sound. Individuals' nutritional status was improved through staff supporting them with eating. The contribution of drinks and snacks to nutrient intake warrants further exploration. Effective communication between food providers and ward areas is important. Mealtimes should reflect patient need - for example, having the main meal in the evening and a lighter option at lunch. This does not clash with the benefits of a flexible breakfast time.

Abstract

AimsWe here present preliminary results from our study to understand better the changes in people’ s experience of food in the months after diagnosis with first episode psychosis (FEP). Weight gain often occurs in the weeks/months after diagnosis and is related to an increase in appetite and food intake. Many drugs that are effective in treating psychosis are associated with changes in the way that people experience reward when they eat.The aim of this project is to increase our understanding of exactly why this happens in terms of an individual's experience of food reward and reduced satiety – and therefore how we can help people with FEP to keep their weight down. At this stage we are looking at the feasibility of applying currently available evaluation tools to people in this situation.MethodA convenience sample was used to recruit 10 service users from RDaSH NHS FT Early Intervention Services. This is a feasibility study which will provide data to underpin a fully powered, larger trial.Rating scales applied were:Power of food questionnaire: measures responsiveness to the food environment.Intuitive Eating Scale: measures an individual's tendency to follow their physical hunger and satiety cues.The loss of control over eating scale (LOCES): measures a global sense of whether individuals experience LOC over eating.Dutch Eating Behaviour Questionnaire (DEBQ): measures restrained eating, emotional eating and external eating.ResultThe ages of the participants ranged from 17-26 years. All were started on Olanzapine at the dose of 5 or 10 mg daily.Baseline total scores for the Power of Food (2.47-3.80)/5 (higher score = more responsiveness) and Intuitive Eating scales (2.10-2.62)/5 (higher score = greater tendency to follow hunger and satiety cues) were in the mid-range, while the LOCES scores varied widely from 1.50-2.38/5.The DEBQ restrained subscale score range was 2.40-2.80/5 (higher indicates greater restraint with food) while the DEBQ external subscale ranged from 2.70—3.00/5 (higher = greater tendency to overeat) and the DEBQ emotional subtotal score was 1.92-1.94/5, in keeping with a relatively low emotional drive to eat.ConclusionOur preliminary results reveal at the beginning of antipsychotic treatment a moderate responsiveness to food and tendency to follow hunger/ satiety cues, with scores for Loss of Control of eating in the low to moderate range and a low emotional drive to eat. The difference between these and the follow-up eating behaviour scores will provide important clues as to the precise changes in eating behaviour with anti-psychotic treatment in FEP.

Abstract

This article reports the findings from a stakeholder consultation to explore the future scope and design of nutrition education for mental health practitioners. The project aimed to consult a range of stakeholders about the scope and design of a nutrition education module in mental health. Focus groups and individual interviews were used to collect data describing the views of 39 stakeholders. A thematic analysis produced five themes to inform the development of an e-learning module. It was discovered that stakeholders felt that nutrition science was an important topic for mental health education. They felt that the need for the education was wider than mental health services and could even be useful outside healthcare. Views about an e-learning approach and the academic level were mixed. Assessing learning through an e-learning platform was also identified as a challenge.

Abstract

The relationship between nutrition and mental health, as well as physical health, is well known. Mental Health Services in the UK and worldwide are recognising the need for nutritional care, however eliciting a change in eating habits in patients/service users is a challenge. The effects of a ‘Psych-Nutritional Intervention’ (PNI) using Motivational Interviewing (n = 30) was compared with a ‘Treatment as Usual’ (TAU) involving standard nutritional advice (n = 21) and a control group (no nutritional support) (n = 22). The sample consisted of young people aged 18–35 years old with a diagnosis of psychosis, who were currently under the care of a UK specialist National Health Service (NHS) Mental Health team (Early Intervention for Psychosis Team). Nutritional knowledge was assessed at baseline across the groups. Readiness to Change eating habits was measured at baseline and a further two time points post intervention. Although there were no significant between group differences, the PNI group elicited both the most progression in terms of readiness to change eating habits, but also the most regression. The use of MI may be anti-therapeutic for those who are not ready to consider make changes to their eating habits, and this requires further investigation. Greater emphasis on the importance of assessing and selecting who is most likely to benefit from interventions is necessary.

Abstract

The Stokes-Brinkman equations model flow in heterogeneous porous media by combining the Stokes and Darcy models of flow into a single system of equations. With suitable parameters, the equations can model either flow without detailed knowledge of the interface between the two regions. Thus, the Stokes-Brinkman equations provide an alternative to coupled Darcy-Stokes models. After a brief review of the Stokes-Brinkman problem and its discretization using Taylor-Hood finite elements, we present a residual-based a posteriori error estimate and use it to drive an adaptive mesh refinement process. We compare several strategies for the mesh refinement, and demonstrate its effectiveness by numerical experiments in both 2D and 3D., 20 pages, 15 figures

Abstract

Psychosis increases the risk of CVD, obesity and type 2 diabetes and reduces life expectancy. There are limited data comparing the dietary habits of community-dwelling first-episode psychosis sufferers – with autonomy over diet – and the general population. The data represent the retrospective evaluation of nutritional data collected between 2007 and 2013 from 143 individuals from the UK population receiving treatment for first-episode psychosis. Differences in mean nutrient intakes between the study cohort and the national sample were tested for statistical significance using independent t tests, incorporating Satterthwaite's correction where required. Mean total energy intake was lower for males (P = 0·049) and higher for females (P = 0·016) in the cohort than in the corresponding subgroups of the national sample. Females in the study cohort consumed 12·9 (95 % CI 4·3, 21·5) g more total fat per d, whilst males consumed 7·7 (95 % CI 0·5, 14·9) g less protein per d than the national sample. Males in the study also showed significantly lower mean intakes than nationally of folate, Fe, Se, vitamin D and Zn, but not vitamin C. The proportion of individuals not meeting the lower reference nutrient intakes, particularly for Se (males 54·0 % and females 57·1 %) and for Fe amongst females (29·6 %), is cause for concern regarding potentially severe deficiencies. Further exploration of dietary habits within first-episode psychosis is warranted to assess whether individuals make beneficial dietary changes for their physical and mental health and wellbeing following dietary change intervention. It would also be pertinent to assess any correlation between diet and mental health symptomology.
