Precision medicine (PM) is increasingly recognized as the way forward for optimizing patient care. Introduced in the field of oncology, it is now considered of major interest in other medical domains like allergy and chronic airway diseases, which face an urgent need to improve the level of disease control, enhance patient satisfaction and increase effectiveness of preventive interventions. The combination of personalized care, prediction of treatment success, prevention of disease and patient participation in the elaboration of the treatment plan is expected to substantially improve the therapeutic approach for individuals suffering from chronic disabling conditions. Given the emerging data on the impact of patient stratification on treatment outcomes, European and American regulatory bodies support the principles of PM and its potential advantage over current treatment strategies. The aim of the current document was to propose a consensus on the position and gradual implementation of the principles of PM within existing adult treatment algorithms for allergic rhinitis (AR) and chronic rhinosinusitis (CRS). At the time of diagnosis, prediction of success of the initiated treatment and patient participation in the decision of the treatment plan can be implemented. The second-level approach ideally involves strategies to prevent progression of disease, in addition to prediction of success of therapy, and patient participation in the long-term therapeutic strategy. Endotype-driven treatment is part of a personalized approach and should be positioned at the tertiary level of care, given the efforts needed for its implementation and the high cost of molecular diagnosis and biological treatment. © 2017 EAACI and John Wiley and Sons A/S. Doel Beoordelen van het effect van de ‘Ziektelastmeter COPD’ op de ziektespecifieke kwaliteit van leven bij patiënten met COPD. Opzet Clustergerandomiseerde gecontroleerde trial. Methode Dit betrof een onderzoek in 39 Nederlandse huisartsenpraktijken en 17 ziekenhuizen bij 357 patiënten met COPD (postbronchodilatoire FEV1/FVC-ratio: < 0,7) van 40 jaar of ouder. We randomiseerden zorgverleners tussen een interventie- en controlegroep, waarbij patiënten uit de interventiegroep behandeld werden met behulp van de ‘Ziektelastmeter COPD’. Dit innovatieve instrument bestaat uit een korte gevalideerde vragenlijst en een aantal objectieve parameters, die samen de integrale gezondheidstoestand visueel weergeven en via een behandelalgoritme leiden tot een individueel zorgplan. Patiënten uit de controlegroep ontvingen de gebruikelijke zorg. De primaire uitkomstmaat was het aantal patiënten met een klinisch relevante verbetering in ziektespecifieke kwaliteit van leven gemeten met de ‘St. George’s respiratory questionnaire’ (SGRQ) na een follow-upduur van 18 maanden. Secundaire uitkomstmaten waren onder andere de SGRQ-totaalscore en de ‘Patient assessment of chronic illness care’(PACIC)-score. Resultaten Na 18 maanden was er een statistisch significant en klinisch relevante toename in de SGRQ-score bij 49 patiënten (34%) in de interventiegroep en bij 33 patiënten (22%) in de controlegroep (oddsratio: 1,85; 95%-BI: 1,08-3,16). Patiënten in de interventiegroep ervoeren een hogere kwaliteit van zorg dan die in de controlegroep (verschil in PACIC-score: 0,32; 95%-BI: 0,14-0,50). Conclusie Gebruik van de ‘Ziektelastmeter COPD’ resulteert in een hogere ziektespecifieke kwaliteit van leven en hogere kwaliteit van zorg voor COPD-patiënten. Dit instrument kan een veelbelovende bijdrage leveren aan de dagelijkse COPD-zorg. We adviseren deze studie te repliceren, zo mogelijk in de buitenlandse gezondheidszorg.

Background: Inhaled corticosteroids (ICSs) and long-acting β2-agonists (LABAs) are used to treat moderate to severe chronic obstructive pulmonary disease (COPD). Objective: To determine whether long-term ICS therapy, with and without LABAs, reduces inflammation and improves pulmonary function in COPD. Design: Randomized, placebo-controlled trial. (ClinicalTrials.gov registration number: NCT00158847) Setting: 2 university medical centers in The Netherlands. Patients: 114 steroid-naive current or former smokers with moderate to severe COPD. Measurements: Cell counts in bronchial biopsies and sputum (primary outcome); methacholine responsiveness at baseline, 6, and 30 months; and clinical outcomes every 3 months. Intervention: Random assignment by minimization method to receive fluticasone propionate, 500 μg twice daily, for 6 months (n = 31) or 30 months (n = 26); fluticasone, 500 μg twice daily, and salmeterol, 50 μg twice daily, for 30 months (single inhaler; n = 28); or placebo twice daily (n = 29). Results: 101 patients were greater than 70% adherent to therapy. Fluticasone therapy decreased counts of mucosal CD3+ cells (-55% [95% CI, -74% to -22%]; P - 0.004), CD4- cells (-78% [CI, -88% to 60%]; P < 0.001), CD8+ cells (-57% [CI, -77% to -18%]; P = 0.010), and mast cells (-38% [CI, -60% to -2%]; P = 0.039) and reduced hyperresponsiveness (P = 0.036) versus placebo at 6 months, with effects maintained after 30 months. Fluticasone therapy for 30 months reduced mast cell count and increased eosinophil count and percentage of intact epithelium, with accompanying reductions in sputum neutrophil, macrophage, and lymphocyte counts and improvements in FEV1 decline, dyspnea, and quality of life. Reductions in inflammatory cells correlated with clinical improvements. Discontinuing fluticasone therapy at 6 months increased counts of CD3+ cells (120% [CI, 24% to 289%]; P = 0.007), mast cells (218% [CI, 99% to 407%]; P < 0.001), and plasma cells (118% [CI, 9% to 336%]; P = 0.028) and worsened clinical outcome. Adding salmeterol improved FEV1 level. Limitations: The study was not designed to evaluate clinical outcomes. Measurement of primary outcome was not available for 24% of patients at 30 months. Conclusion: ICS therapy decreases inflammation and can attenuate decline in lung function in steroid-naive patients with moderate to severe COPD. Adding LABAs does not enhance these effects. - Abstract
The Allergic Rhinitis and its Impact on Asthma (ARIA) initiative commenced during a World Health Organization workshop in 1999. The initial goals were (1) to propose a new allergic rhinitis classification, (2) to promote the concept of multi-morbidity in asthma and rhinitis and (3) to develop guidelines with all stakeholders that could be used globally for all countries and populations. ARIA-disseminated and implemented in over 70 countries globally-is now focusing on the implementation of emerging technologies for individualized and predictive medicine. MASK [MACVIA (Contre les Maladies Chroniques pour un Vieillissement Actif)-ARIA Sentinel NetworK] uses mobile technology to develop care pathways for the management of rhinitis and asthma by a multi-disciplinary group and by patients themselves. An app (Android and iOS) is available in 20 countries and 15 languages. It uses a visual analogue scale to assess symptom control and work productivity as well as a clinical decision support system. It is associated with an inter-operable tablet for physicians and other health care professionals. The scaling up strategy uses the recommendations of the European Innovation Partnership on Active and Healthy Ageing. The aim of the novel ARIA approach is to provide an active and healthy life to rhinitis sufferers, whatever their age, sex or socio-economic status, in order to reduce health and social inequalities incurred by the disease.

- Abstract
The Allergic Rhinitis and its Impact on Asthma (ARIA) initiative commenced during a World Health Organization workshop in 1999. The initial goals were (1) to propose a new allergic rhinitis classification, (2) to promote the concept of multi-morbidity in asthma and rhinitis and (3) to develop guidelines with all stakeholders that could be used globally for all countries and populations. ARIA-disseminated and implemented in over 70 countries globally-is now focusing on the implementation of emerging technologies for individualized and predictive medicine. MASK [MACVIA (Contre les Maladies Chroniques pour un Vieillissement Actif)-ARIA Sentinel NetworK] uses mobile technology to develop care pathways for the management of rhinitis and asthma by a multi-disciplinary group and by patients themselves. An app (Android and iOS) is available in 20 countries and 15 languages. It uses a visual analogue scale to assess symptom control and work productivity as well as a clinical decision support system. It is associated with an inter-operable tablet for physicians and other health care professionals. The scaling up strategy uses the recommendations of the European Innovation Partnership on Active and Healthy Ageing. The aim of the novel ARIA approach is to provide an active and healthy life to rhinitis sufferers, whatever their age, sex or socio-economic status, in order to reduce health and social inequalities incurred by the disease.

Background: Home mechanical ventilation is usually started in hospital as arterial blood gas sampling is deemed necessary to monitor CO2 and O-2 adequately during institution of ventilatory support. A non-invasive device to reliably measure CO2 transcutaneously would alleviate the need for high care settings for measurement and open the possibility for home registration. Objectives: In this study we investigated whether the TOSCA (R) transcutaneous CO2 (PtcCO(2)) measurements, performed continuously during the night, reliably reflect arterial CO2 (PaCO2) measurements in adults with chronic respiratory failure. Methods: Paired measurements were taken in 15 patients hospitalised to evaluate their blood gas exchange. Outcomes were compared 30 min, 2, 4, 6 and 8 h after attaching the sensor to the earlobe. A maximum difference of 1.0 kPa and 95% limits of agreement (LOA) of 1 kPa between CO2 pressure measurements, following the analysis by Bland and Altman, were determined as acceptable. Results: Mean PtcCO(2) was 0.4 kPa higher (LOA -0.48 to 1.27 kPa) than mean PaCO2 after 30 min. These figures were 0.6 kPa higher (LOA -0.60 to 1.80 kPa) after 4 h, with a maximum of 0.72 kPa (LOA 0.35 to 1.79 kPa) after 8 h. The corresponding values for changes in PtcCO(2) versus PaCO2 were not significant (ANOVA). Conclusions: PtcCO(2) measurement, using TOSCA, is a valid method showing an acceptable agreement with PaCO2 during 8 h of continuous measurement.
