ObjectiveBiliary innervation is considered important in regulating the function of bile ducts, whereas the role of innervation in the hepatobiliary system of patients with biliary atresia (BA) remains unknown. This current study aims to investigate the role of innervation in biliary remnants and analyze the relationship between the innervation and prognosis of BA after surgery.MethodsEighty-seven patients with type III BA who underwent the Kasai procedure were consecutively enrolled from January 2017 to September 2020. Innervation and ductules in remnants were examined by pathologists. Liver function, onset of cholangitis, jaundice clearance, and survival with the native liver were recorded. Patients were followed up for 24 months. The relationship between innervation and prognosis was analyzed.ResultsIn total, 67 patients had bile drainage postoperatively, and 21 biliary remnants contained neuronal plexuses where there was no neuron but nerve fiber bundles. Acetylcholinesterase staining was positive in all plexuses. In patients with bile drainage, those with plexuses had improved postoperative liver function, significantly better jaundice clearance 3 or 6 months postoperatively (50.0% vs. 19.1%, or 90.0% vs. 63.8%, respectively), fewer episodes of early cholangitis (10.0% vs. 34.0%), and better survival (80.0% vs. 61.7%) compared to those without. In addition, a larger area of plexuses was associated with a larger area of ductules (R2 = 0.786, p = 0.000), less frequent (p = 0.000) and later cholangitis onset (p = 0.012), and better jaundice clearance (p = 0.063).ConclusionsIncreased cholinergic innervation in biliary remnants may help reduce the onset of cholangitis and lead to better and earlier jaundice clearance. Thus, it improves the postoperative prognosis of patients with BA.

Ischemic stroke (IS) remains a serious threat to human health. Neuroinflammatory response is an important pathophysiological process after IS. Circular RNAs (circRNAs), a member of the non-coding RNA family, are highly expressed in the central nervous system and widely involved in regulating physiological and pathophysiological processes. This study reviews the current evidence on neuroinflammatory responses, the role of circRNAs in IS and their potential mechanisms in regulating inflammatory cells, and inflammatory factors affecting IS damage. This review lays a foundation for future clinical application of circRNAs as novel biomarkers and therapeutic targets.

Epilepsy is a prevalent neurological disorder, rendering the development of automated seizure detection systems imperative. While complex machine learning models are powerful, their training and hardware deployment remain challenging. The reservoir computing system offers a low-cost solution in terms of both hardware requirements and training. In this paper, we introduce a compact reservoir computing system for seizure detection, based on the α-In2Se3 dynamic memristors. Leaky integrate-and-fire neurons are used for post-processing the output of the system, and experimental results indicate their effectiveness in suppressing erroneous outputs, where both accuracy and specificity are enhanced by over 2.5%. The optimized compact reservoir system achieves 96.40% accuracy, 86.34% sensitivity, and 96.56% specificity in seizure detection tasks. This work demonstrates the feasibility of using reservoir computing for seizure detection and shows its potential for future application in extreme edge devices.

Conventional vision systems suffer from lots of data handling between memory and processing units. Inspired by how humans recognize noisy images and the flexible modulation on the timescale of ion dynamics inside an emerging memtransistor, a novel neuromorphic vision system is reported based on the ion‐modulated memtransistors. By controlling the ion‐doping processes under adequate stimuli strengths, both short‐term and long‐term ion dynamics can be utilized to deliver energy‐efficient data processing. When dealing with image reconstructions, the short‐term accumulation effect of the device can help filter noises in a set of received noisy images while enhancing the original pattern information. The increased contrast can help distinguish the actual contents. To demonstrate systematic performances with the reconfiguration of devices, the nonlinear relationship between channel conductance variation and the amplitude of gate pulses into the network‐level simulation is extracted. Also, with the nonvolatile conductance change characteristic, the task of recognizing noisy images is performed to verify the versatility of ion‐modulated memtransistors in the neuromorphic artificial vision systems. Designing an efficient multi-agent hardware system to solve large-scale computational problems through high-parallelism processing with nonlinear interactions remains a challenge. Here, the authors demonstrate that a multi-agent hardware system deploying distributed Ag nanoclusters as physical agents enables parallel, complex computing.

Abstract Background Preterm infants have higher nutrition needs than term infants. The effectiveness of various feeding supplementation was assessed by the improvement of health outcomes in single specific systematic reviews (SRs). The aim of this review was to comprehensively describe the effectiveness of feeding supplementation in promoting health outcomes of preterm infants. Methods A literature search was conducted in the PUBMED, EMBASE, Science Direct, Cochrane library, Web of Science, and Wiley online library. SRs selection followed clear inclusion and exclusion criteria. Pairs of reviewers independently applied the criteria to both titles/abstracts and full texts. Screening and data extraction were performed by using the advanced tables. The methodological quality of SRs and the quality of the evidence were carried out according to the Assessing the Methodological Quality of Systematic Reviews (AMSTAR) tool and the Grades of Recommendation, Assessment, Development, and Evaluation guidelines (GRADE) respectively. A qualitative synthesis of evidence is presented. Results Seventeen SRs were included in the review. Fifteen kinds of feeding supplementation were reported in the SRs. In preterm infants, the effectiveness of feeding supplementation in addition to regular breast-feeding was mainly shown in six aspects: physical health, neurodevelopment, biochemical outcomes, other health outcomes, morbidity and all-cause mortality. And the effectiveness of the interventions on health outcomes in preterm infants was found by most systematic reviews. The methodological quality of all the included SRs was high, and most of the evidences was of low or very low quality. Conclusions Our results will allow a better understanding of the feeding supplementation in preterm infants. Although the feeling supplements may improve the health outcomes of in preterm infants, the existing evidence is uncertain. Therefore, the clinical use of these supplements should be considered cautiously and more well-designed RCTs are still needed to further address the unsolved problems of the included SRs.

Ischemic stroke (IS) is a severe disease with a high disability, recurrence, and mortality rates. Autophagy, a highly conserved process that degrades damaged or aging organelles and excess cellular components to maintain homeostasis, is activated during IS. It influences the blood–brain barrier integrity and regulates apoptosis. Circular RNAs (circRNAs) are novel non-coding RNAs involved in IS-induced autophagy and participate in various pathological processes following IS. In addition, they play a role in autophagy regulation. This review summarizes current evidence on the roles of autophagy and circRNA in IS and the potential mechanisms by which circRNAs regulate autophagy to influence IS injury. This review serves as a basis for the clinical application of circRNAs as novel biomarkers and therapeutic targets in the future.

Atherosclerosis (AS) is a disease dangerous to human health and the main pathological cause of ischemic cardiovascular diseases. Although its pathogenesis is not fully understood, numerous basic and clinical studies have shown that AS is a chronic inflammatory disease existing in all stages of atherogenesis. It may be a common link or pathway in the pathogenesis of multiple atherogenic factors. Inflammation is associated with AS complications, such as plaque rupture and ischemic cerebral infarction. In addition to inflammation, apoptosis plays an important role in AS. Apoptosis is a type of programmed cell death, and different apoptotic cells have different or even opposite roles in the process of AS. Unlike linear RNA, circular RNA (circRNA) a covalently closed circular non-coding RNA, is stable and can sponge miRNA, which can affect the stages of AS by regulating downstream pathways. Ultimately, circRNAs play very important roles in AS by regulating inflammation, apoptosis, and some other mechanisms. The study of circular RNAs can provide new ideas for the prediction, prevention, and treatment of AS.

Background: Intracranial atherosclerotic stenosis (ICAS) is a common cause of first and recurrent ischemic stroke worldwide. Circular RNAs (circRNA)s have been recently suggested as candidate biomarkers in diagnosing and prognosis of ischemic stroke. A few circRNAs even serve as therapeutic targets that improves neurological function after ischemic stroke. However, the roles of circRNAs in ICAS caused ischemic stroke (ICAS-stroke) have not been fully understood. Therefore, in this study, we attempted to find some clues by investigating the different expression profiles of circRNAs between patients diagnosed with ICAS-stroke and normal control (NC)s.Methods: The OE Biotech Human ceRNA Microarray 4 × 180 K (47, 899 probes) screened circRNAs differentially expressed in peripheral blood in a discovery cohort (5 NCs versus five patients with ICAS-stroke). Afterwards, a validation cohort (31 NCs versus 48 patients with ICAS-stroke) was performed by quantitative polymerase chain reaction (qPCR). Gene Ontology (GO), Kyoto Encyclopedia of Genes and Genomes (KEGG) and CircRNA–microRNA-mRNA interaction network was performed to identify potential interactions with microRNAs and pathway-deregulated circRNAs.Results: There were 244 circRNAs differentially expressed in patients diagnosed with ICAS-stroke compared with NCs [fold change (FC) ≥ 2.0 and p-value

Objective The risk factors associated with iodine contrast extravasation immediately after endovascular thrombectomy (EVT) and subsequent hemorrhagic transformation within 24 hours remain unclear. Methods Mixed images, iodine overlay maps, and virtual non-contrast images were reconstructed from 106 consecutive acute ischemic stroke patients who underwent dual energy computed tomography immediately and 24 hours after EVT. Multivariate analyses of clinical and radiological data were performed to explore independent predictors of iodine contrast extravasation and hemorrhagic transformation. Results Sixty-eight (64.2%) patients exhibited pure iodine contrast extravasation after EVT; 30.9% developed hemorrhagic transformation within 24 hours after EVT. The number of stent retriever passes was independently associated with both iodine contrast extravasation (odds ratio 1.608; 95% confidence interval (CI) 1.047–2.469) and subsequent hemorrhagic transformation (odds ratio 1.477; 95% CI 1.003–2.175). Patients with more than two stent retriever passes were more likely to exhibit iodine contrast extravasation (sensitivity = 68.2%, specificity = 81.5%), while those with more than three stent retriever passes more often exhibited hemorrhage after iodine contrast extravasation (sensitivity = 64.6%, specificity = 87.2%). Conclusions The number of stent retriever passes was an independent predictor for both iodine contrast extravasation and subsequent hemorrhagic transformation.

Adjuvants are essential components of vaccines that have been particularly effective at promoting protective humoral immunity to infectious diseases and preventing mortality and morbidity. There are many different types of adjuvants with different modes of action. A better understanding of their potential adjuvanticity could provide guidance for the development of novel adjuvants for cancer immunotherapy. In this article, we review the importance of adjuvants in vaccine development, their mechanisms of action and adjuvant-induced autoimmunity and discuss some strategies for driving the development of new vaccine adjuvants.

Background: Contrast media extravasation can mimic hemorrhage after endovascular thrombectomy (EVT). Dual energy CT (DECT) has the potential to distinguish hemorrhage from iodine contrast.Methods: We retrospectively examined clinical and radiological data from 106 consecutive acute ischemic stroke patients who received EVT and underwent DECT immediately and 24 h after EVT. Iodine overlay map, virtual non-contrast, and mixed images are reconstructed.Results: With the use of DECT, the proportion of all patients diagnosed with hemorrhagic transformation on mixed images immediately after EVT was reduced from 74.5% (79 of 106) to 10.4% (11 of 106), with a very poor consistency (κ = 0.076, p = 0.041). Correspondingly, hemorrhagic transformation on mixed images 24 h after EVT was reduced from 41.5% (44 of 106) to 30.2% (32 of 106), with a moderate consistency (κ = 0.757, p < 0.001).Conclusions: The use of DECT both immediately and 24 h after EVT changes the diagnosis of hemorrhagic transformation in a considerable proportion of acute ischemic stroke patients with EVT. This could affect decision making with respect to antithrombotic strategy.

Abstract Background Endometrial cancers (ECs) are one of the most common types of malignant tumor in females. Substantial efforts had been made to identify significantly mutated genes (SMGs) in ECs and use them as biomarkers for the classification of histological subtypes and the prediction of clinical outcomes. However, the impact of non-significantly mutated genes (non-SMGs), which may also play important roles in the prognosis of EC patients, has not been extensively studied. Therefore, it is essential for the discovery of biomarkers in ECs to further investigate the non-SMGs that were highly associated with clinical outcomes. Results For the 9681 non-SMGs reported by the mutation annotation pipeline, there were 1053, 1273 and 395 non-SMGs differentially expressed between the patient groups divided by the clinical endpoints of histological grade, histological type as well as the International Federation of Gynecology and Obstetrics (FIGO) stage of ECs, respectively. In the gene set enrichment analysis, the cancer-related pathways, namely neuroactive ligand-receptor interaction signaling pathway, cAMP signaling pathway and calcium signaling pathway, were significantly enriched with the differentially expressed non-SMGs for all the three endpoints. We further identified 23, 19 and 24 non-SMGs, which were highly associated with histological grade, histological type and FIGO stage, respectively, from the differentially expressed non-SMGs by using the variable combination population analysis (VCPA) approach and found that 69.6% (16/23), 78.9% (15/19) and 66.7% (16/24) of the identified non-SMGs had been previously reported to be correlated with cancers. In addition, the averaged areas under the receiver operating characteristic curve (AUCs) achieved by the predictive models with identified non-SMGs as predictors in predicting histological type, histological grade, and FIGO stage were 0.993, 0.961 and 0.832, respectively, which were superior to those achieved by the models with SMGs as features (averaged AUCs = 0.928, 0.864 and 0.535, resp.). Conclusions Besides the SMGs, the non-SMGs reported in the mutation annotation analysis may also involve the crucial genes that were highly associated with clinical outcomes. Combining the mutation status with the gene expression profiles can efficiently identify the cancer-related non-SMGs as predictors for cancer prognostic prediction and provide more supplemental candidates for the discovery of biomarkers.

Neuromorphic architectures based on memristive neurons and synapses hold great prospect in achieving highly intelligent and efficient computing systems. Here, we show that a Schottky diode based on Cu-Ta/InGaZnO4 (IGZO)/TiN structure can exhibit threshold switching behavior after electroforming and in turn be used to implement an artificial neuron with inherently stochastic dynamics. The threshold switching originates from the Cu filament formation and spontaneous Cu–In–O precipitation in IGZO. The nucleation and precipitation of Cu–In–O phase are stochastic in nature, which leads to the stochasticity of the artificial neuron. It is demonstrated that IGZO based stochastic neurons can be used for global minimum computation with random walk algorithm, making it promising for robust neuromorphic computation.

Background: It remains unclear whether cortical superficial siderosis (cSS) is associated with dementia and its subtypes. We thus performed a meta-analysis to evaluate the relationship between dementia and cSS.Methods: We searched EMBASE, PubMed, and Web of Science for relevant studies assessing risk of dementia and prevalence of cSS in patients with cognitive impairment. Fixed-effects and random-effects models were performed.Results: Seven eligible studies including 3,218 patients with definite cognitive impairment were pooled in meta-analysis. The prevalence of cSS was 3.4%. The pooled analysis demonstrates odds ratio for cSS and dementia to be 1.60 (95% CI 1.04–2.44; p = 0.031). Subgroup analysis further indicated a significant association between cSS and Alzheimer's disease (AD) (OR = 2.01, 95% CI 1.34–3.02; p < 0.001), but not non-AD dementia (OR = 0.700, 95% CI 0.435–1.128; p = 0.143).Conclusions: Our meta-analysis of available published data demonstrates an increased prevalence of dementia in the subjects with pre-existing cSS, especially for AD. These findings suggest cSS to be a candidate imaging indicator for AD. Further longitudinal research is needed to investigate the clinical relevance.

BackgroundIt remains unclear whether cerebral microbleeds (CMBs) are associated with depression in the general elderly population. We thus performed a meta-analysis to evaluate the relationship between depression and CMBs.MethodsA systematic literature search was conducted in EBSCO, PubMed, and Web of Science for relevant studies that assessed the relationship between depression and the prevalence of CMBs.ResultsFive eligible studies including 7,328 patients were pooled in meta-analysis. The prevalence of CMBs was 18.0%. The prevalence of depression was 11.1%. The pooled analysis demonstrated odds ratio for CMBs and depression to be 1.187 (95% confidence interval 1.005–1.403; p = 0.043) with no evidence of statistical heterogeneity (I2 = 0.0%, p = 0.621).ConclusionOur meta-analysis of available published data indicated an increased prevalence of depression in the subjects with pre-existing CMBs. This finding supports the vascular depression hypothesis. Further studies are needed to investigate the role of CMBs in the pathogenesis and progression of depression, which might provide a potential target for the prevention and treatment.

We take a stochastic optimization approach to opportunity tracking and access in self-similar primary traffic. Based on a multiple time-scale hierarchical Markovian model, we formulate opportunity tracking and access in self-similar primary traffic as a Partially Observable Markov Decision Process. We show that for independent and stochastically identical channels under certain conditions, the myopic sensing policy has a simple round-robin structure that obviates the need to know the channel parameters; thus it is robust to channel model mismatch and variations. Furthermore, the myopic policy achieves comparable performance as the optimal policy that requires exponential complexity and assumes full knowledge of the channel model.
