Abstract

In this 42-year-old-man with suspected cholelithiasis the following finding of the serosal surface of the gallbladder was discovered by laparoscopic examination: Smooth serosal elevation, dark-greenish in color in a size of the small bean; No hypervascularity in the surrounding serosa. This finding was confirmed to be due to intramural gallstones. This is the first description of the laparoscopic finding of the intramural stones of the gallbladder, which may assist in establishing the clinical characteristics of the intramural gallstones.

Abstract

The serum levels of alpha-1-antichymotrypsin (ACT) were studied in 168 patients with various liver diseases and cancers in conjunction with other liver function tests, serum sialic acid, AFP and CEA. The ACT levels in acute viral hepatitis and chronic hepatitis were not significantly altered compared with the normal level (220 +/- 40 microgram/ml), although the level was slightly increased or decreased temporarily during the acute phase of the former. In liver cirrhosis, the mean level was significantly lower than the normal in spite of the absence of signs of hepatic decompensation (168 +/- 51 microgram/ml, p less than 0.001). In contrast to cirrhosis, the levels were increased to various extents in 65% of cases with hepatoma, in spite of the association of liver cirrhosis in the majority of them. Much higher levels were observed in all cases of metastatic liver cancers and cancers of the pancreas and the biliary tract. The elevations were observed even in cases without the increase of AFP or CEA. Both in cirrhosis and cancers, ACT levels were not correlated with any of serum bilirubin and serum enzyme activities, but were positively correlated with the levels of plasma fibrinogen and serum sialic acid. The measurement of serum ACT level can be taken advantage of for the diagnosis and monitoring of liver cirrhosis and liver cancers, particularly of hepatoma without AFP elevation.

Abstract

The activity of neuraminidase (sialidase) in peripheral white blood cells was measured by a fluorometric method using 4-MU NANA as a substrate. The activity in mononuclear cells, which was predominantly lymphocytes, was 2.5 times higher than that in polymorphonuclear cells (neutrophils). The former's activity was directly proportional to the number of the cells, but that of the latter was found to be suppressed by an increasing number of cells. Thus, the increased number of PMN contaminated in MNC fraction somewhat obscured neuraminidase activity affecting suppressively . The enzyme activity in MNC of 25 control subjects (15 males and 10 females) was 254 +/- 73 pmoles per hour per 10(6) cells and there was no difference between the values in males and females (233 +/- 59 vs. 284 +/- 81). The activity in 20 patients with chronic active liver disease was significantly higher than that in controls (551 +/- 135, p less than 0.01). The amount of sialic acid in MNC, which was 1.4 times more than that in PMN, revealed a tendency for a positive correlation between neuraminidase activity. A new finding of the increase of lymphocyte neuraminidase activity was introduced and its pathological significance particularly in liver disease was discussed.

Abstract

Based upon the experimental results in KK and ddY/S-mice, it has become clear that a combination of Coenzyme A and Adenosine triphosphate exercises a favorable influence on treatment of the fatty liver. Forty one patients have been complaining of general fatigue more than one year. Their bodyweight exceeded slightly the standard in their own age. Physical examination revealed slight enlargement of the liver. Laboratory findings: hyperbetalipoproteinemia, hypercholesterinemia, hyperbetaglobulinemia, pathologic BSP-retention, slight elevation of cholin esterase and transaminaseactivity. Glucose toleranse test with 50g resulted in latent or manifest diabetic curve. Immunoreactive insuline (IRl) was also estimated. Liver biopsy: fatty liver. The treatment with a combination of 8mg CoA and 30mg 219 ATP daily in the form of intramuscular injection was introduced into the medical treatment when needle biopsy of the liver revealed steatosis hepatis histologically in these patients. After therapeutic duration of 6 months, laboratory findings as well as histological features of the biopsied liver specimens were improved. Therefore, it can be said that simultaneous i.m. injection of CoA and ATP brings a favorable effect not only on the laboratory findings but also on the histological changes of the fatty liver with diabetic change.

Abstract

In these three years, the authors performed the endoscopic retrograde pancreatocholangiography in 135 patients with diseases of the bile duct, the gallbladder and the pancreas. At the beginning of introduction of the retrograde pancreatocholangiography in our clinic, we have not always succeeded in this diagnostic procedure. Up to now, the duodenofiberscopic retrograde pancreatocholangiography made the diseases of the bile duct, the gallbladder and the pancreas clear in 71 of 135 patients: that is to say, in 39 patients, the diseases of the bile duct and the gallbladder, as well as in 32, the diseases of the pancreas. Nowadays, we can succeed in this diagnostic procedure in more than 80% of cases.Based on these findings, it can be said that the endoscopic retrograde pancreatocholangiography will be capable of being an effective diagnostic tool. It leaves, however, some room for consideration concerning the interpretation of the obtained figures.

冷蔵人胎盤の加水分解によるCE14(ラエンネック(L))は各種アミノ酸を含んでおり,その作用として,抗脂肝作用,組織呼吸の促進効果,肝再生の促進などが,実験的研究から知られている.今回の研究は,本剤が慢性肝疾患の治療に有効な薬剤かどうかを知ることであった.患者を二重盲検により,2群にランダムに分けた.Placeboとしては生理的食塩水を用いた.第I群は,L注射を2週間受けた後,Placeboを2週間受け,この方法をもう一度くり返した.第II群は,始めPlacebo注射を2週間受け,つぎの2週はLの注射を毎日受けた.その後は第I群と同様にもう一度くり返した.肝機能検査を2週毎に行なって,その前と2週後の値を比較した.両群とも,GOT, GPTはL投与期間で下降し,それは統計的に有意であった.これに比して,両群のPlacebo期間におけるGOT, GPTの下降は有意ではなかった.以上より,Lは慢性肝疾患患者に効果があるといえよう.

Abstract

Antibodies reactive with O-phosphotyrosine (PTYR) were detected in 60 out of 621 inpatients, with high frequencies in hematologic and lung malignancies, hepatic diseases, cerebrovascular diseases, and autoimmune diseases. Affinity-purified antibodies proved capable of recognizing PTYR-containing proteins in a human carcinoma cell line, A431, both by immunofluorescent staining and by immunoaffinity chromatography, but had no detectable affinity for phosphorylated serine or threonine, or for the nucleotides tested. In these respects, the antibodies observed in human sera were indistinguishable from anti-PTYR antibodies raised experimentally in rabbits or mice.

Abstract

A method for separation of serum (pseudo) cholinesterase isozymes was studied in order to take advantage of it for clinical research. By employing modifications on a previously reported method, analytical time was shortened to a half (1.5 days) of the original method without abolishing qualitative and quantitative accuracy. Thus, the present method facilitated its clinical application for the study of this isozyme. A normal pattern of the isozyme in Japanese in this method was determined by the analysis of sixteen normal subjects, which appeared to be very consistent in each individual under the present conditions. The distribution of relative activities of respective isozymes measured on a densitogram coincided well with that measured calorimetrically. Abnormalities of the zymogram were newly found in patients with liver cirrhosis and metastatic liver cancer, which seemed to be characteristic to the respective diseases. Isozyme patterns in the liver and ascites were also measured and compared with those in serum.

Abstract

The pattern of serum cholinesterase (ChE) isozyme appeared to be characteristically abnormal in liver cirrhosis and hepatoma. In liver cirrhosis an abnormal fast moving peak was observed in 92.5% of fifty three patients studied. Further, diminishing activities of ChE 3 and 4, accompanied by an emergence of weak bands with unusual rates of flow, were noted in 58.5%. The latter abnormality was always associated with the former one. The pattern in hepatoma was essentially the same with liver cirrhosis, though diversity of bands was also present in some cases. It was ascertained that these abnormalities was due to sialic acid content bound to the enzyme, but was not due to production of abnormal enzyme protein moiety. It was suggested by clinical analysis that the degree of the abnormality of the isozyme may be useful for the diagnosis and prognostic evaluation of liver cirrhosis.

1. 1. The blood concentration of haloperidol was determined in schizophrenic patients according to the three different schedules by the recently developed radioimmunoassay. 2. 2. There exists a significant positive correlation between the blood concentration and the daily dose per kg of body weight of haloperidol. 3. 3. A certain relationship is present between the blood concentration and the development of extrapyramidal symptoms (EPS). 4. 4. The combined use of antiparkinsonian drugs failed to depress the blood level of haloperidol. 5. 5. The blood concentration of haloperidol varied more widely and required much more time in attaining a steady state after administration in a single dose than after administration in divided doses.
