Protocol for a scoping review of supports for pregnancy loss in the workplace

The aim of this project is to explore predictors of cervical cancer screening attendance and HPV vaccination in pre-screening age women. Specifically, this study will examine self esteem, body image, and internalised misogyny; risk perception, illness perception, and cancer worry; cues to action, knowledge; health literacy and misinformation susceptibility, as predictors of attendance to cervical cancer screening and HPV vaccination uptake. In addition, this study will examine a potential link between mothers' attitudes and behaviours around cervical cancer prevention to their daughters' screening and vaccination intentions.

Background Creutzfeldt-Jakob Disease (CJD) is part of a group of rapidly progressive incurable neurodegenerative disorders. CJD is commonly thought of as a neurological disorder but has a wide variety of clinical phenotypes including an initial presentation with psychiatric symptoms. Methods The authors describe a case of probable CJD with an atypical presentation of major depressive disorder (MDD). The literature surrounding the diagnosis and psychiatric manifestations of CJD is briefly reviewed. The following search terms were used: Creutzfeldt-Jakob disease, depression, catatonia, diagnosis. Approximately 30 sources were reviewed, 6 of which were cases of CJD mimicking depression and catatonia. Results A 61 year-old female presented with abrupt onset of MDD with psychosis and catatonic features, as well as broad cognitive deficits. Pharmacotherapy and electroconvulsive therapy were ineffective, with worsening of symptoms during hospitalization. Neurological workup was notable for basal ganglia and cortical abnormalities on magnetic resonance sequences and the presence of tau and 14-3-3 proteins in cerebrospinal fluid, consistent with probable CJD. Discussion CJD typically presents as a rapidly progressive dementia accompanied by myoclonus, pyramidal or extrapyramidal symptoms, visual/cerebellar dysfunction, or akinetic mutism. Diagnosis is made through clinical features and supporting findings on MRI, EEG, and CSF assays. Psychiatric and behavioral symptoms are now increasingly recognized as part of the clinical presentation. Hallucinations, paranoia, depression, and catatonia are some common manifestations, and often fail to respond to standard therapies.discussion Conclusion This case highlights the importance of maintaining a broad differential and consideration of CJD in late onset intractable psychiatric symptoms.

Background: Limited resources for child protection create challenging decision situations for child protective services (CPS) workers at the point of intake. A body of research has examined the factors associated with worker decisions and processes using a variety of methodological approaches to gain knowledge on decision-making. However, few attempts have been made to systematically review this literature. \ud \ud Objective: As part of a larger project on decision-making at intake, this systematic review addressed the question of the factors associated with worker decisions to investigate alleged maltreatment referrals. \ud \ud Methods: Quantitative studies that examined factors associated with screening decisions in CPS practice settings were included in the review. Database and other search methods were used to identify research published in English over a 35-year period (1980-2015). \ud \ud Findings: Of 1,147 identified sources, 18 studies were selected for full data extraction. The studies were conducted in the U.S., Canada, and Sweden and varied in methodological quality. Most studies examined case factors with few studies examining other domains. \ud \ud Conclusions: To inform CPS policy and practice, additional research is needed to examine the relationships between decision-making factors and case outcomes. Greater attention needs to be given to the organizational and external factors that influence decision-making.

Background Transthyretin wild-type (ATTRwt) amyloidosis is a systemic process resulting in deposition of misfolded transthyretin protein in several different tissues throughout the body. It is known to be a cause of progressive, life-threatening cardiomyopathy and lumbar spinal stenosis and carpal tunnel syndrome. Case Description Here we present the case of a 71-year-old man who has clinical manifestations of all 3 entities over several years, providing a picture of the natural history of ATTRwt amyloidosis. This patient eventually underwent a heart transplant because of progressive cardiac amyloidosis (CA) resulting in end-stage heart failure. However, symptoms in carpal tunnel and lumbar spine manifested years before the symptoms of heart failure. ATTRwt amyloidosis may present as a clinical triad of lumbar stenosis, carpal tunnel syndrome, and heart failure. Recently developed medications have shown efficacy in slowing the progress of systemic and cardiac amyloidosis. Conclusions Knowing that extracardiac symptoms may occur first, we propose that sending ligamentum flavum and flexor tenosynovium for pathologic evaluation may be a unique opportunity to screen and treat a population of patients at risk for developing CA and heart failure.

The friends and acquaintances (N = 58) of 10 adolescent suicide victims were interviewed 6 months after the death of the victims, and the rates of psychiatric disorders that had onset after the death were compared with the 6-month incidence of psychopathology in 58 demographically and psychiatrically matched unexposed controls. The exposed group showed higher rates of any new onset major depressive disorder, but the rate of incident suicide attempts was the same in both groups. The median onset of incident depression among the exposed group was within the first month after exposure, and the majority of those exposed youth with incident depression were still depressed at interview 6 months after the death. Adolescent friends and acquaintances of suicide victims experience considerable psychiatric morbidity subsequent to exposure to suicide, most consistent with pathological grief.
