Abstract

BackgroundDespite the importance and popularity of mutual support groups, there have been no systematic attempts to implement and evaluate routine outcome monitoring (ROM) in these settings. Unlike other mutual support groups for addiction, trained facilitators lead all Self-Management and Recovery Training (SMART Recovery) groups, thereby providing an opportunity to implement ROM as a routine component of SMART Recovery groups. ObjectiveThis study protocol aims to describe a stage 1 pilot study designed to explore the feasibility and acceptability of a novel, purpose-built mobile health (mHealth) ROM and feedback app (Smart Track) in SMART Recovery groups coordinated by SMART Recovery Australia (SRAU) The secondary objectives are to describe Smart Track usage patterns, explore psychometric properties of the ROM items (ie, internal reliability and convergent and divergent validity), and provide preliminary evidence for participant reported outcomes (such as alcohol and other drug use, self-reported recovery, and mental health). MethodsParticipants (n=100) from the SMART Recovery groups across New South Wales, Australia, will be recruited to a nonrandomized, prospective, single-arm trial of the Smart Track app. There are 4 modes of data collection: (1) ROM data collected from group participants via the Smart Track app, (2) data analytics summarizing user interactions with Smart Track, (3) quantitative interview and survey data of group participants (baseline, 2-week follow-up, and 2-month follow-up), and (4) qualitative interviews with group participants (n=20) and facilitators (n=10). Feasibility and acceptability (primary objectives) will be analyzed using descriptive statistics, a cost analysis, and a qualitative evaluation. ResultsAt the time of submission, 13 sites (25 groups per week) had agreed to be involved. Funding was awarded on August 14, 2017, and ethics approval was granted on April 26, 2018 (HREC/18/WGONG/34; 2018/099). Enrollment is due to commence in July 2019. Data collection is due to be finalized in October 2019. ConclusionsTo the best of our knowledge, this study is the first to use ROM and tailored feedback within a mutual support group setting for addictive behaviors. Our study design will provide an opportunity to identify the acceptability of a novel mHealth ROM and feedback app within this setting and provide detailed information on what factors promote or hinder ROM usage within this context. This project aims to offer a new tool, should Smart Track prove feasible and acceptable, that service providers, policy makers, and researchers could use in the future to understand the impact of SMART Recovery groups. Abstract

BackgroundEach year, half a million patients with a potential neck (c-spine) injury are transported to Ontario emergency departments (EDs). Less than 1.0% (1/100) of these patients have a neck bone fracture. Even less (1/200, 0.5%) have a spinal cord injury or nerve damage. Currently, paramedics transport all trauma victims (with or without an injury) by ambulance using a backboard, cervical collar, and head immobilizers. Importantly, prolonged immobilization is often unnecessary; it causes patient discomfort and pain, decreases community access to paramedics, contributes to ED crowding, and is very costly. We therefore developed the Canadian C-Spine Rule (CCR) for alert and stable trauma patients. This decision rule helps ED physicians and triage nurses to safely and selectively remove immobilization, without x-rays and missed injury. We successfully taught Ottawa paramedics to use the CCR in the field in a single-center study. ObjectiveThis study aimed to improve patient care and health system efficiency and outcomes by allowing paramedics to assess eligible low-risk trauma patients with the CCR and selectively transport them without immobilization to the ED. MethodsWe propose a pragmatic stepped-wedge cluster randomized design with health economic evaluation, designed collaboratively with knowledge users. Our 36-month study will consist of a 12-month setup and training period (year 1), followed by the stepped-wedge trial (year 2) and a 12-month period for study completion, analyses, and knowledge translation. A total of 12 Ontario paramedic services of various sizes distributed across the province will be randomly allocated to one of three sequences. Paramedic services in each sequence will cross from the control condition (usual care) to the intervention condition (CCR implementation) at intervals of 3 months until all communities have crossed to the intervention. Data will be collected on all eligible patients in each paramedic service for a total duration of 12 months. A major strength of our design is that each community will have implemented the CCR by the end of the study. ResultsInterim results are expected in December 2019 and final results in 2020. If this multicenter trial is successful, we expect the Ontario Ministry of Health will recommend that paramedics evaluate all eligible patients with the CCR in the Province of Ontario. ConclusionsWe conservatively estimate that in Ontario, more than 60% of all eligible trauma patients (300,000 annually) could be transported safely and comfortably, without c-spine immobilization devices. This will significantly reduce patient pain and discomfort, paramedic intervention times, and ED length of stay, thereby improving access to paramedics and ED care. This could be achieved rapidly and with lower health care costs compared with current practices (possible cost saving of Can $36 [US $25] per immobilization or Can $10,656,000 [US $7,335,231] per year). 9. The Translation and Circulation of 'Evidence': Obviousness, Demonstration, and Effect in Educational Research in Germany and England

Abstract

In current educational research communications, especially in English and increasingly also in German publications, the term "evidence" refers to international homogenizing gold standards and is often linked to European evidence policies, large scale assessments and justified and proven knowledge. Against the background of international communication and the related circulation of terms and concepts, this paper analyses German and English OECD publications that recommend the development of educational research in 1970s and 90s. In England, the OECD and thus the external perception of their own education system hardly played a significant role due to years of awareness of the need for reform. Rather, Hargreaves's lecture (1996) about the disappointing effects of educational research when compared with the achievements of evidence-based medicine was decisive for the evidence movement. In addition, non-university institutions have gained legitimacy through acting as "evidence" providers for the school system. Based on analyses of German educational research literature, we show that "evidence" appeared in neurological, medical, technological, and economic texts in the 90s and early 2000s. Usage of "evidence" increased after the PISA shock in 2000 and is now linked to the expression of disciplinary development into empirical educational research. However, based on relatively stable patterns of communication and interpretation in two academic cultures, it is shown that the connectivity of an epistemological term like "evidence" does not necessarily reduce misunderstanding.

Abstract

BackgroundThere are no approved pharmacotherapies for methamphetamine use disorder. Two preliminary phase 2 randomised controlled trials have found mirtazapine, a tetracyclic antidepressant, to be effective in reducing methamphetamine use. The proposed Tina Trial is the first phase 3 placebo-controlled randomised trial to examine the effectiveness and safety of mirtazapine as an outpatient pharmacotherapy for methamphetamine use disorder.MethodsThis is a multi-site phase 3 randomised, double-blind, placebo-controlled parallel trial. Participants are randomly allocated (1:1) to receive either mirtazapine (30 mg/day for 12 weeks) or matched placebo, delivered as a take-home medication. The target population is 340 people aged 18-65 years who have moderate to severe methamphetamine use disorder. The trial is being conducted through outpatient alcohol and other drug treatment clinics in Australia. The primary outcome is measured as self-reported days of methamphetamine use in the past 4 weeks at week 12. Secondary outcomes are methamphetamine-negative oral fluid samples, depressive symptoms, sleep quality, HIV risk behaviour and quality of life. Other outcomes include safety (adverse events), tolerability, and health service use. Medication adherence is being monitored using MEMS® Smart Caps fitted to medication bottles.DiscussionThis trial will provide information on the safety and effectiveness of mirtazapine as a pharmacotherapy for methamphetamine use disorder when delivered as an outpatient medication in routine clinical practice. If found to be safe and effective, this trial will support an application for methamphetamine use disorder to be included as a therapeutic indication for the prescription of mirtazapine.Trial registrationAustralian and New Zealand Clinical Trials Registry ACTRN12622000235707. 33. Young People and Post-Pandemic Futures: Scenario Planning as a Radical Politics of Hope

Abstract

Purpose: In this paper we report on the outcomes of a scenario planning project in Melbourne's (Australia) inner northern suburbs, which was undertaken in the context of an extended lockdown during Melbourne's second wave of COVID-19 infections. In this project, the researchers sought to identify the ways in which young people and youth service providers understood the challenges that the pandemic was creating for young people and the provision of youth services, and through the 5 years up to 2025. Design/methodology/approach: The project was shaped by a scenario planning methodology that produced three research informed scenarios of possible futures for young people in Melbourne's inner north in 2025. The project conducted a series of structured video interviews with young people, and semi-structured interviews with stakeholders that asked participants to reflect on the context of the pandemic, and what the future might hold in relation to young people's pathways and health and well-being, and the futures of their communities and the planet. Findings: The scenario planning methodology revealed many concerns, uncertainties and anxieties that were shared, but which also varied between young people and stakeholders--both about the immediacy of the pandemic, and its aftermaths and intersection with future crises. Originality/value: The scenario planning approach offers sociologies of education and youth a means to do the future-oriented, "hopeful" work that multiple crises for young people demand. Scenario planning is an "affirmative" exercise in hope by which sociologies can "stay with the trouble" that we find ourselves in, and that the pandemic has amplified.

34. Liberal Education in Turbulent Times: Policy, Pedagogy and Their Effects in European Comparison

Abstract

Europe has seen a national populist turn in recent years, a movement away from liberal elites and the rise of 'post-truth' politics. While populist parties may have passed the high point of their success, their politics has had a considerable impact on the mainstream, drawing traditional parties in populist directions. In this article, I consider the consequences of this for those of a liberal persuasion, who value openness and diversity and who are suspicious of those bringing simple solutions to complex problems. I reflect on the findings of a series of comparative education policy and pedagogy studies that I have conducted with colleagues in northern Europe over the past ten years, and in their light, make a number of suggestions for how education can defend liberalism while taking the concerns of national populists seriously.

43. PSB2: The Second Program Synthesis Benchmark Suite

Abstract

For the past six years, researchers in genetic programming and other program synthesis disciplines have used the General Program Synthesis Benchmark Suite to benchmark many aspects of automatic program synthesis systems. These problems have been used to make notable progress toward the goal of general program synthesis: automatically creating the types of software that human programmers code. Many of the systems that have attempted the problems in the original benchmark suite have used it to demonstrate performance improvements granted through new techniques. Over time, the suite has gradually become outdated, hindering the accurate measurement of further improvements. The field needs a new set of more difficult benchmark problems to move beyond what was previously possible. In this paper, we describe the 25 new general program synthesis benchmark problems that make up PSB2, a new benchmark suite. These problems are curated from a variety of sources, including programming katas and college courses. We selected these problems to be more difficult than those in the original suite, and give results using PushGP showing this increase in difficulty. These new problems give plenty of room for improvement, pointing the way for the next six or more years of general program synthesis research., Comment: To be published in GECCO 2021

46. Generative Design of NU's Husky Carbon, A Morpho-Functional, Legged Robot

Abstract

We report the design of a morpho-functional robot called Husky Carbon. Our goal is to integrate two forms of mobility, aerial and quadrupedal-legged locomotion, within a single platform. There are prohibitive design restrictions such as tight power budget and payload, which can particularly become important in aerial flights. To address these challenges, we pose a problem called the Mobility Value of Added Mass (MVAM) problem. In the MVAM problem, we attempt to allocate mass in our designs such that the energetic performance is affected the least. To solve the MVAM problem, we adopted a generative design approach using Grasshopper's evolutionary solver to synthesize a parametric design space for Husky. Then, this space was searched for the morphologies that could yield a minimized Total Cost Of Transport (TCOT) and payload. This approach revealed that a front-heavy quadrupedal robot can achieve a lower TCOT while retaining larger margins on allowable added mass to its design.
