10. Cerebrovascular modelling for the management of aneurysm embolization using an intrasaccular flow diverter made by 3D printing

Abstract

Purpose: Using 3-dimensional (3D) printers, the creation of patient-specific models is possible before and after a therapeutic intervention. There are many articles about replicas for training and simulation of aneurysm clipping. However, no paper has focused on 3D replicas obtained from 3-tesla 3D time of flight (3D-TOF) MR angiography for intrasaccular flow diverter (WEB device) embolization of the cerebral aneurysms. In this paper, we aimed to investigate the feasibility of 3D printing models obtained from 3-tesla 3D-TOF data in the management and training of WEB-assisted embolization procedures. Case presentation: We presented a longitudinal case report with several 3D-TOF MRA prints over time. Three-tesla 3D-TOF data were converted into STL and G-code files using an open-source (3D-Slicer) program. We built patient-specific realistic 3D models of a patient with a middle cerebral artery trifurcation aneurysm, which were able to demonstrate the entire WEB device treatment procedure in the pre-intervention and post-intervention periods. The aneurysmatic segment was well displayed on the STL files and the 3D replicas. They allowed visualization of the aneurysmatic segment and changes within a 6-year follow-up period. We successfully showed the possibility of fast, cheap, and easy production of replicas for demonstration of the aneurysm, the parent vessels, and post-intervention changes in a simple way using an affordable 3D printer. Conclusions: 3D printing is useful for training the endovascular team and the patients, understanding the aneurysm/ parent vessels, and choosing the optimal embolization technique/device. 3D printing will potentially lead to greater interventionalist confidence, decreased radiation dose, and improvements in patient safety. © Pol J Radiol 2022.

19. Prenatal diagnosis of the persistent right umbilical vein, incidence and clinical significance.

Abstract

Prenatal diagnosis of persistent right umbilical vein (PRUV) is important due to accompanying malformations. Pregnant women diagnosed with PRUV were analysed retrospectively. Intrahepatic PRUV was seen in 12 of 10.743 foetuses and its incidence was found to be 0.11%. The gestational week at the time of diagnosis was between 20 and 35 weeks. Six of the cases had additional abnormal sonographic findings (50%) and six cases (50%) were isolated. Major congenital malformations were seen in four (33.3%) foetuses, 75% of which were congenital heart disease (CHD). Genitourinary system anomaly accompanied in two cases (16.6%). Invasive diagnostic tests were applied to three pregnant women and the results were reported as normal karyotype. In PRUV cases, a detailed sonographic examination should be performed, especially the cardiovascular system. Although PRUV cases do not appear to be associated with chromosomal abnormalities, invasive diagnostic tests should be recommended in the presence of concomitant anomalies. What is already known on this subject? The persistent right umbilical vein (PRUV) is a pathological vascular anomaly, in which the left umbilical vein regresses and the right umbilical vein remains open. PRUV can occur in an isolated form that represents its normal variant or be associated with other major or minor anomalies. What do the results of this study add? Additional abnormal sonographic findings were accompanied in 50% of PRUV, major anomaly was detected in 33.3% of them and cardiovascular abnormalities constituted 75% of foetuses with major anomalies. What are the implications of these findings for clinical practice and/or further research? The presence of concomitant anomalies in PRUV cases is not rare and detailed anatomy screening should be done. Abstract

To evaluate the obstetric outcomes of extra ultrasonographic anomalies in patients with cleft lip or/and cleft (CL±P) palate. This retrospective study was conducted in a tertiary referral hospital between December 2017 and March 2021. The patients were analyzed under three groups as follows: isolated CL±P (group 1), CL±P with ultrasonographic anomalies with mild fetal and neonatal consequences (group 2), and CL±P with ultrasonographic anomalies with severe fetal and neonatal consequences (group 3). Fourty cases were analyzed. The abnormal karyotype result (16.7%) were 46,x,t (t15;16)(q26;24), monosomy 18, trisomy 13, 46, XX, 21ps +. In the central cleft, ultrasonographic anomalies were seen in nine patients, which was statistically significantly higher than in isolated CL±P (p=0.004). Seven patients with extra ultrasonographic anomalies underwent termination, one patient with anencephaly died in utero. Preterm birth was the most common obstetric complication in all groups (n=9, 27.2%). In group 3, polyhydramnios was observed statistically significantly more frequently than in group 2 (p=0.033), and the first minute APGAR score was statist ically significantly lower than in group 1 and 2 (p=0.003). The fifth minute APGAR score was statistically significantly lower and the need for the neonatal intensive care unit was statistically significantly higher than group 1 only (p=0.004 and p=0.007, respectively). Polyhydramnios is not found in isolated cases, but only in cases with additional anomalies. Patients with CL±P with either major or minor additional ultrasonographic anomalies have worse fifth minute Apgar results and a greater need for the NICU. [ABSTRACT FROM AUTHOR]

21. Ecballium elateriumextract reduces fibrosis during wound healing in rats

Abstract

AbstractWe explored the effects of subcutaneously injected Ecballium elaterium(L) A. Rich. (EE) extract on skin wound-healing in rats, as well as the effects on the liver and pancreas. Twenty-eight rats were divided into two groups of 14 each: a saline control group (S) and an EE group. Both groups were divided into two subgroups according to the day of sacrifice (S-7 and S-14, and EE-7 and EE-14). All animals received 2.5 cm skin incisions followed by subcutaneous injections of either saline or 2.5 mg/kg EE per margin (5 mg/kg in total). The S and EE groups were compared in terms of the severity and type of local and neighbouring inflammation, vascularization, fibrosis and effects on the liver and pancreas. In addition, apoptosis and vascularity between S and EE groups were compared immunohistochemically with caspase-3 and cd-34 antibodies. There was no significant difference between the staining rates of caspase-3 and cd-34 in the immunohistochemical assay between the S and E groups. Subcutaneous EE was not toxic to the pancreas or liver; the EE-14 group exhibited less fibrosis than the S-14 group. Therefore, it can contribute to the proper closure of the lesion by reducing fibrosis during wound healing.

Any mass on the chest wall may not always be the primary local pathology. A case of lymphoma with an aggressive course may involve the sternum through local invasion and can mimic a chest wall tumour. A 15-year-old boy with mediastinal lymphoma presented with a sternal mass. Partial sternectomy with replacement by methyl methacrylate prosthesis was performed.

Study Design: We investigated CD147 protein expression in uterine smooth muscle tumor samples from patients diagnosed with leiomyoma (n = 22), atypical leiomyoma (BLM) (n = 5), smooth muscle tumor of uncertain malignant potential (STUMP) (n = 14), and LMS (n = 22). The intensity and extensity of immunohistochemical staining were compared to determine its potential role in differential diagnosis. Spearman's rank correlation tests were performed to determine the relationship between CD147 expression and prognostic clinical and pathological criteria in the patients with LMS. Results: CD147 was strongly expressed in 81.8% (n = 18) of the LMS tissue samples. In fact expression of CD147 in LMS tissues was significantly higher than that of the three other uterine smooth muscle tumor types (p = 0.000). However, high CD147 expression was found in only one BLM sample and one STUMP sample. Furthermore, CD147 percent expression positively correlated with Ki67 percent expression (r = 0.466, p<0.05) and mitotic index (r = 0.554, p<0.05), respectively. Conclusion: Our results suggest that immunohisto-chemistry may be a helpful tool in determining whether CD147 is a useful marker in the differential diagnosis of certain uterine smooth muscle tumors. CD147 may also have prognostic value for patients with LMS. Yet, in order to determine the extent of this potential marker's utility as a diagnostic and prognostic indicator, a larger randomized multicenter study must be performed. [ABSTRACT FROM AUTHOR]
