1. Historical use of silver nitrate for the management of epistaxis – evidence-based practice?

Abstract

Objective: Epistaxis is one of the most common emergencies presenting to the ENT service, and silver nitrate cautery is the mainstay of epistaxis treatment in most centres worldwide. This review aimed to ascertain the historical evidence behind current common practice. Method: A review was conducted of historical published literature pertaining to epistaxis management. Results: Silver in medicine dates back to 4000 BC, with silver nitrate first being used in 69 BC. Modern medical use for epistaxis is documented in case reports over the last 200 years. Conclusion: The precise origin and evidence-based practice of using silver nitrate for epistaxis is not well-established or understood. The mechanism of action is questionable; novel research of silver nitrate for this common ENT emergency presentation may be required. [ABSTRACT FROM AUTHOR]

12. Evidence Based Management of Spontaneous Internal Jugular Vein Thrombosis: A Literature Review

Abstract

Internal Jugular Vein (IJV) thrombosis is a rare but potentially fatal condition. First described in 1912 as a complication of peritonsillar abscess it is now known to have a varied aetiology. The leading cause of IJV thrombosis is the use of the IJV for venous access and central venous catheterisation. We present a comprehensive literature review of idiopathic internal jugular vein thrombosis and also present our management experience of this rare condition in an otherwise healthy 60 year old woman. The patient was admitted with a 5 day history of an enlarging right sided neck swelling that was initially associated with redness and pain. Clinically she was stable but there was a tender, soft, diffuse swelling along the length of the right sternomastoid muscle. The overlying skin was mildly erythematous. The ultrasound scan showed occlusive thrombosis of right IJV. Blood cultures, a chest x-ray and computed tomography of chest were all normal. The patient was put on warfarin for 6 months. All the subsequent investigations were normal. Or literature review shows that IJV thrombosis is an important differential diagnosis in a neck swelling. Along with various other factors, it can also occur as a complication of head and neck malignancy, and rarely can be its first manifestation. On other occasions a specific aetiology is never found. Multidisciplinary team approach is required to manage the patient efficiently and the patient needs to be investigated thoroughly to rule out a pathological cause of IJV thrombosis.

Abstract

The reaction $e^+e^−→e^+e^−π^0π^0$ has been analyzed using 97pb$^{−1}$ of data taken with the Crystal Ball detector at the DESY $e^−e^+$ storage ring DORIS II at beam energies around 5.3 GeV. For the first time we have measured the cross section for $γγ→π^0π^0$ for π0π0 mvariant masses ranging from threshold to about 2 GeV. We measure an approximately flat cross section of about 10 nb for W=$mπ^0π^0

Abstract

7 pp. (1990)., We have searched for axion and light Higgs boson production in the channel $ϒ (1S)$ → $(a^0 or h^0) + γ$, where the non-interacting axion a0 and the Higgs boson $h^0$ do not decay in the detector. We find no evidence for an axion and give an upper limit,$ Br(ϒ (1S) → a^0γ) < 4.0 × 10^{−5} (90% CL)$, for long-lived axions. Combining our limit with the previous search in J/ψ decays, we are able to rule out the axion in the standard model with first order QCD corrections. Our $ϒ (1S)$ data also rule out a Higgs boson with mass mh < 86 MeV.

Abstract

Contrast medium (CM) induces a direct toxic effect on renal tubular cells. This toxic effect may have a role in the pathophysiology of CM-induced nephropathy. CM has been shown to affect the endoplasmic reticulum (ER)–related capacity. Unfolded protein response (UPR) is known as a prosurvival response to reduce the accumulation of unfolded proteins and restore normal ER function. However, the role of ER stress–related UPR in the CM-induced renal cell injury still remains unclear. In this study, we examined whether UPR participates in urografin (an ionic CM)-induced renal tubular cells apoptosis. Treatment with urografin in normal rat renal tubular cell line (NRK52E) markedly increased cell apoptosis and decreased cell viability with a dose- and time-dependent manner. The cell necrosis was not increased in urografin-treated cells. Urografin also enhance the induction of ER stress–related markers in NRK52E cells, including glucose-regulated protein (GRP)78 and GRP94 expressions, procaspase-12 cleavage, phosphorylation of PERK (PKR [double-stranded RNA–activated protein kinase]-like ER kinase), and eukaryotic initiation factor 2α (eIF2α). Salubrinal, a selective inhibitor of eIF2α dephosphorylation, effectively decreased urografin-induced cell apoptosis. Furthermore, transfection of GRP78-small interfering RNA in NRK52E cells significantly enhanced urografin-induced cell apoptosis. These results suggest that GRP78/eIF2α-related signals play a protective role during UPR, and the activation of ER stress–related UPR may play an important regulative role in urografin-induced renal tubular injury. Abstract

Using the Crystal Ball detector operating at the DORIS II storage ring we have measured the leptonic partial widthsГee of the Υ(1S) and Υ(2S) resonances. We find $$\Gamma _{ee} (\Upsilon (1S)) = 1.34 \pm 0.03 \pm 0.06keV$$ and $$\Gamma _{ee} (\Upsilon (2S)) = 0.56 \pm 0.04 \pm 0.02keV.$$ The effect onГee of applying different prescriptions for radiative corrections is discussed. We also measureR, the ratio of non-resonant hadronic cross section to the Born cross section of μ pair production, at c.m. energyW=9.39 GeV to beR=3.48±0.04±0.16.

Abstract

We present a study of inclusive p0 and ? production ine+e- annihilation at ". Our analysis is based on integrated luminosities of 46.2 pb-1 taken at the ? (1 S) resonance and 42.1 pb-1 taken in the neighboring continuum. We measure the inclusive p0 and ? spectra for direct ? (1 S) ? 3g decays and continuum processes " and obtain the following mean particle multiplicities per event: $$\begin{array}{*{20}c} {Continuum:} \\ {\begin{array}{*{20}c} {\left\langle {n_{\pi ^0 } } \right\rangle = 3.64 \pm 0.21 \pm 0.36} & {\left\langle {n_\eta } \right\rangle = 0.22 \pm 0.04 \pm 0.03} \\ \end{array} } \\ {\Upsilon (1S) \to 3g:} \\ {\begin{array}{*{20}c} {\left\langle {n_{\pi ^0 } } \right\rangle = 3.39 \pm 0.14 \pm 0.34} & {\left\langle {n_\eta } \right\rangle = 0.35 \pm 0.04 \pm 0.04.} \\ \end{array} } \\ \end{array}$$ We compare our results with predictions of different fragmentation models and with other measurements performed ine+e- annihilation at similar center-of-mass energies.

Abstract

The Crystal Ball detector at thee+e− storage ring DORIS II has been used to analyze exclusive semileptonic decays ofB mesons produced at γ(4S) energies. The decayB→evD* has been observed with a branching ratio of (7.0±1.8±1.4)% using theπ°D decay mode of theD*. In addition we have searched for the exclusive semileptonic decayB±→e±vπ0. We find an upper limit onB±→e±vπ0 of 0.22% at the 90% CL.

Abstract

The Crystal Ball detector has been used at the DORIS II storage ring at DESY to study the reactionγγ→π0π0π0 in theπ0π0π0 invariant mass range from 850 MeV/c2 to 2600 MeV/c2. An enhancement around 1750 MeV/c2 is attributed to theπ2(1670) resonance. The observedπ0π0 invariant mass distribution and theπ0 angular distributions are consistent with those expected for the decay chainπ2→π0f2(1270)→π0π0π0. From our measurements we find the following resonance parameters: two photon partial width ", massM(π2)=(1742±31±49)MeV/c2. and total width ".

Abstract

Using the Crystal Ball detector at thee+e− storage ring DORIS-II, we have searched for the hadronic transitions Υ(2S)→ηΥ(1S) and Υ(2S)→π0Υ(1S) in exclusive decays where the Υ(1S) decays into ane+e− or aµ+µ- pair and the η and π0 are detected by their decay modes η→γγ, η→γγ and π0→γγ. From the number of observed events we derive, after consideration of possible background sources, upper limits on the branching ratiosB(Υ(2S)→ηΥ(1S))<0.7% andB(Υ(2S)→π0Υ(1S))<0.8% at the 90% confidence level.

39. Arsenic and diabetes: Current perspectives

Abstract

Arsenic is a naturally occurring toxic metalloid of global concern. Many studies have indicated a dose–response relationship between accumulative arsenic exposure and the prevalence of diabetes mellitus (DM) in arseniasis-endemic areas in Taiwan and Bangladesh, where arsenic exposure occurs through drinking water. Epidemiological researches have suggested that the characteristics of arsenic-induced DM observed in arseniasis-endemic areas in Taiwan and Mexico are similar to those of non-insulin-dependent DM (Type 2 DM). These studies analyzed the association between high and chronic exposure to inorganic arsenic in drinking water and the development of DM, but the effect of exposure to low to moderate levels of inorganic arsenic on the risk of DM is unclear. Navas-Acien et al. recently proposed that a positive association existed between total urine arsenic and the prevalence of Type 2 DM in people exposed to low to moderate levels of arsenic. However, the diabetogenic role played by arsenic is still debated upon. An increase in the prevalence of DM has been observed among residents of highly arsenic-contaminated areas, whereas the findings from community-based and occupational studies in low-arsenic-exposure areas have been inconsistent. Recently, a population-based cross-sectional study showed that the current findings did not support an association between arsenic exposure from drinking water at levels less than 300 μg/L and a significantly increased risk of DM. Moreover, although the precise mechanisms for the arsenic-induced diabetogenic effect are still largely undefined, recent in vitro experimental studies indicated that inorganic arsenic or its metabolites impair insulin-dependent glucose uptake or glucose-stimulated insulin secretion. Nevertheless, the dose, the form of arsenic used, and the experimental duration in the in vivo studies varied greatly, leading to conflicting results and ambiguous interpretation of these data with respect to human exposure to arsenic in the environment. Moreover, the experimental studies were limited to the use of arsenic concentrations much higher than those relevant to human exposure. Further prospective epidemiological studies might help to clarify this controversy. The issues about environmental exposure assessment and appropriate biomarkers should also be considered. Here, we focus on the review of mechanism studies and discuss the currently available evidence and conditions for the association between environmental arsenic exposure and the development of DM.

Abstract

Angular correlations in the cascade reaction e+e−→Υ(2S)→γχb, χb→γΥ, Υ→μ+μ− or Υ→e+e−, have been used for the first time to investigate the spins of the χb states. The results support the χb spins predicted by the potential models of heavy quarkonia.

Abstract

Physical review / D 38(5), 1365 - 1376 (1988). doi:10.1103/PhysRevD.38.1365, The Crystal Ball detector has been used at the DORIS II storage ring at DESY to study the reaction e+e−→e+e−γ*γ*→e+e−γγ, where X is a narrow resonance with mass between 100 and 3000 MeV. Formation of π0, η, and η’ mesons is observed, and the following meson partial widths are obtained: Γπ0Γ0→γγ=7.7±0.5±0.5 eV, Γη−γγ=0.514±0.017±0.035 keV, and Γη’→γγ=4.7±0.5±0.5 keV. No other narrow resonances are observed, and upper limits are given for the product ΓX→γγBX→γγ. .AE, Published by Inst., Woodbury, NY
