Background: Fulminant myocarditis presentation (FMP) is a rare and severe presentation of myocarditis. The natural history of FMP and its clinical features associated with poor outcomes are incompletely understood because there is a lack of generalizable evidence. Methods: This multicenter retrospective cohort study included patients hospitalized with histologically proven myocarditis who underwent catecholamine or mechanical support from 235 cardiovascular training hospitals across Japan between April 2012 and March 2017. Clinical features and the prognostic predictors of death or heart transplantation within 90 days on the basis of clinical and pathologic findings were determined using the Kaplan-Meier method, log-rank test, and Cox regression analysis. Results: This study included 344 patients with histologically proven FMP (median age, 54 years; 40% female). The median follow-up was 600 days (interquartile range, 36 to 1599 days) and the cumulative risk of death or heart transplantation at 90 days was 29% (n=98). Results from multivariable Cox regression analysis showed that older age, nonsinus rhythm, low left ventricular wall motion ( Conclusions: The results from analyses of data from this multicenter registry demonstrated that patients with FMP are at a higher risk of death or heart transplantation in real-world settings. Kawasaki disease (KD) is an acute self-limited syndrome that predominantly affects children. Coronary sequelae have been identified to be responsible for a small, but significant percentage of young adults who present with myocardial ischemia. In this study, we present a case of an elderly patient with possible coronary sequelae of KD. A 76-year-old man was referred to our outpatient department for silent myocardial ischemia. Axial images of coronary computed tomography showed multiple lumens in the proximal left anterior descending (LAD) artery. Coronary angiography demonstrated braid-like appearance in the proximal and distal segment of the LAD. Coronary intervention was successfully performed for the proximal LAD lesion using directional atherectomy (DCA) catheter. Microscopic examination of the DCA specimens showed the following histological features: tissues in densely hyalinized fibrosis with occasional microcalcification, or those containing a number of smooth muscle cells (SMCs) with myxoid extracellular matrix. There was paucity of cholesterin crystals and aggregation of foamy cells. In addition, scarcely any inflammatory cell filtration was identified. In the section of SMC-containing samples, formation of multiple re-canalized vessels embracing endothelial cells was confirmed. These histopathologic findings indicated that the present coronary artery lesion has a high possibility of very late cardiovascular sequelae caused by arteritis due to KD, rather than arteriosclerosis. This is the oldest adult case with coronary artery disease possibly resulting from KD sequelae. This case highlights that KD sequelae must be considered as a cause of coronary artery lesion even in older patients.

Recently, many UAVs (unmanned aerial vehicles) based on LiDAR (light detection and ranging) systems have been developed for various purpose because of the effective of LIDAR technique and low-cost UAV. In this study, the accuracy of point clouds generated by the developed for a low-cost UAV-based LiDAR systems is evaluated. The system consisting of a multi-beam laser scanner- Velodyne VLP 16 and DJI M600 UAV. The experimental site is undulation with less object in Nagaoka city, Niigata Prefecture, Japan Twelve reflectance makers are arranged as ground control point for the positioning evaluating process. The observed data was collected on Nov. 8th, 2019 with three different flight height at 10m, 20m and 30m. For generating the point clouds, the mounting parameters and sensor parameters are combined. The generated point clouds are corrected by applying bias correction and the 7 parameters transformation. The result is validated using three different experimental setups with three various flight height which indicate that the most accurate and reliable results are obtained. As a result, the point clouds after calibrating attained an accuracy of approximate 0.2 m in vertical and horizontal for both correction methods. In conclusion, the point cloud accuracy is not good enough for generating the topographic map at large scale. However, the stable results and the present accuracy are good for other purposes with less accuracy requirement such as monitoring the crop growth.

Left ventricular outflow tract obstruction (LVOTO) complicated with unstable angina (uAP) has not been described widely, but patients with these two conditions have several problems. Differentiation of the two conditions is also often difficult because the chest symptoms are similar. Moreover, nitrates are commonly used for ischemic heart disease, but have the effect of worsening LVOTO. We experienced three cases of dynamic LVOTO with a sigmoid-shaped septum, and without typical hypertrophic obstructive cardiomyopathy, that were complicated with uAP. In all cases, LVOTO was improved after initial percutaneous coronary intervention (PCI) for the left anterior descending artery lesion. Next, a dobutamine stress test was performed and LVOTO was provoked again in two cases, but not in a case with small acute myocardial infarction of the basal septum during PCI. All cases remained asymptomatic with beta-blocker therapy. Therefore, PCI and beta-blocker administration for LVOTO with uAP resulted in favorable clinical courses in all three cases. These outcomes suggest that revascularization including PCI should have priority in the therapeutic strategy for a case of acute coronary syndrome with LVOTO.

UAV systems are considered effective tools to collect information regarding crops. In this study, the rice growth was observed by a small UAV-based LiDAR system from above. For developing the system, DJI S800 was chosen as a platform on which a non- survey-grade laser scanner HOKUYO UTM30LX-EW was mounted. Field experiments were carried out from late June to late early August 2017 in Nagaoka city, Niigata Prefecture, Japan. Percentile analysis is applied to locate the top and bottom positions of rice plants in three targeted areas. LIDAR-derived plant height is computed by taking the difference between the bottom and the rice plant's top. We consider a method for developing a radio-wave propagation prediction model in a mountainous forested area. A new path loss development approach uses a free-space path loss (FSPL) model and an empirical path loss model. To improve the prediction accuracy, the transmission path distance, free space area, and forest area were calculated separately. We obtained the transmission path distance for free space and forest areas from the digital surface model (DSM), which represents surface elevation information, including vegetation and object height. In this study, the results showed that by combining the empirical model with FSPL for free space area, the accuracy for all the empirical models was improved. We confirmed that the transmission distance calculation of the free space area and forest area with a combination of the empirical models showed a better performance than the model with physical distance. The predicted model results were validated using the actual radio wave propagation in the 920 MHz band measurement data. The overall path loss prediction accuracy was improved for the empirical models average of 8.05 dB on the experimental data.

Since permanent inferior vena cava (IVC) filters increase deep vein thrombosis (DVT), filter retrieval should be performed as possible. Despite the guideline recommendation, IVC filters are not always retrieved in clinical practice. To date, many patients with not-retrieval IVC filters have been prescribed anticoagulant therapy, but the long-term prognosis, including venous thromboembolism (VTE) and bleeding events, remains unknown. In this study, 195 patients who underwent IVC filter implantation between 2006 and 2017 at 3 institutions in Niigata City have been investigated about their deaths, VTE recurrence, and bleeding events. After peaking 2009, the number of IVC filter implantation gradually decreased. During observational period, there were 158 patients with not-retrieval IVC filters (the overall retrieval rate of 19.0%). The not-retrieval group included significantly older and more patients with cancer compared to the retrieval group. Anticoagulation therapy was continued in 88% of the not-retrieval group. During a mean follow-up of 5.0 years, 6 symptomatic DVT events associated with inadequate control of anticoagulation and 13 bleeding events were observed. A total of 52 patients died and only the presence of cancer was prognostic risk factor. Although long-term anticoagulation therapy may be associated with bleeding events, there were few recurrent VTE under optimal anticoagulation. It is anticipated that even if the IVC filter cannot be retrieved, appropriate anticoagulation is useful for prevention of DVT recurrence despite the risk of bleeding.

After taking an estrogen-containing supplement derived from a tropical plant Pueraria mirifica, a 24-year-old woman presented marked QT prolongation and repetitive torsade de pointes. The patient was found to carry a heterozygous KCNQ1-T587M mutation. This is the first report on Pueraria mirifica-related acquired long QT syndrome.

Practical management guidelines for impaired glucose tolerance (IGT) have not been established. Although IGT is a potent marker of cardiovascular disease (CVD), it is still controversial whether its magnitude of CVD risk is comparable to that of frank diabetes. Moreover, information on long-term clinical outcomes of IGT patients undergoing coronary revascularization is limited. The aim of the present work was to investigate the 10-year prognostic impact of IGT in comparison with diabetes in patients with CAD undergoing coronary revascularization. This cohort recruited from two Japanese clinical sites included patients with coronary artery disease (CAD) undergoing percutaneous coronary intervention (PCI) between 2004 and 2008. Patients were categorized into previously known diabetes (PKD, n = 197), newly diagnosed diabetes (NDD, n = 51), and IGT (n = 50) groups according to oral glucose tolerance test results except for PKD. The primary end point was defined as a composite of cardiovascular death, myocardial infarction, stroke, repeat revascularization, and heart failure hospitalization. The cumulative risk of the primary outcome was significantly higher in the PKD and IGT than in the NDD (log-rank test p = 0.017). A Cox proportional hazards model demonstrated that IGT (hazard ratio [HR], 7.91; 95% confidence interval [CI], 1.84–27.58) and creatinine clearance (HR, 7.89, 95% CI, 2.73–19.10) were predictors of long-term CVD risk, while NDD and PKD were not. IGT significantly increased the long-term risk of developing CVD in patients with CAD after PCI compared with diabetes.

Introduction Type 2 diabetes (T2DM) is associated with cardiovascular death, including sudden cardiac death due to arrhythmias. Patients with an implantable cardioverter-defibrillator (ICD) are also at high risk of developing a clinically significant ventricular arrhythmia. It has been reported that sodium–glucose cotransporter 2 (SGLT2) inhibitors can reduce cardiovascular deaths; however, the physiological mechanisms of this remain unclear. It is, however, well known that SGLT2 inhibitors increase blood ketone bodies, which have been suggested to have sympatho-suppressive effects. Empagliflozin (EMPA) is an SGLT2 inhibitor. The current clinical trial titled “Placebo-controlled, double-blind study of empagliflozin (EMPA) and implantable cardioverter-defibrillator (EMPA-ICD) in patients with type 2 diabetes (T2DM)” was designed to investigate the antiarrhythmic effects of EMPA. Methods The EMPA-ICD study is a prospective, multicenter, placebo-controlled, double-blind, randomized, investigator-initiated clinical trial currently in progress. A total of 210 patients with T2DM (hemoglobin A1c 6.5–10.0%) will be randomized (1:1) to receive once-daily placebo or EMPA, 10 mg, for 24 weeks. The primary endpoint is the number of clinically significant ventricular arrhythmias for 24 weeks before and 24 weeks after study drug administration, as documented by the ICD. The secondary endpoints of the study are the change from baseline concentrations in blood ketone and catecholamine 24 weeks after drug treatment. Conclusion The EMPA-ICD study is the first clinical trial to assess the effect of an SGLT2 inhibitor on clinically significant ventricular arrhythmias in patients with T2DM and an ICD. Glycemic variability (GV) is relevant to impaired myocardial salvage in acute ST-elevation myocardial infarction (STEMI). Severity of hypokinesis at the infarct site as assessed from contrast left ventriculography can reportedly predict infarct size in STEMI. We prospectively studied 58 consecutive patients (mean age, 63 ± 11 years) with anterior or inferior STEMI who underwent successful reperfusion therapy. Mean amplitude of glucose excursion (MAGE) was obtained from continuous glucose monitoring system. In patients with chronic heart failure (HF), the clinical importance of sarcopenia has been recognized in relation to disease severity, reduced exercise capacity, and adverse clinical outcome. Nevertheless, its impact on acute decompensated heart failure (ADHF) is still poorly understood. Dual-energy X-ray absorptiometry (DXA) is a technique for quantitatively analyzing muscle mass and the degree of sarcopenia. Fat-free mass index (FFMI) is a noninvasive and easily applicable marker of muscle mass.This was a prospective observational cohort study comprising 38 consecutive patients hospitalized for ADHF. Sarcopenia, derived from DXA, was defined as a skeletal muscle mass index (SMI) two standard deviations below the mean for healthy young subjects. FFMI (kg/m2) was calculated as 7.38 + 0.02908 × urinary creatinine (mg/day) divided by the square of height (m2).Sarcopenia was present in 52.6% of study patients. B-type natriuretic peptide (BNP) levels were significantly higher in ADHF patients with sarcopenia than in those without sarcopenia (1666 versus 429 pg/mL, P < 0.0001). Receiver operator curves were used to compare the predictive accuracy of SMI and FFMI for higher BNP levels. Areas under the curve for SMI and FFMI were 0.743 and 0.717, respectively. Multiple logistic regression analysis showed sarcopenia as a predictor of higher BNP level (OR = 18.4; 95% CI, 1.86-181.27; P = 0.013).Sarcopenia is associated with increased disease severity in ADHF. SMI based on DXA is potentially superior to FFMI in terms of predicting the degree of severity, but FFMI is also associated with ADHF severity.

Unmanned Aerial Vehicles (UAVs) have popularly used as a controllable platform for remote data acquisition. In this study, the DJI spreading wings S800 (DJI S800) was selected as platform to develop an UAV-based system for monitoring the rice crop. The used sensor is Hokuyo UTM30LX laser scanner with 0.37 kg of weight, detection range from 0.1 to 30 m with the wide angle of 270°. The test flight was performed for assessing the accuracy of 3D point clouds at a small and flat experimental site in Kumamoto Prefecture, Japan on Dec. 18th, 2014. Nine ground control points were arranged and their coordinates were identified in the local coordinate system using a total station. By using the GPS data for determining flight direction, the 3D coordinates of point clouds were generated. The location of markers was identified directly from 3D point cloud, and their observed coordinates were manually measured. The 7 parameters transformation was applied with four ground control points. As a result, the 3D point cloud was achieved with the mean absolute error (MAE) of 3.13 m in horizontal and 1.09 m in vertical in cases of non-applying correction method. In case of applying 7 parameters transformation, the absolute error of 3D the point cloud generated from the developed system was improved to 1 m in horizontal and several decimetres in vertical. Although the accuracy of 3D point cloud is not suitable for mapping purpose but is good for the purpose of monitoring the crop in precision agriculture.

Objective Left ventricular outflow tract (LVOT) obstruction is a complication in 15-25% of patients with Takotsubo cardiomyopathy and sometimes leads to catastrophic outcomes, such as cardiogenic shock or cardiac rupture. However, the underlying mechanisms have not been clarified. Methods and Results We experienced 22 cases of Takotsubo cardiomyopathy during 3 years, and 4 of these 22 cases were complicated with LVOT obstruction in the acute phase (mean age 79±5 years, 1 man, 21 women). The LVOT pressure gradient in the acute phase was 100±17 mmHg. Transthoracic echocardiogram (TTE) revealed left ventricular hypertrophy (LVH) in one case and sigmoid-shaped septum without LVH in three cases. The complete resolution of the LVOT obstruction was achieved in a few days with normalization of the left ventricular wall motion following administration of beta-blockers. A dobutamine provocation test after normalization of the left ventricular wall motion reproduced the LVOT obstruction in all cases and revealed the presence of latent LVOT obstruction. Conclusion The manifestation of latent LVOT obstruction in the acute phase of Takotsubo cardiomyopathy is one potential reason for the complication of LVOT obstruction with Takotsubo cardiomyopathy.

GNSS reflectometry involves making measurements from the reflections from the Earth of navigation signals from Global Navigation Satellite Systems such as GPS and QZSS. Mudflats or mud flats, also known as tidal flats, are coastal wetlands that form when mud is deposited by tides. Tidal flats may be viewed geologically as exposed layers of bay mud, resulting from deposition of estuarine silts, clays and marine animal detritus. Most of the sediment within a mudflat is within the intertidal zone, and thus the flat is submerged and exposed approximately twice daily. Tidal flats are important ecosystems. In modern Japan, we have landfilled tidal flat to create a flat ground and used it as a factory area and housing. To protect the ocean ecosystem, it is necessary to investigate the topography change of the tidal flats. We studied to explore the topography of tideland as utilization of GNSS-R. The QZSS satellite stays at a high elevation angle for a relatively long time compared to the GPS satellite. Radio waves of satellites with a low elevation angle strongly reflect, but the delay time of reflected waves is short. A satellite with a high elevation angle has a small reflected wave, but its delay is large, so it is easy to separate the reflected wave. In this research, we report the result of examining GNSS-R using QZSS.

This study was conducted to assess whether any relationships exist between glucose fluctuations and electrocardiographic surrogate markers of reperfusion injury in patients with ST-elevation myocardial infarction (STEMI) undergoing primary percutaneous coronary intervention (PCI).We prospectively studied 63 consecutive patients with STEMI undergoing primary PCI. Patients had either diabetes (n = 30), impaired glucose tolerance (n = 26), impaired fasting glucose (n = 1), or normal glucose tolerance (n = 6). STsegment resolution (STR, %) was measured using electrocardiograms recorded 60 minutes after PCI. STR was categorized as ≥ 30% and < 30%. Glucose fluctuations were assessed by the following parameters obtained from a continuous glucose monitoring system: mean amplitude of glucose excursion (MAGE, mg/dL); and area under curve with reference to mean blood glucose (AUCMBG, mg/ dL/day).Both MAGE and AUCMBG were significantly higher in STR < 30%. In univariate analysis, MAGE ≥ 70 mg/dL (OR = 17.0; 95%CI, 1.93-150.12; P < 0.01), AUCMBG ≥ 20 mg/dL/day (OR = 10.9; 95%CI, 1.92-61.77; P < 0.01), and reperfusion arrhythmias (OR = 7.6; 95%CI, 1.32-44.29; P < 0.05) were significantly associated with suboptimal STR. Multiple logistic regression analysis showed only MAGE ≥ 70 mg/dL was predictive of suboptimal STR (OR = 22.5; 95%CI, 2.43-208.66, P < 0.01).Parameters of glucose fluctuations correlated with electrocardiographic surrogate markers of impaired myocardial salvage in STEMI after reperfusion therapy. Our results suggest that glucose fluctuations may represent a potential therapeutic target to reduce myocardial reperfusion injury in STEMI.

Generally, it is important for commercial products to design their shapes considering not only functionality but also usability. We have been discussing methodologies to analyze 3D polygon data of gripping object. Thanks to our previous works, we obtained much know-hows to design easy-to-grasp objects. In this paper, the authors applied the knowledge to design a smartphone case. The prototype case based on the knowledge is manufactured and its usability was evaluated with electromyogram.

A 70-year-old man with severe symptomatic functional mitral valve regurgitation underwent successful mitral valve repair combined with tricuspid valve ring annuloplasty. Pre-operative coronary angiography (CAG) showed no significant stenosis. One-and-a-half years later, the patient presented with an acute exacerbation of heart failure. Repeat CAG demonstrated tight stenosis in the right coronary artery (RCA) with arterial kinking that corresponded to the same point as the stenosis where the RCA is the closest to the tricuspid valve ring. The new lesion probably occurred as a consequence of the tricuspid valve ring annuloplasty.

This position paper introduces a new paradigm in human friendly computing and systems such that the shape, material and touch of a device case significantly matter. While casing has been considered as an auxiliary, that for mobile devices such as smart-phones becomes quite critical for their use. Since casing is an important part of user experiences of mobile devices, we believe that its integrity within information technologies plays an important role, thus needs to be well established.

A man with a wide-necked renal artery aneurysm underwent combination therapy involving stenting and coiling to embolize it. However, a large section of one of the coils had migrated outside the stent implanted across the orifice of the aneurysm. After repositioning the migrated coil inside the stent, the implantation of another stent together with sandwiching of the coil between two stents repaired the prolapsed coil. This report describes precautions that should be taken during combination therapy involving stenting and coiling to embolize a wide-necked aneurysm.

To prevent side branch occlusion during bifurcational lesion stenting, the placement of a wire in both the main branch and side branch is performed for "side-branch protection". However, this procedure does not always prevent side branch occlusion. A procedure for placing two wires in the side branch, called "two-wire protection of side branches", is considered to be more likely to prevent occlusion compared with one-wire protection of the side branch. We report on three cases in which "two-wire protection of side branches" was effectively performed during the stenting of bifurcational lesions.

Summary Background Peroxisome proliferator-activated receptor-gamma (PPAR-γ) agonists are used as anti-diabetic drugs, and their pleiotrophic action has been reported to improve endothelial function leading to cardioprotective effects. In this study we evaluated the long-term effect of pioglitazone on cardiac function in diabetic patients after percutaneous coronary intervention (PCI) by drug-eluting stent (DES). Methods and results We investigated 54 diabetic patients who received PCI using a sirolimus-eluting stent. We excluded cases of acute myocardial infarction. They were divided into two groups: Group C received only conventional therapy (n = 26) and Group P received additionally pioglitazone 15 mg/day (n = 28). The left ventricular ejection fraction (LVEF) was measured by left ventriculography and analyzed before and 8 months after PCI. In Group C, LVEF did not change significantly: 55.6% vs. 56.7%, before and after PCI respectively (p = 0.58). However, pioglitazone significantly improved LVEF: 54.4% vs. 60.0% (p = 0.014). Multiple linear regression analysis showed that ΔLVEF was significantly related to pioglitazone therapy (p = 0.037). In particular, the combination of pioglitazone and statin improved LVEF (ΔLVEF 9.6% with vs. 2.2% without statin). Conclusions Pioglitazone improved cardiac function after PCI using SES in diabetic patients, especially in combination with a statin.

Specular reflection on the surface of target material causes huge error in the measured spectral reflectance. It is important to eliminate the specular reflection components for estimation of the diffuse reflection components that shows the true spectral characteristics.Today, a study about separating specular and diffuse reflection components has been vigorously conducted in the field of computer vision and the computer graphics. Various methods have been proposed to separate these two components using the polarizing filters and so on. But this method should install the polarizing filters in the instruments to polarize the incident light. Therefore, this method is not suitable to use for a spectral measurements because the optical characteristic is changed.In this paper, we propose a novel method based on dichromatic reflection model with multiple viewing angle reflection data to estimate the diffuse reflection components from the measured spectral reflectance data of the material surface with luster such as leaves of plant and plastics without any additional filters and instruments. Finally, an experiment using a color sample sheet is executed for demonstrating the feasibility of the proposed method.

The possibility to estimate the vegetation cover rate of rice plants using the laser scanner measurement over the rice plants was examined. The vegetation cover rate and 3D point cloud data of rice plants were acquired during the growing stage in the test field. In order to examine a new method without the detection process of the ground, the rice plant's crown was determined by the 3D point cloud data. The observation point density corresponding to the laser pulse incidence depth below the rice plant's crown was calculated from the 3D point cloud data. The vegetation cover rate was compared with the laser pulse penetration depth. As a result, the laser pulse incidence depth was a good parameter to estimate the vegetation cover rate.In this study, the estimation error was 3% using the laser pulse incidence depth at 90% of the accumulated observation point density below the rice plant's crown.

SummaryA 69-year-old woman underwent percutaneous coronary intervention for a severe stenotic lesion in the bifurcation of the mid-left anterior descending artery and first diagonal branch. A single stent was implanted into the left anterior descending artery. After the stent strut was dilated by balloon inflation in the diagonal branch, dissection occurred at the ostium of the diagonal branch and resulted in side branch occlusion due to hematoma. Bailout stenting was performed in the diagonal branch, but thrombus projection occurred in the left anterior descending artery. Aspiration, balloon inflation and thrombolytic therapy were performed, but distal embolism developed. This case illustrates that thrombus projection caused by stenting in a side branch may occur as a rare complication in percutaneous coronary intervention.

Remote sensing is a technology which acquires the physical information of a measuring object indirectly based on the radiation acquired by a remote sensor. Therefore, to systematize the technology of converting the radiation to physical quantity is an important subject for remote sensing to spread in all the natural science fields.We have been developed a portable line spectrum radiometer, which are constructed entirely from commercially available components. Developed radiometer has steadiness and lightness with the commercial digital camera as a measurement unit. These two features are important in field use. And, developed radiometer was proven to be effective by actually measuring the sample color chart in the relative comparison of a spectral pattern.In this paper, we describe the instrument design of developed radiometer. Especially, we discusses it through the examination of the signal-noise characteristics based on the emphasis on the measuring a spectral pattern with the high signal-noise characteristics.
