Successful resection of intra-abdominal tumors using indocyanine green (ICG) fluorescence imaging has not been reported. Here, we report a rare case of an intra-abdominal desmoid-type fibromatosis successfully resected using this technique after intersphincteric resection (ISR) for rectal cancer. One year after ISR for rectal cancer in a 47-year-old man, computed tomography showed a 50-mm intra-abdominal tumor near the left common iliac vein. Surgical resection was performed. The tumor was located in the mesentery of the remnant rectum after ISR. ICG fluorescence imaging confirmed the blood supply to the mesentery of the distal remnant rectum after tumor excision. The anal canal was successfully preserved without creating a permanent colostomy. The tumor was safely resected with resection margins, diagnosed as desmoid-type fibromatosis. No tumor recurrence was observed 6 months post-resection. This was the first case report to demonstrate the utility of this technique for an intra-abdominal tumor resection.

A male in his twentieth was referred to our hospital for jaundice. Computed tomography(CT)showed dilation of the intrahepatic and extrahepatic bile ducts and showed a lesion at the ampulla of Vater, which caused obstructive jaundice. Upper gastrointestinal endoscopy revealed a tumor of protruded-predominant type with raised margins at the ampulla of Vater, and biopsy from the lesion indicated malignancy. With no apparent distant metastasis, radical resection was assumed to be possible, thus we performed subtotal stomach preserved pancreatoduodenectomy. Before the operation, endoscopic retrograde biliary drainage(ERBD)was unsuccessful because of the existence of the tumor, so percutaneous transhepatic cholangio drainage(PTCD)was conducted. After the operation, although pancreatic fistula(ISGPF Grade B)occurred, it improved with conservative treatment, and he discharged at 30 postoperative days. Histopathological examination revealed signet-ring cell carcinoma among the tumor at the ampulla of Vater, which was infiltrating into the pancreas. Final diagnosis was pT3, pN0, M0, pStage ⅡA. Now he is alive without recurrence for 3 and a half years.

A woman in her 50s underwent sigmoid colectomy and D3 lymph node dissection for sigmoid cancer(pT3, N0, M0, Stage Ⅱ: Japanese Classification of Colorectal, Appendiceal, and Anal Carcinoma 9th). She received adjuvant chemotherapy with capecitabine. Seven months after surgery, contrast-enhanced computed tomography( CECT) scan revealed a small mass in the segment 2 (S2) of the liver with dilation of peripheral intrahepatic bile duct, and the size of this mass and the bile duct dilatation were gradually increased. FDG positron emission tomography(FDG-PET)/CT showed abnormal FDG uptakes in the lesion of S2, and EOB-MRI detected other small lesions in the S6 and S7. Considering the results of image examinations, multiple lesions intrahepatic cholangiocarcinoma was firstly assumed. However, immunohistochemistry of the tumor obtained by endoscopic retrograde cholangiopancreatography (ERCP) showed cytokeratin 7-negative. Based on preoperative diagnosis of liver metastasis from colon cancer rather than intrahepatic cholangiocarcinoma, we performed left lobectomy, partial hepatectomy of S6 and S7 and cholecystectomy. In the resected specimen, the tumor was macroscopically located in the intrahepatic bile ducts. Microscopically, there existed atypical epithelial cells with glandular duct-like structure, and the lesions was histopathologically diagnosed as metastasis from colon cancer. She was discharged on the 10th postoperative day, and she is alive without recurrence one year after surgery.

A man in his 50s underwent laparoscopic sigmoid colectomy for sigmoid colon cancer with liver metastasis(cT4aN1M1a, cStage Ⅳa), followed by partial liver resection(S4, S6). One and a half years after the initial surgery, CEA and CA19-9 increased, and contrast-enhanced CT and MRI showed a hypovascular lesion with dilation of the distal pancreatic duct in the pancreatic body. Adenocarcinoma was detected by brushing cytology of the lesion and pancreatic juice cytology by ERCP. From the results of various examinations, the lesion was diagnosed as pancreatic ductal adenocarcinoma. We performed distal pancreatectomy, and initially the histopathological diagnosis was pancreatic body cancer(pT3N1aM0, pStage ⅡB). In a follow-up CT after surgery, a suspected metastatic lymph node was pointed out in the mediastinum, but it was difficult to distinguish between metastasis from colorectal cancer and one from pancreatic cancer. Immunostaining of the tumor tissue and comparative study of the excised specimens of colon and pancreas was performed in order to assume the primary lesion of the lymph node. As a result, both tissues were CK7(-)/CK20(+), and the lesion at first considered to be primary pancreatic cancer was originally the pancreatic metastasis from colon cancer. Bone metastases were also found on FDG-PET/CT around the same time, and then systemic chemotherapy for colorectal cancer was introduced. Four and a half years have passed since the first surgery, and he is still alive and undergoing treatment.

A man in his 60s underwent pancreaticoduodenectomy(PD)for ampullary cancer(pT3, N1, M0, pStage ⅡB). One year after the operation, lymph node recurrence was observed, and gemcitabine/cisplatin regimen was introduced. Once the disease was controlled, but the tolerability of chemotherapy deteriorated due to severe anorexia and general fatigue around the 6th course of the regimen, thus we stopped the chemotherapy. The performance status was remarkably dropped and the oral intake was notably decreased, and subsequently he got hospitalized. Blood examination on admission showed signs of liver failure(Alb 1.9 g/dL, T-Bil 2.0 mg/dL, PT 36.2%, NH3 159μg/dL). CT scan revealed marked deterioration of fatty liver and newly recognized retention of ascites. We diagnosed this clinical state as liver dysfunction caused by aggravation of nonalcoholic fatty liver disease(NAFLD)due to pancreatic exocrine insufficiency after PD. Immediately pancreatic enzyme replacement therapy(pancrelipase)was performed, and liver function gradually improved. He was discharged 17 days after admission. One month after discharge, CT scan showed that the fatty liver was markedly improved and the ascites disappeared. Around the same time, the chemotherapy was able to be resumed with the improvement of general condition.

A 82-year-old man presented with diarrhea and fatigue. He had no past medical or surgical history except chronic renal failure. Locally advanced rectal cancer with invasion to left ureter was detected in computed tomography. Colonoscopy revealed a circular lesion 12 cm from the anal verge. Biopsy showed moderately differentiated adenocarcinoma. There was no sign of distal metastasis and we decided to conduct radical surgery. Robot-assisted laparoscopic lower anterior resection with partial resection of left ureter, and diverting ileostomy were carried out. Besides, urinary tract reconstruction of ureterocystoneostomy using Lich-Gregoir technique was conducted by urologists also with robot assistance. The pathological stage of the disease was pT4b(left ureter)N1bM0, pStage Ⅲc. The resection margin was secured and radical surgery was achieved. The patient was discharged on postoperative day 22nd without postoperative complication. He is alive without recurrence at 6 months after the operation.

Patient undergoing R0 resection have the good survival advantage following surgery for recurrent rectal cancer. Robotic surgery for anastomotic local recurrence of rectal cancer has never been reported before. An 80-year-old woman who had undergone high anterior resection for rectal cancer 1 years previously got colonoscopy. It revealed a 10 mm sized, elevated lesion on the anastomotic site, and a biopsy revealed an adenocarcinoma(tub1). Computed tomography and positron emission tomography-computed tomography showed no signs of distant metastasis, and we decided to conduct radical surgery with robot-assisted laparoscope. Robot-assisted laparoscopic lower anterior resection with resection of left hypogastric nerve was carried out. Histological assessment showed that R0 resection was performed. The patient was discharged on postoperative day 8th without postoperative complication. She is alive without recurrence at 1 year after the last operation.

The case was a 61-year-old woman. She was diagnosed with autosomal dominant polycystic kidney disease(ADPKD)at the age of 38 and started hemodialysis at the age of 42. She was diagnosed with rectal cancer(RS)at the age of 61. Laparoscopic high anterior resection and D3 lymphadenectomy were carried out. Although the intra-abdominal space was limited by the huge renal cysts, laparoscopic surgery can be safely performed by arranging the port closer to the midline, taking the patient's position sufficiently, and using some useful tips. Laparoscopic surgery for the patient with ADPKD was considered a useful approach.

We reported a case of rectal cancer with unresectable liver metastases treated with resection of the primary lesion followed by systemic chemotherapy with curative resection. A woman in her 40s was diagnosed with rectal RS carcinoma and unresectable liver metastasis, mFOLFOX6 plus panitumumab therapy was initiated after laparoscopic high anterior resection of the rectal lesion. After 5 courses of chemotherapy, significant shrinkage of the liver metastatic lesion and increase of the remnant liver volume were observed. Percutaneous transhepatic portal vein embolization( PTPE) was performed with the aim of further preserving remnant liver volume. Since the hepatic reserve was sufficient, the treatment strategy was to perform radical hepatectomy. Extended right hepatic lobectomy, S4 partial resection, and cholecystectomy were performed. The patient didn't relapse at 11 months after hepatectomy.

In Japan, the standard treatment for squamous cell anal cancer(SCAC)has not been established. Herein, we report a case of SCAC that completely responded to chemoradiotherapy(CRT).A woman in her 80s presented with anal pain and bleeding. Computed tomography revealed bilateral inguinal adenopathy and a tumor in the anal canal. Histopathological examination of endoscopic biopsies showed adenocarcinoma. Thus, she was diagnosed with anal canal adenocarcinoma and lymph node metastases:cT3, cN1a(No. 292), cM0, cStage Ⅲc(Japanese Classification of Colorectal, Appendiceal, and Anal Carcinoma, 9th edition). Owing to her advanced age and refusal of a stoma, CRT(S-1, mitomycin C, and radiotherapy)was administered with the expectation that salvage surgery in the form of rectal amputation would eventually be necessary. The tumor noticeably shrank after CRT. The patient is alive to this date,14 months after the final round of CRT.

A woman in her 60s realized heart palpitations and was pointed out anemia. CT revealed a tumor measuring 7 cm, with internal necrosis, originating from the gallbladder and invading the liver, and diagnosed as gallbladder cancer. There existed no distant metastasis and we performed cholecystectomy with partial resection of segment 4a+5 of the liver and lymph node resection. Histopathological examination revealed highly atypical cells with large nuclei and polynuclear cells and poor cell junctions in the specimen, and the tumor was histologically diagnosed as an undifferentiated carcinoma. Metastases were not detected in dissected lymph nodes, and this case was diagnosed as undifferentiated carcinoma of gallbladder, T3a, N0, M0, Stage ⅢA(JSHBPS 6th). She was discharged at 13 days after the operation with no apparent postoperative complications. Postoperative adjuvant chemotherapy with administration of TS-1 was conducted for half a year. Now over 5 years have passed since the operation, and she is alive without recurrence.

The risk factors for recurrence in patients with pStage Ⅱ colorectal cancer still remains controversial. The aim of this study was to investigate the risk factors for recurrence after surgery in patients with pStage Ⅱ colorectal cancer.We retrospectively reviewed 311 patients with primary pStage Ⅱ colorectal cancer who underwent radical resection without neoadjuvant therapy at our institute between January 2014 and December 2019.Of 311 patients, 32 patients(10.3%)developed recurrences after surgery at a median follow-up of 32.9(0.23-74.2)months. The 3-year and 5-year recurrence-free survival(RFS)rate was 88.4% and 87.6%, respectively. A multivariate analysis for RFS showed that only pT4 (HR: 4.06, 95%CI: 1.60-10.29, p=0.003) was an independent risk factor.This study revealed that pT4 was an independent risk factor for recurrence after surgery in patients with pStage Ⅱ colorectal cancer.

Purpose: Neoadjuvant chemotherapy (NAC) followed by surgery is a promising treatment strategy for patients with advanced gastric cancer. Severe toxicity associated with the treatment may reduce the dose intensity of chemotherapy, resulting in the effect of chemotherapy being attenuated. Recently, skeletal muscle mass has been reported to be associated with the treatment outcomes of cancer patients. The purpose of this study was to clarify whether pretreatment skeletal muscle mass is a predictor of adverse events as well as the relationship between changes in skeletal muscle mass and adverse events during NAC. Methods: This study included 41 advanced gastric cancer patients who were treated with NAC followed by surgery. Body composition was assessed before and after NAC. The relationship between the pre-NAC body composition and adverse events was investigated as well as the relationship between changes in body composition and adverse events. Results: Univariate and multivariate analyses revealed that low pre-NAC skeletal muscle mass was the only factor significantly associated with severe diarrhea (p = 0.03). There was no significant difference in body weight before and after NAC, but skeletal muscle mass was significantly reduced after NAC (–5.93 ± 7.69%, p < 0.01). Furthermore, patients who experienced severe diarrhea showed significantly greater relative skeletal muscle decrease than those who did not (p = 0.03). Conclusions: Pre-NAC low skeletal muscle mass was a useful predictor of severe diarrhea. Prevention of severe adverse events may contribute to maintaining the skeletal muscle mass.

There are few reports of using oxaliplatin(L-OHP)for esophageal squamous cell carcinoma.We report a case of long-term disease control after administration of L-OHP for esophagogastric junction squamous cell carcinoma(EG).A woman in her 40s was diagnosed with EG(cT3, cN2, cM0, cStage Ⅲ).She received thoracoscopic, laparoscopic-assisted, subtotal esophagectomy with 3-field dissection after 3 courses of preoperative chemotherapy with DCF(docetaxel hydrate, cisplatin, 5- fluorouracil).Reconstruction was achieved using a retrosternally shifted gastric tube and transesophageal gastro-tubing. Pathological examination showed EG(ypT3, ypN2, ypM0, ypStage Ⅲ)(chemotherapy evaluation: Grade 1a).After 12 months, para-aortic lymph node recurrence(#112aoP, #16a2lat)was observed on a follow-up CT examination.First, we administered 5 courses of chemotherapy with SOX(S-1 100mg/m2 day 1-14 and L-OHP 100 mg/m2 day 1).Recurrent lymph nodes shrunk slightly, and there were no new lesions.Subsequently, there was no other adverse event except for Grade 1 chemotherapy-induced neuropathy.Second, we administered 3 courses of chemotherapy with FOLFOX(5-FU 400mg/m2, L-OHP 85mg/m2, Leucovorin 200 mg/m2 day 1, 5-FU 1,600mg/m2/46 hr)in combination with radiotherapy(total 60 Gy/ 30 Fr).Recurrent lymph nodes shrunk slightly, and there were no new lesions.Subsequently, there was no other adverse event except for Grade 1 chemotherapy-induced neuropathy.No new recurrence was observed 19 months after the first recur- rence.The patient continues to receive weekly nab-PTX(85mg/m2).A regimen that includes L-OHP is useful in the treatment of advanced or recurrent esophageal squamous cell carcinoma.

A 78-year-old man who underwent esophagectomy for middle thoracic esophageal squamous cell carcinoma(pT1bN0M0, pStage Ⅰ)was diagnosed with lymph node recurrence 12 months after the initial surgery. He received chemoradiotherapy(5- fluorouracil plus cisplatin); however, the treatment was terminated at the middle of the treatment course because of progressive disease. He received chemotherapy(docetaxel plus 5-fluorouracil plus cisplatin)as a second-line treatment, which was also canceled due to serious adverse events. Partial response was achieved after the second therapy; therefore, surgical excision was performed. Thirteen months after the second surgery, he was diagnosed with second local recurrence with invasion to the trachea. Another course of chemotherapy(docetaxel[2-weekly]plus 5-fluorouracil plus cisplatin)was administered, which also achieved a partial response. Thus, surgical excision with partial tracheal resection and mediastinal tracheostomy was performed. He has been alive without recurrence for 6 months after the final operation. In case of postoperative solitary lymph node recurrence of esophageal cancer, long-term survival can be expected with multidisciplinary treatment.

Case 1: A man in his 70s was referred to our hospital for further examination of a liver tumor(S3, 3 cm)detected by ultrasonography. Multimodal image examination showed a cystic lesion with solid papillary components located in the S4 accompanied by dilatation of the surrounding intrahepatic bile duct. Although biliary cytology did not indicate confirmed malignancy, the lesion was thought to be an intraductal papillary neoplasm of bile duct(IPNB)with malignant potential, and a left lobectomy was performed. Histopathological examination revealed a papillary tumor in the intrahepatic bile duct which consisted of atypical epithelial cells of pancreatobiliary type, and the lesion was diagnosed as an IPNB with high-grade intraepithelial neoplasia. Case 2: A woman in her 70s was referred to our hospital because of a liver tumor(S4, 8 cm)detected by ultrasonography. Multimodal image examination showed a cystic lesion localized to the liver(S3, 8 cm), and endoscopic retrograde cholangiopancreatography(ERCP)showed continuity of the cyst and the intrahepatic bile duct. The biliary cytology was positive, and the lesion was thought to be a malignant IPNB. After preoperative drainage of the cystic lesion, a left lobectomy was conducted. Histopathological examination showed that the papillary tumor localized to the bile duct and atypical epithelium cells of pancreatobiliary type were infiltrating into the surrounding matrix. We diagnosed this tumor as an IPNB with an associated invasive carcinoma.

A woman in her 40s was hospitalizedfor jaundice. Six years before, she hadbeen diagnosedwith synchronous esophageal andgastric cancers andhadund ergone subtotal esophagectomy andtotal gastrectomy, accompaniedby reconstruction with the pedicled jejunum. Multimodal imaging revealed a tumor at the pancreatic head, probably pancreatic cancer, which induced severe stenosis of the intrapancreatic bile duct. Scraping cytology findings of the lesion via the percutaneous transhepatic cholangial drainage(PTCD)route strengthenedthe suspicion. In the image, although no obvious invasion of the major vessels or apparent distant metastases were detected, an abnormal shadow was found continuously lining the main tumor andpara -aortic region, which was a contraindication for curative resection. Therefore, we performed neoadjuvant chemotherapy with gemcitabine plus S-1. After 3 courses, the lesion size reduced notably, and pancreatoduodenectomy was performed. The pathological diagnosis was pancreatic cancer(ph, ypT3, ypN1a, ypM0, ypStage ⅡB). Except for pancreatic fistulas(Clavien-Dindo Ⅲa), the postoperative clinical course was uneventful, andshe was dischargedon postoperative day 27. To date, the patient is alive without recurrence and is undergoing adjuvant chemotherapy with S-1.

A man in his 60s reported upper abdominal pain; close examination revealed a tumor in the body-tail of the pancreas that was suspected to be infiltrating the stomach. Multiple liver lesions(S3, S4)were also detected. Histological examination by EUS-FNA showed poorly-differentiated carcinoma; thus, this case was diagnosed with unresectable pancreatic cancer with liver metastases(cT3, cN1[No. 7], cM1[P0, H1], cStage Ⅳ: JPS 7th). After 2 kinds of systemic chemotherapy(9 courses of GEM plus nab-PTX and 9 courses of modified FOLFIRINOX), obvious distant metastases or local progression did not appear and conversion surgery was scheduled. Although a metastatic lesion was identified at S5 of the liver just before the surgery, it was assumed that an R0 resection could be achieved; therefore, the operation(distal pancreatectomy with combined proximal gastrectomy, left adrenalectomy, lymph node dissection, partial hepatectomy of S5, and cholecystectomy)was performed. Histopathological examination showed squamous metaplasia of the epithelial tissue combined with glandular formation. This case was, thus, diagnosed as adenosquamous carcinoma of pancreas. This patient was discharged 90 days after the operation. The patient is still alive 2 years and 2 months since the first diagnosis.

Objective: Neoadjuvant therapy followed by surgery has been the standard treatment for advanced esophageal cancer. Severe toxicities may influence body composition, including skeletal muscle mass, and increase postoperative complications. The purpose of this study was to evaluate the influence of sarcopenia, changes in body composition, and adverse events during neoadjuvant chemotherapy (NACT) on postoperative complications in esophageal cancer patients. Methods: A total of 83 patients with esophageal cancer undergoing NACT followed by esophagectomy were included. Body composition was assessed before chemotherapy and before esophagectomy. The relationships between postoperative infectious complications and sarcopenia, changes in body composition, and adverse events during NACT were investigated. Results: Univariate analysis revealed that skeletal muscle loss during NACT, but not preoperative sarcopenia, was significantly higher in the complication (+) group. Febrile neutropenia tended to occur frequently in the complication (+) group. Multivariate analysis demonstrated that skeletal muscle loss was the only factor significantly associated with infectious complications (p = 0.029). Among adverse events, febrile neutropenia was significantly associated with a decrease in skeletal muscle mass. Conclusion: Loss of skeletal muscle mass during NACT was a significant risk factor for postoperative infectious complications in patients with esophageal cancer. Prevention of severe adverse events may reduce postoperative infectious complications.

Case 1 is a 68-year-old woman with locally recurrent rectal cancer(LRRC)developed 5 years after resection of primary rectal cancer. The tumor seized right lateral side in pelvic. We performed tumor excision after preoperative chemoradiation comprised external beam radiation with oral S-1(tegafur/gimeracil/oteracil). He has been relapse-free for 3 years 3months after surgery. Case 2 is a 74-year-old man with LRRC developed 2 years after resection of primary rectal cancer. The tumor was located dorsal to anastomosis site in pelvic. We performed abdominoperineal resection for LRRC after preoperative chemoradiation with oral S-1. He has been relapse-free for 2 years. It was suggested that preoperative radiotherapy combined with oral FU for local recurrence after rectal cancer may contribute to distant and local control.

A 67-year-oldman underwent lower anterior resection for rectal cancer andresection of liver metastatic tumor 5 years later. Seven years and 2 months after the initial surgery, a soft tissue mass was detected in the left diaphragm. Further retrospective review of CT scan images showedthat the diaphragmatic tumor was present just before the hepatectomy. Partial resection of the left diaphragm was performed, and no relapse has occurred since then for 2 years. Most cases of diaphragmatic metastasis are considered to arise from dissemination, but we considered this case as more likely to be hematogenous. When surgery is chosen to treat metastatic tumors of colorectal cancer, checking for other metastasis via preoperative imaging andperforming curative resection is important.

Background Intra-thoracic stomach (ITS) is a rare type of hiatal hernia. Laparoscopic surgery for ITS is technically demanding due to the distorted surgical anatomy. We incorporated assistive esophagoscopy during laparoscopic surgery for ITS. In this study, we assessed the clinical value of esophagoscopy in laparoscopic surgery.

It is sometimes difficult to differentiate between metastatic and primary liver tumors, when the liver tumor occurs simultaneously with a gastric cancer. We encountered a case of resected gastric cancer, which occurred concomitantly with intrahepatic cholangiocarcinoma after S-1 plus cisplatin chemotherapy, in a patient who was previously diagnosed with metastatic liver tumor before treatment. An 80-year-old man was admitted to our hospital because of epigastralgia. Endoscopic study of the upper gastrointestinal tract showed a type 3 tumor at the upper body of the stomach. A plain CT scan showed an irregular, low-density area, which was enhanced by contrast medium in the lateral segment of the liver. We performed an ultrasound- guided needle biopsy, because it was impossible to make a definitive diagnosis by dynamic CT, contrast-enhanced ultrasonography, and MRI. Immunohistochemical analysis for cytokeratin 7/20 resulted in 7 (+)/20 (-) for both the gastric cancer and the liver tumor. Therefore, we diagnosed the patient with gastric cancer, which occurred concomitantly with the metastatic liver tumor, and administered chemotherapy with S-1 plus cisplatin. After 3 courses of the regimen, a reduction in the size of mass was observed in the stomach and the liver. We subsequently performed left hepatectomy and total gastrectomy with lymph node dissection. Microscopic examination revealed the gastric cancer, which occurred simultaneously with the intrahepatic cholangiocarcinoma. The postoperative course was uneventful, and the patient remains well without recurrences.

Skin metastases from esophageal cancer are comparatively rare but have poor prognosis. Here, we report a case of esophageal cancer metastases to the skin. A 70-year-old man was admitted to our hospital for nodules with ulceration in the nose, and biopsy revealed a metastatic carcinoma. FDG-PET indicated FDG accumulation in the skin, liver, and esophagus, while an endoscopic study of the upper gastrointestinal tract showed a type 3 tumor at the upper mid-thoracic esophagus. We diagnosed the patient with metastatic esophageal cancer and administered chemotherapy. Radiation therapy (40 Gy/20 Fr) was simultaneously administered for the tumor in the nose because the patient's quality of life (QOL) decreased daily owing to pain and bleeding caused by metastatic skin cancer. The radiation therapy reduced the size of the tumor in the nose, but the tumor had remained along with the presence of a scar 3 to 6 months after the start of radiation therapy. Radiation effectively promoted the QOL of our patient with skin metastases.

Here, we investigated the efficacy of chemotherapy with trastuzumab for patients with advanced or recurrent gastric cancer.We investigated the progression free survival (PFS) and overall survival (OS) of advanced gastric cancer patients who received first-line chemotherapy with trastuzumab in our hospital.After first-line chemotherapy, the median PFS and median OS of patients who received trastuzumab combined with capecitabine/cisplatin chemotherapy was not significantly longer than those of patients who received trastuzumab combined with S-1/cisplatin chemotherapy (PFS, 138 [95%CI: 118-187] vs 169 [95%CI: 83-251] days, p=0.9684; OS, 393 [95%CI: 240-469] vs 466 [95%CI: 256-482] days, p=0.4703). After second-line chemotherapy, the median PFS of patients who received trastuzumab plus irinotecan chemotherapy was not significantly longer than that of patients who received trastuzumab plus paclitaxel chemotherapy (PFS, 63 [95%CI: 52-266] vs 58 [95%CI: 26-184] days, p=0.5447).

We report the cases of 2 patients with advanced rectal cancer who achieved pathological complete response to treatment with preoperative therapy. Case 1 was a 72-year-old man diagnosed with cT3N1M0 advanced rectal cancer. Neoadjuvant chemoradiotherapy with S-1 (120 mg/day) and radiation (50 Gy/25 Fr) was administered. Eight weeks after the last chemoradiation therapy, we performed laparoscopic Hartmann's operation. Case 2 was a 59-year-old man diagnosed with cT4aN0M0 advanced rectal cancer. He was treated with XELOX plus bevacizumab as neoadjuvant chemotherapy. Four weeks after 4 courses of treatment, he underwent laparoscopic Mile's operation. Neoadjuvant therapy was performed without any complications, and a pathological complete response was achieved in both patients. These cases demonstrated that neoadjuvant therapy can be a promising option for locally advanced rectal cancer.

The prognosis of esophageal cancer patients who undergo non-curative resection is very poor. Here, we retrospectively analyzed the factors associated with non-curative resection. Thirty-five patients with cT3 or T4 thoracic esophageal cancer treated with neoadjuvant chemotherapy or chemoradiotherapy followed by esophageal resection were included in this study. Among the 35 patients, 27 underwent curative resection (R0 group), while 8 underwent non-curative resection (R1R2 group). A comparison of the clinicopathological factors between groups revealed no significant differences in presurgical factors. The pathological T factor was significantly deeper in the R1R2 group than in the R0 group (p=0.0086). Histopathological response tended to be higher in the R0 group (p=0.055). An accurate preoperative diagnosis of T factor is important.

Approximately 20% of patients develop some complications after gastrectomy. These complications should be treated appropriately to achieve a positive outcome. The records of 6 patients with postoperative intra-abdominal abscesses treated with interventional radiology (IVR) were analyzed. The cause of abscess was anastomotic leakage in 4 patients and contaminated surgery after gastric perforation in 2 patients. Intra-abdominal abscesses were detected on postoperative day 12 (median), and an IVR-guided drainage tube was inserted with a median interval of 1 day. The drainage tube was kept in place for 26 days (median), and patients were discharged 6.5 days (median) after drainage tube removal. No patients were converted to open surgery. Early IVR-guided drainage was essential and effective for intra-abdominal abscess treatment after gastrectomy.

Purpose Exfoliated malignant cells, present along staple lines of anastomosis, may be responsible for anastomotic recurrence of colon cancer. We aimed to assess the impact of surgical bowel occlusion around the tumor and intraluminal lavage on the presence of exfoliated malignant cells at anastomosis sites in patients with colon cancer. Methods In this prospective study, 32 patients with colon cancer, requiring right hemicolectomy between January 2007 and September 2008, were randomly assigned to a control group (no surgical bowel occlusion; 18 patients) and a “no-touch” group that underwent surgical bowel occlusion around the tumor before tumor manipulation (14 patients). The fluid used intraoperatively to irrigate the portion of the bowel clamped distal to the tumor was examined cytologically, and exfoliated cells of cytological classes IV and V were considered malignant. Results In the control group, 2 (11.1%) and 10 (55.6%) of 18 patients had exfoliated malignant cells at the terminal ileum and distal colon anastomosis sites, respectively; however, only 1 (7.1%) of the 14 patients in the no-touch group had exfoliated malignant cells at both the sites. The frequency of exfoliated malignant cells at the distal colon anastomosis site was significantly lower in the no-touch group (p = 0.0024). No exfoliated malignant cells were found upon saline irrigation of 400 ml or more in either group. Conclusion Measures, such as surgical bowel occlusion around the tumor and intraluminal lavage, can prevent or eliminate exfoliated malignant cells at anastomotic sites in patients with colon cancer.

Background This study was designed to conduct a retrospective analysis of late toxicity following chemoradiotherapy (CRT) for esophageal carcinoma.

Background and objective: The antiviral neuraminidase inhibitor oseltamivir (OSV) is used to treat influenza. The macrolide clarithromycin (CAM) is used to treat bacterial infections and has anti-inflammatory and immunomodulatory activities. This retrospective study investigated the immunomodulatory effects of CAM in children presenting with influenza A. Methods: The study recruited 40 children with acute influenza, and grouped them according to the treatment received: 5-day treatment with OSV (n = 14), CAM (n = 8), OSV + CAM (n = 12) and untreated (n = 6). The before and after treatment comparisons were made of the level of secretory IgA (sIgA) against influenza A virus (H3N2) and (H1N1), total sIgA, viral RNA copy numbers in nasopharyngeal aspirates and disease symptoms. Results: Infection induced anti-viral mucosal sIgA in the nasopharyngeal aspirates of most patients of all treatment groups. Particularly prominent increases in the levels were found in the CAM and OSV + CAM groups. Low induction of anti-viral sIgA was observed in the OSV group, but the addition of CAM to OSV augmented sIgA production and restored local mucosal sIgA levels. The frequency of residual cough in the OSV + CAM group was significantly lower than in the other groups including the group treated with OSV. Conclusions: CAM boosted the nasopharyngeal mucosal immune response in children presenting with influenza A, even in those treated with OSV who had low production of mucosal anti-viral sIgA, and alleviated the symptoms of influenza.

A 69-year-old man was referred to our hospital for investigation of leukocytosis and a persistent fever of 38 degrees C, but we could find no evidence of a specific infection. The leukocyte count was 18,000/mm(3), and the serum granulocyte colony-stimulating factor (G-CSF) and alpha-fetoprotein (AFP) levels were both elevated, at 66.3 pg/ml and 1,495 ng/ml, respectively. Computed tomography (CT) showed a gallbladder tumor and we performed extended cholecystectomy. Postoperatively, the fever subsided and the leukocyte count, serum G-CSF and AFP level normalized. Histologically, the tumor was a carcinosarcoma of the gallbladder. Immunohistochemical staining of the tumor cells was positive for AFP, but negative for G-CSF. This is the first report of a carcinosarcoma of the gallbladder producing AFP. The laboratory findings and clinical course strongly suggested that the tumor produced not only AFP, but also G-CSF.

症例は69歳の男性で，2004年1月十二指腸癌に対し，膵頭十二指腸切除術を施行した．2006年1月頃より腹満を自覚した．下部消化管内視鏡検査にて，肝彎曲部，横行結腸，S状結腸に全周性の狭窄を認め，バルーン拡張を行った．粘膜面に異常所見を認めず，粘膜下層の生検で悪性所見を認めず．再度腹満が出現し，4月には腸閉塞となった．腹部CTにて上行結腸，S状結腸，回盲部の壁肥厚を，18fluoro-deoxyglucose positron emission tomography（FDG-PET）にて横行結腸に異常集積像を認めた．画像検査から十二指腸癌の腹腔内播種も疑われたが，良性狭窄の可能性も否定しえなかった．同年5月に右半結腸切除術，横行結腸切除術，S状結腸切除術を施行した．病理組織学的検査で，いずれの狭窄部も筋層または粘膜下層に腺管形成を示す異型細胞の浸潤を認め，十二指腸癌の脈管性の大腸転移と診断した．十二指腸を原発とする脈管性の大腸転移は極めてまれであり，文献的考察を加えて報告する．

症例は59歳の女性で, 平成15年3月性交時の違和感を主訴に近医産婦人科を受診, 血中CA125高値と腹水を認め紹介となった. 精査にて腹膜原発表在性漿液性乳頭腺癌と診断され, 術前化学療法(CBDCA/TXL)の後, 両側付属器切除, 大網切除, 骨盤内および傍大動脈リンパ節郭清と同一レジメンでの術後化学療法が施行された. その後, 腫瘍の再発と消失, 血中CA125値の再上昇と正常化を繰り返しながら経過していた. 平成19年1月頃より胃部膨満感が出現し, 血中CA125値の再上昇を認めた. CTおよび上部消化管内視鏡検査にて幽門前庭部後壁に2cm大の腫瘍を認めた. 臨床経過から胃粘膜下腫瘍あるいは表在性漿液性乳頭腺癌の再発と診断し, 平成19年5月胃部分切除術を施行した. 肉眼検査所見では胃漿膜面に異常所見を認めず, 粘膜下腫瘍を強く疑わせる所見であり, 病理検査にて表在性漿液性乳頭腺癌の胃転移と診断された. 胃転移巣切除術後6か月を経た現在, 再発は認められていない.

患者は40歳，女性．人間ドックで後腹膜腫瘤を指摘され，当センターを受診した．腹部CTでは膵体部頭側に38×25mm大で石灰化を伴った造影される腫瘤を認めた．腹部MRIで腫瘤はT1で低信号，T2で淡い高信号を呈し，比較的濃染された．以上より腫瘤は血流豊富で石灰化を伴う後腹膜腫瘍であり，後腹膜原発神経原性腫瘍や悪性腫瘍の可能性も否定できないため，腹腔鏡下腫瘍摘出術を行った．術中腫瘍後面の剥離に難渋し出血も認めたため，開腹に移行して腫瘍を摘出した．肉眼所見では4×3cm大で被膜に覆われ，割面は淡褐色で一部石灰化による灰白色部分の混在を認めた．組織学的にはhyaline vascular型Castleman病と診断された．リンパ増殖性疾患であるCastleman病は腹部領域に石灰化を伴って発生することは極めて稀であり報告した．

Isolated granulomatous inflammation of the appendix is extremely rare, and its etiology is still unknown. Thus, differentiating between idiopathic granulomatous appendicitis and isolated appendiceal Crohn's disease is difficult. We report a case of idiopathic granulomatous appendicitis with long-term follow-up after surgery. A 26-year-old woman was referred to our hospital for investigation of persistent right lower abdominal pain. Abdominal computed tomography showed a mass, suggesting an inflammatory tumor around the appendix; thus, we suspected subacute appendicitis and performed laparoscopically assisted ileocecal resection. Histological examination of the resected specimen revealed several noncaseous epithelioid granulomas in the wall of the appendix, but no foreign bodies, obstructing lesions, or parasites. Stains for acid-fast bacillus and fungi, and serology for yersinia were all negative. The patient has been followed up for 9 years and remains asymptomatic. This clinical course suggests that her isolated granulomatous appendicitis was idiopathic granulomatous appendicitis unrelated to Crohn's disease.

肺腫瘤に対する治療方針の決定に際してはその質的診断が重要になる. 特に悪性腫瘍の術前検査時や切除後に孤立性の肺腫瘤が発見された場合は原発性肺癌, 転移性肺腫瘍, 良性肺腫瘤の鑑別が必須であり, 鑑別診断のためには第一に発生頻度を把握しておく必要がある. 2000年1月から2003年6月までに当院で悪性腫瘍の術前検査あるいは術後の経過観察中に発見され切除された孤立性肺腫瘤症例56例 (男性38例, 女性18例, 平均65.3歳) を検討した. 初回悪性腫瘍の手術から肺切除までの平均観察期間は33.5カ月で, 初回悪性腫瘍は大腸癌19例 (33.9%), 胃癌18例 (32.1%), 乳癌7例 (12.5%), その他12例であり, 結果は原発性肺癌30例 (53.6%), 転移性肺腫瘍15例 (26.8%), 良性肺腫瘤11例 (19.6%) であった. 悪性腫瘍の術後といえども原発性肺癌の発生に留意する必要があると考えられた.

皮膚筋炎合併胃癌において骨格筋転移を来した極めてまれな1例を経験したので報告する. 症例は65歳の男性で, 主訴は筋力低下, 食欲不振. 顔面・前胸部に不定形紅班あり. 両側手指背側にゴットロン兆候あり. CPK高値と胃体上部に3型腫瘍を認めた. 皮膚筋炎合併胃癌と診断し, ステロイド投与開始3週後胃全摘術を施行した. 病理組織学的検査はpor1, pT2 (SS), pN1, pH1, pP0, CY0, pM0, Stage IVであった. 術後1か月目から約7か月間TS-1による化学療法を施行した. CEA値は正常化し筋力低下は改善した. 術後9か月目に右肩背側, 右大腿外側, 右膝上内側に有痛性の腫瘤が出現した. MRIにて右外側広筋, 棘下筋内にリング状に濃染される腫瘤像を認めた. 大腿外側の経皮吸引針生検にて転移性腺癌と診断した. Paclitaxelによる化学療法を開始したが, 肝転移巣の増大, 傍大動脈リンパ節の腫大と腹水を認め, 術後11か月で死亡した.
